Undiagnosed myocardial infarction of the right ventricle presented as Dressler's syndrome. Radioisotopic diagnostic procedures including first-pass and multigated acquisition nuclear angiography ( MUGA ) and thallium-201 perfusion studies enabled a retroactive diagnosis of myocardial infarction, showed it to be in the right ventricle, and clarified the aetiology of the unexplained fever and pleuropericarditis.

The association of pericarditis and pulmonary embolism may be the source of diagnostic error and delay in the administration of anticoagulant therapy. Two cases are reported. Pericarditis occurred late in patients with severe, chronic pulmonary embolism with electrocardiographic changes of acute cor pulmonale. Two physiopathological mechanisms for this association have been proposed. The first, haemodynamic, suggests friction between the pericardium and distended right ventricle and pulmonary artery. The second, an immunological hypothesis, compares the association of pericarditis and pulmonar

A case of Dressler's syndrome with cardiac tamponade is presented. 19 days after an acute lateral myocardial infarction, the patient, 46 years old, was admitted the second time with a characteristic clinical picture of cardiac tamponade. The patient did not receive at any time during his admission an anticoagulant treatment. The clinical diagnosis was supported by fluoroscopic and echocardiographic findings. A pericardiocentesis performed immediately revealed a hemorrhagic pericardial fluid. After withdrawal of 60 ml, the patient's clinical condition improved dramatically and this improvement

Congestive heart failure, shock or severe arrythmias after myocardial infarction can be caused by mechanical complications suitable for surgical correction. These complications - such as aneurysms, rupture of the ventricular wall, septum or papillary muscle, pericardial tamponade - are reliably detected or excluded by echocardiography. Additional aspects related to the surgical techniques can be evaluated, moinly the resectability of left ventricular aneurysms. From these, pseudoaneurysms bearing a high risk of rupture can be differentiated. The causes of systolic murmurs after myocardial infa

This study examines the current incidence of postmyocardial infarction (Dressler's) syndrome. During 1980, 282 patients with documented myocardial infarction were admitted to our coronary care unit. Early postmyocardial infarction pericarditis was present in 18 patients (6.4%). Six of these patients received steroids and the remainder were treated with salicylates or other anti-inflammatory drugs. Anticoagulation was used in 149 patients (53%) during hospitalization. One hundred forty-four (51%) were receiving heparin and 133 (47%) received no anticoagulation. Information on the patient's stat

Three patients developed a polyarthritis in association with Dressler's (post-myocardial infarction) syndrome. Joint involvement was more pronounced in the upper limbs and persisted many months after other features of Dressler's syndrome had settled. In 2 cases the findings in the joints were slight and might have been overlooked but for the persistently raised ESR. There was a prompt symptomatic response to prednisone.

Exertional neck tightness developed 6 months following a myocardial infarction that was complicated by Dressler's syndrome. The patient's symptoms were initially interpreted as angina pectoris but did not respond to antianginal therapy. Findings at cardiac catheterization were indicative of constructive pericarditis. The diagnosis was confirmed at pericardiectomy and the symptoms resolved completely postoperatively. Constrictive pericarditis should be considered as an unusual but important sequel of Dressler's postmyocardial infarction syndrome.

The authors report the case of anteroseptal myocardial infarction, complicated by a late pericardial reaction (5th - 10th week), the development of an aneurysm, and, above all, refractory ascites (12th week) found to be secondary to constrictive pericarditis. Pericardectomy and partial resection of the aneurysm were performed. Constrictive pericarditis is rare after myocardial infarction and its relationship to a forme fruste of Dressler's syndrome remains uncertain. The pathogenesis of the constriction is unknown. This complication (only reported once previously) should be recognised because

A 49-year-old woman with progressive angina pectoris developed chronic Dressler's syndrome following a second myocardial infarction. Control of the chronic pericarditis required long-term steroid therapy. Because of multiple complications generated by the steroid administration, she underwent coronary angiography followed by pericardiectomy and coronary artery bypass surgery. The patient remains asymptomatic without steroid or antianginal medication five years after surgery.
