The clinical and laboratory findings diagnostic of acute myocardial infarction include at least two of the following: (1) a history of pain consistent with myocardial ischemia, (2) electorcardiographic findings consistent with infarction, and (3) a rise in the serum level of specific cardiac enzymes. By the 4th or 5th day of illness, specific criteria can be applied to assign certain patients to a subset with "uncomplicated completed acute myocardial infarction." These criteria include the absence of evidence of (1) continuing cardiac ischemia, (2) left ventricular failure, (3) shock, (4) impo

This review summarizes current knowledge concerning the value of systolic time intervals in coronary artery disease. Although the usual pattern of prolongation of the preejection period (PEP) and shortening of the left ventricular ejection time (LVET) characteristic of left ventricular failure is seen in acute myocardial infarction, the systolic time intervals (as well as all other measures) are profoundly influenced by adrenergic hyperactivity characteristics of this disorder. Adrenergic stimulation normally shortens both the PEP and LVET indexes and decreases the PEP/LVET ratio. The degree o

In the etiology of uni, bi- and trifascicular block pictures the acute myocardial infarction plays an important role. From the topographic relations of the supply of coronary vessels to the coduction system certain correlations between localisation of infarction and kind of blocking are to be explained. The electrocardiographic changes of the individual block pictures are explained. 765 (581 males, 184 females) patients with acute myocardial infarction were examined concerning frequency and prognosis of the left anterior hemiblock and the bifascular block of anterior type (left anterior hemibl

Thirty-seven patients with non-rheumatic subvalvar mitral regurgitation are reported, representing 16% of all patients with mitral regurgitation submitted to open operation over a five-and-a-half-year period. In 22 older patients with `idiopathic' chordal lesions, the commonest finding was rupture of chordae to the posterior leaflet. The aortic leaflet chordae were most frequently involved following myocardial infarction (7 patients) or bacterial endocarditis (3 patients). Three other younger patients had ruptured chordae and two patients had rupture of the posteromedial papillary muscle follo

Current knowledge concerning the major hemodynamic features of acute myocardial infarction has been reviewed and discussed in relation to present concepts of cardiac pathophysiology. The physical examination provides a great deal of information and new, noninvasive methods promise to supplement the bedside appraisal of left ventricular function. Direct hemodynamic methods of serially monitoring patients with acute myocardial infarction are finding increasing application and recently have added considerably to our understanding of this condition. Certain limitations in the use of the central ve

The effectiveness of intravenous lignocaine in suppressing ventricular ectopic activity after acute myocardial infarction was examined in a double-blind trial in 82 patients. Whereas suppression of unifocal ventricular ectopics was achieved by lignocaine in 90% of patients, other forms of potentially more dangerous ectopic activity (multifocal or R-on-T ectopics) seemed more resistant to therapy. Cessation of ectopic activity was also observed in about one-third of the patients in the control group. The incidence of ventricular tachycardia and fibrillation and the mortality during and after th
