4F-MDMB-BICA toxicity: a fatal case and literature review of synthetic cannabinoid fatalities.

We report the death of a 32-year-old woman with a history of tobacco smoking who was found dead in her car after being investigated 18-days prior in hospital following a road traffic accident. Postmortem examination showed no evidence of recent injury. The most significant finding on internal examination was severe single-vessel coronary atherosclerosis. The remainder of the autopsy showed smoking-associated lung changes. Toxicologic studies were negative for common medications, usual street drugs and alcohols, but specialized analytical testing, developed in-house, identified the indazole-bas

Particle size distribution and concentration of Intralipid® 20.

SIGNIFICANCE: Intralipid® is widely used as a tissue-mimicking phantom due to its optical similarity to biological tissues, i.e., high scattering and low absorption of light. Accurate modeling of the optical behavior of phantoms requires detailed knowledge of the properties of its scatterers. AIM: We aim to determine the particle size distribution (PSD), concentration, and refractive index (RI) of particles in Intralipid® 20%. APPROACH: Cryo-electron microscopy, three different flow cytometers, and microfluidic resistive pulse sensing were used to measure the size distribution and concentrat

Antithrombotic Polymers: A Narrative Review on Current and Future Strategies for Their Design, Synthesis, and Application.

Bleeding and thromboembolism are among the leading causes of mortality worldwide. Thrombosis encompasses both arterial forms-primarily associated with atherosclerosis and leading to heart attacks or strokes-and venous forms. Microvascular thrombosis typically arises in the context of sepsis or systemic inflammation, and it became particularly prominent during the COVID-19 pandemic, substantially contributing to increased mortality. Given this burden, the rapid development of new therapies using advanced techniques and materials to prevent and treat these conditions is essential. This review su

Case Report: Elevated Lp(a) as a cause of severe ASCVD in a healthy veteran athlete with low calculated QRISK3 score.

Elevated plasma lipoprotein(a) [Lp(a)] is a causal risk factor for the development of atherosclerotic cardiovascular disease (ASCVD). However, commonly used ASCVD clinical risk-assessment tools in primary care do not include the measurement of Lp(a) levels, potentially under-estimating individual risk. Here we describe the case of a late-40s, asymptomatic, normotensive, non-smoking veteran athlete with a moderately raised low density lipoprotein cholesterol (LDL-C) level and a calculated 10-year QRISK3 score of 4.1%. Despite his low calculated QRISK3 score, significantly elevated Lp(a) levels

Effect of Angiographic Hemodynamic Parameters and Vascular Tortuosity on In-Stent Restenosis in Intracranial Atherosclerosis.

BACKGROUND AND OBJECTIVE: In-stent restenosis (ISR) is an important factor affecting the long-term efficacy of stenting for intracranial atherosclerotic stenosis (ICAS). We aimed to investigate the risk factors of ISR by analyzing the clinical features, angiography-based hemodynamic parameters, and vascular morphology. METHODS: Patients with severe symptomatic ICAS undergoing stenting were retrospectively screened. The vascular morphology was described by angle, relative length (RL), triangulation index (TI). Hemodynamic parameters were assessed by quantitative digital subtraction angiography

Low levels of atherogenic lipoproteins and incident atherosclerotic cardiovascular disease: A pooled cohort primary prevention study.

BACKGROUND: Elevated atherogenic lipoproteins increase risk of atherosclerotic cardiovascular disease (ASCVD), though long-term risk for adults without ASCVD who have low-normal levels has not been well described. METHODS: This study used pooled data from 16,384 individuals in 3 population-based prospective cohorts. At baseline all participants were without ASCVD and were not taking lipid-lowering therapy. We evaluated ASCVD events by baseline LDL-C, non-HDL-C and apoB, including low-normal values. ASCVD risk was assessed using multivariable Cox proportional hazards. RESULTS: The study cohor

Clinician-level variation in lipid management for secondary prevention of atherosclerotic cardiovascular disease: Opportunities for practice improvement.

INTRODUCTION: While prior studies have documented suboptimal lipid management and low rates of low-density lipoprotein cholesterol (LDL-C) goal achievement in patients with atherosclerotic cardiovascular disease (ASCVD), the degree of variability in lipid-lowering therapy (LLT) practice patterns between clinicians is less well described. METHODS: In this cohort study, we evaluated the use of LLTs and achievement of LDL-C <70 mg/dL among adults with ASCVD (coronary artery disease, peripheral arterial disease, or ischemic cerebrovascular disease) followed by a cardiology physician or advanced p

Synthesis, characterization, and nanoparticle formulation and evaluation of rosuvastatin-curcumin conjugate for atherosclerosis management.

The present study describes the rational design, synthesis, and nano formulation of a Rosuvastatin-Curcumin (RS-CU) conjugate aimed at overcoming the limitations of poor bioavailability and systemic adverse effects associated with conventional statin therapy. The RS-CU conjugate was synthesized via an esterification reaction and confirmed through FTIR, NMR, and mass spectrometry. The conjugate was successfully encapsulated into poly(lactic-co-glycolic acid) (PLGA) nanoparticles, which exhibited uniform spherical morphology with an average particle size of 152.6 ± 3.2 nm, a zeta potential of -2

Maternal supraphysiological hypercholesterolemia and its adverse impact on transgenerational cardiometabolic health: a literature review.

AIMS: Among the risk factors leading to cardiovascular disease (CVD), hypercholesterolemia stands out as a key driver of vascular dysfunction and the development of atherosclerotic CVD. This review is aimed to highlight the emergent evidence showing that maternal supraphysiological hypercholesterolemia (MSPH) is a key risk factor for transgenerational CVD risk and to advocate for the development of strategies for the early prediction and prevention of MSPH. DATA SYNTHESIS: Increasing evidence suggests that an individual's lifetime CVD risk may be modified by in utero exposure. However, the co

Giant Cell Arteritis/Polymyalgia Rheumatica and Atypical Pulmonary Carcinoid Tumor: A Paraneoplastic Syndrome?

BACKGROUND Giant cell arteritis (GCA) is the most common large-vessel vasculitis in individuals over age 50 years. Although it typically affects extracranial branches of the carotid artery, central nervous system involvement is rare and can manifest with ischemic stroke. Links between GCA and malignancy have been reported, especially hematologic cancers, but paraneoplastic GCA associated with solid tumors remains exceptional and poorly understood. CASE REPORT We describe a 59-year-old man presenting with temporal headaches, right-hand paresthesia, monocular visual loss, and gait instability. I

Incidence and clinicopathological analysis of portal vein and inferior vena cava thrombosis in autopsy cases of autosomal dominant polycystic kidney disease.

BACKGROUND: With regard to complications of portal vein (PV) and inferior vena cava (IVC) thrombosis in autosomal dominant polycystic kidney disease (ADPKD), several cases have been reported based on imaging findings. However, only one autopsy case has been described and no systematic analysis has been conducted to date. This retrospective study aimed to review autopsy cases from our department over the past 37 years to clarify the frequency and background factors of thrombosis formation in ADPKD. METHODS: Among 4001 autopsies performed at our institution from 1987 to 2023, 10 ADPKD cases wer

Sex differences in human umbilical vein endothelial cells following ox-LDL injury.

BACKGROUND: Numerous sex differences has been described in aterosclerosis including in endothelial dysfunction. Oxidized low-density lipoproteins (ox-LDL) contribute to the formation of atherosclerotic plaque by binding to a membrane glycoprotein expressed by endothelial cells. Ox-LDL also play a key role in mediating endothelial dysfunction during pregnancy. Elevated maternal ox-LDL levels can lead to oxidative stress, inflammation and apoptosis in placental and fetal endothelial cells. The aim of this study was to investigate sex-related differences in the response to ox-LDL-induced damage i

Fatal gelsemium alkaloid poisoning versus cardiovascular cause in a sudden death: a forensic case report.

Gelsemium alkaloids, such as gelsemine and koumine, are highly toxic compounds that can induce rapid respiratory and cardiovascular failure. Their clinical manifestations-dizziness, ptosis, and sudden collapse-closely resemble those of acute myocardial infarction, presenting diagnostic challenges in forensic practice, especially in distinguishing between poisoning and cardiovascular events. We describe a sudden death in a middle-aged male following a restaurant meal. The decedent developed acute dizziness and eyelid drooping shortly after ingestion and died within hours despite resuscitation.

TET2 in epigenetic control of immune cells: Implications for inflammatory responses and age-related pathologies.

Ten-eleven translocation 2 (TET2) is an epigenetic modifier whose canonical activity leads to the removal of cytosine methylation in the genome, which in essence results in the activation of gene expression. This function is particularly well described in the context of hematopoiesis and its alterations that lead to leukemia. However, in recent years, it has become evident that the noncanonical functions of TET2 also play a vital role in its activity. Rather than depending on its catalytic activity, these functions arise from TET2 interactions with other epigenetic modifiers. This review summa

From bench to bedside: the untold story behind the clinical development of enoxaparin.

The path to the development of enoxaparin (Lovenox/Clexane), one of the most widely used low-molecular-weight heparins worldwide, was far from smooth. This narrative review, presented from the personal perspective of the senior author who lived this journey, describes the pivotal research conducted at McMaster University in Canada that transformed enoxaparin from a promising laboratory compound into a cornerstone of prevention and treatment of thrombosis. The story begins in the late 1970s with the observation that low-molecular-weight heparin, produced through controlled depolymerization, exh

Diagnostic Approach to Secondary Hypertension: Atherosclerotic Renal Artery Stenosis.

Secondary hypertension stems from identifiable causes, making targeted treatment essential to control blood pressure (BP) and mitigate the occurrence of complications. We describe a case of resistant hypertension in a 48-year-old male referred to the internal medicine clinic for investigation. A complementary study with abdominal and pelvic computed tomography revealed significant aortic atherosclerosis involving the emergence of the renal arteries, with particularly evident stenosis on the right. Following vascular intervention, the patient's BP stabilized with just two antihypertensive agent

Three-Dimensional Perspectives on Inflammatory Regulation in Coronary Atherosclerosis: Integrated Mechanisms of Endothelial Priming, Lipid Metabolism, and Cytokine Synergy.

Atherosclerosis, a leading cause of global mortality, is a chronic inflammatory disease driven by a vicious cycle of endothelial dysfunction, dysregulated lipid metabolism, and persistent inflammation. This review examines the mechanisms through which diverse triggers initiate the cycle. We discuss key cellular and molecular events, such as the detrimental phenotypic switching of vascular smooth muscle cells. We also describe the processes through which various upstream signals converge on core inflammatory hubs, such as the Toll-like receptor 4 (TLR4)/nuclear factor-κB (NF-κB) pathway and the

Successful Management of Aspirin Intolerance After Ad Hoc PCI: A Case Report and a Developed Algorithm.

In managing atherosclerotic cardiovascular disease, especially after percutaneous coronary intervention (PCI), dual antiplatelet therapy (DAPT), prominently aspirin and a P2Y12 inhibitor, is fundamental. Nonetheless, aspirin hypersensitivity complicates treatment for some patients. Desensitization processes have been identified as a viable method to circumvent this issue. This case report describes a 75-year-old man diagnosed with significant stenosis in the coronary artery who was scheduled for elective PCI of the right coronary artery. The patient's medical record noted a hypersensitivity to

Calcified Artery Preparation and Processing with Preserved Morphology and RNA for Digital Spatial Profiling.

Spatial transcriptomics maps whole-transcriptome profiles directly onto tissue sections, correlating between cellular neighborhoods and gene expression and how these activities drive health and disease. Although the approach has transformed oncology, neuroscience, immunology, developmental sciences, and many fields, it has been less widely used in vascular biology, even though vascular disease remains a leading cause of death worldwide. Two obstacles stand out: arteries and veins offer little flat surface for sectioning, and diseased specimens, often atherosclerotic, thrombosed, plaque-laden,
