Paradoxical Embolism in Young Patients With Acute Coronary Syndrome Without Coronary Atherosclerosis.

Acute coronary syndrome (ACS) in young patients (<50 years) accounts for 2%-10% of all ACS presentations. We describe a case series of young patients who presented with ACS in the absence of coronary atherosclerosis or other identifiable causes such as spontaneous coronary artery dissection and Takotsubo syndrome. Further diagnostic evaluation, often initiated after cryptogenic stroke or a recurrent systemic embolic event, revealed the presence of significant right-to-left shunting because of an atrial septal defect or patent foramen ovale. After exclusion of other etiologies for thromboemboli

Single-cell transcriptomic analysis identifies vascular smooth muscle cell transcriptional programs associated with human carotid plaque remodeling.

BACKGROUND: Vascular smooth muscle cells (VSMCs) contribute to plaque structure and exhibit diverse transcriptional programs in human atherosclerosis. Although human plaque single-cell studies have established substantial VSMC heterogeneity, an important unresolved question is whether discovery-defined programs remain recognizable across independent patients and sequencing platforms without forced integration, and whether those programs retain associations with plaque instability. We therefore focused on program-level reproducibility, inferred regulatory activity, continuous expression organiz

Clinical trajectory and disease progression in patients with cardio-renal-metabolic syndrome: the CaReMap observational study.

BACKGROUND: Cardio-renal-metabolic (CRM) syndrome describes the pathophysiological interconnection among diabetes mellitus (DM), atherosclerotic cardiovascular disease (ASCVD) and heart failure (HF), and chronic kidney disease (CKD). Limited studies simultaneously examine progression trajectories across all three conditions at the population level. We aimed to quantify the coexistence of CRM components, with a specific focus on patients with HF, and analyse survival and disease progression in a real-world setting. METHODS: CaReMap, an observational cohort study, analysed data from an Italian

Characterization of Congestive Heart Failure Phenotype in a Murine Model of JAK2V617F-positive Myeloproliferative Neoplasm.

BACKGROUND: Murine models are widely used to investigate cardiovascular disease (CVD), yet many established models rely on invasive surgery or genetic manipulation to induce hyperlipidemia and therefore may not fully capture the gradual cardiovascular consequences of mutant hematopoiesis. Tie2-Cre + / - FF1 + / - (Tie2FF1) mice express JAK2V617F mutation in both hematopoietic and endothelial cells and develop spontaneous CVD on a standard chow diet. METHODS: We applied a comprehensive, non-invasive cardiovascular phenotyping protocol to 1-year-old Tie2FF1 mice and age-matched controls. Assess

Vaccination as a Strategy for Cardiovascular Prevention: Closing a Gap in Clinical Practice.

Prevention of the consequences of atherosclerotic disease has traditionally focused on controlling individual risk factors such as dyslipidaemia, hypertension and diabetes. In recent years, the term residual risk has been used to describe the persistence of cardiovascular events despite the optimal management of these classical risk factors, with particular attention given to inflammation. Far less attention, however, has been paid to environmental risk factors, which affect entire populations and may account for a substantial proportion of temporal variations in event incidence, as well as th

Coronary Vasculitis and Myocarditis in Acute Systemic Lupus Erythematosus Flares With Macrophage Activating Syndrome.

Cardiovascular disease is a major cause of morbidity and mortality in systemic lupus erythematosus (SLE). Although accelerated atherosclerosis is well recognized, SLE-related coronary vasculitis and microvascular injury remain underdiagnosed. Cardiac magnetic resonance (CMR) offers unique tissue characterization that may reveal atypical patterns of myocardial injury. We describe 2 patients with active SLE who presented with acute cardiac symptoms and elevated cardiac biomarkers. CMR demonstrated both classic inflammatory findings of lupus myopericarditis (subepicardial late gadolinium enhancem

Pathway-Guided and Patient-Centered Therapeutic Strategies in Atherosclerotic Cardiovascular Disease.

Atherosclerotic cardiovascular disease (ASCVD), encompassing coronary artery disease (CAD), cerebrovascular disease (ischemic stroke, IS; and transient ischemic attack, TIA), and peripheral artery disease (PAD) of atherosclerotic origin, remains a leading cause of morbidity and mortality worldwide despite major advances in prevention and treatment. Although atherosclerosis arises from the convergence of endothelial injury, lipid accumulation, inflammation, and thrombosis, the relative contribution of these pathogenic processes varies markedly among individuals, due to the effects of genetics,

Temporal Association Between Aromatase Inhibitor Initiation and New-Onset Atrial Fibrillation: A Case Report.

INTRODUCTION: Breast cancer is the most prevalent cancer among women worldwide with a higher probability of diagnosis in the postmenopausal years. Aromatase inhibitors (AIs) block the aromatase enzyme responsible for converting androgens into estrogens in the peripheral tissues, which become the main source for estrogen production in this population. Estrogen plays a key role in promoting vasodilation and inhibiting vascular injury and atherosclerosis development. Reduced levels can contribute to adipocyte hyperplasia, insulin resistance, and dyslipidemia, all of which are risk factors for atr

Bilateral Medial Medullary Syndrome Following Anterior Odontoid Screw Fixation for a Type II Fracture: A Case Report.

INTRODUCTION: Medial medullary syndrome is an uncommon brainstem stroke, and bilateral involvement is rarer still. It has not, to our knowledge, been described as a complication of anterior screw fixation for a type II odontoid fracture. CASE REPORT: A 28-year-old man sustained a posteriorly displaced type II odontoid fracture with an associated C2 spinous process fracture in a road traffic accident. He was neurologically intact at admission. Surgery was briefly postponed after a transient episode of anisocoria during induction that resolved without deficit and with a normal computed tomograp

U.S. Trends in 10-Year Cardiovascular Risk Scores Over 2 Decades.

BACKGROUND: The PREVENT (Predicting Risk of Cardiovascular Disease Events) equations estimate 10-year risk of total cardiovascular disease (CVD), atherosclerotic CVD (ASCVD), coronary heart disease (CHD), stroke, and heart failure (HF). OBJECTIVES: The objective of the study was to describe the trends in PREVENT-based 10-year CVD risk among U.S. adults. METHODS: We used data from the National Health and Nutrition Examination Survey (1999-2023) to estimate 10-year risk of total CVD, ASCVD, CHD, stroke, and HF among adults aged 30 to 79 years. Crude and age-standardized risk scores were calcul

AI-Assisted Carotid Ultrasound for Nonexpert Use in Primary Care Cardiovascular Prevention: User-Centered Iterative Development Study.

BACKGROUND: Carotid ultrasound is traditionally confined to specialist settings because examination quality and diagnostic reliability are highly dependent on the operator's technical skill and experience. Recent advances in AI-assisted ultrasound may enable task shifting to primary care staff and support the integration of personalized visual risk communication into routine preventive care. Visualization of subclinical atherosclerosis has been shown to strengthen cardiovascular risk communication and support preventive engagement. However, limited evidence exists on how such technology can be

Glycation and receptors: coming of AGE in the midst of danger.

The formation of advanced glycation end products (AGEs), first described in food science, is driven by hyperglycaemia (diabetes mellitus) and the modifications on long-lived proteins (natural ageing); AGE formation is enhanced by oxidative stress and inflammation, which are factors that accompany high-AGE settings of diabetes mellitus and ageing. Exogenous tobacco, ultra-processed foods, pollutants and alcohol contribute to AGE formation. Evidence demonstrates higher concentrations of AGEs in the blood and tissues of patients with diabetes mellitus than in samples from healthy control individu

Quadtree SBFEM and MSPM Based Uncertain Damage Analysis With Random Field Model for Soft Biological Tissues.

This paper presents a numerical model for damage problems in soft biological tissues with random fields, integrating the advantages of the modified stochastic perturbation method (MSPM) and quadtree scaled boundary finite element method (quadtree SBFEM). The anisotropic hyperelastic constitutive model is used, and the damage behavior is described by a gradient-enhanced damage model without mesh dependence. The deterministic problem is formulated by the quadtree SBFEM with the benefit of convenient mesh regeneration directly from images. In order to describe spatial heterogeneous soft biologica

Clinical considerations on the use of evinacumab for the treatment of homozygous familial hypercholesterolemia.

Homozygous familial hypercholesterolemia (HoFH) is a rare genetic disorder that results in severely elevated low-density lipoprotein cholesterol (LDL-C) due to dysfunctional activity of the low-density lipoprotein receptor (LDLR) and impaired clearance of LDL-C. Individuals with HoFH require intensive lipid-lowering therapies (LLTs), ideally starting in early childhood, to aggressively lower LDL-C levels and mitigate risk of early-onset atherosclerotic cardiovascular disease caused by lifetime exposure to high LDL-C levels. However, most conventional LLTs lower LDL-C through an LDLR-dependent

Aging Affects Pancreatic α-Cell Function and Promotes Hyperglucagonemia: Implications in Age-Associated Diabetes.

Aging is a major risk factor in the development of type 2 diabetes mellitus (T2DM). With age, pancreatic β-cells undergo functional and cell identity changes that can compromise insulin secretion, thereby contributing to impaired glucose tolerance and T2DM. However, glucose homeostasis also depends on pancreatic α-cells. Excessive plasma glucagon levels, along with impaired regulation of α-cell secretion, are involved in the pathophysiology of T2DM. Despite their importance, the effects of aging on α-cells have been overlooked. In the present study, we analyzed pancreatic α-cells in aged mice

Treatment patterns in homozygous familial hypercholesterolemia: Undertreatment and delayed adoption of low-density lipoprotein receptor-independent therapies.

BACKGROUND: Homozygous familial hypercholesterolemia (HoFH) is an ultrarare disease characterized by extremely high levels of low-density lipoprotein cholesterol (LDL-C) and low-density lipoprotein receptor (LDLR) dysfunction, making most LDLR function-dependent lipid-lowering therapies (LLTs) less effective. For HoFH, LDLR-independent LLTs are usually required to meaningfully reduce LDL-C levels and atherosclerotic cardiovascular disease risk. OBJECTIVE: To examine treatment patterns and pathways among people with HoFH. METHODS: A retrospective HoFH cohort was formed using closed claims for

MYO1G Facilitates Caveolin 1-Mediated Endocytosis of the N-Terminal Fragments of Gasdermin E to Restrain Pyroptosis and Chemotherapy Efficacy.

UNLABELLED: The N-terminal fragments of gasdermin (GSDM-NT) form pores on the plasma membrane that initiate pyroptosis. However, the presence of GSDM-NT pores does not necessarily result in cell death, allowing some cells to survive GSDM-mediated pyroptosis. Understanding the regulators of response to the formation of GSDM-NT pores is crucial for revealing the strategies to harness the potential of pyroptosis for treating cancer. In this study, we found that myosin 1G (MYO1G) enabled cells to resist death driven by membrane rupture. Mechanistically, MYO1G tethered GSDME-NT pores to caveolin 1

Metabolic reprogramming in diabetic panvascular disease: Molecular mechanism and therapeutic strategies.

Diabetes mellitus substantially increases the risk of cardiovascular, cerebrovascular, renal, retinal and peripheral vascular complications. These are collectively referred to as diabetic panvascular disease. A traditional approach that treats each complication separately is no longer sufficient. An emerging framework places metabolic reprogramming at its center. This review examines how the diabetic environment, including hyperglycemia, insulin resistance, and dyslipidemia, rewires core metabolic pathways in vascular cells. Major changes include the diversion of glucose through the polyol, he
