Background and objective Extracranial carotid and vertebral artery (VA) atherosclerosis is an important contributor to ischemic stroke and recurrent cerebrovascular events. Color Doppler ultrasound is widely used as an accessible first-line modality, whereas CT angiography (CTA) and MR angiography (MRA) provide additional anatomical information regarding stenosis, plaque morphology, occlusion, and vascular variants. The objective of this study was to describe extracranial carotid and VA abnormalities in ischemic stroke patients using color Doppler ultrasound, CTA, and MRA and to assess aggrega

Dyslipidemia is clinically heterogeneous, and LDL-C alone may not fully capture atherogenic particle burden, metabolic risk, treatment response, or inherited disease. This case-based review presents three dyslipidemia phenotypes that illustrate common diagnostic and management challenges. The first case describes diet-sensitive LDL particle and lipoprotein insulin resistance changes despite LDL-C values near conventional targets in a patient with subclinical coronary atherosclerosis. The second highlights persistent low HDL-C and HDL particle number despite substantial LDL-C reduction with non

BACKGROUND AND OBJECTIVES: Internal jugular vein (IJV) compression at the skull base represents a rare cause of cerebral venous outflow obstruction and may mimic Eagle's syndrome. Unlike classic Eagle's syndrome, this condition results from venous compression between the hypertrophic C1 transverse process and the styloid process, producing symptoms such as pulsatile tinnitus, headache, and cognitive disturbances. We describe our experience highlighting the diagnostic protocol and management strategies.
METHODS: We retrospectively reviewed 5 patients with symptomatic IJV compression syndrome.

ABSTRACT: Cardiovascular disease (CVD) remains the leading cause of death globally, affecting populations in both developed and low- and middle-income countries. Although age-adjusted mortality rates from coronary artery disease (CAD) have declined in regions such as the United States and Europe, sudden cardiac death continues to account for a significant proportion of CVD-related fatalities. The concept of the "cardiovascular continuum," introduced more than a decade ago, describes the progressive nature of CVD-beginning with modifiable risk factors such as hypertension, diabetes, and smoking

Background, objective, and methods: Antiphospholipid syndrome (APS) is classically regarded as a cause of thrombosis and stroke in younger patients and is often considered a rare etiology beyond the sixth decade. We retrospectively describe seven older adults (ages 67-90 years; median 76; four females) with ischemic stroke in whom antiphospholipid antibodies (aPL) were identified during evaluation, most after one or more cerebrovascular events. Results: Six of the seven had radiographic or clinical evidence of prior cerebrovascular disease, and several had experienced recurrent events despite

Endothelial cells (ECs) line the vasculature and integrate hemodynamic, inflammatory, and metabolic cues that are crucial in maintaining vascular health. Endothelial dysfunction is a driver of thromboinflammatory conditions including atherosclerosis, thrombosis, stroke, and sepsis-induced multiorgan dysfunction syndrome. Therapies for these conditions that are given systemically fail to target the site of disease and can be associated with off-target side effects. EC-targeted nanoparticles represent a novel method of concentrating the drug- or gene-based therapy in ECs at the site of disease.

BACKGROUND: Fisher described several distinct vascular pathologies underlying lacunar infarction. However, subsequent literature and clinical practice have often misattributed symptomatic lacunar stroke primarily to lipohyalinosis and intracranial branch ostial atherosclerosis. Classic neuropathologic studies indicate that many lacunar strokes are instead caused by microatherosclerosis - small atheromatous plaque within penetrating arteries. This study aims to quantify the proportion of symptomatic lacunar infarcts in clinical practice attributable to microatherosclerosis.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have substantially improved long-term oncologic outcomes in patients with advanced malignancies; however, ICI-related cardiovascular adverse events have emerged as a growing clinical challenge. Accumulating clinical evidence confirms that ICI treatment markedly increases the risk of atherosclerotic cardiovascular disease (ASCVD). Although cardiovascular toxicities secondary to ICIs are relatively infrequent, these complications carry severe and potentially fatal clinical consequences. Herein, we describe a rare case of acute non-ST-segment elevat

The chronic low-grade inflammation driven by NLR family pyrin domain containing 3 (NLRP3) inflammasome is increasingly acknowledged as an underlying mechanism of myocardial infarction, heart failure, atrial fibrillation, hypertension, and atherosclerosis. Extracellular Ca2+, amino acids, and other ligands link to intracellular Ca2+ overload, mitochondrial dysfunction, reactive oxygen species generation, thioredoxin-interacting protein activation, and NF-κB-dependent inflammasome priming via the calcium-sensing receptor (CaSR). Experimental studies indicate that CaSR may activate the NLRP3 infl

BACKGROUND: Optimal antithrombotic therapy for ischemic stroke due to symptomatic large-artery atherosclerosis in patients with atrial fibrillation remains uncertain. We describe real-world prescribing patterns and outcomes across antithrombotic regimens.
METHODS: We retrospectively analyzed consecutive patients with atrial fibrillation and ischemic stroke related to symptomatic large-artery atherosclerosis (intracranial or extracranial or both) at 4 US comprehensive stroke centers. Patients were grouped by discharge regimen: single anticoagulant, single antiplatelet (sAP), single anticoagula

Hypophosphatasia (HPP) is a rare recessive or dominant autosomal disease caused by loss-of-function variants in the ALPL gene, which encodes tissue-nonspecific alkaline phosphatase (TNSALP). Molecules involved in the pathogenesis of HPP are alkaline phosphatase (ALP), inorganic pyrophosphate (PPi; an inhibitor of bone mineralization), pyridoxal-5'-phosphate (PLP; responsible for the neurological complications) and phosphoethanolamine. Asfotase alfa is a recombinant, bone-targeted, human TNSALP approved to treat patients with HPP. Its clinical effectiveness has been demonstrated in real-world s

PURPOSE OF REVIEW: To assess and present the current state of gene editing for reduction of atherosclerotic cardiovascular disease (ASCVD) with emphasis on technology and ongoing trials for genetic editing of protein convertase subtilisin/kexin type 9 (PCSK9).
RECENT FINDINGS: There are two ongoing clinical trials in humans to edit PCSK9 using CRISPR-Cas9 base editing technology packaged within lipid nanoparticles (LNPs). Preliminary interim and preclinical data suggest that this strategy is effective for long-term reduction of PCSK9 and LDL-C following a single IV infusion. Monoclonal antibo

PURPOSE OF REVIEW: Lipoprotein(a) [Lp(a)] is an independent, genetically determined, and causal risk factor for atherosclerotic cardiovascular disease (ASCVD) and calcific aortic valve stenosis. Elevated Lp(a) affects approximately 20% of the global population and is not meaningfully modified by lifestyle intervention or existing lipid-lowering pharmacotherapy. This review summarizes the biology of Lp(a) as a therapeutic target, evaluates the efficacy of currently available lipid-lowering agents against this target, and describes the mechanistic principles, clinical evidence, and unresolved qu

Acute coronary syndrome (ACS) in young patients (<50 years) accounts for 2%-10% of all ACS presentations. We describe a case series of young patients who presented with ACS in the absence of coronary atherosclerosis or other identifiable causes such as spontaneous coronary artery dissection and Takotsubo syndrome. Further diagnostic evaluation, often initiated after cryptogenic stroke or a recurrent systemic embolic event, revealed the presence of significant right-to-left shunting because of an atrial septal defect or patent foramen ovale. After exclusion of other etiologies for thromboemboli

BACKGROUND: Vascular smooth muscle cells (VSMCs) contribute to plaque structure and exhibit diverse transcriptional programs in human atherosclerosis. Although human plaque single-cell studies have established substantial VSMC heterogeneity, an important unresolved question is whether discovery-defined programs remain recognizable across independent patients and sequencing platforms without forced integration, and whether those programs retain associations with plaque instability. We therefore focused on program-level reproducibility, inferred regulatory activity, continuous expression organiz

BACKGROUND: Cardio-renal-metabolic (CRM) syndrome describes the pathophysiological interconnection among diabetes mellitus (DM), atherosclerotic cardiovascular disease (ASCVD) and heart failure (HF), and chronic kidney disease (CKD). Limited studies simultaneously examine progression trajectories across all three conditions at the population level. We aimed to quantify the coexistence of CRM components, with a specific focus on patients with HF, and analyse survival and disease progression in a real-world setting.
METHODS: CaReMap, an observational cohort study, analysed data from an Italian

BACKGROUND: Murine models are widely used to investigate cardiovascular disease (CVD), yet many established models rely on invasive surgery or genetic manipulation to induce hyperlipidemia and therefore may not fully capture the gradual cardiovascular consequences of mutant hematopoiesis. Tie2-Cre + / - FF1 + / - (Tie2FF1) mice express JAK2V617F mutation in both hematopoietic and endothelial cells and develop spontaneous CVD on a standard chow diet.
METHODS: We applied a comprehensive, non-invasive cardiovascular phenotyping protocol to 1-year-old Tie2FF1 mice and age-matched controls. Assess

Prevention of the consequences of atherosclerotic disease has traditionally focused on controlling individual risk factors such as dyslipidaemia, hypertension and diabetes. In recent years, the term residual risk has been used to describe the persistence of cardiovascular events despite the optimal management of these classical risk factors, with particular attention given to inflammation. Far less attention, however, has been paid to environmental risk factors, which affect entire populations and may account for a substantial proportion of temporal variations in event incidence, as well as th

Cardiovascular disease is a major cause of morbidity and mortality in systemic lupus erythematosus (SLE). Although accelerated atherosclerosis is well recognized, SLE-related coronary vasculitis and microvascular injury remain underdiagnosed. Cardiac magnetic resonance (CMR) offers unique tissue characterization that may reveal atypical patterns of myocardial injury. We describe 2 patients with active SLE who presented with acute cardiac symptoms and elevated cardiac biomarkers. CMR demonstrated both classic inflammatory findings of lupus myopericarditis (subepicardial late gadolinium enhancem
