Diet and atherosclerosis.

Because of the statistical establishment of elevated blood lipids as a risk factor in the development of atherosclerotic heart disease, most of the attempts to regulate blood lipids by diet are centered on the fat in the diet. The levels of blood lipids and the course of experimental atherosclerosis can be affected by other dietary components such as type and amount of protein, carbohydrate, and nonnutritive fiber. Interaction among the dietary components further affects serum lipids and atherosclerosis.

Myocardial infarction in young women.

This study of 24 women under age 40 years with myocardial infarction demonstrates that even in young women myocardial infarction is most commonly due to coronary atherosclerotic heart disease. Other causes of coronary occlusion were documented in 17% of these patients, indicating that these lesser causes of myocardial infarction are more common in young women than in older persons or in young men. In those patients with coronary atherosclerosis one or more significant risk factors could usually, but not always, be documented. The clinical manifestation of the coronary occlusion in the study gr

[The spontaneous course of coronary disease].

The natural history of coronary atherosclerotic heart disease was studied in a retrospective analysis of 160 patients, who underwent coronary arteriography from 1965 to 1969. All had at least one angiographically documented significant stenosis of more than 50%; the survivors were followed for at least 5 years and a maximum of 9 years. For the entire group, 5-year survival was 81%. Subdivided into single, double or triple vessel disease categories, the 5-year survival rates were 91, 85 and 63% respectively.

Value of exercise testing for screening asymptomatic men for latent coronary artery disease.

Treadmill exercise testing identifies a group of men at high risk for coronary atherosclerotic heart disease. However, the predictive value and sensitivity limitations are obvious. An abnormal electrocardiographic response does not absolutely predict the presence of coronary atherosclerotic heart disease, and a normal response does not rule out this possibility. Thus in appropriate instances when the minimal risk of coronary angiography is justified this procedure can be used to determine the anatomic correlation of exercise-induced functional ST-segment changes.

[Proceedings: Results of the surgical treatment of valvulopathies and ischemic heart disease in patients older than 65 years].

53 patients aged from 65 to 76 years underwent surgery for valvulopathies (23 patients), atherosclerotic heart disease (24 patients) or both diseases combined (6 patients). All these cases were in NYHA fonctional classes III and IV except for one patient in class II. Hospital mortality after surgical treatment of these cardiopathies was 3.8% (2 deaths). The late mortality of 19% (10 deaths), though high, was of non-cardiac origin in 7 of these patients. The clinical and hemodynamic findings in the two patients who died in the postoperative period were compared with those in the others. These t

Pathology of atherosclerotic heart disease in sudden death. I. Organizing thrombosis and acute coronary vessel lesions.

The significance of organizing thrombosis was investigated in 121 cases of sudden death with atherosclerotic heart disease. Organizing thrombosis was observed in one-third of the cases, and sites at which it was present showed a higher prevalence of acute lesions -- thrombus, plaque rupture, and hemorrhage -- than sites at which it was not present. Acute and subacute coronary lesions were almost entirely restricted to vessels with 75% or greater obstruction. Evidence was presented suggesting that the organization of thrombosis in the coronary arteries was a more extended process than that obse

Hyperventilation syndrome: a frequent cause of chest pain.

Chest pain is frequently a prominent symptom of the hyperventilation syndrome (HVS) and must be distinguished from angina pectoris due to coronary atherosclerotic heart disease (CAHD). The association between hyperventilation and chest pain may be apparent if psychoneurotic traits or anxiety are present. Many patients with HVS are not overtly anxious or neurotic, but in the great majority, a careful history and physical examination will indicate whether chest pain is due to HVS or CAHD. The failure to make this clinical differential diagnosis, which often leads to unnecessary coronary angiogra

Pathology of atherosclerotic heart disease in sudden death. II. The significance of myocardial infarction.

The role of myocardial infarction was investigated in 121 cases of sudden death with atherosclerotic heart disease. In addition to supporting other reports which have demonstrated the importance of chronic occlusion of the coronary vessels in relation to the high rate of infarctions found in such cases, the authors presented evidence which showed differences in occurrence in the sites of remote and recent infarctions, a lack of concordance between the sites of recent infarctions, and acute thrombosis in coronary vessels proximal to these lesions. No relationship between patient age or prior sy

Arterial embolism. An analysis of 115 surgical procedures.

Arterial embolus is usually a serious complication of rheumatic or atherosclerotic heart disease. One hundred-fifteen surgical procedures performed in 86 patients with arterial emboli in a 10 years' period in the Department of Adult Thoracic and Cardiovascular Surgery of our university are analyzed regarding the etiology, the treatment, and the results of this complication. Most of the patients were delayed cases showing mild to severe degrees of trophic, sensory and motor disturbances in the limbs without gangrene. These cases are also subjected to vascular surgery (removal of the embolus) as
