Two patients who had previously experienced old myocardial infarction and who died suddenly after an attack of chest pain were examined and discussed. In both cases two of the three main coronary arteries showed severe stenosis with canalization. Ruptured atheromatous plaque was found in the unblocked coronary artery. Pulmonary diffusive capacity by the carbon monoxide method was evaluated in 43 patients two years after myocardial infarction, and without evidence of other types of heart disease. The patients did not have primary lung disease or clinic bronchitis. Special interest was given to the effects of cigarrette smoking and moderate pulmonary congestion. A significant decrease in diffusion capacity was observed in smokers and former smokers compared to none smokers. A coronary angiography was taken to 80 patients whose twelve leads exercise ECG had been found positive. This group included 38 patients with old myocardial infarction and 42 with coronary insuficiency. When looking for a relation between: 1. The amount of positivity in the exercise ECG and the number on injured vessels, and 2. the ischemia located in the effort test and the blocked vessels shown by the angiography. An analytical study was performed to estimate the magnitude of the visual Q wave duration bias produced by pressurized ink round stylus electrocardiographic (ECG) recorders. With a paper speed of 25 mm/sec and the ECG tracing width of 0.25 mm, the visually measured Q waves are on the average 8 msec too short. The corresponding error with the older type flat stylus recorder is less than 2 msec. CPK MB isoenzymes were qualitatively identified using an electrophoresis method in 166 patients with acute cardiovascular disease during a six month period. Forty-three had total CPK activity in the normal or borderline range (0--75 U/Liter at the time the MB isoenzyme was first identified. Lymphocyte transformation and serum levels of immunoglobulins and complement were measured in groups of patients with old myocardial infarction, with peripheral vascular disease and in age- and sex-matched controls. There were no significant differences between the groups in phytohaemagglutinin (PHA) induced lymphocyte transformation, in IgA or in serum complement levels. The levels of IgG and IgM were significantly raised in patients with old proven infarcts while the level of IgM was raised in patients with peripheral vascular disease.

Myocardial 201Tl uptake and regional blood flow by the microsphere technique were determined in anesthetized dogs undergoing either 20 min of coronary occlusion and 100 min of reperfusion (N = 10) or 120 min of occlusion (N = 4). In both groups, 201Tl was injected intravenously after 10 min of occlusion. Concentration of 99mTc-pyrophosphate (99mTc-PPi) in the area of left ventricular aneurysms is discussed. In three cases clinical, laboratory, and electrocardiographic (ECG) evidence did not indicate that these patients currently had an acute myocardial infarction, but each patient had a clinical history and an ECG picture compatible with an old myocardial infarction. Cardiac catheterization revealed a large left ventricular aneurysm in all three cases. The reason for the preferential uptake of the radionuclide in the area of these aneurysms is not certain at this time.

During the four year period from 1972 to 1975, eleven patients, eight with recurrent and three with first attacks of ventricular fibrillation, underwent aortocoronary bypass graft and/or resection of ventricular aneurysm. Using an experimental canine model, we have employed echocardiography to study segmental dyskinesis produced by acute coronary occlusion. Characteristic alterations in posterior wall and septal motion occur after acute coronary ligation; these resemble clinical abnormalities described in acute and old myocardial infarction. In 13 patients with old myocardial infarction diagnosed with use of the electrocardiogram, coronary angiogram and left ventriculogram and in 11 patients without infarction, thallium-201 imaging with a color display computer system was carried out.
