Acute type A aortic dissection: assessment of surgically treated and autopsied cases.

BACKGROUND: Acute type A aortic dissection (TAAD) is the most frequently encountered pathological condition of the aorta and constitutes one of the most fatal cardiovascular emergencies. This study aimed to determine the risk factors in aortic dissection and aortic dissection-related mortality by examining the characteristics of surgically treated TAAD cases and autopsied TAAD cases. METHODS: A total of 157 cases from two centers were included in the study between 2003 and 2022: 141 cases of acute TAAD who underwent surgical treatment at the Department of Cardiovascular Surgery, and 16 autops

Body size in young adulthood is associated with thoracic aortic events later in life.

Objectives. Larger body size is associated with larger thoracic aortic diameter, a risk factor for thoracic aortic disease (TAD). Studies of body size-indexed aortic diameters suggest wider aortas may be less harmful in large people, but the association between body size and TAD events is unknown. This study aims to assess if body size is associated with TAD events. Methods. Swedish 18-year-olds attending mandatory military conscription 1969-1997 were included. Body size was assessed as height, weight, body surface area (BSA) and body mass index (BMI) stratified into quartiles. A composite end

Giant Coronary Artery Aneurysm Arising From a Septal Perforator: Diagnosis and Percutaneous Management.

OBJECTIVE: Coronary artery aneurysms (CAAs) are rare, and management is often uncertain. We present a case of a massive CAA arising from a large septal perforator branch of the left anterior descending coronary artery that was treated successfully with coiling with complete obliteration of the aneurysmal cavity. KEY STEPS: The coiling strategy was 2-fold: intrasaccular coiling to promote thrombosis of the aneurysmal cavity and coil embolization of the feeding septal perforator branch to eliminate residual flow. Angiography confirmed complete occlusion of flow to the aneurysm and the septal br

Basal Metabolic Rate and Risk of Aortic Aneurysm and Dissection: Results From Two Cohort Studies.

INTRODUCTION: Basal metabolic rate (BMR) may play a key role in the prevalence of aortic aneurysm and dissection. This study aimed to explore the association between BMR and the risk of abdominal aortic aneurysm (AAA), thoracic aortic aneurysm (TAA), and aortic dissection. METHODS: Mendelian randomization (MR) analyses were conducted to examine the causal relationship between BMR and aortic aneurysm and dissection. The data from 360,292 participants in the UK Biobank were analyzed using Cox regression models and restricted cubic spline. The results were further validated in a Chinese cohort o

Lipoprotein(a) and risk of subsequent Stanford type A aortic dissection in patients with acute coronary syndrome: a multicenter case-control study with Mendelian randomization analysis.

BACKGROUND AND AIMS: Lipoprotein(a) [Lp(a)] is associated with cardiovascular risk, but its role in Stanford type A aortic dissection (AD) following acute coronary syndrome (ACS) remains unclear. We evaluated whether Lp(a) at ACS presentation predicts subsequent type A AD, its incremental value, and to assess potential causal effects using a complementary two-sample Mendelian randomization (MR) analysis. METHODS AND RESULTS: In this bi-center retrospective case-control study, 222 patients with type A AD after ACS and 751 ACS controls were included. Overlap weighting balanced eight covariates.

Subacute aortic dissection presenting as heart failure due to aortic valve insufficiency and right coronary artery dissection.

Subacute thoracic aortic aneurysm dissection (TAAD) often presents with a clinical profile distinct from its acute form, which can closely mimic common cardiac conditions, risking diagnostic delay. We report a case of subacute TAAD presenting initially with progressive dyspnoea due to severe aortic regurgitation (AR), culminating in congestive heart failure with reduced left ventricular ejection fraction. Diagnosis was confirmed with CT angiography. Surgical repair was performed using the Hemi-Yacoub technique. The postoperative course was complicated by paroxysmal atrial fibrillation, which n

The Takotsubo Syndrome: Where the Sympathetic Stress Reflex and the Dive Reflex Conflict.

Understanding of the pathophysiology of Takotsubo syndrome (TTS) remains incomplete. This is a hypothesis-driven editorial, proposing a new autonomic mechanism that may explain several important and puzzling features of TTS. Rather than involving a single reflex pathway, we suggest that TTS may result from the interaction and conflict of two evolutionarily conserved autonomic reflex responses. The first is the sympathetic stress response (SSR), a cardiovascular predominantly sympathomimetic reaction characterised by heightened adrenergic activity, tachycardia and cardiovascular stress. The sec
