Spontaneous coronary artery dissection (SCAD) is an increasingly recognised cause of acute coronary syndrome predominantly affecting women (>90% of cases) that is frequently associated with other arteriopathies, such as fibromuscular dysplasia (FMD) and migraine. We present a case of multi-vessel SCAD in a woman in her 40s presenting with myocardial infarction in whom incidental widespread FMD was found, including a massive right renal artery aneurysm requiring ex vivo resection, repair and autotransplantation. The case underscores the need for routine angiographic screening for FMD, which has

OBJECTIVES: In recent years, several epidemiologic studies have shown that pulse pressure (PP) is a powerful predictor of mortality from many cardiovascular diseases. However, few studies have reported the association between PP and adverse events during hospitalization in patients with type A acute aortic dissection (TAAAD). The aim of this study was to evaluate the relationship between admission PP and in-hospital all-cause mortality, in patients with TAAAD of relatively stable patients.
METHODS: Patients with TAAAD of relatively stable patients admitted from January 2015 to December 2021 w

Patients with Stanford type A aortic dissection (TAAD) have high postoperative mortality. This study aimed to develop a prediction model for in-hospital death after surgery in patients with TAAD. The derivation cohort came from a meta-analysis. Major risk factors were counted. The corresponding hazard ratio was reported to establish a prediction model for in-hospital death in patients with TAAD. Validation cohorts from 2 centres were used to evaluate the prediction model. The meta-analysis included 24 cohort studies with a total of 11,404 patients and 1,554 patients died early after surgery. R

Congenital coronary artery anomalies complicated with aortic dissection are rare. We experienced a patient with a single coronary artery presenting with a type A dissecting aortic aneurysm. A 77-year-old woman who experienced sudden back pain and was diagnosed with type A acute aortic dissection was initially treated conservatively because the false lumen thrombosed entirely. Computed tomography taken during 5 weeks of hospitalization incidentally revealed an anomalous single coronary artery arising from the left sinus of Valsalva, and the right coronary artery orifice was absent. One month af

BACKGROUND: Acute type A aortic dissection (ATAAD) with coronary ostial involvement poses significant surgical challenges. We describe two surgical approaches to managing coronary involvement and assess their outcomes.
METHODS: Between January 2019 and December 2023, 617 acute type A aortic dissection patients with coronary involvement were enrolled. Based on our institutional surgical protocol, 507 patients underwent isolated coronary ostial reconstruction, whereas 110 received protective coronary artery bypass grafting (CABG) after coronary ostial reconstruction or closure in cases of sever

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is an increasingly recognized cause of acute coronary syndrome or sudden cardiac death, primarily affecting relatively young women (median age, 51 years) without typical cardiovascular risk factors. SCAD has a genetic component, with genome-wide association studies identifying multiple risk loci. Thoracic aortic dissection (type A) shares some genetic overlap with SCAD, suggesting potential common predispositions.
METHODS: We performed genetic screening or whole-genome sequencing of 17 patients with SCAD (94% women) with a first- or se

Objective: This multicenter study compares outcomes of open thoracoabdominal aortic aneurysm (TAAA) repair in patients < 60 years with (n = 106), versus without (n = 167), hereditary aortopathy. Methods: We conducted a retrospective analysis of 273 consecutive open TAAA repairs (2000-2024) at two European centers. The primary endpoint was early outcome. We used a Kaplan-Meier curve to assess survival, and logistic regression to identify predictors. Results: Operative death rates were similar (hereditary: 13/106 [12.3%] vs. non-hereditary: 22/167 [13.2%], p = 0.83). Hereditary aortopathy patien

In this case report, we describe the feasibility and technical considerations of an off-label use of the new Gore® Aortic Extender (W.L. Gore & Associates), which is part of the Gore® TAG® Thoracic Branch Endoprosthesis (W.L. Gore & Associates), to repair a huge false aneurysm originating from an ascending aortic prosthesis. The patient's medical history reported an acute type A aortic dissection 10 years ago, for which he was treated with a Bentall procedure. This was complicated by obstruction of the left main coronary after it was reconstructed and thus a coronary artery bypass was also per

BACKGROUND Circumferential acute aortic dissection is a rare and fatal condition. We present a surgical case of a 76-year-old woman presenting with Stanford type A acute aortic dissection with intimo-intimal intussusception. CASE REPORT A 76-year-old woman living on a remote island with no significant medical history visited a local general hospital because of sudden anterior thoracic strangulation. Contrast-enhanced computed tomography revealed Stanford type A acute aortic dissection, and the patient requested helicopter transport to our hospital for surgery. During preparation for transport,

BACKGROUND: The genetic risk of intracranial aneurysm (IA) development has been ascribed to the genetic risk of smoking exposure and hypertension. The relationship of IA to other cardiovascular traits and the contribution of IA risk loci to aberrant gene programs within cerebrovascular cell types remains unclear.
METHODS: We performed a genome-wide association study in the Million Veteran Program and Finnish cohort study testing association of roughly 25 million DNA variants with unruptured IA (4694 cases and 877 091 controls) in individuals of European, African, and Hispanic ancestries. Meta

Isolated abdominal aortic dissection is a rare but potentially fatal condition. Timely diagnosis and management are crucial to reduce mortality and possible complications. Though computed tomography (CT) is the diagnostic imaging modality of choice, point-of-care ultrasound (PoCUS) holds certain promises in diagnosing aortic dissection. PoCUS is readily available at the bedside, has high specificity and sensitivity, and is useful, especially in unstable cases or when advanced imaging access is limited or time-consuming. PoCUS can also aid in risk stratification and assist in clinical decision-

BACKGROUND AND AIMS: Folate metabolites have been identified as prognostic markers in cardiovascular, oncologic, and congenital diseases. However, few studies have explored the association between serum folic acid levels and aortic rupture. The aim of this study was to evaluate the predictive value of serum folic acid level for pre-operative adverse events in patients with symptomatic aortic aneurysm/aortic dissection.
METHODS AND RESULTS: Patients with symptomatic aortic aneurysm/aortic dissection, presenting to the emergency department (ED), were enrolled in the study. Both univariate and m

OBJECTIVES: We present a case in which a coronary aneurysm distorted the architecture of the left anterior descending coronary artery (LAD), causing extreme difficulties in wiring followed by uncrossable balloon catheters. We demonstrated how we completed the angioplasty using dual 360° looping guidewires followed by unlooping of the guidewires.
KEY STEPS: LAD wiring was performed with a 360° looping guidewire technique. The LAD was wired again with a buddy wire in the same looping fashion. The LAD guidewire was unlooped using the 2 guidewires and a balloon catheter.

The association between Helicobacter pylori (H. pylori) infection and the development of thoracic aortic aneurysm (TAA) remains inconclusive. Globally, approximately 58% of the population is infected with Helicobacter pylori. It is typically associated with chronic gastritis, peptic ulcers, and gastric cancer1. Mendall et al. were the first to report a serological correlation between H. pylori seropositivity and coronary heart disease2, a finding that has gradually spurred interest in the potential role of the bacterium in the pathogenesis of cardiovascular diseases. Previous studies have demo

BACKGROUND: Thoracic aortic aneurysm (TAA) is a serious condition characterized by dilation of the thoracic aorta, often leading to aortic dissection or rupture. Current treatments involve surgical and pharmacological interventions and do not effectively address the underlying molecular mechanisms. This study explores the effects of ketogenic diet (KD) on TAA, focusing on histone deacetylase 1 (HDAC1) and vascular smooth muscle cells (VSMCs) function.
METHODS: A β-aminopropionitrile monofumarate (BAPN)-induced TAA mouse model was used. Mice were divided into groups receiving either a standard

Bicuspid aortic valve (BAV) and thoracic aortic aneurysms and dissections (TAAD) are recognized in syndromic connective tissue diseases (CTD), but most cases occur sporadically. The extent to which non-syndromic BAV or TAAD predisposes to additional arteriopathies, particularly in younger individuals, remains unclear. We retrospectively analyzed 1438 patients (mean age = 48.0, 67.7% female), excluding those with CTDs. Participants were ≤60 years old and categorized by the presence of BAV and/or TAAD. We examined co-existing arterial pathologies, including fibromuscular dysplasia, spontaneous c

Spontaneous coronary artery dissection (SCAD) is a rare but critical cause of acute myocardial infarction (AMI). Diagnosing SCAD solely based on angiographic findings is often challenging; therefore, intravascular imaging modalities such as intravascular ultrasound (IVUS) and optical coherence tomography (OCT) are often required. Angioscopy allows for direct visualization of the vascular lumen, enabling detailed assessment. Herein, we present a unique case of a young man who presented with AMI. Coronary angiography (CAG) revealed diffuse stenosis in the first septal branch; therefore, IVUS, OC

BACKGROUND: Ascending aortic dissection rarely presents as acute myocardial infarction, and when it does, its diagnosis is even more challenging.
CASE SUMMARY: We present a case of a young male with corrected congenital heart disease and a giant ascending aorta aneurysm. He was admitted to hospital for chest pain and was diagnosed with acute anterior myocardial infarction. Imaging tests showed dissection of the ascending aorta. He required complex cardiovascular surgery for Dacron tube implantation to treat the giant ascending aortic aneurysm.
DISCUSSION: This case is a reminder of how impor

Acute type A aortic dissection complicating pregnancy typically occurs in the third trimester (50%-60%) or postpartum period (30%-50%). Decision for single-stage "deliver-then-repair" vs emergency aortic surgery with in utero protection is multifactorial. A 36-year-old woman with 25-week-gestation twins presented with chest pain radiating to back. Echocardiogram showed dilated root with severe regurgitation. Urgent aortogram revealed Stanford type A (DeBakey type II) aortic dissection from coronary sinuses to ascending aorta. The multidisciplinary team decided for a Bentall procedure, which wa

Background/Objectives: After years of work in the field of aortic arch surgery, the technique has evolved, making this procedure relatively safe, with lasting results. Due to the increasing long-term survival and overall aging of the patient population, more patients require aortic arch reoperation. In the present study, the safety of aortic arch reoperations was analyzed in the long term, focusing on risk factors for mortality. Methods: Between 1999 and 2023, 108 patients were included in our study who underwent reoperation on aortic arch after prior operation on the aorta, the aortic valve,
