Pulsed field ablation as a bailout for ventricular tachycardia ablation: a case report.

BACKGROUND: Catheter ablation of ventricular tachycardia (VT) using radiofrequency energy has proven to be an effective therapy. The outcome in patients presenting with an intramural septal substrate has however been disappointing. New tools and energy such as pulsed field ablation (PFA) might improve our efficacy to treat arrhythmia in this setting. CASE SUMMARY: A 64-year-old patient with valvular and ischaemic cardiomyopathy had recurrence of arrhythmic storm despite previous endocardial ablation and stereotactic body radiation therapy targeting the interventricular septum. A new attempt u

Identification of clinical phenotypes and heterogeneous treatment effects of surgical revascularization in ischaemic cardiomyopathy: a machine learning consensus clustering analysis.

AIMS: To identify ischaemic cardiomyopathy (ICM) patients with different phenotypes for evaluating their outcomes and heterogeneous treatment effects (HTEs) of coronary artery bypass grafting (CABG). METHODS AND RESULTS: We applied a machine learning-based consensus, K-Medoids clustering analysis to the Surgical Treatment for Ischemic Heart Failure trial. We compared the risk of all-cause mortality and cardiovascular mortality among different phenotypes. The survival benefits of CABG compared with medical therapy alone were assessed in the identified phenotypes for evaluating HTEs. The consen

The value of cardiac CT based inflammatory risk assessment in predicting cardiovascular events: a case report.

BACKGROUND: Vascular inflammation plays a critical role in the development of coronary artery disease (CAD). Measurement of coronary inflammation from coronary computed tomography angiography (CCTA) using the perivascular fat attenuation index (FAI) Score could provide unique prognostic information and guide the clinical management of patients. In this context, we also refer to an artificial intelligence-based risk prediction tool (AI-Risk algorithm), which integrates FAI Score with clinical risk factors and plaque burden to estimate the long-term probability of a fatal cardiac event. Prediction of ventricular arrhythmias and sudden cardiac death by quantification and location of late gadolinium enhancement on cardiac magnetic resonance: a systematic review and meta-analysis.

AIMS: In non-ischaemic cardiomyopathy (NICM), late gadolinium enhancement (LGE) detected by cardiovascular magnetic resonance is related to ventricular arrhythmia (VA) and sudden cardiac death (SCD) risk. The incremental prognostic value of quantifying LGE volume or mass beyond its mere presence, however, remains unresolved. The aim was to evaluate whether LGE quantification improves the prediction of SCD. METHODS AND RESULTS: PubMed, Embase, and Web of Science were searched on 20 November 2024 for observational studies that related quantified LGE burden to ventricular arrhythmia (VA)/SCD in

Guideline-directed medical therapy after revascularization and outcomes in ischaemic cardiomyopathy.

AIMS: Guideline-directed medical therapy (GDMT) remains the cornerstone for treating patients with heart failure (HF) and reduced ejection fraction (HFrEF) and coronary artery disease (CAD). However, real-world implementation of GDMT (GDMTi) is suboptimal. This subanalysis of the RevascHeart study evaluates the impact of GDMTi on long-term mortality in patients with de novo ischaemic heart failure (HF) undergoing revascularization. METHODS: Among 409 patients with HFrEF (left ventricular ejection fraction ≤40%) and CAD, 275 one-year survivors who underwent revascularisation shortly after inde

Infected left ventricular thrombus confirmed by FDG PET/CT presenting as persistent Streptococcus anginosus bacteraemia in ischaemic cardiomyopathy: a case report.

BACKGROUND: Infected left ventricular (LV) thrombus is a rare but serious complication in patients with heart failure with reduced ejection fraction (HFrEF). It is associated with persistent bacteraemia, systemic embolization, and stroke. Diagnosis requires a high index of clinical suspicion, supported by multimodal imaging and coordinated multidisciplinary management. CASE SUMMARY: We report a 69-year-old man with ischaemic cardiomyopathy (EF 30%), prior multi-territory stroke due to LV thrombus, and multiple comorbidities, who was admitted with reduced GCS following ICU treatment for Strept

Catheter ablation vs. anti-arrhythmic drug therapy for ventricular tachycardia in ischaemic heart disease: a meta-analysis of randomized controlled trials.

AIMS: Ventricular tachycardia (VT) in ischaemic heart disease (IHD) requires complex management strategies including catheter ablation (CA) and anti-arrhythmic drugs (AADs). The aim of this study is to compare efficacy and safety of CA vs. AADs in patients with IHD and VT. METHODS AND RESULTS: We performed a meta-analysis of randomized controlled trials (RCTs) enrolling patients with IHD and ICD randomized to CA or AADs. Primary outcome was appropriate ICD therapy. Secondary outcomes included inappropriate ICD therapy, cardiovascular (CV) re-hospitalization, all-cause/CV mortality, and advers

Dynamic obstruction of right coronary ostium by a papillary fibroelastoma: a case report on alternative causes of chest pain and cardiac imaging assessment.

BACKGROUND: Papillary fibroelastomas (PFEs) are one the most common primary cardiac tumours. Although they are benign, their location involves a high risk of adverse events, which have been reported mainly in the form of embolic events, but angina pectoris is another possible, less frequent manifestation. CASE SUMMARY: We present a case of a 72-year-old woman with chest pain symptoms, who was found to have a PFE. As part of pre-operatory study a computed tomography (CT) scan was performed which documented dynamic obstruction of the right coronary artery. Surgery was brought forwards and mass

A surgical calm to the storm: managing refractory electrical storms with bilateral thoracoscopic sympathectomy.

OBJECTIVES: Refractory electrical storms (ES) present a significant challenge in patients with structural heart disease, often persisting despite optimal medical therapy, catheter ablation, and ICD therapy. This study evaluates the efficacy and safety of bilateral thoracoscopic sympathectomy (BCSD) in reducing arrhythmic burden and improving clinical outcomes in this high-risk population. METHODS: A retrospective review was conducted at CHU Grenoble-Alpes, including 11 patients with ES secondary to cardiomyopathies treated with BCSD between 2019 and 2024. Baseline characteristics, surgical ou

First in-human microelectrode recordings of vagus nerve activity in heart failure.

We know that ischaemic cardiomyopathy leads to compensatory changes in the cardiovascular system to offset the reduction in cardiac output; there is a marked increase in cardiac sympathetic drive, as measured by noradrenaline spillover, and a marked increase in sympathetic vasoconstrictor drive to the skeletal muscle vascular bed, as measured by recording muscle sympathetic nerve activity from a peripheral nerve. Conversely based on assessment of heart rate variability there is believed to be a decrease in cardiac parasympathetic drive. Accordingly we might expect a decrease in ongoing neural

A case report: SPEAR-CRT: first description of use of left bundle branch area pacing tools and techniques to place an endocardial LV lead via the interventricular septum in a patient with ostial coronary sinus occlusion.

BACKGROUND: Cardiac resynchronization therapy (CRT) is a well-established treatment with heart failure (HF) and conduction delays. Complex anatomy, such as coronary sinus (CS) stenosis can render LV lead placement challenging or impossible. Alternate strategies such as epicardial and left bundle branch area pacing (LBBAP) exist, though have their own disadvantages. CASE SUMMARY: A 72-year-old male with a history of ischaemic cardiomyopathy, LV dysfunction, and pacemaker dependence presented to hospital in decompensated HF after two failed attempts to upgrade to a CRT system, the latest includ

Refractory ventricular tachycardia: clinical challenges and management strategies.

This case report highlights the clinical course of a young patient with a history of ischaemic cardiomyopathy and severely impaired left ventricular (LV) systolic function following a delayed anterior myocardial infarction, which was further complicated by the presence of large LV thrombus. The patient subsequently presented with persistent ventricular tachycardia (VT) refractory to multiple anti-arrhythmic medications and antitachycardia pacing (ATP). VT ablation was contraindicated due to the LV thrombus, and the failure of conventional medical therapy. Heart transplantation was considered a
