Perfusion-induced myocardial ischaemia is observed in several situations: chronic coronary insufficiency, unstable angina, myocardial infarction, and during coronary angioplasty or bypass surgery. Oxygen-derived free radicals are liberated in large quantities during myocardial reperfusion ischaemia. Though very toxic in experimental studies, the responsibility of these free radicals in myocardial injury remains to be demonstrated clinically. Oxidant stress, characterised by an imbalance between the free radical attack and insufficient cellular defense seems partially responsible for reperfusio

BACKGROUND: Brain natriuretic peptide is a novel natriuretic peptide that is secreted predominantly from the ventricles, and its plasma levels have been shown to be markedly increased in patients with chronic congestive heart failure. This study was designed to examine the plasma levels of brain natriuretic peptide as well as atrial natriuretic peptide in patients with acute myocardial infarction.
METHODS AND RESULTS: We examined the plasma levels of brain natriuretic peptide as well as atrial natriuretic peptide in 50 consecutive patients (36 men and 14 women; mean age, 66 years) with acute

BACKGROUND: Ventricular tachycardias occurring in the chronic phase of myocardial infarction are caused by reentry. Areas of slow conduction, facilitating reentry, are often found in the infarcted zone. The purpose of this study was to elucidate the mechanism of slow conduction in the chronic infarcted human heart.
METHODS AND RESULTS: Spread of activation was studied in infarcted papillary muscles from hearts of patients who underwent heart transplantation because of infarction. Recordings were carried out on 10 papillary muscles that were superfused in a tissue bath. High-resolution mapping

"Oxygen radicals" is a term used as a synonym for a group of reactive radical and non-radical species formed by successive univalent reduction of oxygen, interaction of the primarily formed oxygen radicals and by reaction of oxygen radicals with other molecules and ions of the biological system e.g. polyunsaturated fatty acids and halogenides. Besides physiological reactions (metabolism of xenobiotics and arachidonic acid; phagocytosis), the high reactivity of oxygen radicals leads also to unspecific reactions with biomolecules resulting in cell damage. In the last fifteen years, the participa

To study the pattern of various medical diseases seen during specialist medical rural practice, we undertook an analysis of 1362 consecutive cases seen over an eight year period at a rural centre in Rajasthan. Analysis showed that infective diseases e.g. tuberculosis, acute infections, and infection related diseases of the gastrointestinal tract, skin, nervous system and heart formed the largest group with 724 patients (53.2%) as compared with chronic non-infective systemic diseases which were seen in 638 patients (46.8%). Cardiac diseases were seen in 306 patients (22.5%) which included 110 c

Our previous observations that minimal coronary resistance (MCR) decreases by 60% in a model of chronic ischemia suggest that angiogenesis and vascular remodeling occurred. To test this hypothesis we conducted quantitative morphometry on the arteriolar (ART) and capillary (CAP) beds of 14 pigs subjected to chronic ischemia. We induced chronic ischemia by ameroid occlusion of the left circumflex coronary artery for 2 to 8 weeks. We measured numerical densities (ND) and total cross sectional areas (CSA) of the ART and CAP in the ischemic regions. In the same pigs minimal coronary resistance (MCR

The central aim of this review was to examine the application of intervention therapy for CAD in the elderly population. The data reviewed indicates that it is no longer appropriate to use age 70 or 75 as the upper limit of eligibility for thrombolytic intervention in patients with acute myocardial infarction. Elderly who are physiologically active without contraindications to thrombolytic therapy should be considered eligible. Additional controlled trials specifically targeted at the elderly population are needed to better define the precise dosing regimen and the magnitude and extent of blee

A study of the immune and fibrinolytic systems of the body in 216 patients with different clinical forms of atherosclerosis (exertion and rest stenocardia, arrhythmias, atherosclerotic hypertension), chronic obstructive bronchitis revealed that the antiatherogenous effect of chronic obstructive bronchitis is predetermined on the one side by an increased function of the monocytic-macrophagal link of immunity and on the other by activation of the fibrinolysis system.

Catecholamines acting through beta 1- and beta 2-adrenergic receptors cause positive inotropic and chronotropic effects in the human heart. However, recent evidence suggests that in the human heart other receptor systems can also affect heart rate and contractility. Positive inotropic effects can be mediated by receptor systems acting through accumulation of intracellular cyclic adenosine monophosphate (cAMP; Gs-protein-coupled receptors such as 5-hydroxytryptamine(5-HT)4-like, histamine H2, and vasoactive intestinal peptide) or by receptor systems acting independently of cAMP, possibly throug

We review the recent literature examining lipid changes during stressful experiences, and the psychological and constitutional differences that influence lipid levels at rest and that may modulate lipid response to stress. Mild forms of chronic or episodic stress are apparently not associated with alterations in lipids and lipoproteins, but severe forms of real or perceived stress do appear to alter lipid levels. Acute laboratory stress is frequently associated with short-term alterations in lipids and lipoproteins, but the significance of these changes is unclear. Several individual character

The major components of atherosclerotic plaque, ultimately responsible for clinical effects, are deposited lipids--mostly cholesteryl esters and cholesterol, derived largely from the lower-density lipoproteins of the blood--and proliferated, modified arterial smooth muscle cells with their synthesized connective tissue products. Advanced plaques vary widely in the proportion of the two components, but evidence indicates that lipid deposition--especially of lipoprotein elements--often occurs in the lesion-prone intimal areas of the artery prior to the buildup of smooth muscle cells. The 1980s w

Computed tomography (CT) represents the optimum theoretical approach to x-ray imaging. This conclusion results from an awareness of CT's capacity to solve the fundamental limitation of all forms of x-ray imaging--the superimposition of anatomic structures. Because CT is potentially a fully three-dimensional method, this problem is addressed in a manner not subject to the risks, complications, and technical limitations of selective angiography, subtraction angiography, tomography, and other techniques. Ultrafast CT provides a broad spectrum of useful quantitative cardiac data during one minimal

Naeye (1) has long linked SIDS with low utero-placental blood flow and foetal hypoxia. Gestational hypotension, maternal smoking and disorders of the foetal membranes are considered by Naeye to promote brainstem and neurological abnormalities. Naeye also found that SIDS' victims often showed a growth lag after birth and suggested a reduced oxygen environment after birth which pointed to chronic under-ventilation of the lungs. Another abnormality observed by Naeye was that brown fat was retained for an abnormally long time. Now Mitchell (New Zealand Herald, 27.11.90) has concluded a 3-year stud

Recent studies suggest that granulocytes (PMNs) play a role in the pathogenesis of acute and chronic myocardial ischemia and extension of myocardial injury. A positive correlation was also found between leukocyte count and severity of coronary artery disease. Rabbit derived antiserum dependent-reduction of circulating PMNs in the dog or using monoclonal antibody anti-CD11b/CD18 of PMNs resulted in smaller myocardial infarcts. Granulocytes can release a variety of mediators tissue injury and synergize with these different mediators and other cells resulting in amplification of neutrophil stimul

It is claimed that long-term treatment with beta-blockers improves cardiac function and exercise capacity in patients with various forms of congestive heart failure. This was first reported by Waagstein and coworkers in patients with idiopathic dilated cardiomyopathy in 1975 and was later confirmed in 8 further studies in this type of patient. A total of 211 patients with idiopathic dilated cardiomyopathy were treated for 12-19 months. About two thirds of the patients have improved to some extent. Seven other studies reported favourable long-term effects of beta-blockers in 120 patients with o

Substantial evidence has accumulated that in the human heart both beta1- and beta2-adrenoceptors coexist. As a rule, the amount of beta2-adrenoceptors is higher in the atria (about 30% of the total beta-adrenoceptor population) than in the ventricular myocardium (about 20%). Both beta1- and beta2-adrenoceptors couple to adenylate cyclase and mediate positive inotropic effects of isoproterenol and epinephrine on isolated, electrically driven cardiac preparations. In the atria, stimulation of both beta1- and beta2-adrenoceptors causes maximal increases in contractile force; in the ventricular my

Direct visual examination of the interior surfaces of intact human coronary arteries is now a reality, both from intraoperative and transluminal approaches. As an additional investigation to angiography it improves the knowledge about the corresponding morphology of angiographically visible changes. Since the first report about percutaneous and intraoperative coronary angioscopy by Spears et al. in 1982 important indications concerning the intraoperative approach were defined such as the control of coronary atherectomy, intraoperative balloon dilatation and laser angioplasty, the inspection of

It seems likely that a number of medical and surgical treatments have had an impact on the overall mortality from ischaemic heart disease in recent years although there is comparatively little data upon which to base conclusions. The two forms of treatment which have been shown, in controlled trials, to reduce mortality are beta blockade after infarction and coronary artery bypass surgery in chronic ischaemic heart disease. It is of interest that these treatments seem to be most often applied in countries that have had the greatest fall in mortality, although it is not suggested that they have

A 72% reduction in the number of premature ventricular complexes, a 75 and 65% reduction in pairs and repetitive forms, respectively, are commonly accepted criteria for the evaluation of the efficacy of short-term antiarrhythmic treatment. On the other hand, a three-to-ten fold increase in the frequency of premature ventricular complexes is considered as a proarrhythmic effect. The aim of this paper is to verify if these criteria can still be applied on a long-term basis. Twenty-eight subjects without a demonstrable underlying organic heart disease and 21 patients with stable chronic ischaemic
