Identification of key driver genes in idiopathic pulmonary arterial hypertension by single-cell RNA sequencing and experimental validation.

Idiopathic pulmonary arterial hypertension (IPAH) is characterized by irreversible pulmonary vascular remodeling. This pathological remodeling is mainly driven by the aberrant proliferation and migration of pulmonary artery smooth muscle cells (PASMCs). However, the molecular mechanisms underlying these dysfunctions remain incompletely understood. In this study, we integrated single-cell RNA sequencing (scRNA-seq) analysis with in vivo and in vitro validation to identify key driver genes implicated in IPAH. The GSE169471 dataset was acquired from the Gene Expression Omnibus and processed via q

Neutrophil extracellular traps-related biomarkers in idiopathic pulmonary arterial hypertension: A machine learning-based identification and experimental validation study.

Neutrophil extracellular traps (NETs) are increasingly recognized as critical mediators in vascular inflammation and remodeling, yet their molecular mechanisms in idiopathic pulmonary arterial hypertension (IPAH) pathogenesis remain largely unexplored. This study employed integrated bioinformatics approaches and experimental validation to identify NETs-related biomarkers in IPAH. We performed weighted gene co-expression network analysis (WGCNA) on merged transcriptomic datasets (GSE117261 and GSE48149, comprising 40 IPAH and 34 control samples), identifying a blue module significantly correlat

Amonafide Targeting NTSR1-PI3K/AKT/mTOR Signaling Attenuates Vascular Remodeling in Pulmonary Arterial Hypertension.

BACKGROUND: Pulmonary arterial hypertension (PAH) is a progressive disease driven by pulmonary vascular remodeling, largely due to the abnormal proliferation and phenotypic switching of pulmonary artery smooth muscle cells. METHODS: Levels of topoisomerase II α were evaluated in the lungs from patients with idiopathic PAH, 2 rodent PAH models (SU5416 combined with hypoxia and monocrotaline-induced), and in pulmonary artery smooth muscle cells stimulated with platelet-derived growth factor BB (PDGF-BB). The therapeutic potential of amonafide, a topoisomerase II inhibitor, was also evaluated in

Survival and Hemodynamic Effects of Parenteral Prostanoids in High-Altitude Group 1 Pulmonary Hypertension: A Retrospective Cohort Study.

BACKGROUND: In Latin America, the evidence regarding the use of prostanoids at high altitude in patients with Group 1 pulmonary hypertension (PH-1) is limited. Therefore, it is essential to describe the clinical characteristics and outcomes associated with prostanoid treatment in this population. METHODS: This retrospective study involved patients with PH-1. Hemodynamic and clinical variables were compared before the initiation of prostanoids and during clinical follow-up. Overall survival was analyzed using Kaplan-Meier estimation, with survival rates calculated at 3 and 5 years. The strengt

Association of initial diagnostic setting and referral delay with mortality in systemic sclerosis: a nationwide database cohort study.

BACKGROUND: Early diagnosis and prompt referral are critical for improving outcomes in systemic sclerosis (SSc), yet disparities in diagnostic capabilities between healthcare levels may delay treatment and impact survival. This study aimed to compare mortality risk among SSc patients based on their diagnostic pathway: initial diagnosis at a secondary care hospital before referral versus prompt referral and diagnosis at a tertiary center. METHODS: Using National Health Security Office cohort data (fiscal years 2016-2023), we identified 17,036 patients aged ≥ 18 years with SSc (ICD-10 M34) diag

Noninvasive surrogate of main pulmonary artery stiffness is associated with right ventricular function in experimental pulmonary arterial hypertension.

Main pulmonary artery (MPA) stiffening increases right ventricular (RV) afterload and is associated with worse outcomes in pulmonary arterial hypertension (PAH). Measures of pulmonary artery compliance (PAC) and vascular resistance (PVR) require invasive catheterization, limiting clinical application. Echocardiographic assessment of MPA pulsatility, the relative change in MPA diameter, may be useful but not well validated. We investigated the relationship between MPA pulsatility and passive stiffness, PAC, PVR, RV function, and RV-PA coupling in sugen-hypoxia (SuHx) rats and children with idio

Beyond PVR: dynamic physiologic assessment enhances prognostic evaluation in pulmonary hypertension associated with chronic obstructive pulmonary disease.

OBJECTIVES: Pulmonary hypertension associated with chronic obstructive pulmonary disease (PH-COPD) is associated with poor outcomes, yet resting haemodynamics incompletely capture disease severity. We evaluated whether integrating submaximal cardiopulmonary exercise testing variables with transthoracic echocardiographic and pulmonary function test parameters improves prognostic assessment in PH-COPD beyond resting haemodynamics alone. SETTING: Single-tertiary pulmonary vascular disease referral centre in the USA. PARTICIPANTS: 67 patients with PH-COPD who underwent right heart catheterisatio

Integrated transcriptomic and machine learning analysis identifies female-associated candidate biomarkers in Idiopathic pulmonary arterial hypertension.

BACKGROUND: Idiopathic pulmonary arterial hypertension (IPAH) exhibits a marked female predominance. Despite this "estrogen paradox," the molecular basis of sex-associated pathogenesis remains incompletely understood, limiting the development of precision diagnostic strategies. OBJECTIVE: To develop a sex-stratified computational framework that integrates transcriptomic profiling and ensemble machine learning to identify female-associated candidate biomarkers in IPAH. METHODS: Sex-stratified differentially expressed genes (DEGs) were derived from pulmonary arterial endothelial cells (GSE3030

Association of pulmonary artery radiation with pulmonary hypertension after lung cancer radiotherapy.

BACKGROUND: Thoracic radiotherapy (RT) results in pulmonary fibrosis that has been hypothesised to increase the risk of pulmonary hypertension (PH); however, dose effects to cardiopulmonary structures predicting PH have not been systematically studied. METHODS: Multi-institutional retrospective cohort analysis of patients with non-small cell lung cancer treated with thoracic RT. The pulmonary arteries (PA), distal pulmonary vasculature, lungs and heart were segmented. Radiotherapy dose parameters (mean, maximum and volume (V) receiving X Gy in 5 Gy increments) were extracted. Area under the r

Phase 1b ATMOS trial of the inhaled sGC activator mosliciguat in patients with PAH/CTEPH.

BACKGROUND: Mosliciguat, a soluble guanylate cyclase (sGC) activator, is designed for lung-targeted delivery via dry powder inhalation. Nitric oxide (NO) binds sGC's prosthetic heme group, catalysing cyclic guanosine monophosphate (cGMP) production, resulting in increased vasodilation, reduced inflammation/apoptosis, reverse vascular remodelling and antifibrotic effects. sGC is redox sensitive, losing heme in pulmonary hypertension (PH) oxidative stress conditions. Mosliciguat's novel 'activator' mechanism directly binds the sGC heme-binding pocket to induce cGMP, allowing it to target NO-inse

Decoding right ventricular geometry: novel 3D echocardiography-derived global shape analysis across health and disease states.

AIMS: While pre-defined reference shapes have been used to assess morphological changes in the left ventricle, standardized methods for evaluating right ventricular (RV) remodelling are lacking. This study aimed to develop and test a new 3D echocardiography (3DE)-based method for quantifying RV shape in a large cohort of healthy individuals and across various disease states. METHODS AND RESULTS: 3DE-derived RV mesh models were reconstructed in 1043 healthy subjects from the World Alliance of Societies of Echocardiography (WASE) study and in 581 patients with severe aortic stenosis, heart fail

Diffuse Myocardial Injury With Diabetes-Related Advanced Glycation End Product Accumulation in Dilated Cardiomyopathy: Pathological Insights From an Autopsy Case.

Diabetes mellitus is a systemic metabolic disorder associated with diverse long-term complications, among which cardiovascular involvement is a major cause of morbidity and mortality. A central pathogenic mechanism linking diabetes to tissue injury is the formation and accumulation of advanced glycation end products (AGEs), which exert deleterious effects through receptor-mediated signaling via the receptor for AGEs (RAGE) and receptor-independent collagen cross-linking. However, pathological evidence directly demonstrating the contribution of AGE accumulation to advanced cardiac dysfunction i

Deep Learning-Based Prediction of Cardiopulmonary Disease in Retinal Images of Premature Infants.

IMPORTANCE: Bronchopulmonary dysplasia (BPD) and pulmonary hypertension (PH) are leading causes of morbidity and mortality in premature infants. OBJECTIVE: To determine whether images obtained as part of retinopathy of prematurity (ROP) screening might contain features associated with BPD and PH in infants and whether a multimodal model integrating imaging features with demographic risk factors might outperform a model based on demographic risk alone. DESIGN, SETTING, AND PARTICIPANTS: A deep learning model was used to study retinal images collected from patients enrolled in the multi-instit

Pulmonary hypertension in cardiac tamponade: An observational cohort study of in-hospital mortality and echocardiographic findings.

BACKGROUND: Patients with pulmonary hypertension (PHTN) (i.e., chronic PHTN) have right ventricular hypertrophy, elevated right-sided heart pressures, and frequently have pericardial effusions. When evaluating these patients for cardiac tamponade, the hypertrophy and elevated pressure in right heart may be protective from tamponade by counteracting the pressure from the pericardial effusion. However, these patients may be harmed if echocardiographic signs of tamponade (e.g., right ventricular diastolic collapse) are obscured. STUDY OBJECTIVE: The effect of PHTN on patients with cardiac tampon

High-flow nasal cannula oxygenation reduces desaturation risk during diagnostic flexible bronchoscopy under deep sedation: a randomized controlled trial.

BACKGROUND: Deep sedation for flexible bronchoscopy (FB) improves procedural conditions but exacerbates desaturation risks. High-flow nasal cannula (HFNC) may mitigate this situation, yet efficacy under deep sedation remains unproven. METHODS: In this randomized trial (ChiCTR2400083597), 340 ASA I-II patients undergoing FB under deep sedation (MOAA/S ≤ 1) received conventional nasal cannula (5 L/min; NC) or HFNC (25/45/65 L/min). The primary outcomes were the incidence of intraoperative desaturation (SpO₂ < 90% > 10s) and intraoperative nadir SpO₂ value. The intraoperative VAS scores for coug

Large-Scale Disease-Wide Association Study Identified Predisposition Links Between Influenza and Cardiovascular Diseases.

BACKGROUND: Influenza remains a major global health threat and, akin to "long COVID," has been linked to prolonged multi-organ comorbidities ("long flu"). Cardiovascular diseases (CVDs) are increasingly recognized in the pathogenesis of influenza, yet most prior work emphasizes acute or short-term outcomes and rarely contrasts multiple endpoints over extended horizons. METHODS: This study conducted a disease-wide association study (DWAS), assembling individuals with clinically coded influenza without pneumonia (n = 5136) and population comparators (n = 314,673). Using age- and sex-adjusted Co

Versatile hiPSC Models and Bioengineering Platforms for Investigation of Atrial Fibrosis and Fibrillation.

Atrial fibrillation (AF) is the most common sustained heart rhythm disorder. It is estimated that AF affects over 52 million people worldwide, with its prevalence expected to double in the next four decades. AF significantly increases the risk of stroke and heart failure, contributing to 340,000 excess deaths annually. Beyond these life-threatening complications, AF results in limitations in physical, emotional, and social well-being causing significant reductions in quality of life and resulting in 8.4 million disability-adjusted life-years per year, highlighting the wide-ranging impact of AF

Pathogenesis, histopathology, and mechanisms of pulmonary pathology secondary to chronic left heart failure in animals.

The pulmonary consequences of congestive heart failure in domestic animals, particularly from clinical and gross morphologic perspectives, have been described. However, the full spectrum of mechanistic and microscopic alterations, especially at the molecular and cellular levels, remains less well integrated in the context of veterinary pathology. We examine the pathogenesis and consequences of pulmonary hypertension secondary to left heart failure, in which chronic elevation of hydrostatic pressure triggers complex molecular, physiologic, and morphologic responses. These include the epithelial
