A rare case of spotted fever group rickettsiosis in southern Iran: diagnostic challenges and review.

BACKGROUND: Mediterranean spotted fever (MSF), caused by Rickettsia conorii and transmitted by the brown dog tick (Rhipicephalus sanguineus), is endemic in Mediterranean regions. In non-endemic countries such as Iran, spotted fever group (SFG) rickettsioses are rarely reported and present significant diagnostic challenges due to overlap with common illnesses like measles and dengue fever. CASE PRESENTATION: A 58-year-old man from Hormozgan Province, southern Iran, with diabetes and ischemic heart disease, presented with two weeks of high fever, chills, weakness, maculopapular rash, and severe

Co-circulation of Crimean-Congo hemorrhagic fever virus and spotted fever group Rickettsia in cattle and ticks from cattle in Kosovo.

BACKGROUND: Changes in climate and land use as well as expanding livestock production around the world, including southeastern Europe, result in growing public health risk of tick-borne pathogens. Cattle and other livestock serve as important hosts sustaining tick populations as well as tick-borne zoonotic and animal pathogens. This study aimed to investigate the prevalence and co-circulation of the spotted fever group (SFG) Rickettsia spp. in ticks from cattle and Crimean-Congo hemorrhagic fever virus (CCHFV) in cattle in Kosovo. METHODS: Ticks from pastured cattle in the Malishevë district,

Surveillance of vector-borne infections: Dogs as sentinel animals for Orientia and Rickettsia exposure in five regions in Chile.

Domestic dogs have been proposed as sentinels for various infectious diseases, including rickettsial infections. In Chile, the only endemic human rickettsiosis is scrub typhus caused by Ca ndidatus Orientia chiloensis, an emerging infectious disease, discovered in recent years in southern Chile. The geographic distribution of scrub typhus and other rickettsial infections across Chile is incompletely understood. This study using domestic dogs as sentinel investigated the potential exposure to members of the three main groups of rickettsiae, scrub typhus orientiae (STGO), spotted fever group ric

How rash and eschar came to clinical attention in scrub typhus and Japanese spotted fever.

BACKGROUND: Early diagnosis of scrub typhus and Japanese spotted fever remains challenging because initial manifestations are often nonspecific. In endemic areas, nearly 30% of patients were not correctly diagnosed at the first visit to a participating site. Previous studies have mainly treated rash and eschar as present-or-absent findings. We examined how rash and eschar came to clinical attention in patients correctly diagnosed at the first visit to a participating site and in those diagnosed later. METHODOLOGY/PRINCIPAL FINDINGS: We analyzed 217 patients with scrub typhus or Japanese spott

Surveillance for Metastriate Ticks and Spotted Fever Group Rickettsioses in Southern Illinois.

Spotted fever group rickettsioses (SFGR) are a growing public health concern in the United States. The increase in SFGR cases, coupled with the geographical expansion of tick species, including Amblyomma americanum and A. maculatum, necessitates a better understanding of Rickettsia species prevalence in newly established areas. Amblyomma maculatum is concerning because of its role as a Rickettsia parkeri vector. This study investigated the prevalence of Rickettsia species in ticks collected across 16 counties in southern Illinois, a region experiencing increasing reported SFGR cases. We collec

Molecular detection of Rickettsia helvetica in lesion and blood samples after tick bites in Serbia: two cases with One Health implications.

Rickettsia helvetica is a spotted fever group rickettsia associated mainly with Ixodes ricinus and increasingly detected in European ticks. We describe two tick-bite-associated human cases in Serbia in May 2025. A 65-year-old woman (Case 1) developed pruritic erythematous lesions after two adult female I. ricinus attachments acquired in the Valjevo area. Rickettsia spp. DNA was detected in capillary blood from both lesions, and sequencing of the PCR amplicons identified R. helvetica, while whole blood was PCR-negative and serology did not show reproducible seroconversion. An 8-year-old girl (C

Molecular survey of Rickettsia japonica in ticks and wild boars in Ibaraki Prefecture: a newly recognized endemic area of Japanese spotted fever in eastern Japan.

Japanese spotted fever (JSF) is a tick-borne disease caused by Rickettsia japonica, a member of the spotted fever group rickettsiae. Recently, Japan has experienced a marked increase in JSF cases, with endemic regions expanding from western to eastern areas. However, the specific tick species carrying R. japonica in these newly affected eastern regions, as well as the factors influencing this geographic dissemination, remain unclear. In this study, we investigated the tick species carrying R. japonica and assessed the potential involvement of wild boars (Sus scrofa) in the dispersal of these t

Exploring spotted-fever and typhus group rickettsiae seroprevalence and associated risk factors among rural inhabitants from Cauca department, Colombia.

Rickettsia species are vector-borne bacteria, some of which cause rickettsioses, an underrecognized cause of acute undifferentiated febrile illness. While several regions of Colombia are considered endemic for rickettsial infections, information from other areas such as the Cauca department remains limited. Human serum samples and epidemiological data were collected from 506 rural inhabitants in eight rural localities across four municipalities of Cauca department, Colombia, between August and November 2017. Antibodies against SFG (spotted-fever group) and TG (typhus group) rickettsiae were de

Molecular detection and identification of spotted fever group Rickettsia in ticks infesting dairy cattle in northern Algeria.

Ticks are major vectors of pathogens affecting livestock and public health worldwide. In Algeria dairy cattle are heavily infested by ticks, but the prevalence and diversity of spotted fever group (SFG) Rickettsia, emerging human pathogens, remain poorly understood. This study aimed to detect and characterize Rickettsia species in ticks collected from dairy cattle in northern Algeria. A total of 770 ticks were manually collected from cattle in three provinces (Blida, Medea and Batna) between 2024 and 2025. From these, 128 ticks were randomly selected for molecular screening. DNA was extracted

Spotted fever Rickettsia and relapsing fever Borrelia in rodents from southern India.

UNLABELLED: Bacterial zoonoses constitute a significant proportion of emerging infectious diseases. Yet, the diversity and transmission cycles of many zoonotic bacteria remain poorly characterized, particularly in tropical, biodiverse regions where host and vector diversity are high, and the burden of zoonotic disease in human populations is substantial. In this study, we investigated the presence of five pathogenic bacterial genera-Rickettsia, Borrelia, Orientia, Leptospira, and Coxiella-in rodents from southern India. We detected low circulation of Rickettsia (7.26%), Borrelia (6.45%), and L
