Weight reduction drives improvement in insulin resistance independent of OSA phenotype and CPAP treatment in the RICCADSA cohort.

BACKGROUND: Insulin resistance is common in adults with obstructive sleep apnoea (OSA), yet the extent to which longitudinal metabolic improvement is driven by OSA phenotype, continuous positive airway pressure (CPAP) treatment, or changes in body weight remains unclear. We examined 1-year changes in insulin resistance across OSA and non-OSA subgroups in the non-diabetic RICCADSA cohort. METHODS: In the RICCADSA study, non-diabetic participants (n = 332) were categorized into four predefined subgroups: non-sleepy OSA randomized to CPAP (n = 72), non-sleepy OSA randomized to no CPAP (n = 81),

Impact of the POPulation Medicine Multimorbidity Intervention in Xishui County (POPMIX) on people at high risk for COPD who smoke: protocol for the POPMIX-Smoking cluster randomised controlled trial.

INTRODUCTION: Tobacco use is a major contributor to the burden of chronic obstructive pulmonary disease (COPD) and other non-communicable diseases in China. People at high risk for COPD who smoke, particularly those with pre-existing chronic conditions, often remain underserved by conventional smoking cessation programmes. Population medicine offers a promising framework for proactively identifying high-burden diseases, managing multimorbidity and prioritising interventions for vulnerable populations. METHODS AND ANALYSIS: This protocol describes a stratified, two-arm cluster randomised contr

Acute hemodynamic effects of TPN171H in pulmonary arterial hypertension: a randomized, controlled, phase 2a trial.

Pulmonary arterial hypertension (PAH) has a poor prognosis despite available treatments. TPN171H, structurally modified from traditional Chinese medicine (Epimedium), was reported to have a high affinity for phosphodiesterase type 5 and exhibited anti-inflammatory and vasodilatory effects in preclinical studies. This phase 2a randomized trial (NCT04483115) evaluated the hemodynamic effects and safety of TPN171H in PAH. Sixty patients with PAH were randomly assigned to receive placebo, TPN171H (2.5, 5, or 10 mg) or tadalafil (20 or 40 mg) and evaluated for hemodynamic changes for 24 h. The prim

Clinical Characteristics and Management Outcomes of Severe Multi-Valvular Rheumatic Heart Disease: A Hospital-Based Analysis.

BACKGROUND: Multi-valvular involvement represents the most severe form of rheumatic heart disease (RHD), presenting complex clinical challenges requiring a comprehensive understanding for optimal patient management. In resource-limited settings, late presentation and inadequate secondary prophylaxis result in advanced disease with high complication burdens. OBJECTIVE: To analyze the clinical presentation, diagnostic findings, treatment strategies, and outcomes, specifically in-hospital complication rates, mortality, functional improvement, and six-month follow-up outcomes in patients with sev

The interplay of maturity-onset diabetes of the young, obesity, uncontrolled hypertension, and cannabinoid hyperemesis in the progression to end-stage renal disease: a case report.

BACKGROUND: Maturity-onset diabetes of the young (MODY) is a rare monogenic diabetes with significant renal risk, especially when combined with hyperglycemia, obesity, and other factors. While chronic cannabis use may further accelerate kidney decline, no previous reports have described rapid progression to ESRD in a young patient with MODY, obesity, hypertension, and cannabinoid hyperemesis, as presented here. CASE PRESENTATION: A 24-year-old man with class II obesity, uncontrolled presumed MODY, CKD stage 5, diabetic gastroparesis, hypertension, and chronic marijuana use was diagnosed with

Safety and Efficacy of a Novel Atrial Flow Regulator in Patients With Severe Pulmonary Arterial Hypertension: The AFR-Prophet Study.

BACKGROUND: Pulmonary arterial hypertension (PAH) is a progressive, often fatal disease. Balloon atrial septostomy may relieve symptoms or serve as a bridge to transplant, but carries risks of variable shunting and reocclusion. The atrial flow regulator (AFR) provides a sustained interatrial shunt with a defined shunt size. METHODS: AFR-Prophet is a prospective, multicenter study evaluating the mechanism of action and safety of AFR in high-risk PAH. Of 32 screened patients, 25 underwent the procedure. The primary end point was serious adverse device effects or procedure-related events at 90 d

A Life Without Lemons: A Case of Severe Pulmonary Arterial Hypertension Secondary to Vitamin C Deficiency in a Child With Autism Spectrum Disorder.

Pulmonary arterial hypertension (PAH) is a rare, serious, and reversible complication of vitamin C deficiency in children. Vitamin C deficiency, amongst other micronutrient deficiencies, is common in children with ASD due to restrictive dietary intake. In cases of PAH of unknown cause in paediatric patients, serum vitamin C testing should be considered, with vitamin C supplementation as indicated.

Pulmonary Hemorrhage Secondary to Superior Vena Cava Thrombosis Mimicking Upper Gastrointestinal Bleeding: A Case Report.

Upper gastrointestinal bleeding (UGIB) is a frequent cause of emergency department admission and is most commonly attributed to peptic ulcer disease or portal hypertension-related varices. In critically ill patients, however, pulmonary bleeding may be misinterpreted as hematemesis when blood is swallowed or expelled during retching or airway compromise. In patients undergoing long-term hemodialysis, catheter-related central venous thrombosis can precipitate atypical bleeding presentations. We report the case of a 45-year-old man with chronic kidney disease on maintenance hemodialysis who prese

Electrocardiographic markers of right heart strain and their association with arrhythmias in patients with COPD.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is frequently associated with cardiovascular complications, including right heart dysfunction and cardiac arrhythmias. Chronic hypoxia and pulmonary vascular remodeling may lead to structural and electrical cardiac changes. Electrocardiography (ECG) may provide simple markers of right heart strain that could help identify COPD patients at increased arrhythmic risk. OBJECTIVE: To evaluate the association between ECG markers of right heart strain and arrhythmias and to identify independent predictors using multivariable analysis. METHODS

The effect of high altitude on exercise capacity and cardiorespiratory function in patients with pulmonary vascular disease after an overnight stay at 2500 m: a randomised crossover study.

AIM: We investigated the effect of an overnight stay at high altitude (HA; 2500 m) on constant work rate exercise test (CWRET) time and cardiorespiratory function in patients with pulmonary vascular disease (PVD) and the impact of supplemental oxygen therapy (SOT) in case of severe altitude-induced hypoxaemia. METHODS: In a randomised crossover trial, stable patients diagnosed with pulmonary arterial or distal chronic thromboembolic pulmonary hypertension (PVD, PAH/CTEPH) performed a symptom-limited cycling CWRET at 470 m and after an overnight stay at 2500 m. For safety, patients with an oxy

Uncontrolled Hypertension from Inappropriate Ventilator Settings in Obstructive Sleep Apnea: A Case Report.

This case report explores a typical instance of poorly controlled secondary hypertension associated with obstructive sleep apnea (OSA), specifically exacerbated during bilevel positive airway pressure (PAP) therapy, which was an inappropriate ventilator mode. The patient exhibited uncontrolled blood pressure and a high residual apnea-hypopnea index (AHI) while on bilevel therapy. Following a guided retitration of PAP therapy, both blood pressure control and residual AHI improved substantially. This report aims to provide an in-depth analysis of pathophysiological mechanisms underlying PAP ther

The high-altitude baby: Neonatal development and survival in hypoxic environments.

High altitude exposes the human fetus and newborn to chronic hypobaric hypoxia, markedly reducing oxygen availability during critical windows of development. While adults tolerate moderate hypoxia through well-described acclimatization mechanisms, neonates (particularly those born preterm) exhibit substantially greater vulnerability. Despite more than one million infants being born annually at high or very high altitude, neonatal care practices are still largely derived from sea-level evidence, often neglecting altitude-specific physiology. This review synthesizes current evidence on the effec

Facilitating early diagnosis of chronic thromboembolic pulmonary hypertension with dynamic chest radiography: Protocol for a multicenter, assessor-blinded, case-wise randomized superiority reader study (FIND-DCR).

INTRODUCTION: Chronic thromboembolic pulmonary hypertension (CTEPH) is a treatable cause of pulmonary hypertension but remains under-recognized and is often diagnosed with delay. Limited access to lung ventilation-perfusion (V/Q) scintigraphy, especially outside tertiary centers, is one contributor. Dynamic chest radiography (DCR), with a pulmonary circulation analysis program, can provide a rapid, non-invasive, and widely deployable assessment of pulmonary perfusion. We describe the protocol of a multicenter reader study testing whether adding DCR-based analysis to standard initial work-up im

Cardiac Tamponade Secondary to Severe Uremic Pericarditis in the Modern Dialysis Era: A Case Report.

Uremic pericarditis is currently an uncommon complication of end-stage renal disease (ESRD) in developed countries due to improved access to renal replacement therapy and earlier initiation of hemodialysis. Nevertheless, delayed or interrupted dialysis may still result in life-threatening manifestations, including massive pericardial effusion and cardiac tamponade. We present the case of a 44-year-old man with ESRD requiring hemodialysis, insulin-dependent type 2 diabetes mellitus, morbid obesity, hypertension, and heart failure with preserved ejection fraction who was admitted because of prog
