Canagliflozin, a sodium-glucose cotransporter-2 inhibitor, reduces lung fibrosis in left heart dysfunction.

BackgroundLeft heart failure is the most common cause of pulmonary hypertension and increases morbidity and mortality. We investigate the use of canagliflozin, an antidiabetic Sodium-glucose Cotransporter-2 Inhibitor, which is first-line therapy in congestive heart failure, on pulmonary fibrosis in a porcine model of chronic myocardial ischemia.MethodologySixteen Yorkshire swine, eight in a normal diet control arm (NDC) and eight in the canagliflozin arm (CAN), underwent left thoracotomy and ameroid constrictor placement on the left circumflex artery. Seven weeks after placement, the swine und

Critical Care Dilemma: Complex Congenital Pulmonary Airway Malformation With Pulmonary Hypertension in a Neonate.

Congenital pulmonary airway malformations (CPAMs) are the most prevalent congenital lung lesions, yet their postnatal trajectory remains heterogeneous. Although many antenatally detected macrocystic lesions remain clinically silent, rapid postnatal enlargement can precipitate tension physiology, air-leak syndromes, and secondary pulmonary hypertension - events that mandate urgent departure from elective surgical timetables. We report a term female infant with a right-sided type 1 CPAM diagnosed by 20-week ultrasound. Despite an initial asymptomatic course, the lesion expanded abruptly at four

Adverse pregnancy outcomes and lung function later in life: The CARDIA lung study.

BACKGROUND: Pregnancy events are linked to future maternal health, although relationships between adverse pregnancy outcomes (APOs) and maternal respiratory health are unknown. METHODS: This is a secondary analysis of CARDIA, a multicenter cohort study. We included all women who had delivered a live infant. APOs were defined as a history of ever having a small-for-gestational-age infant, preterm birth, gestational diabetes mellitus, or hypertensive disorder of pregnancy. Women were categorized as having no, one or >1 pregnancy with any APOs. Outcomes included year 30 lung function, annual cha

Severe Pulmonary Hypertension Secondary to Pulmonary Artery Compression With Resolution Following Hodgkin Lymphoma Therapy.

BACKGROUND: Extrinsic compression of the pulmonary artery (PA) has been described with mediastinal and other thoracic masses. This compression can lead to acquired pulmonic stenosis and pulmonary hypertension. CASE SUMMARY: A 27-year-old woman was diagnosed with Hodgkin lymphoma with a mediastinal mass compressing her PA leading to severe pulmonary hypertension, without hemodynamic compromise. She was treated with chemotherapy and had rapid resolution of the pulmonary artery compression on subsequent transthoracic echocardiogram 1 month after starting treatment. DISCUSSION: The current liter

Outcomes of CDH patients receiving PDA ligation: a propensity score matched analysis.

PURPOSE: Neonates with congenital diaphragmatic hernia (CDH) have varying severity of pulmonary hypertension (PH) caused by developmental and structural abnormalities of the pulmonary vasculature and exacerbated by hypoxia induced vasoconstriction. Failure of the ductus arteriosus to close postnatally can intermittently shunt blood either to or away from the lungs. We hypothesize patent ductus arteriosus (PDA) ligation can be performed. METHODS: We queried the Congenital Diaphragmatic Hernia Study Group registry to identify patients from 2007 to 2020 who underwent PDA ligation. We excluded pa

Anesthesia Management Involving Remimazolam in a Patient With Pulmonary Hypertension and Right Ventricular Failure: A Case Report.

In the anesthetic management of patients with pulmonary hypertension (PH), it is essential to minimize right ventricular (RV) load and prevent abrupt changes in pulmonary vascular resistance (PVR). We report a case in which safe anesthetic management was achieved without circulatory collapse by using remimazolam in a patient with PH. An 84-year-old woman was scheduled to undergo percutaneous left atrial appendage closure for non-valvular atrial fibrillation. She had developed secondary PH and RV failure secondary to atrial fibrillation and systemic sclerosis, necessitating careful general anes

Staging Aortic Stenosis Based on Cardiac Damage: A New Tool for Risk Prediction, Clinical Decision-Making, and Trial Design.

Staging based on extra-valvular cardiac damage is an intuitive approach to categorizing patients with aortic stenosis (AS) that is easily applied using widely available echocardiographic tools. As discussed in this review, it has been shown to be a powerful tool for risk stratification that complements conventional approaches. The original and most widely used framework identifies stage 0 when there is AS without additional cardiac damage; stage 1 when there is left ventricular damage (hypertrophy, systolic or diastolic dysfunction with evidence of elevated filling pressures); stage 2 when the

Inpatient outcomes among children with Down syndrome: a Kids' Inpatient Database study.

BACKGROUND: Children with Down Syndrome (DS) are more likely to have multi-system comorbidities leading to more frequent hospitalizations than the general population. We aim to evaluate whether racial differences contribute to hospitalization outcomes and mortality among children with DS. METHODS: Hospital discharge records were obtained for children (< 21 y) with DS hospitalized between 2006 and 2019 from the Kid's Inpatient Database. The primary exposure was the Black race. Primary outcomes were invasive mechanical ventilation (IMV) and mortality. Secondary outcomes were non-invasive mechan

Effects of Dehydroepiandrosterone in Pulmonary Hypertension (EDIPHY): A Randomized, Double-Blind, Placebo-Controlled Crossover Trial.

RATIONALE: The adrenal steroid dehydroepiandrosterone (DHEA) and its sulfated form (DHEA-S) are deficient in pulmonary arterial hypertension (PAH) and lower levels are associated with worse right ventricular (RV) function, a decisive factor for survival in PAH. OBJECTIVES: We sought to determine whether DHEA improved RV function as measured by cardiac magnetic resonance imaging (MRI) in PAH. METHODS: We conducted a randomized, double-blind, placebo-controlled crossover clinical trial of DHEA in participants with PAH at a single center. All participants and study staff were blinded. The prima

From Pulmonary Hyperlucency to Pulmonary Hypertension: A Case Report of Swyer-James-MacLeod Syndrome.

Swyer-James-MacLeod syndrome (SJMS) is a rare condition characterized by unilateral pulmonary hyperlucency and vascular alterations secondary to post-infectious bronchiolitis obliterans. It is also an underdiagnosed disease due to the nonspecificity of its symptoms, which highlights the importance of complementary imaging studies for its identification. Although treatment is mostly conservative, in selected cases, surgical management can improve the prognosis and quality of life of patients. We present the case of an adult female with recurrent respiratory infections in childhood who had an in

Impact of Remdesivir on long-term outcomes in lung transplant recipients with SARS-CoV-2 infection: a retrospective cohort analysis.

BACKGROUND: The COVID-19 pandemic has presented considerable challenges for lung transplant (LTx) recipients, a population inherently at high risk for severe complications. Although early data from the pandemic indicated worrisome outcomes for these patients, understanding the current context of SARS-CoV-2 infections is essential to inform ongoing management strategies. The efficacy of anti-viral treatments, including Remdesivir (RDV), remains uncertain. This study aims to evaluate the effect of RDV on clinical outcomes in LTx recipients infected with SARS CoV-2, primarily those with a clinica

Seralutinib for the Treatment of Pulmonary Arterial Hypertension in Adults: TORREY Open-Label Extension Study.

INTRODUCTION: Seralutinib is an inhaled tyrosine kinase inhibitor targeting platelet-derived growth factor receptor (PDGFR) α/β, colony stimulating factor 1 receptor (CSF1R), and mast/stem cell growth factor receptor kit (c-KIT) kinases. TORREY, a phase 2, double-blind, randomized, placebo-controlled study of seralutinib in pulmonary arterial hypertension (PAH), met its primary endpoint, demonstrating a significant reduction in pulmonary vascular resistance (PVR) over placebo after 24 weeks (NCT04456998; EudraCT 2019-002669-37). We present results (as of December 5, 2024) from an open-label ex

SUL-150 Limits Vascular Remodeling and Ventricular Failure in Pulmonary Arterial Hypertension.

Pulmonary arterial hypertension (PAH) is a rare, progressive, and incurable disease characterized by an elevated pulmonary blood pressure, extensive remodeling of the pulmonary vasculature, increased pulmonary vascular resistance, and culminating in right ventricular failure. Mitochondrial dysfunction has a major role in the pathogenesis of PAH and secondary right ventricular failure, and its targeting may offer therapeutic benefit. In this study, we provide proof-of-concept for the use of the mitochondrially active drug SUL-150 to treat PAH. PAH was induced in rats by monocrotaline, followed

Revived role of reverse Potts shunt in management of paediatric severe pulmonary hypertension.

Pulmonary artery hypertension in children is a progressive, fatal disease secondary to a variety of causes leading to right ventricular dysfunction. Traditional treatments include lung transplantation (LTx) and targeted therapy. However, due to the shortage of donor sources, heavy medical economic burden, immune rejection, and other problems, many patients are discouraged. In the past 20 years, the Potts palliative shunt, which was initially used for the treatment of cyanotic heart disease, has been gradually revived. Studies have shown that reverse Potts shunt can improve postoperative cardia
