Plasma Proteomics Linking Primary and Secondary diseases: Insights into Molecular Mediation from UK Biobank Data.

Diabetes, hypertension, and dyslipidemia are major risk factors for cardiovascular (CVD), cerebral, and renal diseases (RD). However, the underlying molecular mechanisms, particularly the biological paths linking these primary conditions to downstream diseases remain incompletely understood. In this study, we investigated the role of plasma proteins as mediators of secondary disease development using data from ~50,000 UK Biobank participants. Across three primary diseases and 18 subsequent conditions, we identified 1,461 significant mediation pathways involving 395 unique plasma proteins. Nota

Managing a Patient With Hypertensive Crisis.

Renal artery stenosis (RAS) is a common yet under-recognized cause of secondary hypertension and acute kidney injury, especially in elderly patients with multiple comorbidities. We report the case of a 74-year-old woman with longstanding hypertension, chronic kidney disease stage 3, and tobacco use who presented with flash pulmonary edema and hypertensive emergency. Despite a negative renal duplex ultrasound, persistent clinical suspicion prompted further evaluation with renal angiography, which revealed significant right RAS. Following successful stenting, the patient showed marked improvemen

Personalised exercise-rehabilitation for people with multiple long-term conditions (PERFORM): a randomised feasibility study.

OBJECTIVE: Existing exercise-based rehabilitation services, such as cardiac and pulmonary rehabilitation, are traditionally commissioned around single long-term conditions (LTCs) and therefore may not meet the complex needs of adults with multiple long-term conditions (MLTCs) or multimorbidity. The aim of this study was to assess the feasibility and acceptability of the newly developed personalised exercise-rehabilitation programme for people with multiple long-term conditions (PERFORM) and the trial methods. DESIGN: A parallel two-group mixed-methods feasibility randomised controlled trial (

Increased Risk of Pulmonary Complications with Long Cephalomedullary Nails in Geriatric Intertrochanteric Femur Fractures: A Retrospective Review of 881 Patients.

BACKGROUND: Despite extensive literature comparing short and long cephalomedullary nails (CMNs), the impact of CMN length on pulmonary complications in geriatric patients with intertrochanteric (IT) fractures is not well established. This study compared postoperative pulmonary complications in geriatric patients with IT fractures treated with short (<235 mm) or intermediate (235-240 mm) CMNs (SINs) versus long (≥300 mm) CMNs (LNs). METHODS: Patients aged ≥65 years with AO Foundation/Orthopaedic Trauma Association (AO/OTA) 31A1-A3 fractures treated with SINs or LNs between 2008 and 2020 were r

Beyond the Acute Phase: Persistent Pulmonary Findings After COVID-19 in Hungary.

Objective The objective of this one-year study, conducted at Kenézy Gyula Hospital in Hungary and involving a cohort of 50 hospitalized patients with confirmed SARS-CoV-2 infection, was to assess the long-term impact of the disease on lung morphology. Using follow-up chest CT imaging performed three months post-infection, the study aimed to identify persistent abnormalities such as ground-glass opacities (GGOs), fibrotic-like changes, and other post-inflammatory sequelae. By correlating radiological findings with clinical and laboratory data, the study sought to provide insights into the exten

Clinical validation of a machine-learned, point-of-care system to IDENTIFY pulmonary hypertension.

BACKGROUND: Pulmonary hypertension (PH) is a collection of diverse disorders, defined by mean pulmonary artery pressure (mPAP) ≥21 mmHg (most recent guidelines) or ≥25 mmHg (previous guidelines, that underpins the field's past work) measured by right heart catheterisation (RHC). Considering the difficulties in diagnosing PH and the subsequent treatment delays, there is a need for novel diagnostics to enable prompt detection. METHODS: An algorithm to assess mPAP elevation was validated using subjects with elevated mPAP from RHC (positive cohort) and subjects with low probability of PH by strin

Radioiodine Plus Low-Dose Lenvatinib in Radioiodine-Sensitive High-Volume Metastatic Differentiated Thyroid Cancer: A Pilot Study.

BACKGROUND: Radioactive iodine (RAI) therapy is the standard-of-care for metastatic differentiated thyroid cancer (DTC), but ~60% of patients show resistance before achieving satisfactory response. METHODS: Eligible patients were randomly assigned to RAI therapy plus lenvatinib (RAI-L arm) or standard RAI therapy alone (RAI arm). All patients were administered RAI therapy (5.5-7.4 GBq) every 6-9 months, and patients in RAI-L arm were additionally administered lenvatinib (10 mg once daily). The primary endpoint was objective response rate (ORR). Secondary endpoints included progression-free su

A Hard Lump to Chew: Delayed Diagnosis of Foreign Body Aspiration in an Elderly Man Leading to Severe Respiratory Failure.

Foreign body aspiration (FBA) in adults is uncommon and often associated with risk factors such as advanced age and chronic alcohol abuse. It can constitute a serious emergency with acute symptoms secondary to airway obstruction, but there are cases in which it goes unnoticed, causing late complications with insidious symptoms and recurrent respiratory infections. We present the case of a 77-year-old male patient with a history of hypertension, type 2 diabetes mellitus, dyslipidemia, hyperuricemia, probable chronic obstructive pulmonary disease, chronic alcohol abuse, and gonarthrosis, who was

Prevalence and Characteristics of Sleep Apnea in Patients With Heart Failure: A Single-Center Study.

Introduction Heart failure is a common outcome of chronic cardiovascular diseases and carries a poor prognosis. Sleep apnea is an important comorbidity in heart failure patients, increasing the risk of mortality. Obstructive sleep apnea (OSA) is more commonly associated with heart failure with preserved ejection fraction, while central sleep apnea (CSA) prevalence increases with worsening New York Heart Association (NYHA) functional class and decreasing ejection fraction. The primary endpoint of the study is the prevalence of sleep apnea in heart failure patients. Secondary endpoints were the

Effects of pulmonary arterial hypertension therapies on arterial oxygenation in patients with pulmonary hypertension associated with lung disease: a systematic review and meta-analysis.

BACKGROUND: Pulmonary hypertension (PH) associated with lung disease entails a poor prognosis. There is concern about the potential deleterious effect on gas exchange of drugs approved for pulmonary arterial hypertension (PAH) in patients with lung disease. We conducted a systematic review on the effects of drugs approved for PAH on arterial oxygenation in randomised clinical trials (RCTs) conducted in patients with PH associated with lung disease. METHODS: Five databases were searched until May 2025. We included RCTs with PAH therapies in patients with PH associated with lung disease that re

Comparative efficacy and safety of urapidil and nitroglycerin for blood pressure control in acute cardiogenic pulmonary edema.

BACKGROUND: Acute cardiogenic pulmonary edema (ACPE) is a life-threatening manifestation of acute heart failure characterized by abrupt pulmonary congestion and hypertension. Rapid blood pressure (BP) reduction with intravenous vasodilators is a cornerstone of emergency management. Nitroglycerin is widely used but may be limited by side effects, while urapidil, an α1-adrenergic antagonist with additional central serotonergic activity, has been proposed as an alternative. METHODS: In this prospective cohort study at a tertiary university emergency department, adults with hypertensive ACPE rece

Atherosclerotic renal artery stenosis in the post-CORAL Trial Era. A narrative review.

Atherosclerotic renal artery stenosis (ARAS) represents a common manifestation of systemic atherosclerosis and remains an underrecognized cause of secondary hypertension, chronic kidney disease, and cardiovascular morbidity. Although often clinically silent, progressive narrowing of the renal artery may result in renovascular hypertension, ischemic nephropathy, or cardiac destabilization syndromes such as recurrent pulmonary edema. The pathophysiology of ARAS extends beyond simple flow limitation, involving renin-angiotensin-aldosterone system activation, oxidative stress, microvascular rarefa

Outcomes of Pulmonary Arterial Hypertension in Pregnancy: A Tertiary Care Retrospective Study.

BACKGROUND AND AIMS: Pulmonary arterial hypertension (PAH) is a rare disease affecting mainly women of childbearing age. It is associated with high maternal mortality. Hence, obstetricians and cardiologists recommend against pregnancy, thereby stressing the need for effective contraception such as an intrauterine contraceptive device (IUD), and in case pregnancy occurs, early termination of pregnancy should be considered. The present study aims to retrospectively analyse 12 cases presenting with PAH in pregnancy and their maternal and fetal outcomes. METHODS: Retrospective analysis of 12 case

Alcoholic Cirrhosis in the Hispanic Population of the United States: A Retrospective Analysis.

BACKGROUND: The Hispanic population is the fastest-growing ethnic group in the USA and is projected to comprise 30% of the US population by 2050. Despite socioeconomic disadvantages and often presenting with more severe disease phenotypes, previous studies in chronic diseases have shown that Hispanics experience lower overall inpatient mortality compared with other ethnic groups - a phenomenon known as the "Hispanic Paradox". In alcoholic liver cirrhosis (ALC), this paradox is particularly evident: Hispanics frequently develop more advanced forms of alcoholic liver cirrhosis, yet survival outc

Systemic sclerosis: pathogenic mechanisms and their implications for treatment.

Systemic sclerosis (SSc) is a rare systemic autoimmune disease characterized by a triad of pathogenic mechanisms, including: a) microvascular hyperreactivity secondary to endothelial dysfunction, b) dysregulated immune activation of both innate and adaptive immunity, with the production of autoantibodies targeting nuclear antigens (e.g., anticentromere antibodies, anti-RNA polymerase III antibodies, and anti-topoisomerase I antibodies), and c) fibrosis of the skin and internal organs, such as the lungs, due to excessive extracellular matrix deposits produced by dysregulated myofibroblasts. Early Echocardiographic and Serum Biomarkers Predict Thrombotic Microangiopathy, Endotheliopathy, and Survival After Pediatric Hematopoietic Stem Cell Transplant.

Many life-threatening complications of hematopoietic stem cell transplantation (HSCT) develop secondary to endothelial injury and dysfunction, including transplant-associated thrombotic microangiopathy (TA-TMA), sinusoidal obstruction syndrome, idiopathic pneumonia syndrome, and engraftment syndrome. These endotheliopathies are often accompanied by cardiovascular compromise. Echocardiographic abnormalities including pericardial effusions and elevated right ventricular pressure have previously been identified as early indicators of TA-TMA. Additional echo-derived parameters and serologic metric
