Hearing loss, hearing-aid use, and mortality risk: individual participant data meta-analysis of population-based studies from Australia, Iceland, and the USA and a replication study from Norway.

BACKGROUND: Hearing loss is associated with poorer health, depression, cognitive decline, and perhaps, with increased mortality risk. Whether hearing aids ameliorate this association is not well understood. We conducted a meta-analysis with a replication study to examine associations between hearing loss, current use of hearing aids, and mortality. METHODS: Hearing threshold data, all-cause mortality, and cardiovascular disease (CVD)-related mortality (available across datasets) were analysed using individual participant data (IPD) meta-analysis of three combined population-based studies: Blu

Physiological Significance and Functions of AA.

AA has several physiological functions. Its various metabolites also have many significant actions and thus participate in innumerable physiological and pathological processes. There is strong evidence to suggest that AA and its metabolites play a significant role in the pathophysiology of diabetes mellitus (DM), hypertension (HTN), and coronary heart disease. In addition, high-fat diet (HFD), high-cholesterol diet (HCD), calorie-dense foods (CDF), high-salt and low-potassium diet, low magnesium, lack of exercise, sedentary lifestyle, and aging alter the generation of AA from its dietary precu

AA in Hypertension and Preeclampsia.

Human essential hypertension is driven by a network of interacting nutritional, metabolic, inflammatory, and endothelial mechanisms in which AA and other PUFAs play a critical role. Beyond excess sodium intake, population data supports the role of inadequate calcium, potassium, and magnesium intake and low antioxidant vitamin status in shaping blood pressure regulation. These nutrients influence vascular smooth muscle tone and act as cofactors that support Δ6 and Δ5 desaturase activities, thereby governing tissue availability of AA, EPA, and DHA and downstream formation of vasodilator and anti

AA in Brain-Memory, Dementia, Depression, Anxiety, Alzheimer's Disease, Affective and Psychiatric Disorders, and Coronary Heart Disease (CHD).

The human brain is rich in AA and DHA, which are crucial for normal brain growth and development and memory. They form a major structural component of neurons of the cerebral cortex and photoreceptor cells in the retina. AA and DHA support synaptic plasticity and contribute to efficient neurotransmission. Adequate amounts of AA and DHA support neuronal growth and the formation of new synapses and thus are crucial for memory and cognitive function.Depression, anxiety, schizophrenia, and other neurological diseases occur more frequently in those with coronary heart disease (CHD) and vice versa.

Immunophenotypic Assessment of Natural Killer Cell Degranulation and Monocyte Function from Cryopreserved Peripheral Blood Mononuclear Cells.

Natural killer (NK) cells and monocytes are central components of the innate immune system, playing critical roles in host defense, immune surveillance, and inflammation. Functional alterations in these cell populations are implicated in cancer, infectious diseases, cardiovascular disease, and responses to environmental and psychosocial stressors. Accurate measurement of NK cell and monocyte distribution and function is therefore essential for both basic and translational immunology. In large-scale or longitudinal studies, cryopreserved peripheral blood mononuclear cells (PBMCs) are commonly u

Global, regional, and national burden of exposure to chewing tobacco, 1990-2023: a systematic analysis of the Global Burden of Disease Study 2023.

BACKGROUND: Exposure to chewing tobacco is a known health risk factor but has received less attention than smoking in global tobacco control efforts. As the evidence base linking chewing tobacco use to poor health grows, it is essential to closely track the changing use of chewing tobacco distinct from other forms of smokeless tobacco, as well as the estimated burden associated with its use. These data are essential to inform policy makers who seek to identify and avert the health burden associated with these dangerous products. METHODS: We estimated the prevalence of chewing tobacco use and

Mechanisms underlying rheumatic heart disease immune-mediated pathogenesis: T-cell responses, cytokine networks, and immune dysregulation as drivers of valvular injury.

Acute rheumatic fever is an immune-mediated sequela of Streptococcus pyogenes (group A Streptococcus) infection, presenting with fever, arthritis, chorea, rash, and, crucially, carditis. Without effective secondary prophylaxis and care, acute rheumatic fever can progress to chronic rheumatic heart disease, a leading yet under-recognised cause of cardiovascular morbidity and death that disproportionately affects children and young adults aged 5-24 years in low-resource settings. Robust evidence implicates the role of antigenic mimicry-particularly between streptococcal M protein and cardiac or

Disrupted interhemispheric functional connectivity between the bilateral precentral gyri and associated transcriptomic signatures in acute basal ganglia ischemic stroke.

Acute basal ganglia ischemic stroke (BGIS) often leads to motor dysfunction, but the mechanisms of functional reorganization, particularly the underlying molecular basis, remain unclear. This study aimed to investigate the local and global functional reorganization patterns and their molecular correlates in acute BGIS. Resting-state fMRI data were acquired from 59 patients with acute BGIS and 35 age- and sex-matched healthy controls (HCs). Regional homogeneity (ReHo) and seed-based functional connectivity (FC) analyses were used to assess local and global brain connectivity, respectively. We i

Vascular effects of dietary nitrate supplementation in older adults with coronary artery disease: A randomized, placebo-controlled, crossover pilot trial.

Coronary artery disease (CAD) is characterized by endothelial dysfunction, arterial stiffening, and abnormal central hemodynamics, which are independent predictors of adverse cardiovascular events. Dietary nitrate supplementation using beetroot-derived products is a promising strategy, but its vascular effects in adults with CAD remain unclear. This randomized, double-blind, placebo-controlled, crossover pilot trial evaluated the safety, feasibility, and vascular effects of dietary nitrate supplementation to inform the development of a full-scale trial. Eight older adults with confirmed CAD (6

Adaptive electrocardiogram (ECG) signal denoising using particle swarm optimization-based non-local means (NLM) filtering.

An ECG signal is usually corrupted by AWGN (additive white Gaussian noise), baseline wandering (BW), power line interference (PLI), and motion artifacts (EM), which may impact the reliable diagnosis of arrhythmias. This study proposes a hybrid ECG denoising framework that integrates particle swarm optimization with non-local means filtering to suppress diverse ECG artifacts while preserving signal morphology. In the first experiment, ECG signals from MIT-BIH Arrhythmia Database are artificially contaminated using AWGN and PLI to measure their denoising capability. In the second experiment, rea

Paediatric patient with FLNC and CTNNA3 variants presenting with frequent premature ventricular contractions and systolic dysfunction: a case report.

We present the case of an 8-year-old male with frequent premature ventricular contractions and mild left ventricular dysfunction, who was found to carry heterozygous variants in both FLNC and CTNNA3. Medical therapy resulted in improved systolic function and reduced arrhythmia burden. This rare combination suggests a potential genotype-phenotype correlation in paediatric arrhythmogenic cardiomyopathy.

A case of endocardial mapping for the epicardial pure origin of premature ventricular complex using the time difference between bipolar and unipolar potential.

In endocardial mapping of premature ventricular complexes of intramural or epicardial origin, we must be aware that pure origin and exit do not always coincide. In this study, we describe a case of endocardial mapping for epicardial-side premature ventricular complexes of pure origin using the time difference between bipolar and unipolar potentials.

Impact of Virtual Reality on Transcatheter Aortic Valve Implantation: A Prospective Randomized Controlled Trial.

BACKGROUND: Accurate preprocedural planning is crucial for a successful transcatheter aortic valve implantation to ensure patient safety and valve longevity. Through 3-dimensional visualization, virtual reality (VR) offers the potential to enhance this process. The study investigated whether the inclusion of VR in preprocedural planning can improve the procedural preparation, impact intraprocedural parameters, and improve short-term patient outcomes. METHODS: This randomized, prospective, controlled study included 140 patients who underwent transcatheter aortic valve implantation at the Unive

Prediction of severe aortic stenosis using peripheral arterial waveforms during brachial-ankle pulse wave velocity testing.

With the increasing use of minimally invasive catheter-based therapies, early identification of severe aortic stenosis (AS) has become essential. The slow upstroke waveform (Tardus) observed in carotid artery wave analysis is a known feature of AS, and quantitative assessments of Tardus can estimate the severity of AS. Prolongation of ejection time (ET) and upstroke time (UT) is a characteristic of Tardus. However, the clinical relevance of ET and UT measured during brachial-ankle pulse wave velocity (baPWV) assessment remained unclear. This study aimed to develop a predictive model for screen

Interpretable machine learning model for early prediction of deep vein thrombosis in elderly ICU patients: multicenter development and external validation.

BACKGROUND: Deep vein thrombosis (DVT) is a frequent complication in elderly intensive care unit (ICU) patients, while reliable prediction tools for this population remain limited. This study aimed to develop and externally validate an interpretable machine learning model for early prediction of DVT in critically ill elderly patients. METHODS: In this multicenter retrospective study, ICU patients aged ≥ 75 years from two tertiary medical centers in China were enrolled. Patients with pre-existing DVT were excluded. Exercise and Thoracic Aortic Aneurysms: A Paradigm Shift Toward Informed, Personalized Activity Guidelines.

Exercise is widely recognized for its cardiovascular benefits; however, its safety and role in patients with thoracic aortic aneurysms (TAAs) remains complex due to its distinctive biomechanical vulnerabilities. Fundamentally different from abdominal aortic aneurysms, TAAs develop from distinct pathophysiological processes that affect their hemodynamic responses to stressors and influence their risk of adverse aortic events. Exercise induces acute hemodynamic changes, including increased stroke volume, systolic and pulse pressures, and transient aortic stiffening. These changes impose mechanic

H2FPEF Score and Charlson Comorbidity Index as Independent Predictors of Cardiovascular Outcomes in Elderly Patients Undergoing Geriatric Rehabilitation.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is difficult to diagnose in older adults. The H2FPEF score estimates the likelihood of HFpEF and has been associated with adverse prognosis, but its performance has not been evaluated in geriatric inpatients with a high comorbidity burden. HYPOTHESIS: We hypothesized that the H2FPEF score predicts adverse outcomes in geriatric inpatients independently of comorbidity burden. METHODS: This prospective cohort included consecutive patients admitted to a geriatric rehabilitation unit. All patients underwent functional and echocard

Cardiopulmonary exercise testing utilization and clinical outcomes among patients with heart failure across racial and ethnic groups.

BACKGROUND AND AIMS: Cardiopulmonary exercise testing (CPET) carries a Class I recommendation for advanced heart failure (HF) therapy candidacy. Whether CPET utilization and outcomes differ by race is unknown. METHODS: Using TriNetX, adults with HF (ICD-10 I50.x, 2010-23) were stratified by race/ethnicity and CPET receipt (CPT 94621) within 1 year of diagnosis. All-cause mortality and HF hospitalization were assessed at 1, 3, and 5 years after 1:1 propensity score matching for demographics, HF subtype, comorbidities, and guideline-directed medical therapy. RESULTS: Among 807 379 patients wit
