Apolipoprotein A1 attenuates epirubicin-induced cardiotoxicity with GPX4-linked ferroptosis-defense modulation.

Anthracycline-induced cardiotoxicity (AIC), in which ferroptosis-related cardiomyocyte injury has been implicated, remains a major challenge in oncology. This study investigates the therapeutic potential of an apolipoprotein A1 (ApoA1) mimetic peptide, L-4 F, in epirubicin-induced cardiotoxicity (EIC) in a rat model. Rats received epirubicin (2.5 mg/kg/week, i.p.) for six weeks to establish the EIC model, with or without concurrent ApoA1 mimetic treatment (L-4 F, 1 mg/kg/day). Cardiac function was assessed by echocardiography. Myocardial tissues underwent single-nucleus RNA sequencing (snRNA-s

Catheter Ablation of Idiopathic Premature Ventricular Contractions Using Robotic Magnetic Navigation: Extensive Experience and Review.

AIMS: This retrospective analysis of our extensive experience focuses on idiopathic premature ventricular contractions (PVC) ablation guided by robotic magnetic navigation (RMN). Critical evaluation is given to the causes and impact of acute procedural failure. METHODS: This retrospective cohort study enrolled 824 patients (mean PVC burden 21.0 ± 6.6%) undergoing first-time RMN-guided PVC ablation. Acute failure was defined as recurrent clinical PVC during the observation period after the last radiofrequency (RF) application. The Navigation Index was calculated as the ratio of RF delivery tim

Cycle length variation assists in differentiation of atrial, ventricular, and sinus arrhythmias in horses at rest.

OBJECTIVE: Criteria for arrhythmia classification in horses are insufficient, particularly when differentiating atrial premature complexes (APCs) from sinus arrhythmia (SA). This study investigated RR and PP interval changes between SA, APCs, and ventricular premature complexes (VPCs). METHODS: 30 resting ECGs were reviewed: 10 each containing SA, APC, and VPC. Sequences of 10 RR and PP intervals were recorded for each arrhythmia (20 events/ECG). Percent RR and PP deviations were calculated (1) between sequential intervals (%ΔSEQPP and %ΔSEQRR, respectively) and (2) compared to averaged basel

Inulin- and Butyrate-Mediated HIF-1α Butyrylation Prevent Diabetic Ferroptosis and Cardiomyopathy by Maintaining Iron Homeostasis.

UNLABELLED: Abnormal iron metabolism and ferroptosis contribute to diabetic cardiomyopathy (DCM); however, the regulatory mechanisms remain unclear. Inulin improves systemic metabolism and cardiac function, but whether it alleviates myocardial ferroptosis and DCM is unknown. Using two distinct type 2 diabetes (T2D) mouse models-the high-fat diet combined with streptozotocin-induced T2D model and the db/db mouse model-in combination with an iron chelator, we found that imbalanced iron homeostasis and ferroptosis drive DCM. Inulin restored cardiac iron homeostasis, inhibited ferroptosis and DCM,

Postextrastimulus Termination of Ventricular Tachycardia: An Indicator of a Good Ablation Site?

Entrainment mapping, or continuous resetting of a re-entrant tachycardia, provides a powerful tool for distinguishing between focal and re-entrant ventricular tachycardias (VTs) and for identifying critical circuit components, such as the entrance, critical isthmus (CI), and exit sites. However, this technique is not always feasible and, in some cases, may itself terminate VT during the entrainment attempt. Whereas local capture of the CI without global ventricular capture (nonglobal capture [NGC]) can terminate VT by creating a bidirectional block (orthodromic or nonorthodromic capture) withi

Matching Electrode Configuration to Signal Sensing for Cardiac Electroanatomic Mapping: A Critical Appraisal.

The management of cardiac arrhythmias has been transformed by electroanatomical mapping, yet it is often underappreciated how dependent mapping is to electrode configuration, sensing modality, catheter geometry, and signal-processing characteristics. This review systematically evaluates how these features influence mapping, in the context of catheters in contemporary linear, circular, grid, star-shaped and basket configurations. We compare direct sensing strategies in unipolar and bipolar configurations, monophasic action potentials, and discuss post-processing approaches including omnipolar,

When Swallowing Triggers VT.

We describe a case of swallowing-induced ventricular tachycardia in a young, otherwise healthy male with a structurally normal heart which was refractory to multiple anti-arrhythmic medication and was successfully treated with ablation of a cardiac ganglion in the crux of the heart. We use this case to describe an unusual disease presentation and highlight current evidence related to evaluation and management of ventricular arrhythmias and add to the limited body of evidence related to swallowing induced tachycardia.

The Effect of Aortic Regurgitation on Left Ventricular Flow Dynamics Assessed by 4D-Flow MRI.

BACKGROUND: Aortic regurgitation (AR) alters normal blood flow patterns in the left ventricle (LV), affecting diastolic filling dynamics, and is commonly quantified by the regurgitant fraction (RF). However, conventional diagnostic methods have notable limitations, prompting growing interest in four-dimensional flow magnetic resonance imaging (4D-flow MRI) as a tool for quantitatively assessing AR severity. In parallel, viscous energy dissipation (VED), which captures the energy loss caused by viscous forces within blood flow, provides additional important insight into the hemodynamic burden i

Validation of an Automated Artificial Intelligence-Driven Planner for Transcatheter Aortic Valve Replacement.

BACKGROUND: Preprocedural planning with multislice computed tomography (MSCT) is an essential prerequisite for successful transcatheter aortic valve replacement (TAVR). However, preprocedural planning is time-consuming and may contain clinically meaningful interobserver variability. Artificial intelligence (AI) based automated planning methods may provide anatomical MSCT measurements while reducing the time load required for the analysis. Previous studies on the validation of AI-based TAVR planning methods excluded anatomical variants or lacked data on actual TAVR prosthesis size agreement. Mi

Dapagliflozin After Transcatheter Aortic Valve Implantation According to Frailty in DapaTAVI Trial.

BACKGROUND: Frailty is common in patients with severe aortic stenosis undergoing transcatheter aortic valve implantation and portends worse outcomes. Whether the benefit of posttranscatheter aortic valve implantation dapagliflozin differs by frailty is uncertain. METHODS: In this post hoc analysis, we investigated the efficacy of dapagliflozin according to frailty status in the DapaTAVI (Dapagliflozin in Patients Undergoing Transcatheter Aortic Valve Implantation) trial. Frailty was assessed through the Fatigue, Resistance, Ambulation, Illnesses, and Loss of Weight scale (robust=0, prefrail=1

The Influence of Pre-operative Mitral Regurgitation in Post Operative Recovery of Ventricular Function in Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery (ALCAPA).

We attempted to study the influence of pre-operative mitral regurgitation on the recovery of ventricular function in paediatric patients with repaired anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA). Eighty-eight patients under 18 years who underwent ALCAPA repair from 2003 to 2022 were included. Baseline clinical and echocardiographic data, including MR and left ventricular ejection fraction (LVEF), were recorded pre-operatively and at multiple post-operative follow-ups (discharge, 1 month, 6 months, 1 year, and annually thereafter). MitraNav: Design and Modeling of a Robotic System for Transseptal Mitral Valve Replacement with Dock Catheter Task-Space Control.

PURPOSE: Transcatheter mitral valve replacement (TMVR) is a promising therapy for reducing mitral regurgitation in select patients. Direct tip control of multi-segment catheters can simplify manipulation and enable precise, stable positioning. This paper introduces MitraNav, the first robotic system developed for dock-based TMVR, focusing on system design and task-space control of the dock catheter. METHODS: Built on a previously validated guide-sheath robot, MitraNav is upgraded to support concentric multi-catheter manipulation via a unified actuation mechanism. It suits both dock and balloo

Opportunistic CT Fatty Muscle Fraction for Outcome Prediction in Patients Undergoing Transcatheter Mitral Valve Edge-to-Edge Repair.

BACKGROUND: Frailty is a risk factor for adverse outcomes in patients undergoing mitral valve transcatheter edge-to-edge repair (M-TEER) and is interrelated with sarcopenia. This study aimed to investigate the prognostic value of fatty muscle fraction (FMF), measured from routine preinterventional computed tomography, as an imaging biomarker for frailty in patients undergoing M-TEER. METHODS: In patients undergoing M-TEER between 2010 and 2022 at the Heart Center Bonn who received preprocedural contrast-enhanced computed tomography, skeletal muscle quantity and quality at the L3 vertebral lev

Simulation of Transcatheter Therapies for Atrioventricular Valve Regurgitation in an Open-Source Finite Element Simulation Framework.

PURPOSE: Transcatheter edge-to-edge repair (TEER) and annuloplasty devices are increasingly used to treat mitral valve regurgitation, yet their mechanical effects and interactions remain poorly understood. This study aimed to establish an open-source finite element modeling (FEM) framework for simulating patient-specific mitral valve repairs and to evaluate how TEER, annuloplasty, and combined strategies influence leaflet coaptation and valve mechanics. A central objective was to demonstrate how such simulations may support surgical planning by identifying optimal interventions. METHODS: A pa

Ultrasonic manifestations of diffuse pericardial mesothelioma masquerading as pericardial effusion: a case report.

BACKGROUND: Primary pericardial mesothelioma (PPM) is an exceedingly rare, highly invasive cardiac neoplasm with insidious onset and non-specific clinical symptoms. Due to the lack of typical early manifestations and effective routine diagnostic methods, PPM is often misdiagnosed and missed, resulting in delayed treatment and poor prognosis. CASE PRESENTATION: A 42-year-old male presented with 6 months of recurrent chest tightness and pain, worsening over 2 months, and two prior admissions for unexplained pericardial effusion. On admission, he had tachycardia, distant heart sounds, bilateral

Zero-fluoroscopy approach to left atrial appendage closure using intracardiac echo guidance.

BACKGROUND: Left atrial appendage closure (LAAC) with the Watchman™ device is typically performed with the use of transesophageal echocardiography (TEE) and fluoroscopy. LAAC guided by intracardiac echo (ICE) without the use of fluoroscopy has not been previously described. OBJECTIVE: In this series, we describe the safety and feasibility of Watchman™ implantation using only ICE imaging and without the use of fluoroscopy. METHODS: 214 patients undergoing LAAC with the Watchman FLX™ or Watchman FLX™ Pro device and using a planned fluoroscopy-free approach were studied. ICE was used for echoca

Active Helicobacter pylori Infection and Atrial Fibrillation in Patients Hospitalized With Acute Myocardial Infarction.

OBJECTIVE: Chronic Helicobacter pylori (H. pylori) infection may promote systemic inflammation and has been proposed as a contributor to atrial fibrillation (AF), but clinical evidence is inconsistent. The aim of the study was to explore whether active H. pylori infection was associated with AF in patients hospitalized with acute myocardial infarction (AMI). METHODS: This cross-sectional analysis included adults from a two-center observational AMI cohort. Active H. pylori infection was assessed during hospitalization using a urea breath test (UBT). AF was defined as AF documented before admis

Late gadolinium enhancement dispersion mapping predicts major arrhythmic events in patients with prior myocardial infarction.

BACKGROUND: Myocardial scar heterogeneity is a key determinant of ventricular arrhythmias after myocardial infarction (MI). Late gadolinium enhancement dispersion mapping (LGE-DM) quantifies structural complexity, but its prognostic role in ischemic cardiomyopathy remains unevaluated. METHODS: We retrospectively included 364 consecutive patients with prior MI (>90 days). For each LGE image, a parametric map was generated and each enhanced pixel scored (0-8) based on surrounding signal distribution; the global dispersion score (GDS) was calculated as the mean pixel score, quantifying pixel-to-
