This article describes a case involving a patient who presented with chest pain persisting for three days, elevated levels of cardiac troponin, and widespread ST segment elevation in the ECG leads. Subsequent diagnosis revealed esophageal perforation resulting from the accidental ingestion of a fish bone, leading to pericardial effusion and pneumopericardium. This case highlights the importance of healthcare professionals conducting a comprehensive investigation of the patient’s medical history when confronted with widespread ST segment elevation and elevated cardiac troponin levels to avoid m

BACKGROUND: Purulent pericardial effusions are rare, comprising less than 1% of pericardial disease. Patients with systemic lupus erythematosus are particularly vulnerable owing to immunosuppression, and distinguishing infectious from autoimmune causes of pericardial effusion can be challenging. This distinction is critical, as treating a flare with immunosuppression can worsen an occult infection. We describe a unique case of a patient with systemic lupus erythematosus who developed purulent pericardial effusion secondary to methicillin-sensitive Staphylococcus aureus, and we highlight asympt

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease with frequent cardiac involvement. A 49-year-old male with a 20-year history of SLE presented with a rash and lip swelling concerning for angioedema versus anaphylaxis. During bedside rounds, point-of-care-ultrasound (POCUS) revealed a small posterior pericardial effusion, prompting a formal transthoracic echocardiogram (TTE). He subsequently developed acute substernal chest pain with ECG findings of isolated ST elevations and PR depressions, in the inferior leads (II, III, and aVF). Despite concerns for acute coronary syndrome

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by inflammation and immune-mediated damage with mucocutaneous, musculoskeletal, cardiopulmonary, and renal involvement. It typically affects younger women; however, late-onset presentations can occur and often display distinct clinical features. We report the case of a septuagenarian woman who presented with massive pericardial effusion and type 3 cardiorenal syndrome. Further evaluation revealed class IV lupus nephritis, confirming a diagnosis of late-onset SLE according to the 2019 European League Against Rheuma

Systemic lupus erythematosus (SLE)-associated pericardial effusion is generally sterile, and purulent pericarditis is rare; however, delayed recognition can be fatal. In active SLE, immunologic abnormalities - such as hypocomplementemia and impaired neutrophil function - together with concurrent immunosuppressive therapy, may predispose patients to invasive infections. We report a rare clinical course in which left-sided infective endocarditis in a patient with active SLE progressed to purulent pericarditis and cardiac tamponade, underscoring the need for rapid diagnostic consideration and tim

Initially recognized as a key cardiac feature of systemic lupus erythematosus and rheumatoid arthritis, autoimmune pericarditis has gained increasing attention given the recent advances in cardiac imaging, biomarker assessment, and understanding of immune-mediated mechanisms. The prevalence of pericardial involvement varies considerably among autoimmune diseases. Patients may have a small pericardial effusion that remains clinically silent but could also present with acute, recurrent, or chronic pericarditis. A minority develop severe complications such as tamponade or constrictive physiology.

INTRODUCTION: Histiocytic Myocardial Inflammatory Disease (HMID) is a rare and under-recognized form of myocarditis, characterized by predominant histiocytic infiltration within the myocardial tissue. Unlike more common lymphocytic and eosinophilic forms, HMID remains diagnostically challenging, particularly in children. Here we present a pediatric case, outlining the clinical course and the difficulties encountered, and highlighting the histopathological and immunohistochemical features essential for establishing the diagnosis.
CASE DESCRIPTION: A 4-year-old girl accompanied by her parents,

OBJECTIVES: Autoantibodies represent a serological hallmark of SSc. Heart involvement in SSc patients includes diastolic dysfunction, conduction block, pericardial effusion, altered left ventricular ejection fraction (LVEF), valvular pathology, as well as myocardial inflammation and/or fibrosis. To characterize the role of anti-RNA polymerase III (RNAPIII) antibody positivity in the pathophysiology of coronavirus disease 2019 (COVID-19)-related cardiac involvement in SSc patients.
METHODS: We retrospectively analysed four SSc patients with anti-RNAPIII antibodies who developed cardiac complic

Primary pericardial angiosarcoma is an exceedingly rare and highly aggressive malignancy arising from pericardial endothelial cells, with a reported median survival of approximately 7 months. Owing to its non-specific clinical presentation, diagnosis is frequently delayed, particularly in tuberculosis-endemic regions, where symptoms may be attributed to tuberculous pericarditis. We report the case of a 25-year-old Nepalese male who presented with a six-month history of progressively worsening dyspnea (MMRC grade 1 to 4) and an ECOG performance status of 3. Initial evaluation revealed a large p

Tuberculous pericarditis is a rare but important cause of constrictive pericarditis in developed countries, while remaining a significant public health concern in endemic regions. Myocardial involvement is uncommon and often underrecognized. A previously healthy 47-year-old man presented with a five-year history of progressive bilateral lower-limb edema and exertional dyspnea, with marked deterioration over the last five months, including severe fatigue and involuntary weight loss of 8 kg. On admission, imaging revealed biventricular systolic dysfunction, pericardial thickening with calcificat

UNLABELLED: An 83-year-old man was admitted to our hospital with chest and back pain accompanied by a high fever. His electrocardiogram showed ST elevation in the inferior leads, and transthoracic echocardiography revealed a moderate amount of pericardial effusion. Contrast-enhanced computed tomography revealed lung cancer adjacent to the pericardial sac and obstructive pneumonia. Emergent cardiac catheterization was performed; however, no coronary artery occlusion was detected. Based on these findings, a diagnosis of acute pericarditis was made. The patient was treated with colchicine, ibupro

We report the case of a woman in her 40s with type II diabetes mellitus and a known ostium secundum atrial septal defect (23 mm, left-to-right shunt) diagnosed one year back. The patient presented with progressive dyspnea on exertion for the last three months, along with low-grade fever and fatigue. Clinical and echocardiographic evaluation revealed massive pericardial effusion with features of cardiac tamponade. Pericardial drainage via a pigtail catheter yielded 650 mL of exudative fluid. Pericardial effusion was negative for fungal culture, malignant cytology, and tubercular workup. Adenosi

INTRODUCTION: Ascites, the abnormal accumulation of fluid within the peritoneal cavity, is commonly associated with decompensated liver cirrhosis, primary or secondary malignancy, right-sided heart failure, and peritoneal tuberculosis. The underlying pathophysiology is usually related to one of portal hypertension, hypoalbuminaemia, or peritoneal inflammation. When these common causes are excluded, autoimmune aetiologies such as systemic lupus erythematosus (SLE) should be considered, albeit being rare. SLE is a chronic, multisystem autoimmune disease with diverse manifestations, and serositis

PURPOSE: To evaluate the diagnostic utility and inherent limitations of postmortem computed tomography (PMCT) in homicide investigations by correlating imaging findings with conventional autopsy results across diverse injury mechanisms.
MATERIALS AND METHODS: This retrospective study analyzed 44 homicide victims who underwent pre-autopsy PMCT and subsequent forensic autopsy. Imaging findings were systematically compared with autopsy (the reference standard). Lesions were categorized as PMCT-detectable or PMCT-occult, stratified by injury mechanism: sharp force (n = 13), blunt force (n = 11),

A 7-year, 6-month-old, female neutered, Pug dog presented with lethargy, hyporexia, polydipsia, abdominal distension, and dyspnea. Point-of-care ultrasonography identified tricavitary effusions and cardiac tamponade. Pericardiocentesis yielded 83 mL of sanguineous scintillating fluid. Clinical examination, bloodwork, echocardiography, electrocardiography, thoracic and abdominal computed tomography scans, and pericardial fluid analysis including culture and sensitivity did not identify any cardiovascular, endocrine, neoplastic, or infectious causes of the pericardial effusion. Pericardial fluid
