Effusion Unveiled: Impending Cardiac Tamponade as the Initial Manifestation of Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized primarily by inflammatory polyarthritis, although extra-articular manifestations, including pericardial disease, are well recognized. While pericardial effusions are relatively common, large symptomatic effusions causing cardiac tamponade as the initial manifestation of RA are exceedingly rare. We report a 75-year-old woman with hypertension and dyslipidemia who presented with progressive dyspnea and fatigue one month after an episode of right-hand monoarthritis with an initially unrevealing rheumatologic evaluation. Kartagener syndrome with pulmonary mycetoma and recurrent thromboembolism: A case report.

RATIONALE: Kartagener syndrome, a variant of primary ciliary dyskinesia, is defined by chronic sinusitis, bronchiectasis, and situs inversus. While recurrent infections are seen, a combination of thromboembolic disease, pulmonary mycetoma, cardiac dysfunction, and serological evidence of autoimmune disorders is very uncommon. PATIENT CONCERNS: A 27-year-old man with Kartagener syndrome and situs inversus totalis complaining of worsening dyspnea, chest pain, fever, hemoptysis, and ankle swelling. His past medical history included episodes of deep vein thrombosis, recurrent pulmonary embolism,

Standardized Spark-Pattern Cut-Mode Electrosurgical Bailout Workflow for Difficult Left Bundle Branch Area Pacing Lead Advancement: Procedural and Follow-Up Outcomes From a 15-Patient Case Series.

BACKGROUND: Electrosurgical facilitation can aid difficult left bundle branch area pacing lead advancement, but standardized delivery patterns, reassessment logic, and follow-up lead performance remain insufficiently defined. METHODS: We retrospectively analyzed 15 consecutive patients undergoing standardized spark-pattern cut-mode electrosurgical bailout after conventional mechanical left bundle branch area pacing lead advancement failed. The generator was operated in unipolar cut-mode at 20 W; spark-pattern delivery denoted brief sliding contact to the exposed cathode segment followed by im

Risk Stratification for Pericardial Effusion After Leadless Pacemaker Implantation - Multicenter Study.

BACKGROUND: Postoperative pericardial effusion (PPE) is a known complication associated with the use of leadless pacemakers (LPMs). The risk score (RS) is a useful tool for risk stratification. This study evaluated the utility of risk stratification using the RS in Japanese patients who received the Micra LPM. METHODS AND RESULTS: We retrospectively enrolled 443 patients who received Micra LPMs that were implanted at 4 institutions between August 2017 and August 2024. Patients were risk stratified using the RS into 3 groups: low risk (score ≤0), intermediate risk (score 1), and high risk (sco

A dual-stream multi-scale cross-attention network for pericardial effusion detection.

Early detection of pericardial effusion (PE) is clinically important because delayed diagnosis may cause severe cardiac complications. We propose a Dual-Stream Multi-Scale Cross-Attention Network (DS-MSCA) for automatic PE detection using simultaneously acquired electrocardiogram (ECG) and phonocardiogram (PCG) signals. Multi-scale Inception-ResNet encoders capture waveform and rhythm variations, while bidirectional cross-attention models cardiac electromechanical coupling. On a clinical dataset of 149 patients, DS-MSCA achieved 93.26% accuracy, 95.28% sensitivity, and a 95.01% F1-score. Atten

Recurrent rupture of a pancreatic mucinous cystic neoplasm with suspected main pancreatic duct communication, presenting with a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion.

Spontaneous rupture of pancreatic mucinous cystic neoplasm (MCN) is rare, particularly outside pregnancy, and mediastinal extension has not been reported. We report a 34-year-old woman with a pancreatic tail MCN demonstrating continuity with the main pancreatic duct on computed tomography (CT), suggesting ductal communication. The neoplasm ruptured dorsally, and cyst fluid extended through the aortic hiatus, resulting in a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion. Pleural fluid amylase was 1541 IU/L. Endoscopic ultrasonography showed a cyst-in-cy

A Case of a Cardiac Re-Rupture Caused by the Removal of Pericardial Patches Late after a Sutureless Repair for a Cardiac Rupture.

INTRODUCTION: Surgical treatment is the only reliable therapeutic option for a left ventricular free wall rupture (LVFWR). There are 2 major surgical techniques, namely, sutureless repair and sutured repair; a ventricular pseudoaneurysm is a late postoperative complication. We herein report a case of a re-ruptured left ventricular pseudoaneurysm after a sutureless bovine pericardial patch repair. CASE PRESENTATION: The patient was a 79-year-old woman who underwent a sutureless pericardial patch repair for a blow-out-type left ventricular rupture following an anterior myocardial infarction 10

Cardiac Chemodectoma With Recurrent Pericardial Effusion and Cardiac Tamponade in an Alaskan Malamute: A Case Report.

Chemodectomas (aortic body tumors) are rare, slow-growing neoplasms arising from chemoreceptor tissue at the heart base in dogs, most frequently reported in brachycephalic breeds; to the authors' knowledge, their occurrence in nonbrachycephalic large breeds, such as the Alaskan Malamute, is uncommonly documented. An 8-year-old intact male Alaskan Malamute (body weight 44.8 kg) was referred with a history of lethargy, inappetence, exercise intolerance, and ascites. Echocardiography confirmed extensive pericardial effusion with cardiac tamponade and a multinodular, irregularly shaped heart-base

Myxoedema coma in a patient with alcohol dependence: a diagnostic pitfall.

Myxoedema coma is a rare but life-threatening manifestation of hypothyroidism with high associated mortality. We present the case of a man in his sixties with known hypothyroidism and alcohol dependence who collapsed at home with profound hypothermia, refractory hypoglycaemia, hypoxia and reduced consciousness. Initial findings were easily attributable to excess alcohol; however, clinical features including periorbital oedema and bedside echocardiogram revealed moderate pericardial effusion, which prompted further investigation. Thyroid function tests confirmed severe hypothyroidism with eleva

Surgical outcomes of constrictive pericarditis and exploratory associations with inflammatory features: a 17-year single-center experience.

BACKGROUND: Constrictive pericarditis (CP) remains a challenging disease with evolving etiological patterns and increasing emphasis on phenotype-based management. Pericardiectomy is the definitive treatment for CP. However, contemporary data on the etiological spectrum and the clinical relevance of inflammatory phenotypes in patients undergoing pericardiectomy are limited. We report the etiological spectrum, clinical phenotypes, and short- and long-term outcomes of pericardiectomy at a single center and explore whether a preoperative inflammatory phenotype is associated with adverse perioperat

From oesophageal cancer to constrictive pericarditis: oesophagopericardial fistula complicated by Streptococcus equi infection-a case report.

BACKGROUND: Oesophagopericardial fistula is a rare but life-threatening condition that may lead to purulent pericarditis, which in turn carries a high risk of progression to constrictive pericardial disease. CASE SUMMARY: A 50-year-old horse handler with advanced oesophageal carcinoma presented with acute chest pain, fever, and shock. Echocardiography showed a large pericardial effusion and pericardiocentesis yielded purulent fluid. Streptococcus equi was isolated. Computed tomography confirmed an oesophagopericardial fistula. The patient underwent percutaneous drainage, broad-spectrum antimi

Outcomes of a Simplified Two-Catheter Approach for Catheter Ablation of SVT in Pediatric Patients.

BACKGROUND: Catheter ablations are an established treatment for supraventricular tachycardia (SVT) in pediatric patients with high success and low complication rates typically. This study evaluated the procedural outcomes, safety, and recurrence rates of pediatric patients undergoing SVT ablation using a two-catheter approach. METHODS: A retrospective review was performed of 150 pediatric patients who underwent electrophysiology studies and catheter ablation for SVT. All patients were younger than 21 years, with arrhythmia mechanisms including atrioventricular nodal reentrant tachycardia (AVN

Penetrating cardiac injury with no initial radiological findings and negative E-FAST: The critical role of clinical judgment.

We report the case of a man in his forties admitted to hospital after being stabbed in the precordial region. The initial chest X-ray and FAST examination revealed nothing abnormal. Despite apparent haemodynamic stability, the patient developed progressive dyspnoea, diaphoresis and signs of circulatory compromise. Due to clinical deterioration, an emergency exploratory thoracotomy was performed. Intraoperative findings revealed haemothorax, haemopericardium, and a perforation of the right atrium associated with pericardial rupture. The cardiac injury was successfully repaired, and the patient

Giant Intrapulmonary Solitary Fibrous Tumour With a Possible Liver Metastasis: A Rare Case.

Intrapulmonary solitary fibrous tumours (SFTs) are exceptionally rare mesenchymal neoplasms, particularly when presenting as giant thoracic masses. We report a case of a 57-year-old woman with progressive chest pain and dyspnoea who initially presented with pleural effusion and hypoglycaemia and received empirical anti-tuberculosis therapy without improvement. Contrast-enhanced CT revealed a left lower lobe mass enlarging to 11 × 16.5 × 15.2 cm on repeat imaging, with rightward cardiac displacement, pericardial effusion and a hepatic nodule suspicious for metastasis to the liver. When Tuberculosis Left the Heart in a Cage: Multimodality Imaging Revealing Constrictive Pericarditis in a Young Adult.

Tuberculous pericarditis (TBP) is a potentially severe manifestation of extrapulmonary tuberculosis that may lead to pericardial effusion and constrictive physiology and, in some patients, progress to fixed constrictive pericarditis. Diagnosis can be particularly challenging in young immunocompetent patients without overt pulmonary involvement. We report the case of a 20-year-old immunocompetent man presenting with a one-month history of weight loss, profuse night sweats, nocturnal fever, and typical pericarditic chest pain. Clinical examination revealed fever and signs of systemic venous cong
