Bacterial aortic endarteritis with sepsis as a rare complication of aortic dissection in a cat.

A 14-year-old domestic shorthair cat was examined because of respiratory distress. Thoracic radiography showed a prominent aortic arch and no other abnormalities. Echocardiography documented a mobile flap within the ascending aorta creating 2 lumens, consistent with aortic dissection (AD), mild pericardial effusion with tamponade, left ventricular concentric hypertrophy, and normal left atrial size. Systemic hypertension (200 mmHg) was the presumed cause of AD, prompting administration of telmisartan, furosemide, and amlodipine. The cat clinically improved, became normotensive, and was dischar

Pericardial Diseases in Lagos: A 5-Year Institutional Experience of Surgically Managed Cases.

UNLABELLED: Pericardial diseases are common in clinical practice and may occur in isolation or form part of systemic disease. The clinical presentation of patients with pericardial disease is dependent on multiple factors, including the rate and volume of pericardial fluid accumulation, chronicity of the inflammatory process, and the degree of pericardial fibrosis and calcification. Cardiac tamponade and pericardial constriction are life-threatening sequelae of pericardial diseases, occurring in the acute and chronic stages, respectively. The management of pericardial diseases is multidiscipli

Psychosis and Pericardial Effusion in Neuropsychiatric Systemic Lupus Erythematosus: Diagnostic Challenges in a Language-Discordant Encounter.

Neuropsychiatric systemic lupus erythematosus (NPSLE) is a diagnostically complex manifestation of systemic lupus erythematosus (SLE) that may initially present with significant psychiatric symptoms, contributing to delayed recognition and treatment. We describe a 36-year-old non-English-speaking female patient with a recent history of intermittent medical and psychiatric encounters over 1 year, during which progressive psychosis was attributed to a primary psychiatric illness. Her course was characterized by fragmented care across institutions, limited collateral history, language discordance

Successful repair of a penetrating right atrial gunshot injury via right posterolateral thoracotomy in a resource-limited hospital: a case report.

BACKGROUND: Penetrating cardiac injuries are uncommon but frequently fatal, particularly after gunshot wounds. Diagnosis may be challenging when pleural bleeding predominates, and pericardial fluid is minimal because blood may decompress through a pericardial defect into the thoracic cavity. CASE PRESENTATION: A 25-year-old man sustained a penetrating chest gunshot wound during wartime and presented to a resource-limited hospital in hemorrhagic shock with a massive right hemothorax. Right tube thoracostomy immediately drained approximately 900 mL of blood, but he remained hemodynamically unst

Traumatic High Cervical Spine Injury Presenting as Cardiac Arrest: A Case Report.

Traumatic high cervical spinal cord injury is a potentially devastating condition that may cause profound cardiovascular and respiratory instability through disruption of descending sympathetic pathways. In severe cases, autonomic dysfunction may contribute to marked hypotension, bradycardia, and cardiac arrest. We report the case of a 47-year-old man who sustained blunt trauma of incompletely characterized mechanism in a workplace accident and was found in cardiac arrest by a prehospital emergency medical team. Advanced life support was initiated, and return of spontaneous circulation was ach

Rupture of Left Ventricular Aneurysm in Infant: Case Report.

Rupture of ventricular aneurysm is an unusual pathology in children. This case describes a preterm infant who had been in the hospital since birth due to necrotizing enterocolitis, which was complicated by narrowing of the ascending colon that required surgery. Throughout the entire period, the infant exhibited signs of infection, first caused by Staphylococcus haemolyticus and subsequently by Staphylococcus epidermidis. Several echocardiographic examinations did not reveal any significant changes; however, the final echocardiography revealed a structure connected to the left ventricular cavit

Tricuspid Stenosis Due to Cardiac Sarcoma.

Primary cardiac sarcomas are rare, aggressive malignant tumors with high recurrence rates and poor prognosis. Their initial presentation is often vague, contributing to diagnostic delay and rapid clinical deterioration. We report the case of a 32-year-old man who presented with gastrointestinal symptoms and constitutional features, initially suggesting a hepatobiliary or infectious cause. As his condition progressed, signs of right-sided heart failure emerged, prompting transthoracic echocardiography, which revealed a large right atrial mass causing tricuspid inflow obstruction and a significa

Clinical manifestations and cardiac abnormalities in systemic Amyloid A Amyloidosis.

BACKGROUND: Cardiac involvement in systemic amyloid A (AA) amyloidosis is traditionally considered rare. Respectively, contemporary data describing cardiac characteristics of patients with systemic AA amyloidosis are scarce. METHODS: All patients with a diagnosis of systemic amyloidosis were identified from electronic medical records of a tertiary medical center. Patients with non-AA amyloidosis, concomitant multiple myeloma, lack of histological confirmation, or absence of cardiac evaluation were excluded. Probable cardiac involvement was diagnosed based on predefined criteria. RESULTS: The

Effusion Unveiled: Impending Cardiac Tamponade as the Initial Manifestation of Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized primarily by inflammatory polyarthritis, although extra-articular manifestations, including pericardial disease, are well recognized. While pericardial effusions are relatively common, large symptomatic effusions causing cardiac tamponade as the initial manifestation of RA are exceedingly rare. We report a 75-year-old woman with hypertension and dyslipidemia who presented with progressive dyspnea and fatigue one month after an episode of right-hand monoarthritis with an initially unrevealing rheumatologic evaluation. Standardized Spark-Pattern Cut-Mode Electrosurgical Bailout Workflow for Difficult Left Bundle Branch Area Pacing Lead Advancement: Procedural and Follow-Up Outcomes From a 15-Patient Case Series.

BACKGROUND: Electrosurgical facilitation can aid difficult left bundle branch area pacing lead advancement, but standardized delivery patterns, reassessment logic, and follow-up lead performance remain insufficiently defined. METHODS: We retrospectively analyzed 15 consecutive patients undergoing standardized spark-pattern cut-mode electrosurgical bailout after conventional mechanical left bundle branch area pacing lead advancement failed. The generator was operated in unipolar cut-mode at 20 W; spark-pattern delivery denoted brief sliding contact to the exposed cathode segment followed by im

Risk Stratification for Pericardial Effusion After Leadless Pacemaker Implantation - Multicenter Study.

BACKGROUND: Postoperative pericardial effusion (PPE) is a known complication associated with the use of leadless pacemakers (LPMs). The risk score (RS) is a useful tool for risk stratification. This study evaluated the utility of risk stratification using the RS in Japanese patients who received the Micra LPM. METHODS AND RESULTS: We retrospectively enrolled 443 patients who received Micra LPMs that were implanted at 4 institutions between August 2017 and August 2024. Patients were risk stratified using the RS into 3 groups: low risk (score ≤0), intermediate risk (score 1), and high risk (sco

A dual-stream multi-scale cross-attention network for pericardial effusion detection.

Early detection of pericardial effusion (PE) is clinically important because delayed diagnosis may cause severe cardiac complications. We propose a Dual-Stream Multi-Scale Cross-Attention Network (DS-MSCA) for automatic PE detection using simultaneously acquired electrocardiogram (ECG) and phonocardiogram (PCG) signals. Multi-scale Inception-ResNet encoders capture waveform and rhythm variations, while bidirectional cross-attention models cardiac electromechanical coupling. On a clinical dataset of 149 patients, DS-MSCA achieved 93.26% accuracy, 95.28% sensitivity, and a 95.01% F1-score. Atten

Recurrent rupture of a pancreatic mucinous cystic neoplasm with suspected main pancreatic duct communication, presenting with a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion.

Spontaneous rupture of pancreatic mucinous cystic neoplasm (MCN) is rare, particularly outside pregnancy, and mediastinal extension has not been reported. We report a 34-year-old woman with a pancreatic tail MCN demonstrating continuity with the main pancreatic duct on computed tomography (CT), suggesting ductal communication. The neoplasm ruptured dorsally, and cyst fluid extended through the aortic hiatus, resulting in a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion. Pleural fluid amylase was 1541 IU/L. Endoscopic ultrasonography showed a cyst-in-cy

A Case of a Cardiac Re-Rupture Caused by the Removal of Pericardial Patches Late after a Sutureless Repair for a Cardiac Rupture.

INTRODUCTION: Surgical treatment is the only reliable therapeutic option for a left ventricular free wall rupture (LVFWR). There are 2 major surgical techniques, namely, sutureless repair and sutured repair; a ventricular pseudoaneurysm is a late postoperative complication. We herein report a case of a re-ruptured left ventricular pseudoaneurysm after a sutureless bovine pericardial patch repair. CASE PRESENTATION: The patient was a 79-year-old woman who underwent a sutureless pericardial patch repair for a blow-out-type left ventricular rupture following an anterior myocardial infarction 10

Cardiac Chemodectoma With Recurrent Pericardial Effusion and Cardiac Tamponade in an Alaskan Malamute: A Case Report.

Chemodectomas (aortic body tumors) are rare, slow-growing neoplasms arising from chemoreceptor tissue at the heart base in dogs, most frequently reported in brachycephalic breeds; to the authors' knowledge, their occurrence in nonbrachycephalic large breeds, such as the Alaskan Malamute, is uncommonly documented. An 8-year-old intact male Alaskan Malamute (body weight 44.8 kg) was referred with a history of lethargy, inappetence, exercise intolerance, and ascites. Echocardiography confirmed extensive pericardial effusion with cardiac tamponade and a multinodular, irregularly shaped heart-base
