UNLABELLED: Primary malignant pericardial mesothelioma (PMPM) is a rare malignancy, accounting for 0.7% of all mesotheliomas and has non-specific clinical features such as dyspnea and chest pain. A 66-year-old man visited a referring physician with a chief complaint of worsening dyspnea on exertion. Imaging revealed irregular pericardial thickening and hemorrhagic effusion. Despite initial empirical management with antibiotics and steroids under the diagnosis of constrictive pericarditis (CP) of unknown etiology, his condition deteriorated rapidly; he was transferred to our hospital for advanc

IMPORTANCE: Low-dose oral minoxidil (LDOM) is increasingly used off-label to treat pediatric hair disorders; however, standardized guidance for patient selection, dosing, safety, and monitoring in patients younger than 12 years is lacking.
OBJECTIVE: To establish expert consensus recommendations for LDOM use in pediatric patients (age <12 years) with hair disorders.
EVIDENCE REVIEW: Survey development was informed by a review of the literature on LDOM pharmacology, safety, and clinical use, with emphasis on pediatric populations. A modified Delphi process was conducted from June 30, 2025, to

BACKGROUND: Current techniques for instrumented posterolateral lumbar fusion include the use of titanium rods and pedicle screws to provide mechanical fixation, while autograft and/or allograft bone is used to promote osseous union across the fusion site. Although bone graft substitutes may be used for this purpose, they can resorb before a solid fusion is established, leading to nonunion. Tetracalcium phosphate-phosphoserine (TTCP-PS) is a novel, bioresorbable, osteoconductive, wet-field mineral-organic bone adhesive and bone graft substitute that has been previously shown to promote bone hea

BACKGROUND: Pericarditis is typically diagnosed on the basis of clinical criteria (chest pain, pericardial rub, ECG changes and pericardial effusion). However, the diagnosis can be challenging as these findings are often transient. We evaluated the clinical utility of cardiac magnetic resonance (CMR) in the diagnosis of pericarditis.
METHODS: Consecutive patients referred for the evaluation of pericarditis by CMR between 2010 and 2024 to a tertiary hospital in Sydney, Australia, were retrospectively screened. Patients were stratified according to the presence (CMR-pos) or absence (CMR-neg) of

Cefdinir is a third-generation cephalosporin with a wide variety of indications and a strong safety profile. A 24-year-old man began taking cefdinir for an uncomplicated urinary tract infection. Two days later, he began experiencing pleuritic, sharp chest pain and diarrhea and was shown to have mildly elevated troponins. Due to the pleuritic nature of his chest pain improving when leaning forward, as well as elevated troponins and a new small pericardial effusion on echocardiogram, he was diagnosed with myopericarditis thought to be secondary to cefdinir. Cefdinir was subsequently held with di

PURPOSE: Information from electronic health records (EHRs) may be incorporated into computable phenotype algorithms in efforts to overcome inaccuracies of algorithms based on administrative claims data alone. However, such efforts can be resource-intensive and unsuccessful. Assessing the feasibility of computable phenotyping for a health outcome of interest (HOI) before proceeding is therefore recommended.
METHODS: We developed a systematic fitness-for-purpose (FFP) assessment process to implement concepts outlined in a previously described general framework for computable phenotyping incorpo

A 66-year-old man with acute myeloid leukemia with myelodysplasia-related changes (AML-MR) was admitted in April 2024 after presenting with increased peripheral blood blasts. At the initial visit, he had atrial fibrillation but no pericardial effusion. Although treatment with venetoclax and azacitidine reduced peripheral blood blasts, a concurrent cardiac tamponade was observed in May of the same year. Pericardial drainage was performed, and cytology and flow cytometry of the pericardial fluid confirmed a diagnosis of pericardial infiltration due to AML. Pericardial effusion resolved with repe

Phagocytic activity of mesothelial cells is a rare phenomenon requiring careful differentiation from histiocytic hemophagocytosis to ensure appropriate patient management, as these phenomena could exhibit distinct clinical implications. In this study, we report a case of phagocytic mesothelial cells found in pericardial effusion that developed 18 months after allogeneic peripheral blood stem cell transplantation for chronic active Epstein-Barr virus (EBV) infection. The patient presented with fever and dyspnea, alongside mild chronic graft-versus-host disease, affecting the shoulders and skin.

This is a case report of a 62-year-old male patient who developed coronary perforation during a percutaneous coronary intervention. He presented with a non-ST-elevation MI and had a history of hypertension and hypercholesterolaemia, with an elevated BMI. Coronary angiography revealed severe calcific coronary disease within an angulated proximal left anterior descending artery. Following a heart team meeting, the consensus was for complex percutaneous coronary intervention to the left anterior descending artery, with or without left main stem involvement. Following initial balloon dilatation wi

Diffuse large B-cell lymphoma (DLBCL) is an aggressive malignancy and the most common subtype of non-Hodgkin lymphoma, typically presenting with lymphadenopathy. Pericardial involvement is extremely rare. We report the case of a patient who presented with cough and shortness of breath and was admitted for acute hypoxic respiratory failure. During hospitalization, the patient developed atrial fibrillation with rapid ventricular response (RVR). Transthoracic echocardiography revealed a pericardial effusion, which increased in size on repeat imaging 1 week later. The patient subsequently underwen

A 66-year-old patient with an anterior ST-elevation myocardial infarction (STEMI), treated with primary angioplasty and stenting of the proximal left anterior descending artery, developed an extensive left ventricular thrombosis that was initially sessile but later, after the introduction of anticoagulant therapy, became pedunculated and mobile. Given the high cardioembolic risk, the case was extensively discussed by the Heart Team, which recommended surgical thrombectomy through a transmitral approach. The procedure, performed with fibroscopic assistance, allowed complete removal of the throm

BACKGROUND Intramural esophageal hematoma (IEH) is a rare disease characterized by clinical manifestations such as chest pain, hematemesis, dysphagia, or odynophagia. Patients who have undergone coronary stent implantation often have several high-risk factors that can predispose them to IEH. Therefore, special attention should be paid to the etiology of chest pain after percutaneous coronary intervention (PCI). CASE REPORT We report the case of a 67-year-old man who presented with acute chest pain after PCI. He had stents implanted in the left circumflex artery and the right coronary artery in

Posterior pericardiotomy is a well-established approach for preventing pericardial effusion following cardiac surgery. The procedure can be easily performed after a full sternotomy; it decreases the number of manifestations of retained blood syndrome. However, there is a lack of data on the feasibility and efficacy of posterior pericardiotomy in ministernotomy. In this video tutorial, we demonstrate our technique for performing a posterior pericardiotomy in ascending aortic replacement performed through J-shaped ministernotomy.

Primary mediastinal B-cell lymphoma is a rare subtype of non-Hodgkin lymphoma. Typical symptoms include cough, chest pain, and dyspnea; however, cardiac tamponade as the primary manifestation is exceedingly rare. We hereby present a case of a 83-year-old woman, who presented to our emergency department with dyspnea, cough and hypotension. On admission, echocardiography (transthoracic and transesophageal), computed tomography and cardiac magnetic resonance demonstrated a large 70 x 38 mm pericardial mass, with pericardial effusion and signs of cardiac tamponade. BACKGROUND AND PURPOSE: Stereotactic arrhythmia radiotherapy (STAR) is a promising treatment for therapy-refractory ventricular tachycardia (VT), and is associated with a substantial reduction in VT burden with only modest short-term toxicity. However, detailed organ-of-interest dosimetry and long-term safety data remain scarce.
MATERIALS AND METHODS: To study the association between dosimetry and safety after STAR, we conducted a two-centre, two-platform study on dose to (extra)cardiac organs-of-interest. Patients with therapy-refractory VT were treated with a single fraction of 20-25 Gy and

A 67-year-old female underwent an en-bloc double lung transplantation for idiopathic pulmonary fibrosis. The post-transplant course was complicated by refractory septic shock. Despite empiric antimicrobial coverage with vancomycin, meropenem, micafungin, and posaconazole, she continued to require high-dose vasopressors and remained unable to wean from veno-venous extracorporeal membrane oxygenation (ECMO). Blood cultures were positive for Candida Kefyr (teleomorph: Kluyveromyces marxianus), the same organism that had been isolated from donor bronchoalveolar lavage cultures 5 days prior to tran

BACKGROUND: Sinus node modification for drug-refractory inappropriate sinus tachycardia (IST) is limited by the risk of phrenic nerve injury, particularly when the target substrate is located along the high right atrium and superior vena cava region. Various mechanical and electrical strategies have been proposed to mitigate this risk; however, effective insulation of the phrenic nerve during epicardial ablation remains challenging.
CASE SUMMARY: We report a patient with highly symptomatic, drug-refractory IST who underwent sinus node modification after failure of conventional endocardial abl
