Purulent pericarditis is an uncommon but potentially fatal condition that may rapidly progress to cardiac tamponade. Contiguous spread from adjacent thoracic disease is a recognized mechanism; however, imaging documentation of a presumed mediastinal-pericardial pathway in association with necrotic mediastinal lymphadenopathy remains uncommon. We describe the case of a 72-year-old man with recently diagnosed low-grade prostate adenocarcinoma and a concomitant radiologically suspected primary right upper lobe lung malignancy. Baseline FDG PET/CT performed on 19 March 2026 demonstrated intensely

Objective: We aim to characterize patients undergoing pericardiocentesis for clinically significant pericardial effusion and explore differences in inflammation-based biomarkers according to malignancy status and one-year outcome. Methods: We retrospectively evaluated 113 consecutive patients who underwent pericardiocentesis between January 2021 and January 2026. Clinical, laboratory, pericardial fluid, cytopathological, recurrence, and mortality data were analyzed. NLR, PLR, SII, SIRI, and PIV were assessed as secondary exploratory biomarkers. Results: Malignancy-associated pericardial effusi

A 29-year-old male with a history of systemic lupus erythematosus (SLE) with class IV/V lupus nephritis, recurrent pericardial effusions, hypertension, and type 2 diabetes mellitus presented for worsening chest tightness and shortness of breath. Echocardiography revealed a large, free-flowing pericardial effusion circumferential to the heart, up to 2.9 cm at its widest diameter, with mild atrial and right ventricular collapse. Due to concern for tamponade, interventional cardiology was consulted, and a pericardiocentesis was performed, yielding 820 mL of serosanguinous fluid, and a pericardial

BACKGROUND: Cardiac tamponade is a life-threatening emergency characterized by reduced cardiac output and circulatory failure. It most commonly occurs during cardiac surgery, interventional cardiology procedures, pericardiocentesis, or central venous catheterization, as well as in the setting of acute myocardial infarction or aortic dissection. To the best of our knowledge, cardiac tamponade during video-assisted thoracoscopic surgery (VATS) lobectomy has not been reported and may has an insidious presentation, making diagnosis challenging in the absence of obvious pericardial injury.

INTRODUCTION: Feline infectious peritonitis (FIP) is a fatal disease caused by feline coronavirus, most commonly associated with effusions in body cavities, while pericardial involvement remains rarely described.
MATERIAL AND METHODS: A three-year clinical study (2021-2024) included 256 cats with PCR-confirmed effusive FIP, of which 37 presented fluid accumulation exclusively in the pericardial sac. Diagnosis was based on real-time PCR analysis of pericardial fluid, echocardiographic examination and measurement of serum N-terminal pro-B-type natriuretic peptide (NT-proBNP) concentrations.

BACKGROUND: Transthoracic echocardiography (TTE) is essential for making clinical decisions regarding cardiovascular disorders. Cardiology trainees frequently conduct and interpret TTE examinations; however, discrepancies between their interpretations and those of attending cardiologists can impact patient management. The objective of this study was to assess the concordance between cardiology trainees and attending cardiologists in the interpretation of TTE findings.
METHODS: This study included 374 inpatients and outpatients referred to for TTE examinations. TTE studies were independently p

BACKGROUND: Penetrating cardiac injuries from firearms are highly lethal, with the majority of victims dying at the scene from exsanguination. A minority of patients who reach hospital alive harbor a retained intracardiac or pericardial projectile, posing a clinical dilemma: while extraction has historically been favored to prevent migration, embolization, infective endocarditis, conduction abnormalities, and lead toxicity, contemporary evidence supports selective conservative management in hemodynamically stable patients without active hemorrhage, structural cardiac injury, or arrhythmia. INTRODUCTION: Rheumatic heart disease (RHD) remains a leading cause of cardiovascular morbidity and mortality among pregnant women in resource-limited settings. Pregnancy induces significant hemodynamic changes that can decompensate pre-existing valvular heart disease, leading to life-threatening complications for both mother and fetus.
CASE PRESENTATION: A 27-year-old pregnant woman (gravida 6, para 5) at approximately 34-38 weeks presented with progressive shortness of breath, palpitations. Based on the modified World Health Organization (mWHO) classification, she was categorized as Class I

Spear guns are rarely implicated in homicidal events and are infrequently described in forensic literature, with most published reports focusing on accidental or suicidal injuries and on the surgical management of surviving victims. We report a case of homicide caused by a trident-fishing harpoon, a device designed for underwater use. The fatal injury followed a sudden verbal dispute between two individuals over a parking space on a city street. Autopsy revealed three circular entrance wounds in the precordial region arranged in an oblique pattern, corresponding to the three-pronged configurat

Eosinophilic granulomatosis with polyangiitis (EGPA) is a small- and medium-vessel necrotizing vasculitis characterized by asthma, eosinophilia, and multiorgan involvement. Cardiac manifestations, though uncommon, are more prevalent in antineutrophil cytoplasmic antibody (ANCA)-negative patients and carry significant morbidity. Diagnosis can be challenging due to variable presentations and the lack of a definitive test. We present a 60-year-old woman with asthma and chronic rhinosinusitis who developed progressive dyspnea and was found to have a large pericardial effusion with tamponade physio

Systemic sclerosis (SSc) is a rare autoimmune connective tissue disorder characterized by immune dysregulation, microvascular injury, and progressive fibrosis involving the skin and internal organs. Although Raynaud phenomenon and cutaneous sclerosis are the hallmark manifestations, atypical presentations may significantly delay diagnosis. Peripheral neuropathy is an uncommon neurological manifestation and rarely precedes the diagnosis of SSc by several years. We report the case of a 62-year-old female who presented with a 20-year history of progressive distal paresthesia involving both upper

BACKGROUND: Intracardiac echocardiography (ICE) is increasingly used for left atrial appendage (LAA) assessment during atrial fibrillation (AF) procedures. We describe a novel, thrombus-mimicking artifact on ICE, termed the "Floating Lotus" sign.
METHODS: In this retrospective multicenter study, we analyzed data from 172 patients undergoing ICE-guided AF ablation and/or LAA occlusion. The incidence, imaging features, and clinical correlates of the "Floating Lotus" sign were assessed.
RESULTS: The "Floating Lotus" sign was observed in 48 patients (27.9%). It manifested as a mobile, echogenic

Cardiac tamponade is a life-threatening complication characterized by the accumulation of fluid or blood within the pericardial sac surrounding the heart, leading to increased intra-pericardial pressure and impaired ventricular filling. This is a known complication of cardiothoracic procedures, including coronary artery bypass grafting (CABG) and mitral valve repairs, and can rapidly progress to decreased cardiac output, hypotension, and eventual cardiac collapse. We report the case of a 62-year-old male with an unremarkable past medical history who was admitted to the hospital due to recurren

BACKGROUND: Pulsed field ablation (PFA) is Food and Drug Administration (FDA)-cleared for pulmonary vein isolation in atrial fibrillation. Using a keyword-based natural language processing pipeline applied to free-text narratives in the FDA MAUDE database, we aimed to characterize and compare adverse event and device malfunction profiles across the four cleared PFA systems.
METHODS: We retrieved 6930 MAUDE reports for FDA product code QZI (pulsed field ablation catheter) spanning December 13, 2023 through April 30, 2026, covering all four FDA-cleared PFA systems. Reports were deduplicated and

Primary cardiac tumors are rare, and cardiac lymphomas are particularly uncommon. Their presentation can mimic benign tumors, but perioperative management is highly challenging due to risks of tumor-related obstruction or invasion of surrounding cardiac structures. We present a 76-year-old woman with rapidly progressing dyspnea, found to have large biatrial masses, significant pericardial and pleural effusions, and evidence of tamponade. The patient underwent successful surgical resection of bilateral atrial tumors after careful anesthetic preparation and intraoperative monitoring. Histology r
