Myxoedema coma in a patient with alcohol dependence: a diagnostic pitfall.

Myxoedema coma is a rare but life-threatening manifestation of hypothyroidism with high associated mortality. We present the case of a man in his sixties with known hypothyroidism and alcohol dependence who collapsed at home with profound hypothermia, refractory hypoglycaemia, hypoxia and reduced consciousness. Initial findings were easily attributable to excess alcohol; however, clinical features including periorbital oedema and bedside echocardiogram revealed moderate pericardial effusion, which prompted further investigation. Thyroid function tests confirmed severe hypothyroidism with eleva

Surgical outcomes of constrictive pericarditis and exploratory associations with inflammatory features: a 17-year single-center experience.

BACKGROUND: Constrictive pericarditis (CP) remains a challenging disease with evolving etiological patterns and increasing emphasis on phenotype-based management. Pericardiectomy is the definitive treatment for CP. However, contemporary data on the etiological spectrum and the clinical relevance of inflammatory phenotypes in patients undergoing pericardiectomy are limited. We report the etiological spectrum, clinical phenotypes, and short- and long-term outcomes of pericardiectomy at a single center and explore whether a preoperative inflammatory phenotype is associated with adverse perioperat

From oesophageal cancer to constrictive pericarditis: oesophagopericardial fistula complicated by Streptococcus equi infection-a case report.

BACKGROUND: Oesophagopericardial fistula is a rare but life-threatening condition that may lead to purulent pericarditis, which in turn carries a high risk of progression to constrictive pericardial disease. CASE SUMMARY: A 50-year-old horse handler with advanced oesophageal carcinoma presented with acute chest pain, fever, and shock. Echocardiography showed a large pericardial effusion and pericardiocentesis yielded purulent fluid. Streptococcus equi was isolated. Computed tomography confirmed an oesophagopericardial fistula. The patient underwent percutaneous drainage, broad-spectrum antimi

Outcomes of a Simplified Two-Catheter Approach for Catheter Ablation of SVT in Pediatric Patients.

BACKGROUND: Catheter ablations are an established treatment for supraventricular tachycardia (SVT) in pediatric patients with high success and low complication rates typically. This study evaluated the procedural outcomes, safety, and recurrence rates of pediatric patients undergoing SVT ablation using a two-catheter approach. METHODS: A retrospective review was performed of 150 pediatric patients who underwent electrophysiology studies and catheter ablation for SVT. All patients were younger than 21 years, with arrhythmia mechanisms including atrioventricular nodal reentrant tachycardia (AVN

Penetrating cardiac injury with no initial radiological findings and negative E-FAST: The critical role of clinical judgment.

We report the case of a man in his forties admitted to hospital after being stabbed in the precordial region. The initial chest X-ray and FAST examination revealed nothing abnormal. Despite apparent haemodynamic stability, the patient developed progressive dyspnoea, diaphoresis and signs of circulatory compromise. Due to clinical deterioration, an emergency exploratory thoracotomy was performed. Intraoperative findings revealed haemothorax, haemopericardium, and a perforation of the right atrium associated with pericardial rupture. The cardiac injury was successfully repaired, and the patient

Giant Intrapulmonary Solitary Fibrous Tumour With a Possible Liver Metastasis: A Rare Case.

Intrapulmonary solitary fibrous tumours (SFTs) are exceptionally rare mesenchymal neoplasms, particularly when presenting as giant thoracic masses. We report a case of a 57-year-old woman with progressive chest pain and dyspnoea who initially presented with pleural effusion and hypoglycaemia and received empirical anti-tuberculosis therapy without improvement. Contrast-enhanced CT revealed a left lower lobe mass enlarging to 11 × 16.5 × 15.2 cm on repeat imaging, with rightward cardiac displacement, pericardial effusion and a hepatic nodule suspicious for metastasis to the liver. Core needle b

When Tuberculosis Left the Heart in a Cage: Multimodality Imaging Revealing Constrictive Pericarditis in a Young Adult.

Tuberculous pericarditis (TBP) is a potentially severe manifestation of extrapulmonary tuberculosis that may lead to pericardial effusion and constrictive physiology and, in some patients, progress to fixed constrictive pericarditis. Diagnosis can be particularly challenging in young immunocompetent patients without overt pulmonary involvement. We report the case of a 20-year-old immunocompetent man presenting with a one-month history of weight loss, profuse night sweats, nocturnal fever, and typical pericarditic chest pain. Clinical examination revealed fever and signs of systemic venous cong

Human intravenous immunoglobulins for recurrent pericarditis: a multicentre cohort study.

AIMS: Based on limited data, intravenous immunoglobulins (IVIG) have been proposed as possible last therapeutic option for recurrent pericarditis (RP). The aim of this multicentre registry was to evaluate the efficacy and safety of IVIG in these patients after failure of other medical therapies. METHODS AND RESULTS: This multicentre cohort study enrolled consecutive patients with RP treated with IVIG. The primary outcome was the pericarditis recurrence rate after treatment with IVIG. A total of 43 patients (median age 41.7 ± 14.4 years, 65.1% women) were included. The median duration of disea

Diagnostic pearls and potential pitfalls of free-breathing motion-corrected late gadolinium enhancement: a head-to-head comparative study.

OBJECTIVE: To compare the free-breathing motion-corrected (MOCO) sequence with the segmented breath-hold (BH) sequence in subjects undergoing late gadolinium enhancement (LGE) scans of cardiac MRI. MATERIALS AND METHODS: Retrospectively enrolled 71 consecutive subjects underwent MOCO-LGE and BH-LGE sequences in random order. Semi-quantitative image quality (IQ) scores, image contrast-to-noise ratios (CNRs), myocardial scar size, and thickness of pericardial effusion were evaluated. RESULTS: Compared to BH-LGE images, MOCO-LGE images showed improved CNR of the scar to the remote myocardium (1

Use of Point-of-Care Ultrasound During Cardiac Arrest in the Emergency Department: A Multicentre Retrospective Study.

BACKGROUND: Point-of-care ultrasound (POCUS) is widely used in emergency departments (ED) and has potential value during cardiac arrest for identifying reversible causes (e.g., pericardial effusion, pneumothorax and pulmonary embolism) and guiding resuscitation. However, its role remains inconsistently applied. This study describes the use of POCUS during ED cardiac arrests, including frequency, association with rhythm type and impact on interventions. METHODS: A retrospective review was conducted across three Sydney EDs from November 2020 to October 2023 (3 years). Patients were included if

[ESC guidelines on myocarditis and pericarditis : First combined European guidelines for myocarditis and pericarditis].

The new guidelines for myocarditis and pericarditis from the European Society of Cardiology (ESC) are the first guidelines for myocarditis and for the first time they also combine the two entities. A key aspect is the introduction of the term inflammatory myopericardial syndrome (IMPS), an umbrella term that covers the spectrum of both diseases and enables uniform diagnostics and treatment but in particular draws attention to the fact that there are overlapping causes, diagnostic approaches and treatment options. Other major aspects relate to noninvasive diagnostics using multimodal imaging. T

Subclinical Hypothyroidism as a Risk for Coronary Artery Bypass Grafting.

BACKGROUND: The relationship between hypothyroidism and cardiovascular disease is well established. However, data on subclinical hypothyroidism (SH) and its impact on major adverse cardiovascular events (MACEs) and postoperative complications following coronary artery bypass grafting (CABG) remain limited. This study was aimed at evaluating whether SH is associated with an increased risk of these outcomes. METHODS: From 2010 to 2019, 863 patients who underwent CABG for cardiovascular diseases at a reference center were included. The primary outcomes included MACE (composite and individual eve

A Rare but Severe Long-Term Complication of an Implantable Chamber.

Long-term implantable ports may cause severe complications even years after placement. This case highlights the importance of verifying catheter integrity and tip position before reusing a dormant central venous access device, as delayed mechanical erosion can lead to pericardial effusion and life-threatening cardiac tamponade.

Percutaneous Optical Coherence Tomography Reveals Coronary Artery Rupture and Pseudo-aneurysm.

BACKGROUND: A patient with coronary rupture and subsequent pseudoaneurysm was successfully recorded by angiography and optical coherence tomography (OCT). CASE SUMMARY: A 61-year-old man presented with angina. Coronary angiography revealed diffuse stenosis of the left anterior descending artery. After stent implantation, postdilatation angiography showed contrast extravasation, and OCT confirmed vessel rupture. However, the patient remained hemodynamically stable, and serial bedside echocardiography detected no pericardial effusion. At more than 1-year follow-up, angiography revealed aneurysm

Cardiac tamponade presenting as obstructive shock in the first trimester pregnancy - a case report.

BACKGROUND: Cardiac tamponade is a critical emergency that needs to be recognized and treated without delay. There is a significant knowledge gap regarding cardiac tamponade in early pregnancy, with no reported cases or guidelines on its evaluation and management. The impact of hemodynamic instability on maternal and fetal outcomes remains unclear. CASE PRESENTATION: We present a case of 24-year-old gravida 2 para 1 living 1 (G2P1L1) antenatal mother with no prior co-morbidities who developed sudden cardiovascular collapse secondary to cardiac tamponade caused by massive pericardial effusion.
