Pediatric ECPR as Bridge to Surgery: Successful Management of Refractory Cardiac Arrest in High-Risk ICD Lead Extraction.

BACKGROUND: Refractory cardiac arrest remains a major challenge in children, with poor survival despite advances in cardiopulmonary resuscitation (CPR). Extracorporeal cardiopulmonary resuscitation (ECPR), defined as rapid deployment of extracorporeal membrane oxygenation (ECMO) during ongoing CPR, provides circulatory and respiratory support and can serve as a bridge to definitive interventions. While increasingly reported in adults, pediatric use remains limited due to anatomical and physiological challenges, with evidence largely restricted to case reports and small series. Sex Differences in Early Complications After Cardiac Implantable Electronic Device Implantation: Myth or Reality?

BACKGROUND: Early complications after cardiac implantable electronic device (CIED) implantation remain clinically relevant and have been widely examined in large registry studies. Advanced age, comorbidities, device complexity, and antithrombotic therapy are recognized predictors of early complications. This study planned to examine the effect of sex on early (≤ 30 days) device-related complications and to determine independent predictors of early major adverse cardiac events (MACE). METHODS: This retrospective, single-center study enrolled 1807 sequential patients (729 women, 1078 men) under

Fatal arrhythmia associated with myocardial contusion caused by a horse kick: A forensic autopsy case.

BACKGROUND: Horse-related injuries often involve high-energy blunt trauma that can be fatal. In cases of chest trauma associated with horse kicks, death is typically attributed to structural cardiac injuries such as cardiac rupture or tamponade. Fatal arrhythmia associated with myocardial contusion following a horse kick has rarely been reported in the forensic literature. CASE PRESENTATION: A 48-year-old man was kicked in the left lower chest by the right hind limb of a Thoroughbred horse while handling the animal. The patient collapsed immediately after the impact. Emergency medical personn

Feasibility, procedural efficiency, and early imaging outcomes of concomitant pulsed field ablation and left atrial appendage closure: a prospective single-centre study.

AIMS: Concomitant pulsed field ablation (PFA) for atrial fibrillation (AF) with left atrial appendage closure (LAAC) offers a single-procedure approach for arrhythmia control and thromboembolic risk reduction. This study assessed the workflow, safety, and feasibility of combined PFA and LAAC in routine practice. METHODS AND RESULTS: We prospectively analysed patients undergoing zero-fluoroscopy PFA, with low fluoroscopy for LAAC. Pre-procedural planning used CT imaging and AI-based models for device selection and landing-zone assessment. A single transseptal puncture facilitated intracardiac

Early Outcomes With a Next-Generation Dual-Seal Left Atrial Appendage Occluder: Results From the VERITAS Study.

BACKGROUND: Percutaneous left atrial appendage occlusion (LAAO) is an alternative stroke prevention strategy in atrial fibrillation patients deemed unsuitable to tolerate long-term oral anticoagulation. Although current LAAO devices have been shown to provide stroke protection, they have been associated with peridevice leak and pericardial effusion. The VERITAS study evaluates the safety and effectiveness of Abbott's next-generation dual-seal LAAO device. OBJECTIVES: The purpose of this study was to assess the VERITAS primary endpoints and key clinical outcomes through 45 days. METHODS: In t

Primary synovial sarcoma of the pericardium diagnosed after multiple biopsies following recurrent episodes of haemorrhagic pericardial effusion following infection: a case report.

BACKGROUND: Primary malignant pericardial tumours are extremely rare, and synovial sarcoma arising from the pericardium is an exceptionally uncommon entity with poor survival. Diagnosis is often delayed because initial pericardial biopsies can be non-diagnostic, and distinguishing between inflammatory disease and malignancy may be challenging. CASE SUMMARY: An 11-year-old boy developed recurrent fever and chest pain 15 days after Coronavirus disease 2019 infection. Echocardiography demonstrated pericardial effusion, and pericarditis was suspected. Despite drainage of haemorrhagic effusion, pe

High-Output Heart Failure Associated With Arteriovenous Fistula.

BACKGROUND: Arteriovenous fistulas (AVFs) are the preferred vascular access for hemodialysis, but they can cause high-output heart failure (HOHF) by imposing significant hemodynamic stress. Despite its clinical significance, AVF-related HOHF remains under-recognized in hemodialysis patients. CASE SUMMARY: A 73-year-old woman on hemodialysis presented with progressive dyspnea. She had undergone upper-arm AVF surgery 1 year earlier. Transthoracic echocardiography revealed pericardial effusion and left ventricular enlargement. Right heart catheterization with simultaneous echocardiography showed

Modified transseptal technique without needle puncture: insights from a 4100-procedure registry.

OBJECTIVE: Fluoroscopy-guided transseptal puncture (TSP) remains essential for left atrial access; however, it carries rare but serious risks, including cardiac tamponade. We report outcomes from one of the largest single-center registries assessing a modified needle-free technique for TSP. METHODS: A total of 4100 procedures requiring TSP were analyzed. RESULTS: Subjects with patent foramen ovale (PFO) (n = 322, 7.8%) were excluded from the analysis. The modified needle-free rotational probing of the fossa ovalis with the assembly technique was applied in 1335 patients under fluoroscopic gu

Outcomes of Intracardiac Versus Transesophageal Echocardiography to Guide Left Atrial Appendage Occlusion: A Real-World Propensity-Matched Study.

Intracardiac echocardiography (ICE) and transesophageal echocardiography (TEE) are both used to guide left atrial appendage occlusion (LAAO). Earlier comparative analyzes predate widespread adoption of 3-dimensional ICE, and ICE was often reserved for patients with comorbidities precluding TEE. With continued technological advancements and increasing adoption of ICE, contemporary real-world comparisons are warranted. We conducted a large propensity-matched analysis using the TriNetX database to compare early outcomes between ICE- and TEE-guided LAAO from 2015 through 2025. Adults undergoing LA

Delayed Constrictive Pericarditis Following Presumed Esophagopericardial Communication After Esophageal Stenting.

Esophagopericardial fistula is a rare but life-threatening complication of esophageal stenting and malignancy. Although acute purulent pericarditis is a well-recognized consequence, delayed presentation as constrictive pericarditis years after the initial insult is exceptionally uncommon. A 74-year-old woman presented with a two-month history of progressive dyspnea. She had undergone esophageal stent implantation for esophageal cancer two years earlier, followed by chemoradiotherapy. Transthoracic echocardiography demonstrated preserved biventricular systolic function and constrictive physiolo

Signals of pericardial and myocardial events associated with antiangiogenic drugs: a disproportionality analysis on FAERS®.

PURPOSE: Antiangiogenic drugs (AADs) are an heterogenous group of molecules widely used in the treatment of cancers. Based on their mechanism of action, AADs are associated with an increased reporting of cardiovascular adverse events (AEs). However, pericarditis, pericardial effusion and myocarditis were not documented for most of AADs. The aim of this study was to determine whether AADs may be associated with pericarditis, pericardial effusion and myocarditis in a real-world setting. METHODS: We performed an extensive descriptive and disproportionality analysis of pericarditis, pericardial ef

Cytokine-driven recurrent pericarditis in Down syndrome: Role of pericardial fluid analysis and cardiac magnetic resonance imaging.

Recurrent pericarditis is uncommon in children with Down syndrome (DS) and may present diagnostic challenges. We report the case of a child with DS who developed massive pericardial effusion with minimal systemic inflammatory markers. Pericardial fluid analysis revealed markedly elevated interleukin-6 levels, suggesting localized cytokine-driven inflammation. Cardiac magnetic resonance (CMR) imaging revealed limited subepicardial enhancement with preserved ventricular function, findings consistent with pericarditis and secondary myocardial involvement rather than primary myocarditis. Combinati

Electrical Alternans as an Early Clue to a Rapidly Evolving Large Pericardial Effusion: The Role of Point-of-Care Ultrasound.

Electrical alternans is an uncommon electrocardiographic finding classically associated with significant pericardial effusion and may raise suspicion for cardiac tamponade in the appropriate clinical context. We report the case of a 65-year-old man who presented with 3 days of progressive dyspnea 10 days after hospitalization for acute pericarditis, during which transthoracic echocardiography showed no pericardial effusion. On presentation, he was tachycardic at 110 beats/min and had jugular venous distension, while electrocardiography demonstrated prominent electrical alternans. Although he w

Pericardio-peritoneal communication following emergency subxiphoid pericardiocentesis in acute complex percutaneous coronary intervention: a case report.

BACKGROUND: Emergency pericardiocentesis represents a life-saving intervention in patients with acute cardiac tamponade. During complex percutaneous coronary intervention (PCI), abrupt haemodynamic deterioration secondary to pericardial tamponade may necessitate immediate subxiphoid drainage. Although the procedure is generally safe when performed by experienced operators, catheter malposition can result in ineffective pericardial decompression and may be associated with unexpected and potentially serious complications. CASE SUMMARY: An 83-year-old patient with multiple cardiovascular risk fa

Iatrogenic Large Bowel Perforation Following Image-Guided Pericardiocentesis: A Case Report.

BACKGROUND Image-guided pericardiocentesis is a diagnostic and potentially life-saving intervention, yet even under guidance, rare complications can occur. Among them, bowel perforation with subsequent fecal pericarditis is an exceptionally uncommon and often under-recognized occurrence. CASE REPORT We describe the case of a 72-year-old man with multiple comorbidities, including acute colonic pseudo-obstruction (Ogilvie syndrome), Parkinson disease, and chronic kidney disease, who underwent urgent fluoroscopy-guided pericardiocentesis for a large pericardial effusion. After the procedure, his

Cardiac Paroxysmal Orthodeoxia Syndrome: A comprehensive process of this challenging disease starting from the clinical suspicion till the therapeutic procedure.

BACKGROUND: The platypnea-orthodeoxia syndrome is a rare disorder characterized by both dyspnea and arterial desaturation in the upright position that is relieved by returning to a recumbent position. In addition to PFO (as discussed in our case) and other interatrial defects, this syndrome has also been described with intrapulmonary shunting and with disorders such as pericardial effusion, constrictive pericarditis, emphysema, amiodarone pulmonary toxicity, pneumonectomy, and cirrhosis. METHODS: We describe a case of platypnea-orthodeoxia syndrome (POS) due to patent foramen ovale (PFO) occu

Retrograde Cerebral Perfusion in Type A Aortic Dissection Repair Is Associated With Higher Rates of Postoperative Complications.

Background The comparative short- and long-term outcomes of retrograde versus antegrade cerebral perfusion during circulatory arrest for acute type A aortic dissection repair remain debated. Methods In this single-center retrospective study, postoperative outcomes were compared between retrograde and antegrade cerebral perfusion. Retrograde cerebral perfusion was employed in older patients (median age = 72 vs. 63 years; p = 0.030), with comparable body mass index (29 vs. 28 kg/m²; p = 0.649). Results Relative to antegrade cerebral perfusion, retrograde cerebral perfusion was associated with in

Composite Hemangioendothelioma of the Right Atrium Presenting With Cardiac Tamponade in an Adolescent.

Primary cardiac vascular tumors are rare in children. We report a 14-year-old girl with hemorrhagic pericardial effusion, cardiac tamponade, and a large right atrial mass. Echocardiogram identified the lesion and tamponade physiology. Pericardial drainage and stabilization preceded computed tomography (CT) and cardiac magnetic resonance imaging. The mass was excised under cardiopulmonary bypass, and the atrial defect was reconstructed with an autologous pericardial patch. Histopathology and immunohistochemistry favored composite hemangioendothelioma of low malignant potential. Postoperative FD
