The Influence of Pre-operative Mitral Regurgitation in Post Operative Recovery of Ventricular Function in Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery (ALCAPA).

We attempted to study the influence of pre-operative mitral regurgitation on the recovery of ventricular function in paediatric patients with repaired anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA). Eighty-eight patients under 18 years who underwent ALCAPA repair from 2003 to 2022 were included. Baseline clinical and echocardiographic data, including MR and left ventricular ejection fraction (LVEF), were recorded pre-operatively and at multiple post-operative follow-ups (discharge, 1 month, 6 months, 1 year, and annually thereafter). MitraNav: Design and Modeling of a Robotic System for Transseptal Mitral Valve Replacement with Dock Catheter Task-Space Control.

PURPOSE: Transcatheter mitral valve replacement (TMVR) is a promising therapy for reducing mitral regurgitation in select patients. Direct tip control of multi-segment catheters can simplify manipulation and enable precise, stable positioning. This paper introduces MitraNav, the first robotic system developed for dock-based TMVR, focusing on system design and task-space control of the dock catheter. METHODS: Built on a previously validated guide-sheath robot, MitraNav is upgraded to support concentric multi-catheter manipulation via a unified actuation mechanism. It suits both dock and balloo

Opportunistic CT Fatty Muscle Fraction for Outcome Prediction in Patients Undergoing Transcatheter Mitral Valve Edge-to-Edge Repair.

BACKGROUND: Frailty is a risk factor for adverse outcomes in patients undergoing mitral valve transcatheter edge-to-edge repair (M-TEER) and is interrelated with sarcopenia. This study aimed to investigate the prognostic value of fatty muscle fraction (FMF), measured from routine preinterventional computed tomography, as an imaging biomarker for frailty in patients undergoing M-TEER. METHODS: In patients undergoing M-TEER between 2010 and 2022 at the Heart Center Bonn who received preprocedural contrast-enhanced computed tomography, skeletal muscle quantity and quality at the L3 vertebral lev

Simulation of Transcatheter Therapies for Atrioventricular Valve Regurgitation in an Open-Source Finite Element Simulation Framework.

PURPOSE: Transcatheter edge-to-edge repair (TEER) and annuloplasty devices are increasingly used to treat mitral valve regurgitation, yet their mechanical effects and interactions remain poorly understood. This study aimed to establish an open-source finite element modeling (FEM) framework for simulating patient-specific mitral valve repairs and to evaluate how TEER, annuloplasty, and combined strategies influence leaflet coaptation and valve mechanics. A central objective was to demonstrate how such simulations may support surgical planning by identifying optimal interventions. METHODS: A pa

Influence of hematocrit-dependent blood viscosity on hemodynamics of mitral paravalvular leak.

PURPOSE: Paravalvular leak (PVL) is a pathological condition that may arise following prosthetic mitral valve implantation and is characterized by regurgitant blood flow through a gap between the prosthetic valve ring and the native cardiac tissue. High-velocity flow jets generated within the leak region may produce elevated shear stresses capable of inducing mechanical damage to erythrocytes and subsequent hemolysis. The purpose of this study is to investigate the influence of blood viscosity, modulated by hematocrit variability, on flow characteristics and hemolytic potential in mitral PVL.

Severe continuous prosthetic mitral regurgitation from chordal rupture after valve-sparing replacement: a case report.

BACKGROUND: After mitral valve replacement with preservation of the subvalvular apparatus, prosthetic dysfunction resulting from retained native structures is extremely rare. When it occurs, it may directly threaten the patient's life. We report an unusual case of severe regurgitation resulting from preserved chordal rupture. The key distinction of this case is the presentation of late, severe continuous prosthetic regurgitation and reversibility. CASE PRESENTATION: A 54-year-old man had undergone double mechanical valve replacement for severe rheumatic mitral stenosis with mild regurgitation

Outcomes of rescue procedures in patients undergoing unsuccessful deployment of transcatheter mitral valve replacement.

BACKGROUND: Perivalvular leak (PVL), valve embolization, and migration of valve in mitral position are critical complications of transcatheter mitral valve replacement (TMVR). To date, rescue strategies, and outcomes following unsuccessful initial TMVR deployment remain poorly documented. METHODS: Between March 2016 and December 2024, 86 patients with high risk of open-heart surgery underwent TMVR with a balloon expandable valve at a single institution. 19.8% (17/86) of patients experienced unsuccessful first TMVR (unsuccessful group) due to valve embolization, migration, significant PVL, or

Explainable machine learning and unsupervised anatomical phenotyping of mitral valve prolapse in patients with severe mitral regurgitation.

BACKGROUND: Mitral annular disjunction (MAD) is increasingly recognized in mitral valve prolapse (MVP), but its anatomical embedding within degenerative MVP remains incompletely defined. We assessed whether explainable supervised learning and unsupervised clustering could identify MAD-associated MVP phenotypes. METHODS: We analyzed a public surgical cohort of 979 patients with MVP and clinically significant mitral regurgitation. Etiology was classified as Barlow disease (BD) or fibroelastic deficiency (FED). Logistic regression, Random Forest and XGBoost were evaluated for MAD prediction usin

Endoscopic secondary chordae transfer and edge-to-edge repair for the treatment of asymmetric anterior leaflet prolapse.

Anterior leaflet prolapse remains a challenging lesion in mitral valve repair, particularly in the presence of asymmetric degenerative disease. We present the case of a 59-year-old patient with severe mitral regurgitation due to asymmetric anterior leaflet prolapse in the setting of Barlow's disease, treated through a totally endoscopic right minithoracotomy. Intraoperative valve analysis demonstrated prolapse of the A2M2 and A3 segments with elongated secondary chordae. Mitral valve repair was performed using secondary chordae transfer, followed by asymmetric edge-to-edge repair at the A2-P2

The durability of different anchoring points and anchoring types in the left ventricle in an ex vivo porcine model.

OBJECTIVE: Surgical mitral valve repair, including anchoring in different parts of the mitral apparatus, is the standard of care for severe mitral regurgitation. Various percutaneous approaches have been developed and are currently under development, but are struggling with durable results. This study investigates the durability of different anatomic anchoring points and anchoring types in the left ventricle. MATERIAL AND METHODS: Anchoring was tested with PTFE sutures or a 5 mm helix at three locations: the head of the papillary muscle, the lateral wall of the ventricle, and the apex of the

Radiographic Comparison of Pulmonary Lobar Vasculature Dimensions in Medically Managed and Drug-Naive Dogs With Left Heart Failure Secondary to Myxomatous Mitral Valve Disease.

Myxomatous mitral valve disease (MMVD) is the most common acquired cause of left heart failure (LHF) in dogs, and radiographic identification of pulmonary venous enlargement is traditionally used to support the diagnosis of LHF. However, the absence of venous enlargement has been anecdotally observed after initiation of therapy. Therefore, we aimed to compare radiographic pulmonary vessel size in LHF patients with ongoing medical management ± receiving acute rescue therapy. Dogs with echocardiographically confirmed MMVD and radiographic cardiogenic pulmonary edema were retrospectively selected

Definition and Prognosis of Mitral Regurgitation in Transthyretin Amyloid Cardiomyopathy: A Multicenter International Cohort Study.

BACKGROUND: In transthyretin amyloid cardiomyopathy, mitral regurgitation (MR) severity may be underestimated by conventional (semi-)quantitative echocardiographic criteria due to restrictive low-flow hemodynamics. We sought to define and validate disease-specific MR risk thresholds and compare their prognostic performance with guideline definitions. METHODS: In a multicenter cohort study (8 centers, 4 countries), 1101 patients with newly diagnosed transthyretin amyloid cardiomyopathy (derivation, n=722; validation, n=379) underwent standardized echocardiography with blinded core laboratory a
