Clinical Significance of Extreme Left Atrial Enlargement in Atrial Functional Mitral Regurgitation.

The left atrium (LA) size varies in patients with atrial functional mitral regurgitation (AFMR) and its clinical impact remains unclear. This study aimed to assess the distribution of LA volume index (LAVI) and the impact of LAVI on clinical outcomes in a large cohort of patients with ≥ moderate AFMR. Patients with AFMR diagnosed by transthoracic echocardiography in 2019 were retrospectively enrolled from the REVEAL-AFMR registry. The endpoint was a composite of cardiovascular death and heart failure hospitalization, compared across the LAVI groups. Of the 877 patients (age 78 ± 9 years, 45% m

Durability of Mitral Valve Repair in Adult Connective Tissue Disease.

BACKGROUND: Connective tissue diseases (CTDs) are known risk factors for the development of mitral valve disease. Despite this risk, mitral valve repair (MVr) in patients with CTD remains underused. This study sought to investigate the survival and durability of MVr in adult patients with CTD. METHODS: All patients with suspected or confirmed Marfan, Loeys-Dietz, and Ehlers-Danlos syndromes who underwent MVr at a single institution between 2012 and 2023 were identified through database review. Survival of this cohort was compared with survival in patients with degenerative mitral regurgitatio

Comparison of high-flow nasal cannula oxygen therapy versus nasal cannula in sedated transoesophageal echocardiography in patients with mitral regurgitation: a prospective, randomized controlled clinical trial.

BACKGROUND: Patients with moderate-to-severe mitral regurgitation (MR) undergoing pre-procedural transesophageal echocardiography (TEE) for transcatheter edge-to-edge repair (TEER) are at high risk for hypoxia during propofol sedation. We evaluated the efficacy and safety of high-flow nasal cannula (HFNC) oxygen therapy compared with conventional nasal cannula in this population. METHODS: This prospective, single-center, randomized controlled trial enrolled 263 patients with moderate-to-severe MR scheduled for pre-procedural TEE under propofol sedation. Patients were randomized to receive eith

Impact of Prosthesis-Patient Mismatch After Surgical Mitral Valve Replacement: Systematic Review and Meta-Analysis.

The long-term clinical impact of mitral prosthesis-patient mismatch (PPM) after mitral valve replacement (MVR) remains incompletely defined. We aimed to systematically assess the prevalence of mitral PPM and its association with long-term mortality, cardiac death, and heart failure-related hospitalizations. We conducted a systematic review and meta-analysis of observational studies reporting Kaplan-Meier time-to-event data in patients with and without mitral PPM. Sixteen studies, including 10,872 patients, were analyzed. The pooled prevalence of any mitral PPM was 43% (95% confidence interval

Characterizing Antithrombotic Regimens After Isolated Mitral Valve Repair: A Retrospective Descriptive Study and Interview of Cardiac Surgeons.

The evidence for optimal antithrombotic therapy after isolated surgical mitral valve repair (MVr) is limited resulting in variable use of antithrombotic regimens in practice. This study aimed to characterize what antithrombotic regimens are being used and elucidate rationale. We conducted a retrospective descriptive study of adult patients after isolated surgical MVr at two cardiac surgery centers to characterize the type of antithrombotics used, timing of initiation, and duration. We then conducted semi-structured interviews of cardiac surgeons across Canada to elucidate post-MVr antithrombot

Impact of Preoperative Left Ventricular Dimensions on Early Outcomes After Mitral Valve Repair for Degenerative Mitral Regurgitation.

BACKGROUND: Current guidelines include left ventricular end-systolic diameter (LVESD) as a threshold criterion for operative intervention in degenerative mitral regurgitation (DMR). We examined a contemporary mitral repair cohort to determine when LVESD served as the operative trigger and whether preoperative left ventricular end-diastolic diameter (LVEDD) was associated with early postoperative outcomes. METHODS: We analyzed DMR patients who underwent mitral valve repair (2021-2024) at a single center. Left ventricular dimensions were obtained from echocardiographic reports at preoperative c

Acute mitral regurgitation following giant coronary artery aneurysm resection: a case report.

BACKGROUND: Giant coronary artery aneurysms carry a high risk of rupture, thrombosis, and distal embolization. When percutaneous coronary intervention is not feasible in symptomatic cases, surgical resection combined with coronary artery bypass grafting is recommended. However, surgical ligation can interrupt flow to downstream branch arteries and precipitate regional myocardial ischemia. CASE PRESENTATION: In a 70-year-old woman with coronary artery disease, coronary angiography showed a 20-mm aneurysm with 90% stenosis of the proximal left anterior descending (LAD) artery, 90% stenosis of t

Mitral regurgitation detection and central/eccentric classification using transformer-based deep learning in multi-view echocardiography.

BACKGROUND: Fully automated diagnostic echocardiography allows for broader screening, earlier diagnosis for patients with mitral regurgitation (MR). This feasibility study developed a deep learning (DL) framework to automatically detect MR in echocardiography videos and classify regurgitation samples into central or eccentric regurgitation. METHODS: We designed a Transformer-based deep learning model for fully automatic Doppler video detection of MR. This framework can automatically detect MR and classify central or eccentric regurgitation. The algorithm was trained, validated, and tested usi

Comparative Outcomes of Transcatheter Edge-to-Edge Repair and Surgical Mitral Valve Repair or Replacement for Degenerative Mitral Regurgitation: A Systematic Review, Meta-Analysis, and Trial Sequential Analysis.

Traditionally, surgical mitral valve repair or replacement (SMVR) has been the mainstay of treatment for mitral regurgitation (MR), providing a long-lasting way to restore valve competence. On the other hand, transcatheter edge-to-edge repair (TEER) has emerged as an option, especially for high surgical risk patients, demonstrating favorable results from short-term to intermediate-term follow-up. This study aims to evaluate and compare the clinical outcomes of TEER versus SMVR in patients with degenerative mitral regurgitation (DMR). This systematic review was conducted in accordance with the

Association of Mitral Annular Area With Effective Regurgitant Orifice Area Independent of Jet Momentum: A Three-Dimensional Echocardiography Cohort Analysis.

BACKGROUND: Mitral regurgitation (MR) severity is traditionally assessed using hemodynamic parameters, but the contribution of structural remodeling, specifically mitral annular area (MAA), remains incompletely understood. We hypothesized that MAA is independently associated with effective regurgitant orifice area (EROA) after accounting for jet momentum. METHODS: This prospective study enrolled 120 patients with chronic MR (60 degenerative, 60 functional) who underwent comprehensive two-dimensional echocardiography (2DE) and three-dimensional transesophageal echocardiography (3D TEE). Feasibility and technical considerations of mitral TEER in adults with surgically repaired congenital heart disease: descriptive cohort of OCEAN-mitral registry.

Advances in congenital heart surgery have led to a growing population of adults with congenital heart disease (ACHD). Mitral regurgitation (MR) often develops after congenital heart surgery, yet the role and safety of transcatheter edge-to-edge repair (TEER) in this setting remain poorly defined. This descriptive cohort study from the OCEAN-Mitral registry aimed to evaluate the feasibility, procedural characteristics, and short-term outcomes of TEER in adults with surgically repaired congenital heart disease (CHD). Among 3,764 consecutive TEER cases registered between 2018 and 2023, 20 patient

Totally endoscopic mitral valve replacement using two-dimensional endoscopic support and percutaneous peripheral cardiopulmonary bypass: a case report.

BACKGROUND: Minimally invasive mitral valve surgery has gained increasing acceptance for selected mitral valve procedures owing to its potential to minimize surgical trauma and enhance postoperative recovery. However, the adoption of advanced technologies such as three-dimensional endoscopic systems may be limited in resource-constrained environments. CASE PRESENTATION: We report the case of a 62-year-old Vietnamese female patient with a history of rheumatic heart disease and mitral regurgitation who underwent totally endoscopic mitral valve replacement using two-dimensional endoscopic visual

DASH Closure: A Dual Access Strategy for Hemodynamic Closure of Mitral Paravalvular Leaks.

BACKGROUND: Paravalvular leaks (PVLs) are common complications following prosthetic valve implantation, significantly impacting patient outcomes due to associated heart failure and hemolysis. While percutaneous closure is a recognized alternative to surgery, challenging anatomies and multiple leaks can be difficult to manage with traditional single-access techniques. METHODS: We present a case of a patient with a mechanical mitral prosthesis and two clinically significant mitral PVLs-one anterior and one posterolateral. The patient presented with symptoms of heart failure and was experiencing

The Heart Under Stress: What Happens When Both Ventricles Fail?

We present the case of a 52-year-old male with severe primary mitral regurgitation, with biventricular dilatation and systolic impairment. Pre-operative exercise stress echocardiography revealed absence of left and right ventricular contractile reserve suggestive of increased surgical risk possibly requiring postoperative mechanical circulatory support. In the immediate postoperative period, the transesophageal echocardiogram revealed severe left ventricular systolic impairment with very low cardiac output, resulting in significant challenges in weaning the patient off cardiopulmonary bypass.

Fulminant recurrent myocarditis complicated by papillary muscle rupture and cardiogenic shock managed with ExtraCorporeal membrane oxygenation and impella support.

We present a case of acute-on-recurrent viral myocarditis complicated by papillary muscle rupture of the mitral valve. This rare clinical scenario, combining refractory cardiogenic shock with acute severe mitral valve regurgitation, illustrates the therapeutic challenges associated with temporary mechanical circulatory support (tMCS). Although mitral valve replacement would theoretically correct the mechanical complication, cardiac surgery in the setting of profound cardiogenic and septic shock carries a high risk of failure. In this context, the combination of veno-arterial extracorporeal mem

Real-time three-dimensional transesophageal echocardiography and three-dimensional printing: detection of multiple perivalvular leaks after mitral valve replacement-a case report and literature review.

BACKGROUND: Accurate assessment of perivalvular leak (PVL) morphology following valve replacement remains a significant clinical challenge. Precise characterization of PVL anatomy is essential, as different leak patterns require distinct management strategies. We report a case in which two complementary imaging modalities—real-time three-dimensional (RT-3D) transesophageal echocardiography (TEE) and three-dimensional (3D) printing technology—were successfully utilized to evaluate PVL morphology and guide the subsequent surgical approach. Mid-term outcomes after totally video guided thoracoscopic mitral valve surgery.

OBJECTIVE: Totally video-guided thoracoscopic surgery (TVTS) has emerged as a minimally invasive alternative to conventional surgery with full sternotomy for mitral valve surgery. Evaluation of clinical outcomes of TVTS mitral valve surgery is essential for guiding patient selection and surgical planning. METHODS: We conducted a retrospective study of 368 consecutive patients who underwent TVTS for mitral valve repair at a tertiary academic center to evaluate the safety and efficacy of this minimally invasive approach. RESULTS: Mitral valve repair was accomplished using the respect technique i
