Risk of Atrial Fibrillation Recurrence After Modified Cox-Maze IV Surgery: Comparing Atrial Functional Mitral Regurgitation and Rheumatic Mitral Regurgitation.

BACKGROUND: The comparative efficacy of the modified Cox-Maze IV procedure (CMP IV) in treating atrial fibrillation (AF) in patients with atrial functional mitral regurgitation (AFMR) and those with rheumatic mitral regurgitation (RMR) remains inadequately characterized. Therefore, this study aimed to evaluate and compare postoperative AF recurrence rates and clinical outcomes between these two etiologically distinct groups. METHODS: A retrospective analysis was conducted on 221 patients (AFMR group: n = 53; RMR group: n = 168) with concomitant long-standing persistent AF who underwent mitral

Associations of Mild or Trace Valvular Abnormalities With Cardiac Remodeling in Older Adults: The ARIC Study.

BACKGROUND: The impact of mild/trace valvular abnormalities on cardiac remodeling is not well understood, despite their association with poor cardiovascular outcomes. METHODS: Using data from 4684 participants in the ARIC (Atherosclerosis Risk in Communities) Study (2011-2013), aged 66 to 90 years, we examined cross-sectional associations of mild/trace valvular abnormalities (aortic stenosis, aortic regurgitation, and mitral regurgitation) with cardiac parameters using multivariable linear regression models. Cardiac parameters included cardiac structure (eg, left ventricular mass index), syst

Early Mitral Valve Surgery and Survival in Asymptomatic Severe Primary Mitral Regurgitation.

IMPORTANCE: The optimal timing of mitral valve (MV) surgery in patients with asymptomatic severe primary mitral regurgitation (MR) remains uncertain because randomized clinical trials are lacking. OBJECTIVE: To evaluate the association of early MV surgery with long-term survival in patients with asymptomatic severe primary MR using a target trial emulation framework. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used a clinical valve registry from 3 tertiary hospitals in South Korea between 2000 and 2022. Patients with asymptomatic severe primary MR were analyzed using a clone-censor-

[Mitral valve disease: diagnosis and management revisited].

Mitral valve disease ranks among the most frequent valvular conditions and carries significant morbidity and mortality if left untreated. This review addresses anatomy, the Carpentier classification, and echocardiographic severity grading, as well as current management of primary and secondary mitral regurgitation (MR) and mitral stenosis. Transcatheter therapies, including edge-to-edge repair and transcatheter mitral valve replacement, receive particular attention and are discussed in the context of four clinical cases from a specialized valve center.

Evaluation of serum pancreatic lipase activity in dogs with myxomatous mitral valve disease.

BACKGROUND: Previous studies have evaluated the association between myxomatous mitral valve disease (MMVD) and pancreatic lipase activity, but medications remain unconsidered. HYPOTHESIS/OBJECTIVES: Elevated serum pancreatic lipase activity is related to cardiac medication in dogs with MMVD. ANIMALS: Eighty-two client-owned dogs without clinical signs of pancreatitis (MMVD, n = 67 [stage B1 = 24; stage B2 = 22; non-medicated stage C = 9; medicated stage C = 12]; controls, n = 15). METHODS: Retrospective cross-sectional study. Dogs were classified into MMVD stage and control groups. Serum pa

Incremental Value of Right Ventricular Free-Wall Strain in Ventricular Functional Mitral Regurgitation: Insights from a Multicenter Study.

BACKGROUND: Ventricular functional mitral regurgitation (VFMR) is a heterogeneous disease with widely variable outcomes, yet current risk stratification relies predominantly on left ventricular (LV) indices. This study aims to evaluate the incremental value of right ventricular free-wall longitudinal strain (RVfwLS) beyond conventional LV parameters and LV longitudinal strain (LVLS) in an unselected VFMR population. METHODS: This multicenter retrospective cohort included consecutive patients with moderately severe or greater VFMR diagnosed between 2010 and 2022. Right ventricular free-wall lo

Impact of Adjustable Pulmonary Artery Band on Outcomes of Anomalous Left Coronary Artery From the Pulmonary Artery (ALCAPA) Repair.

Objective: Anomalous left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital anomaly resulting in myocardial ischemia, global left ventricular (LV) dysfunction, and mitral regurgitation (MR). Surgical placement of a pulmonary artery band (PAB) at ALCAPA repair may assist with recovery of patients with severe LV dysfunction. We assessed the outcomes of patients with ALCAPA and severe dysfunction treated with, and without a PAB. Methods: We performed a retrospective analysis of cases presented to our center between January 2007 and December 2023. Baseline and follow-up data

Transcatheter Management of a Mitral Paravalvular Leak and an Adjacent Large Left Ventricular Pseudoaneurysm Using a Covered Stent-Nitinol Occluder Combination.

Paravalvular leak (PVL) and left ventricular pseudoaneurysm can develop after surgical valve replacement, often as a result of suture dehiscence and annular disruption, potentially causing refractory heart failure. While re-operation poses a high risk, transcatheter PVL management remains equally difficult due to the possibilities of device instability and interference with mechanical prosthesis leaflets. We present an early postoperative mitral PVL that was connected to a large left ventricular pseudoaneurysm, in which standard PVL closure devices would have restricted leaflet movement. A nov

Safety of same-day discharge after mitral transcatheter edge-to-edge repair.

BACKGROUND: The safety of same-day discharge following mitral transcatheter edge-to-edge repair (M-TEER) in a nationwide setting remains unclear. METHODS: Using the administrative databases on Medicare fee-for-service beneficiaries, we identified patients aged 65-99 years undergoing elective M-TEER from 2016 to 2021 and who were discharged home on the same day or the next day after the procedure. We applied a linear probability model to examine the relationship between same-day discharge and patient outcomes, adjusting for patient demographics, underlying comorbidities, and hospital fixed eff

Quantitative Three-Dimensional Transesophageal Echocardiography Analysis of Mitral Valve Geometry for Transcatheter Edge-to-Edge Repair Strategy in Atrial Functional Mitral Regurgitation.

BACKGROUND: Atrial functional mitral regurgitation (AFMR) is a distinct subtype of secondary mitral regurgitation (MR) in patients with atrial fibrillation (AF). Although mitral transcatheter edge-to-edge repair (M-TEER) is increasingly performed under three-dimensional (3D) transesophageal echocardiography (TEE) guidance, detailed characterization of 3D mitral valve geometry relevant to procedural strategy remains limited. OBJECTIVES: To investigate the impact of mitral valve geometrical parameters on preprocedural planning and outcomes of M-TEER in AFMR. METHODS: Patients with AFMR undergo

Sex Differences in Clinical Presentation and Outcome of Degenerative Mitral Regurgitation Quantified: The MIDA-Q Registry.

BACKGROUND: Degenerative mitral regurgitation (DMR) due to valve prolapse predominates in men, but sex-specific differences are poorly understood due to a lack of quantitative and sex-stratified studies. METHODS: The international MIDA-Q (Mitral Regurgitation International Database-Quantitative) registry enrolled 3274 men and 2331 women with greater than or equal to mild DMR prospectively quantified, comprehensively characterized, and long-term follow-up. RESULTS: Women were more symptomatic despite lower effective regurgitant orifice (ERO, 27±18 versus 38±22 mm2, standardized mean differenc

Estimating the Effects of MTEER in U.S. Practice: A Transportability Analysis of the COAPT Trial.

BACKGROUND: Although mitral transcatheter edge-to-edge repair (MTEER) was approved for secondary mitral regurgitation after the COAPT trial, findings of other MTEER trials have been mixed, raising questions about the applicability of the COAPT results to contemporary clinical practice. OBJECTIVES: We used transportability methods to estimate the treatment effects of COAPT trial interventions applied to 2 target populations: 1) trial-eligible patients representative of U.S. clinical practice; and 2) treatment-candidate patients with secondary mitral regurgitation representative of U.S. clinica

Cardiac Imaging in Structural Valve Clinical Trials: Secondary Mitral Regurgitation.

Transcatheter mitral valve interventions including transcatheter edge-to-edge repair and transcatheter mitral valve replacement have emerged as vital therapies for secondary mitral regurgitation (SMR) in patients at high or prohibitive surgical risk. Advanced multimodality imaging is central to patient selection, procedural guidance, and outcome assessment. This review delineates the roles of echocardiography (transthoracic and transesophageal), cardiac computed tomography, and cardiac magnetic resonance in structural valve clinical trials. We examine the imaging criteria used for patient sele

Residual Mitral Regurgitation as a Long-Term Predictor of Mortality After Mitral TEER.

BACKGROUND: The MITRA-PRO registry revealed residual mitral regurgitation (MR) as an important predictor of 1-year and 3-year mortality following mitral transcatheter edge-to-edge repair (M-TEER). OBJECTIVES: The 5-year follow-up of the MITRA-PRO registry (N = 1,491) aimed to assess the impact of residual MR on 5-year mortality. Furthermore, time-dependent predictors of mortality after M-TEER should be assessed. METHODS: Patients with a MitraScore of ≤3 were classified as patients with mild residual MR after M-TEER, while a MitraScore of ≥4 was defined as relevant residual MR. Adjusted HRs w

Impact and outcomes of failure mode after mitral valve repair.

OBJECTIVE: Mitral valve repair (MVr) is the standard of care for degenerative mitral regurgitation (MR). However, repair failure occurs, stemming either from progression of disease (nontechnical failure, NTF) or suboptimal initial repair (technical failure, TF). This study assesses the impact and outcomes of failure mode in patients requiring reoperation after MVr. METHODS: A single-center retrospective analysis was conducted on patients reoperated for failed MVr for degenerative MR from 2014 to 2023. Failures requiring open surgical reintervention were classified as NTF or TF, and outcomes w
