Safety and efficacy of simple system for artificial chordae length adjustment in patients undergoing minimally invasive mitral valve repair.

BACKGROUND: For patients who require mitral valve repair, it is often difficult to determine the appropriate artificial chordae length. The Memo 3D/4D ReChord system, developed for use as a temporary chordal guide system, is comprised of a complete prosthetic ring containing yellow loops that function as a reference for automatically determining neo-chordae height. The present study was conducted to evaluate the safety and efficacy of this system when used with a minimally invasive right mini-thoracotomy approach. METHODS: Sixty-three patients (40 males, 64.4 ± 13.3 years old) using this syste

Intraoperative Discovery of Bioprosthetic Mitral Valve Thrombus on TEE During HeartMate 3 Implantation: Implications for Anesthetic Management and Surgical Planning.

We present the case of a 73-year-old female with chemotherapy-induced cardiomyopathy, severe mitral regurgitation status post bioprosthetic mitral valve (MV) replacement one year prior, and atrial fibrillation, admitted for evaluation for left ventricular assist device (LVAD) implantation. While her preoperative transthoracic echocardiogram (TTE) did not reveal a discrete mass, it showed a mean mitral gradient of 7 mmHg; intraoperative transesophageal echocardiography (TEE) identified a 2 cm × 1.2 cm thrombus on the bioprosthetic MV. Given these findings, the surgical team decided to replace t

Early and Intermediate Outcomes of Cavitron Ultrasonic Surgical Aspiration to Treat Mitral Annular Calcification.

OBJECTIVES: Severe mitral annular calcification (MAC) carries an increased risk of perioperative complications in patients undergoing mitral valve surgery and may be considered a prohibitive surgical risk. Cavitron ultrasonic surgical aspiration (CUSA) has been reported as a method for controlled debridement of severe MAC during mitral valve surgery; however, experience in the literature is limited. METHODS: We assessed 30-day and intermediate-term outcomes in 67 consecutive patients (mean age: 72 [SD 9] years; female sex: 65.7% [44/67]) with severe MAC who underwent mitral valve repair or re

Patch exclusion of caseating annular calcification cavity in the atrioventricular groove during mitral valve replacement.

Caseous liquefaction is a rare variant of mitral annular calcification that can lead to cavity formation in the atrioventricular groove. We report a case of mitral valve replacement in a 76-year-old woman with a large caseous cavity, coronary artery disease, severe mitral valve stenosis and atrial fibrillation. Preoperative computed tomography showed a large caseous cavity (3 x 3.5 cm) in the posterior annulus extending into the atrioventricular groove. After unroofing and evacuation, the cavity was excluded with a pericardial patch. The lower edge of the patch was sutured to the ventricular s

Contemporary Operator Procedural Volumes and Outcomes for TAVR and MTEER in the US.

IMPORTANCE: Recent evidence suggests that hospital-level associations between procedural volume and outcomes for transcatheter aortic valve replacement (TAVR) and mitral transcatheter edge-to-edge repair (MTEER) may be plateauing. Less is known about the operator volumes-outcomes association in the contemporary era. OBJECTIVE: To determine whether an operator-level volume-outcomes association exists for TAVR and MTEER in the contemporary era. DESIGN, SETTING, AND PARTICIPANTS: This cohort study examined data from patients undergoing TAVR or MTEER between January 2020 and December 2023 includ

Right Ventricular Function After Mitral Valve Surgery: Insights From the United Kingdom Mini Mitral Study.

BACKGROUND: Right ventricular (RV) function is frequently reduced after cardiac surgery, with persistent impairment associated with increased mortality. This study aimed to compare RV function after mitral valve repair through right minithoracotomy vs sternotomy. METHODS: In the UK Mini Mitral trial, patients were randomized to mitral valve repair through right minithoracotomy (small lateral pericardial incision) or sternotomy. Prespecified secondary outcomes included assessment of cardiac function by blinded echocardiography preoperatively and at early (12 weeks) and late (52 weeks) time poi

Impact of mitral valve intervention and frailty on outcomes of severe mitral regurgitation due to severe mitral annular calcification.

BACKGROUND: Mitral annular calcification (MAC) can be complicated by mitral regurgitation (MR). However, data on outcomes of severe MR caused by MAC are limited. This study evaluated outcomes of severe MR due to MAC and the prognostic significance of frailty and comorbidities in guiding management. METHODS: In this single-center, retrospective cohort study, we reviewed our echocardiographic database to identify patients with isolated severe MR due to severe MAC. The primary endpoint was all-cause mortality. The Society of Thoracic Surgeons (STS) risk score, Charlson Comorbidity Index (CCI), a

Transcatheter Closure of a Residual Mid-Leaflet Mitral Valve Cleft With a Siege Vascular Plug.

Transcatheter mitral valve interventions have emerged as valuable alternatives to surgical repair in high-risk patients, including transcatheter closure of mitral valve perforations. However, there is little experience with percutaneous mitral valve interventions in pediatric patients or patients with congenital heart disease. We report a case of transcatheter closure of a residual mid-leaflet mitral valve cleft in a 19-year-old patient with moderate mitral regurgitation through two defects in the anterior leaflet of a repaired cleft mitral valve using a retrograde approach and a novel device,

Outcomes of Transcatheter Mitral Valve Implantation for Failed Bioprostheses and Annuloplasty Rings.

OBJECTIVES: Transcatheter mitral valve implantation (TMVI) is increasingly used in failed bioprostheses and annuloplasty rings as a less invasive alternative to redo-surgery in selected patients, with promising results. This retrospective single-centre study aims to provide additional insight into clinical outcomes and its determinants following TMVI. METHODS: Data of all TMVI procedures (n = 88) performed between February 2014 and June 2024 were retrospectively analysed. The primary end-points were 1-year mortality and the Mitral Valve Academic Research Consortium (MVARC) composite end-point

A Comprehensive Echocardiographic Guide for Mitral Valve Repair - A Narrative Review.

In patients with severe mitral regurgitation (MR), mitral valve repair is the preferred and recommended treatment; thus, precise diagnosis of the mechanism, location, and the specific scallop causing MR is decisive and paramount. Occasionally, mitral valve repair is inadequate (residual MR), or result in mitral stenosis, or is associated with systolic anterior motion (SAM) of the mitral valve and left ventricular outflow tract obstruction. Two- and three-dimensional Transesophageal echocardiography (2D and 3D TEE) can precisely diagnose the mechanisms, location and scallops causing MR, identif

2D Contour: A Simplified Approach for Left Atrial Reservoir Strain.

BACKGROUND: Left atrial strain reservoir (LASr) has emerged as a useful parameter in assessing diastolic dysfunction and heart failure with preserved ejection fraction. LASr is typically assessed using speckle-tracking echocardiography (STE), which was developed to measure strain in the left ventricle. Despite its strength demonstrated in several clinical studies, the application of STE to the left atrium presents well-known limitations: thin walls, anatomical discontinuities, far-field location, and inter-vendor variability. The present study aims to evaluate the efficacy of an alternative me

Transcatheter edge-to-edge repair plus guideline-directed medical therapy versus guideline-directed medical therapy alone for symptomatic functional mitral regurgitation: a comprehensive, up-to-date meta-analysis of randomised trials.

BACKGROUND: Transcatheter edge-to-edge repair (TEER) is among the treatments for functional mitral regurgitation (FMR), but its benefits over guideline-directed medical therapy (GDMT) alone are discordant. We conducted a meta-analysis of randomised trials comparing long-term outcomes between these treatment strategies. AIMS: We aimed to compare long-term clinical outcomes between TEER plus GDMT and GDMT alone in symptomatic moderate-to-severe FMR. METHODS: Major electronic databases were searched for randomised trials comparing TEER plus GDMT with GDMT alone in FMR. The primary outcome was d

Simultaneous transcatheter aortic and mitral valve-in-valve replacement: A case report.

RATIONALE: There are fewer reports in China about re-performing combined transcatheter transcatheter aortic and mitral valve-in-valve replacement after bioprosthetic valve failure. Due to the higher difficulty of the combined secondary bivalve procedure and the greater risk of pericardial tamponade and malignant arrhythmias, the professional demands on the surgeon and team are extremely high. It is particularly important to provide quality perioperative care for such patients. PATIENT CONCERNS: This study reports a case of bioprosthetic valve failure after undergoing transcatheter aortic and

Atrial Secondary Mitral Regurgitation Outcomes Following Mitral Transcatheter Edge-to-Edge Repair: Results From the EXPANDed Studies.

BACKGROUND: Atrial secondary mitral regurgitation (aSMR) is a distinct subtype of SMR characterized by normal leaflets, annular dilatation, left atrial (LA) enlargement, and preserved left ventricular function. Treatment pathways for aSMR are undefined, and limited data exist regarding outcomes following mitral transcatheter edge-to-edge repair (MTEER). The analysis aimed to evaluate outcomes in patients with aSMR treated with MTEER from the EXPANDed (Evaluation of the MitraClip X System Post-MArket Real-World CliNical Outcomes Database ) studies.
