Mitral valve prolapse (MVP) affects 1-3% of the general population and is largely considered benign. However, a small subset of MVP patients develops complex ventricular arrhythmias (VAs) and sudden cardiac death, including patients with nonsignificant mitral regurgitation (MR). This subgroup of arrhythmic MVP (aMVP) is variably associated with features such as mitral annular disjunction, bileaflet myxomatous prolapse, and regional myocardial fibrosis, with arrhythmic risk disproportionate to hemodynamic burden. The EHRA defines aMVP as MVP with complex VAs (frequent premature ventricular cont

BACKGROUND: Situs inversus totalis with mirror-image dextrocardia is a rare anatomical condition that creates specific challenges for minimally invasive mitral valve surgery. In totally endoscopic procedures, the reversed cardiac orientation may affect thoracic access, peripheral cannulation, left atrial exposure and accurate identification of mitral valve segments. We report a case of left-sided three-dimensional totally endoscopic mitral valve repair in this setting, with emphasis on operative planning and intraoperative segment orientation.
CASE PRESENTATION: A 39-year-old woman presented

Variants in the filamin C (FLNC) gene are increasingly recognized as causes of cardiac and skeletal muscle disease. Cardiomyopathy-associated FLNC variants span dilated cardiomyopathy (DCM), nondilated left ventricular cardiomyopathy (NDLVC), arrhythmogenic cardiomyopathy (ACM), hypertrophic cardiomyopathy (HCM), and restrictive cardiomyopathy (RCM), but their clinical interpretation depends strongly on variant class and evidence of pathogenicity. FLNC truncating variants (FLNCtv), including nonsense, frameshift, and canonical splice-site variants, usually cause haploinsufficiency through nons

BACKGROUND: Double-orifice mitral valve (DOMV) is a rare congenital anomaly. Repair strategies for mitral valve prolapse in the setting of DOMV remain technically challenging.
CASE SUMMARY: We report a case of DOMV presenting with severe mitral regurgitation (MR) due to chordal rupture and annular dilatation, successfully treated with totally endoscopic mitral valve repair. Regarding the surgical approach, preserving the fibrous bridge, we repaired a DOMV via an endoscopic approach with neochords and an edge-to-edge stitch for leaflet prolapse, and a flexible band for annular dilatation, achi

Background: Relative apical sparing of longitudinal strain is widely used as a diagnostic marker of cardiac amyloidosis. However, similar deformation patterns have been reported in other cardiac diseases, raising concerns regarding disease specificity. A comprehensive multimodality synthesis of the relative apical sparing pattern (RASP) across disease entities is lacking. Methods: A systematic review and meta-analysis were conducted according to PRISMA guidelines. PubMed, Scopus, and EMBASE were searched through December 2025 for studies reporting RASP or regional longitudinal strain values al

A 17-year-old male presented with intermittent palpitations for 6 months, acutely worsening over 24 h. Physical examination revealed Marfanoid features (arachnodactyly, positive thumb/wrist signs, pectus carinatum) and cardiac abnormalities (irregular rhythm, 3/6 systolic murmur with mid-late click). Echocardiography demonstrated: (1) Aortic sinu (AS) dilation (41 mm); (2) Bileaflet mitral valve prolapse (Barlow's Disease) with myxomatous degeneration (anterior leaflet 3.8 mm, posterior 4.2 mm); (3) Mitral annular disjunction (MAD, 13 mm gap). Transesophageal echo revealed left atrial appendag

BACKGROUND: Lipoprotein(a) [Lp(a)] is a genetically determined cardiovascular risk factor increasingly linked to calcific valvular disease. Although its role in aortic stenosis is well established, the association between Lp(a) and mitral valve pathology, particularly mitral annular calcification (MAC), mitral stenosis (MS), and mitral regurgitation (MR), remains uncertain.
OBJECTIVE: To evaluate the association between elevated Lp(a) levels and mitral valve disease in a large, real-world cohort.
METHODS: We conducted a retrospective observational analysis using the TriNetX research network,

INTRODUCTION: Hypermobile Ehlers-Danlos syndrome (hEDS) is a connective tissue disorder that may involve the cardiovascular system. While mitral valve prolapse (MVP) and aortic root dilatation (ARD) have been documented in adults with hEDS, their prevalence in children remains unclear.
METHODS: This study evaluated 28 pediatric patients with hEDS and 28 age- and body surface area-matched healthy controls using two-dimensional echocardiography, Doppler, Tissue Doppler Imaging (TDI), and Speckle Tracking Echocardiography (STE), including global longitudinal strain (GLS) analysis.
RESULTS: Nine

Cavalier King Charles Spaniels (CKCS) are predisposed to a hereditable form of myxomatous mitral valve disease (MMVD) with a high incidence and a high risk of disease progression. Gender is recognized as a risk factor for the development of the disease in the breed. However, patient's morphological traits associated with disease progression have not been identified. The aim of this study was to analyze the association between preclinical MMVD progression and morphometric features in CKCSs. This was a retrospective, time-to-event, longitudinal study. Medical records of 302 CKCSs were reviewed f

BACKGROUND: Individuals with Loeys-Dietz syndrome 3 are at risk for thoracic aortic dissection even if their aortas are not substantially dilated. A pathogenic variant of SMAD3 is a genetic trigger, but the cellular programs that underlie the heightened aortic risk remain poorly understood.
METHODS: We ascertained the cardiovascular profile of 57 individuals in a 4-generation family with a missense variant in SMAD3 (p.Arg287Gln). Aortic medial architectures were quantified from aortas harvested during thoracic aortic replacement surgery, with 2-dimensional and 3-dimensional profiling of the e

Valvular heart diseases (VHDs), including mitral regurgitation, aortic stenosis, mitral stenosis, and mitral valve prolapse represent a significant global health burden particularly among older adults. Digital auscultation platforms can transform traditional cardiac assessments that are primarily subjective and clinician-driven into a data-driven diagnostic tool enabling advanced signal processing, feature extraction and machine learning (ML)-based interpretation. This creates new opportunities for early, objective, and precise disease screening, diagnosis, longitudinal monitoring, and persona

BACKGROUND: Mitral valve prolapse (MVP) reflects a connective tissue abnormality of the mitral leaflets, but systemic manifestations beyond the valve in nonsyndromic MVP remain uncertain. Cardiovascular magnetic resonance (CMR) allows a comprehensive assessment of cardiac chambers, aortic dimensions, and skeletal indexes. We investigated systemic morphofunctional features suggestive of connective tissue alterations in nonsyndromic MVP.
METHODS: In this single-centre, retrospective case-control study, 120 patients with MVP without more than mild mitral or aortic regurgitation, bicuspid aortic

BACKGROUND: Marfan syndrome (MFS) is a heritable connective tissue disorder in which cardiovascular complications drive morbidity. Contemporary sex-based differences in vascular and rhythm manifestations remain incompletely defined.
OBJECTIVES: The purpose of this study was to compare aneurysm burden, dissection prevalence, arrhythmias, valvular phenotypes, and cardiovascular interventions between men and women with confirmed MFS.
METHODS: We performed a retrospective cohort study of adults with confirmed MFS evaluated from 2018 to 2024 at Mayo Clinic. Diagnoses and vascular outcomes were co

Papillary muscle fibrosis (PMF) is being increasingly recognised as a cause of ventricular arrhythmias; therefore, its identification is clinically relevant. PMF has been described in multiple cardiac conditions such as ischaemic cardiomyopathy, mitral valve prolapse and non-ischaemic cardiomyopathies such as hypertrophic cardiomyopathy (HCM), cardiac amyloidosis and cardiac sarcoidosis. Although cardiac magnetic resonance (CMR) imaging is a central tool in its diagnosis, PMF can be overlooked if the papillary muscles are not carefully assessed, especially in cases where the abnormal papillary

BACKGROUND Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited renal disorder, typically presenting with hypertension, hematuria, or progressive renal dysfunction. However, it can have a broad range of initial presentations and, in uncommon cases, may mimic other abdominal disorders. This report presents a case of ADPKD manifesting as an epigastric mass in a patient without a family history, highlighting its importance in the differential diagnosis of upper abdominal masses. CASE REPORT A 62-year-old woman presented to our primary care clinic with a 5-day history

LINS1 gene plays a role in cognition and brain development. Mutations in this gene have been identified as the cause of a neurodevelopmental disorder as well as dysmorphisms, motor symptoms, behavioral problems, seizures, microcephaly, mitral valve prolapse, and Q-T prolongation. We recruited two related families with a total of four children affected by LINS1-associated neurodevelopmental disorder. All four subjects have global developmental delay. Congenital heart disease is prominent: Tetralogy of Fallot in two (one with a discontinuous left pulmonary artery), left-sided cardiac hypoplasia

UNLABELLED: The development of efficient treatment medicines with multitarget modes of action is necessary because breast cancer continues to be one of the leading causes of cancer-related mortality among women worldwide. Although esculetin (6,7-dihydroxycoumarin), a naturally occurring phenolic compound, has demonstrated promising anticancer activity, its molecular mechanism against breast cancer remains poorly understood. Therefore, this study aimed to investigate the therapeutic potential and underlying molecular mechanisms of esculetin against breast cancer using an integrated computationa

BACKGROUND: Connective tissue disorders (CTDs) such as Marfan syndrome, Loeys-Dietz syndrome, and Ehlers-Danlos syndromes are traditionally defined by vascular fragility and aortic complications. Increasing evidence, however, demonstrates that CTDs also predispose to atrial and ventricular arrhythmias, often independent of overt cardiomyopathy or severe valvular disease.
CONTENT: This review integrates emerging data on the arrhythmogenic substrates of CTDs. Four mechanistic bridges are emphasized: (1) diffuse myocardial fibrosis driven by dysregulated TGF-β signaling, (2) structural valve and

BACKGROUND: Among healthy subjects, women have smaller cardiac size than men even after indexing for body surface area (BSA), as well as higher left ventricular ejection fraction (LVEF) and left ventricular (LV) global longitudinal strain (GLS). Despite this, little is known about sex differences in LV remodeling in patients with mitral valve prolapse (MVP) undergoing mitral valve repair (MVr). The aim of this study was to assess pre- and post-MVr sex differences in LV size and function in patients with MVP.
METHODS: Patients with MVP who underwent surgery between 2007 and 2024 at two heart v
