Prevalence of mitral valve regurgitation in 102 apparently healthy Dachshunds.

INTRODUCTION: Mitral regurgitation (MR) is one of the main features of myxomatous mitral valve disease which affects Dachshunds. This study aims to investigate the prevalence of MR and tricuspid regurgitation, valvular thickening, and prolapse based on echocardiography in apparently healthy Dachshunds. ANIMALS, MATERIALS AND METHODS: One-hundred two privately owned, apparently healthy Dachshunds, recruited via the Swedish Dachshund Club,a were included in the study. It is a prospective observational study, where the presence of a cardiac murmur, echocardiographic evidence of MR and tricuspid

Prevalence and Importance of Tricuspid Valve Prolapse in Patients with Primary Mitral Regurgitation.

The presence and impact of tricuspid valve (TV) prolapse in patients with mitral valve (MV) prolapse and severe mitral regurgitation (MR) have not been widely reported. The aim of our study was to describe the prevalence of TV prolapse, and the associated echocardiography features, in a large cohort of patients with MV prolapse undergoing surgery, and to explore its potential clinical impact. A total of 803 patients were included, of which 87 (11%) were diagnosed with TV prolapse, while 716 (89%) patients showed no TV prolapse. Patients with TV prolapse were more often diagnosed with Barlow's

Role of Cardiac Magnetic Resonance (CMR) as a Diagnostic and Risk Stratification Tool for a Patient With Arrhythmic Mitral Valve Complex Phenotype and Tricuspid Annular Disjunction (TAD).

Arrhythmogenic mitral valve disease (AMVD) is a newly established entity that carries a high risk of developing ventricular arrhythmia ranging from premature ventricular contraction to sudden cardiac death. Therefore, there is an urgent need for risk prediction and stratification by using different imaging modalities. A 77-year-old patient presented with a syncopal episode. Physical examination was unremarkable. Resting electrocardiogram (ECG) was normal. Echocardiography showed mild systolic dysfunction and mild aortic stenosis. Seven days of Holter monitor recording demonstrated frequent epi

Extent and Progression of Cardiac Damage in Patients With Primary Mitral Regurgitation Undergoing Surgical Repair.

OBJECTIVES: The progression of extra-mitral valve (MV) cardiac damage in patients with primary mitral regurgitation (MR) following surgical repair has not been described. We aimed to investigate the evolution of extra-MV cardiac damage after MV repair and to assess its prognostic significance. METHODS: Patients with severe primary MR undergoing surgical repair at 5 referral centres were included when echocardiographic follow-up assessment was available. Based on echocardiographic parameters at baseline and at a median follow-up of 7 (interquartile range [IQR] 4-11) months after MV repair, pat

Cardiac surgical outcomes and predictors of redo sternotomy in patients with Marfan syndrome: a retrospective multicentre cohort study.

BACKGROUND: Marfan syndrome (MFS) is associated with progressive aortic and valvular disease requiring cardiac surgery, but contemporary data on repeat cardiac operations in adults remain limited. METHODS: We performed a retrospective multicentre cohort study of 783 adults with MFS evaluated across three Mayo Clinic sites. Cardiac operations, postoperative complications, echocardiographic findings and redo sternotomy events were identified by chart review. Multivariate logistic regression was used to evaluate factors associated with redo sternotomy. RESULTS: Overall, 454 patients (58.0%) und

Valve-Specific Anatomy and Structural Determinants of Susceptibility to Infective Endocarditis: A Review.

Background/Objectives: Infective endocarditis (IE) is a life-threatening cardiovascular infection with in-hospital mortality of 15-30% despite modern therapy. Contemporary IE demonstrates non-random valve involvement: aortic and mitral 35-45%, tricuspid 5-10% (30-50% in intravenous drug users [IVDU]), and pulmonary < 1%. These patterns implicate valve-specific anatomy and hemodynamics as central determinants of susceptibility. This narrative review examines the reported distribution of IE across the aortic, mitral, tricuspid and pulmonary valves and summarises the anatomical, haemodynamic, str

Blood-nulled late gadolinium enhancement increases sensitivity for subtle basal myocardial scar in mitral valve prolapse with annular disjunction.

Mitral valve prolapse (MVP) with mitral annular disjunction (MAD) is an increasingly recognized arrhythmogenic phenotype in which focal myocardial fibrosis of the basal inferolateral wall serves as a substrate for ventricular arrhythmia and a target for cardiac magnetic resonance (CMR) risk stratification, yet this fibrosis is often subtle and may be obscured on conventional bright-blood late gadolinium enhancement (LGE) imaging where subendocardial scar blends with the adjacent bright blood pool. We describe a 67-year-old woman with known MVP and a 3-mm posterior MAD referred for CMR followin

Characteristics and Clinical Features of Mitral Annular Disjunction in the Pediatric Population.

OBJECTIVE: Mitral annular disjunction (MAD), a structural abnormality associated with mitral valve prolapse (MVP), is understudied in pediatric populations. This study aims to evaluate the clinical and laboratory characteristics of pediatric patients with MAD detected by transthoracic echocardiography (TTE). METHODS: This retrospective study included 27 patients (aged 5-17 years, median 13, 81% female) diagnosed with MAD via TTE between 2019 and 2024 at a pediatric cardiology clinic. In addition to the traditional parameters with TTE, the Pickelhaube sign was investigated with tissue Doppler

Impact of Myocardial Fibrosis and Female Sex on Life-Threatening Arrhythmias and Sudden Cardiac Death in Mitral Valve Prolapse.

BACKGROUND: Mitral valve prolapse (MVP) is a common cardiac condition that is generally benign, yet a subset of affected patients develop malignant ventricular arrhythmias and sudden cardiac death. Female sex and myocardial fibrosis detected by late gadolinium enhancement on cardiovascular magnetic resonance imaging have been proposed as risk factors though their independent contributions to arrhythmic risk remain uncertain. METHODS: We conducted a prospective cohort study of 550 consecutive patients with MVP referred for cardiovascular magnetic resonance at a tertiary academic center between

Myocardial late gadolinium enhancement is strongly associated with ventricular arrhythmias in mitral valve prolapse.

The prognosis of patients with mitral valve prolapse (MVP) largely depends on the presence of ventricular arrhythmias. This study aims to identify association between Cardiac MRI (CMR) based findings with ventricular arrhythmias in MVP patients. Consecutive patients (n = 45, mean age 56 ± 17 y) with MVP underwent CMR scans. Premature ventricular beats (PVC) and ventricular tachycardia (VT) were recorded by 24-hour Holter monitoring and exercise stress testing. Myocardial late gadolinium enhancement (M -LGE) was calculated as percentage of left ventricular (LV) mass and was analyzed by tertiles

Phenotypic Heterogeneity in Mitral Valve Prolapse: Insights from a Case Series.

Mitral valve prolapse (MVP) is the most common valvular heart disorder and shows marked phenotypic heterogeneity, ranging from benign forms to malignant variants associated with ventricular arrhythmias (VA), mitral annular disjunction (MAD), myocardial fibrosis, and sudden cardiac death (SCD). We present a case series of three patients with bileaflet MVP and mild mitral regurgitation (MR), followed longitudinally for more than a decade. Clinical assessment, electrocardiography, advanced echocardiography, 24-h Holter monitoring, exercise testing, and cardiac magnetic resonance imaging were used

Immune Mechanisms of Heart Valve Development, Homeostasis, and Disease.

Valvular heart disease is among the most common cardiovascular conditions, usually presenting as aortic stenosis or mitral regurgitation. Clinically significant structural and functional anomalies can arise through congenital malformation, genetic variants, cardiovascular dysfunction, infectious disease sequelae, or chronic inflammation that can progress over time, necessitating surgical replacement or repair. The most common types of valve disease requiring surgical intervention are mitral valve prolapse, often with myxomatous changes due to congenital malformation or infectious disease, and

Infective Endocarditis-Associated Glomerulonephritis Caused by Streptococcus cristatus: A Case Report.

INTRODUCTION: Infective endocarditis (IE) can trigger systemic embolic and immune-mediated complications, including infection-related glomerulonephritis. When the infectious focus is IE, the condition is termed IE-associated glomerulonephritis (IEAGN). A major diagnostic pitfall is that IEAGN may present with purpura, nephritic urinary abnormalities, systemic inflammation, and antineutrophil cytoplasmic antibody (ANCA) positivity, closely mimicking primary small-vessel vasculitis and risking inappropriate immunosuppression if infection is not promptly excluded. CASE PRESENTATION: A 52-year-ol

Infective Endocarditis Presenting With Scalp Allodynia and a Giant Cell Arteritis-Like Vasculitic Phenotype.

BACKGROUND The clinical manifestations of infective endocarditis (IE) are highly varied, and diagnostic delays are common and potentially life-threatening. Due to their overlapping clinical presentations, IE may be misdiagnosed as giant cell arteritis (GCA). Although IE mimics GCA, scalp allodynia associated with IE has not previously been reported. CASE REPORT A 60-year-old man with mitral valve prolapse presented with fever, generalized stiffness, and low back pain refractory to oral antibiotics and low-dose glucocorticoids. He also reported a stinging sensation on his scalp while washing hi

A Case of Myxoma-Like Degeneration in a Patient With Marfan Syndrome Mimicking Vegetation of Infective Endocarditis.

Infective endocarditis (IE) is often considered first in the differential diagnosis when fever, a heart murmur, and valvular vegetation are present. IE caused by Mycoplasma pneumoniae has rarely been reported. We report a case in which IE was initially suspected based on the presence of fever, a heart murmur, and an IE vegetation-like structure on the anterior leaflet of the mitral valve identified by echocardiography following admission for pneumonia caused by M. pneumoniae. However, IE was ultimately excluded, and the vegetation-like structure was determined to represent myxoma-like degenera

Inheritance of Imaging Parameters of Arrhythmic Risk in Mitral Valve Prolapse: A Pedigree Study.

BACKGROUND: A subset of patients with mitral valve prolapse (MVP), a highly heritable condition, experience sudden cardiac arrest (SCA) or sudden cardiac death (SCD). However, the inheritance of phenotypic imaging features of arrhythmic MVP remains unknown. METHODS: We recruited 23 MVP probands, including 9 with SCA/SCD and 14 with frequent/complex ventricular ectopy. Participants underwent interviews, 2-dimensional and speckle-tracking echocardiography, and 48-hour Holter/event monitoring. Each individual was categorized as having a normal mitral valve, MVP, or borderline MVP. We assessed mi
