Time-robust myocardial [68Ga]Ga-FAPI PET biomarker reflects aortic stenosis severity and predicts post-TAVI outcomes.

BACKGROUND: Aortic stenosis (AS) induces myocardial remodeling and fibroblast activation, yet modifiable biomarkers capable of capturing active fibrogenesis and predicting post-transcatheter aortic valve implantation (TAVI) recovery are currently scarce. Fibroblast activation protein (FAP)-targeted PET serves as a noninvasive tool to visualize activated fibroblasts in vivo. We evaluated a time-robust, blood-pool-normalized myocardial [68Ga]Ga-FAPI PET imaging biomarker that reflects AS burden and predicts outcomes after TAVI. METHODS: Nineteen patients with severe symptomatic AS underwent [68

Survival outcomes of elderly patients undergoing transcatheter aortic valve implantation after 3 years of follow-up.

BACKGROUND: Severe aortic stenosis is typically treated with transcatheter aortic valve implantation (TAVI) in elderly patients with contraindications for surgery or elderly patients who have a high risk for surgical aortic valve replacement. Currently, there is a paucity of data on the survival outcomes for patients older than 85 years who underwent TAVI. AIM: To determine survival and predictors of mortality in patients older than 85 years who underwent TAVI. METHODS: A retrospective cohort study was conducted on 64 patients ≥ 85 years of age who underwent TAVI between 2010 and 2023 at the

Sex-specific anatomical challenges in TAVR: A closer look at bicuspid aortic valve stenosis.

BACKGROUND: This paper seeks to examine the findings of the recently published study by Gitto et al., titled "Clinical Outcomes in Women and Men with Raphe-Type Bicuspid Aortic Valve Stenosis Undergoing Transcatheter Aortic Valve Replacement," and to conduct a further analysis of the potential influencing factors. METHODS: A critical analysis was conducted on this published multicenter registry study, with a focus on the finding that female patients have a higher risk of valve migration after transcatheter aortic valve replacement (TAVR). In-depth discussions were carried out from the perspec

Venoarterial Extracorporeal Membrane Oxygenation Rescue for Catastrophic Grade 3 Bone Cement Implantation Syndrome in a Patient With Aortic Valve Stenosis and Moderate Pulmonary Hypertension: A Case Report.

Bone cement implantation syndrome (BCIS) is a rare but potentially fatal complication of cemented arthroplasty, characterized by hypoxia, hypotension, and/or cardiovascular collapse. Although right ventricular failure is considered a key mechanism, its pathophysiology - potentially driven by pulmonary microembolization, complement activation, and the release of histamine - remains poorly defined. Due to the high mortality of severe BCIS, evidence regarding optimal management is limited. An 80-year-old woman with moderate-to-severe aortic valve stenosis and moderate pulmonary hypertension (PH)

Management of severe asymptomatic aortic stenosis: An updated overview.

Aortic stenosis is the most frequent valvular heart disease in industrialized countries, affecting about 10% of individuals aged>75years, with 2-4% presenting severe disease; nearly half of them are asymptomatic. No medical therapy halts aortic stenosis progression, and aortic valve replacement remains the only curative option. Managing asymptomatic patients is challenging because of a persistent risk of sudden death, and the potential for irreversible myocardial damage if intervention is delayed, but also because aortic valve replacement carries procedural risks and long-term prosthesis-relat

A Predictive Analysis of Readmission in Older Adults After Transcatheter Aortic Valve Replacement.

BACKGROUND: Aortic valve stenosis in older adults presents a growing healthcare challenge, with transcatheter aortic valve replacement (TAVR) being widely used in patients aged ≥75 years owing to its minimally invasive approach. Despite its benefits, readmission remains common and is associated with adverse outcomes. However, evidence from rapidly aging countries such as South Korea is limited, highlighting the need for context-specific strategies. OBJECTIVE: To identify the key predictors associated with hospital readmission in older adults after TAVR. METHODS: This retrospective study anal

Sutureless aortic valves: timing, technique, and evidence.

PURPOSE OF REVIEW: This review synthesizes contemporary evidence for sutureless aortic valve replacement (SuAVR). It focuses on indications, imaging-guided sizing, operative technique, outcomes, and complications to clarify where SuAVR meaningfully complements or substitutes conventional surgical aortic valve replacement and transcatheter aortic valve implantation. RECENT FINDINGS: Modern studies show SuAVR consistently shortens cardiopulmonary bypass and cross-clamp times by 25-50%, produces low discharge gradients and larger effective orifice areas, and is particularly advantageous in elder

Caldesmon-1-mediated actin dynamics is essential for osteogenic differentiation of aortic valve interstitial cells.

The precise molecular pathways driving fibrosis and calcification in aortic valve leaflets remain poorly defined. Here, we present the first data indicating a role for caldesmon-1 (CALD1) in calcified aortic valve disease (CAVD) pathogenesis. Analysis of publicly available single-cell RNA sequencing (scRNA-seq) datasets revealed that CALD1 shows prominent upregulation in aortic valve stenosis (AS) cases when compared to normal subjects. Histological examination demonstrated that CALD1 protein expression is elevated in calcified AS valves and co-localises with α-smooth muscle actin (a myofibrob

Next-day discharge after TAVR in an all-comer nonagenarian population.

BACKGROUND: Given the aging of population in western countries, nonagenarians have been increasing. Subsequently, the number of nonagenarians affected by severe aortic stenosis and requiring transcatheter aortic valve replacement (TAVR) is expected to increase. Next-day discharge (NDD) after TAVR has been explored in several studies, demonstrating its feasibility and safety but no data in elder populations have been reported so far. METHODS: The aim of this study was to evaluate the safety of NDD following TAVR in nonagenarians. From November 2011 to May 2024, a total of 104 nonagenarian pati

Young Transcatheter Aortic Valve Implantation: Shifting Risk and Outcomes.

OBJECTIVES: Recent data suggest that 50% of patients ≤65 years of age undergo transcatheter aortic valve implantation (TAVI). It has also been suggested that these patients are typically high risk with poor life expectancy. We sought to evaluate predicted risk, procedural and longitudinal outcomes, and cost in patients ≤65 years who received TAVI in the Premier Healthcare Database. METHODS: All patients aged 65 and younger receiving TAVI (2017-2024) were assessed. A predicted risk of index surgical aortic valve mortality model, incorporating the Fried frailty index, was fitted to simulate the

Assessment of the Safety and Effectiveness of the FLOWer Antiembolic Filter Supporting Transcatheter Aortic Valve Implantation.

BACKGROUND: Embolic events are life-threatening complications of transcatheter aortic valve implantation (TAVI) with clinically relevant stroke rates of 2%-4%. AIMS: This study aims to evaluate the safety and effectiveness of FLOWer, a novel embolic protection device (EPD) used in TAVI procedures. METHODS: Data on the FLOWer device were obtained from its CE-mark clinical trial (the NAUTILUS study, NCT04704258). Safety outcomes were the 30-day rate of major adverse events (MAEs, including all deaths, ischemic strokes, and acute kidney injury), while effectiveness data consisted of the number

Transcatheter versus surgical aortic valve replacement in low-risk patients with severe aortic stenosis: a systematic review and meta-analysis.

BACKGROUND: The optimal revascularization strategy for low-risk patients with severe aortic stenosis (AS), transcatheter (TAVI) versus surgical aortic valve replacement (SAVR), remains a subject of ongoing debate, particularly regarding long-term outcomes. We aimed to synthesize the evidence on the comparative safety and efficacy of TAVI and SAVR in this population. METHODS: We conducted a systematic review and meta-analysis, searching PubMed, Scopus, Web of Science, and Cochrane Library until March 2025. We included randomized controlled trials (RCTs) and comparative observational studies enr

Impact of a Noninterruptive Echocardiogram Report-Embedded Nudge on Rates of Referral to Cardiac Specialty Care and Aortic Valve Replacement in Patients with Severe Aortic Stenosis: A Multicenter Intervention.

BACKGROUND: Undertreatment of severe aortic stenosis (AS) including absence and delays in referral to cardiac specialty care remains common, particularly when patients are followed by noncardiac specialty providers. Electronic health record (EHR)-based nudges improve adherence to treatment but have not been widely evaluated in valvular heart disease. METHODS: In this multicenter study across 3 diverse US health systems, an automated, noninterruptive EHR-embedded nudge was implemented within the echocardiography report of patients with a first diagnosis (index echo) of severe AS. The study pri

Balloon-Expandable Versus Self-Expanding Valves in Low-Flow, Low-Gradient Aortic Stenosis: Does Valve Type Matter?

BACKGROUND: The optimal choice between balloon-expandable (BE) and self-expanding (SE) transcatheter heart valves (THVs) in patients with low-flow low-gradient aortic stenosis (LFLG AS) undergoing transcatheter aortic valve replacement (TAVR) remains unclear. AIMS: This study aims to compare outcomes of BE THVs and SE THVs in patients with LFLG AS. METHODS: This was a retrospective, single-institution cohort study of TAVR procedures between 2012 and 2024. Patients with LFLG AS (mean gradient < 40 mmHg and ejection fraction < 50% or stroke volume index < 35 mL/m2) were stratified by respectiv

Computed Tomography Derived Procedure Simulations for Redo Transcatheter Aortic Valve Replacement.

BACKGROUND: Transcatheter aortic valve (TAV) failure may require redo transcatheter aortic valve replacement (redo-TAVR). The multiple layers of metal after redo-TAVR may jeopardize coronary artery patency and accessibility and result in prosthesis-patient mismatch. Dedicated software may use multi-slice computed tomography (MSCT) imaging to simulate TAV deployment relative to the prior TAV and the individual's anatomy and predict coronary accessibility and TAV expansion. AIM: We aimed to validate redo-TAVR simulations created in FEops HEARTguide. METHODS: This multicenter observational stud

Determinants of Unmet Demand for Surgery: The Case of Transcatheter Aortic Valve Replacement.

OBJECTIVES: To examine the determinants of unmet transcatheter aortic valve replacement (TAVR) needs and their impact on patient survival among Medicare beneficiaries with aortic stenosis. METHODS: We developed a county-level mismatch score measuring the gap between actual TAVR procedures performed and expected need based on population differences. Counties were classified as metropolitan, semiurban, or rural. Factors associated with larger mismatches were identified, and mortality rates among aortic stenosis (AS) patients were examined in relation to mismatch scores. We analyzed Medicare dat

Lipoprotein(a) and Cardiovascular Disease: From Genetic Risk Factor to Therapeutic Target.

Lipoprotein(a) [Lp(a)] is a causal, genetically determined risk factor for atherosclerotic cardiovascular disease (ASCVD) and calcific aortic valve stenosis (CAVS). Although elevated Lp(a) affects approximately 20% of the global population, specific pharmacological options have long been unavailable, leaving a major gap in residual risk management. This review synthesizes current understanding of Lp(a) molecular architecture, genetics, and metabolism, and integrates mechanistic evidence linking Lp(a) to pro-atherogenic, pro-inflammatory, and pro-thrombotic pathways. We summarize epidemiologica
