Electromechanical Window Negativity and Arrhythmic Risk in Arrhythmogenic Mitral Valve Prolapse.

BACKGROUND: Mitral valve prolapse (MVP) is generally a benign valvular heart disease, but a subset of patients is at particular risk of malignant ventricular arrhythmias (AMVP). OBJECTIVES: The authors evaluated the electromechanical window (EMW) as an arrhythmia-risk marker in (A)MVP (the sum of all MVP and AMVP patients). METHODS: Healthy controls, patients with MVP without ventricular arrhythmia (MVP/-VA), and patients with AMVP with nonsustained (AMVP/nsVA) or sustained ventricular arrhythmia (AMVP/VA) (ventricular arrhythmia >30 seconds) were retrospectively included from 3 centers (der

Percutaneous Multivalvular Endocarditis Debulking With a Whole-Body Embolic Protection Device.

BACKGROUND: Percutaneous debulking of left-sided endocarditis and thrombus has previously relied on cerebral-only embolic protection devices. The Emboliner is an investigational nitinol-mesh filter that provides whole-body embolic protection and has been studied in transcatheter aortic valve replacement. SUMMARY: We describe its first reported use as an adjunct to AngioVac-based debulking in 2 patients: 1 with aortic valve endocarditis and 1 with combined tricuspid and mitral endocarditis. DISCUSSION: The Emboliner was successfully deployed in both cases, with over 90% of vegetation removed

Pelacarsen and Lp(a)HORIZON: Interpreting the Disconnect Between Lipoprotein(a) Lowering and Cardiovascular Outcomes.

Lipoprotein(a) [Lp(a)] is a genetically determined causal risk factor for atherosclerotic cardiovascular disease and calcific aortic valve stenosis. Pelacarsen, a hepatocyte-directed antisense oligonucleotide targeting apolipoprotein(a), was evaluated in Lp(a)HORIZON, the first large phase 3 cardiovascular outcomes trial specifically designed to test whether pharmacological Lp(a) lowering reduces cardiovascular events. Topline results announced in September 2026 indicated that pelacarsen lowered Lp(a) but did not reduce the primary composite cardiovascular endpoint in patients with established

Aspirin for primary prevention in patients with high lipoprotein(a): A promising therapeutic option while awaiting future therapies?

Elevated lipoprotein(a) [Lp(a)] is a genetically determined cardiovascular risk factor, linked to both atherosclerotic cardiovascular disease and aortic valve stenosis. Plasma Lp(a) levels between 30-50mg/dL suggest a moderate risk, while levels of 51-180mg/dL indicate high risk, and concentrations exceeding 180mg/dL are linked to a very high cardiovascular risk. Currently, there are no approved therapies specifically indicated to lower Lp(a). Recent evidences indicate a role of aspirin in reducing cardiovascular events in primary prevention in patients with high Lp(a). In this review, we disc

Tracking Active Fibroblast-Mediated Tissue Remodelling in Cardiomyopathies: Translational Insights from FAPI PET Imaging.

Myocardial fibrosis is a major pathological component of cardiac remodelling and contributes to ventricular stiffness, impaired myocardial function, heart failure progression, and arrhythmic vulnerability across a broad spectrum of cardiomyopathies. In hypertrophic cardiomyopathy (HCM), fibrotic remodelling encompasses both diffuse interstitial matrix expansion and focal replacement fibrosis and is closely associated with disease severity and adverse clinical outcomes. Cardiac magnetic resonance (CMR), using late gadolinium enhancement (LGE) and quantitative parametric mapping, is central to n

Lipoprotein(a): Particle vs mass and prevalent cardiovascular disease in the cardiovascular risk in Young Finns study population.

BACKGROUND: Lipoprotein(a) [Lp(a)] can be reported as mass (mg/dL) or particle concentration (nmol/L). Because Lp(a) acts at the particle level, molar concentration may better reflect biologically relevant exposure. OBJECTIVE: To compare molar- and mass-based Lp(a) in relation to cardiovascular outcomes, risk classification, LPA genetic variation, and apo(a) isoform phenotype in the Young Finns Study. METHODS: Molar Lp(a) was measured using a traceable Roche Tina-quant Gen.2 assay and mass using a nontraceable Thermo Fisher assay. Associations with cardiovascular outcomes and aortic valve st

Coronary microvascular dysfunction in patients undergoing transcatheter aortic valve implantation: A systematic review and meta-analysis.

BACKGROUND: Coronary microvascular dysfunction (CMD) is a complication of severe aortic stenosis. Its prevalence after transcatheter aortic valve implantation (TAVI) and prognostic significance are understudied, as are the post-TAVI trajectories of microvascular indices. AIMS: The aims of this study were to determine the pooled prevalence of CMD, the influence of the diagnostic modality, the post-TAVI coronary flow reserve (CFR) and microvascular resistance changes, and the prognostic impact of baseline CMD on major adverse cardiovascular events (MACE) in patients with severe aortic stenosis

Artificial Intelligence-Derived Computed Tomography Phenotyping for Cardiovascular Risk Stratification in Transcatheter Mitral Valve Replacement.

Background: Despite high procedural success of transcatheter mitral valve replacement (TMVR), mortality remains substantial, and conventional risk scores inadequately reflect the specific risk profile of this high-risk population. Artificial intelligence (AI)-derived analysis may enable TMVR-specific risk assessment using routinely acquired computed tomography (CT). Methods: In a retrospective European multicenter registry, AI-based segmentation of harmonized real-world pre-procedural cardiac CT with variable thoracic and upper-abdominal coverage was applied in patients undergoing TMVR with th

Diagnostic Performance of a Deep Learning Model Using Phonocardiogram and Single-Lead ECG Signals for Detecting Moderate-or-Greater Aortic Stenosis: Multicenter External Validation.

BACKGROUND: Valvular heart disease, particularly aortic stenosis (AS), often remains asymptomatic until advanced stages, making early detection challenging. Although cardiac auscultation is a traditional screening tool, its diagnostic accuracy is limited. We aimed to externally validate a deep learning-based model (eAS model) using phonocardiogram (PCG) and single-lead electrocardiogram (ECG) signals to detect moderate-or-greater AS. METHODS: PCG and ECG recordings were acquired using a PCG device (Cardio-EGG). Transthoracic echocardiography served as the reference standard. The external vali

RNA-Based Therapies for Lipoprotein(a) Reduction: Antisense Oligonucleotides, Small Interfering RNAs, and Emerging Platforms.

PURPOSE OF REVIEW: Lipoprotein(a) [Lp(a)] is an independent, genetically determined, and causal risk factor for atherosclerotic cardiovascular disease (ASCVD) and calcific aortic valve stenosis. Elevated Lp(a) affects approximately 20% of the global population and is not meaningfully modified by lifestyle intervention or existing lipid-lowering pharmacotherapy. This review summarizes the biology of Lp(a) as a therapeutic target, evaluates the efficacy of currently available lipid-lowering agents against this target, and describes the mechanistic principles, clinical evidence, and unresolved qu

Exploratory Analysis of the Innate Immune System in Aortic Stenosis Patients With and Without ASCVD.

Calcific aortic valve disease (CAVD) leads to aortic stenosis (AS) through inflammation and fibro-calcific remodeling. Although CAVD and atherosclerosis share features, not all patients with AS develop atherosclerotic cardiovascular disease (ASCVD), suggesting mechanistic heterogeneity. This case-control study evaluated inflammatory profiles and valvular remodeling in 128 patients with AS (59.4% with ASCVD) and 105 controls. Immune cell composition and monocyte markers were analyzed by flow cytometry, whereas cytokine production was assessed after peripheral blood mononuclear cell (PBMC) stimu

Restoration of frequency-dependent myocardial reserve after TAVI in severe aortic stenosis: an invasive hemodynamic study.

Severe aortic stenosis (AS) and chronic pressure overload may impair frequency-dependent myocardial reserve despite preserved left ventricular ejection fraction (LVEF). We investigated whether acute afterload reduction with transcatheter aortic valve implantation (TAVI) restores force-frequency relationships (FFR) and relaxation frequency relationships (RFR). In this prospective study, patients with severe normal-flow, high-gradient AS and preserved LVEF underwent invasive left ventricular pressure recording during incremental atrial pacing at 80 and 100 beats/min before and immediately after

Impact of Continuous Baseline and Discharge Blood Pressure on Outcomes After Transcatheter Aortic Valve Implantation.

INTRODUCTION: Evidence linking pre- and post-transcatheter aortic valve implantation (TAVI) blood pressure (BP) with cardiovascular outcomes is scarce. This study investigated the associations of pre- and post-TAVI BP and their changes as a continuous variable with cardiovascular outcomes. METHODS: Patients undergoing TAVI at a tertiary university hospital between 2011 and 2025 with office BP available before and after TAVI were included. The primary outcome was a composite of major adverse cardiac events (MACE) at 1 year. Multivariable Cox proportional hazards models incorporating restricted

Transcatheter Aortic Valve Explants: Calcification Patterns and Leaflet Thickening to Inform Repeat Interventions.

BACKGROUND: Degeneration is an inevitable outcome for bioprosthetic heart valves. In the setting of transcatheter aortic valve (TAV) replacement (TAVR), calcifications and leaflet thickening have implications for feasibility and outcomes of repeat interventions, including leaflet modification techniques. In this study, we aimed to investigate the frequency and pattern of leaflet calcification and thickening in explanted TAVs to inform repeat interventions. METHODS: TAV explants were obtained from the Explanted THV registry and clinical institutions. Micro-computed tomography imaging and histo

Advancing aortic stenosis assessment: Validation of fluid-structure interaction models against 4D flow MRI data.

Systematic in vivo validations of computational models of the aortic valve (AV) remain scarce, despite successful validation against in vitro data. Utilizing a combination of computed tomography and 4D flow magnetic resonance imaging data, we developed patient-specific fluid-structure interaction models of the AV immersed in the aorta for five patients in the pre-transcatheter AV replacement configuration. Our computational models are subjected to rigorous validation against 4D flow measurements. Our results demonstrate the models' capacity to accurately replicate flow dynamics. In addition, w

Incidence of Worsening Aortic Regurgitation After Mapping and Ablation in the Aortic Cusps.

BACKGROUND: Ventricular arrhythmias from the aortic cusps (ACs) represent a subset of left ventricular outflow tract arrhythmias. Although ablation in this region can be highly effective, concerns persist regarding aortic valve injury and iatrogenic aortic regurgitation (AR). Contemporary data on AR incidence after AC mapping/ablation remain limited. OBJECTIVES: This study sought to determine the incidence, severity, and clinical significance of AR in a multicenter cohort after mapping and/or radiofrequency ablation in the ACs or commissures. METHODS: We retrospectively analyzed patients at
