Targeting Lipoprotein(a) in Cardiovascular Disease: Emerging Therapies and Clinical Implications.

Lipoprotein(a) [Lp(a)] is a genetically determined apolipoprotein B-containing lipoprotein independently associated with atherosclerotic cardiovascular disease (ASCVD) and calcific aortic valve stenosis (CAVS). Circulating Lp(a) concentrations are largely inherited, minimally responsive to lifestyle modification, and not adequately addressed by conventional lipid-lowering pharmacotherapy. Patients with established ASCVD continue to experience recurrent cardiovascular events despite intensive guideline-directed therapy, and this residual risk reflects mechanisms - including persistently elevate

Association between advanced chronic kidney disease and left-sided transcatheter heart valve re-intervention.

BACKGROUND: Evidence regarding whether advanced chronic kidney disease (ACKD) serves as an independent risk factor for shorter time to valve re-intervention remains limited. OBJECTIVES: This study investigates the prognostic association between ACKD and transcatheter heart valve re-intervention (THVr) in patients with left-sided bioprosthetic valves. METHODS: We conducted a retrospective study of patients who underwent THVr between November 2011 and March 2025 following prior surgical or transcatheter bioprosthetic aortic or mitral valve replacement. Patients with estimated glomerular filtra

Circulating serotonin is associated with calcific aortic valve stenosis: a two-phase metabolomics study.

BACKGROUND: Calcific aortic valve stenosis (CAVS) is the most prevalent valvular heart disease in the elderly, yet no pharmacological therapy can halt its progression, partly due to incomplete understanding of disease-associated metabolic alterations and the lack of circulating biomarkers for disease detection. OBJECTIVE: We aimed to characterize metabolic alterations associated with CAVS and evaluate a candidate circulating marker for CAVS detection. METHODS: In the discovery cohort, patients with severe CAVS and matched controls without aortic valve disease were enrolled. Untargeted serum

EACTS Expert Consensus Document on aortic valve repair and valve-sparing aortic root replacement procedures.

Aortic valve repair is a complex and evolving field that requires multidisciplinary teamwork among heart specialists to diagnose and treat various causes and mechanisms of aortic regurgitation and proximal thoracic aortic aneurysms across all age groups. From a surgical standpoint, aortic valve repair includes all procedures aimed at restoring or maintaining native valve function. This European Association for Cardio-Thoracic Surgery (EACTS) Expert Consensus Document critically evaluates the current evidence on the common causes and mechanisms of aortic regurgitation, the use of echocardiograp

Life-expectancy paradox and the selection of older patients with aortic stenosis for valve intervention.

The natural history of symptomatic severe aortic stenosis (AS) heralds a poor prognosis-a paradigm that rightly drives valve intervention. A widely voiced corollary holds that once patients exceed their nation's average life expectancy, aortic valve replacement offers no prognostic benefit. We argue that this corollary is statistically incoherent and potentially clinically harmful. Life expectancy at birth is a period average across a whole cohort, weighted by deaths at younger ages; it is not a ceiling on individual survival. Expectancy is conditional on survival already achieved: the longer

[Aortic valve disease - Interventional aortic valve replacement in asymptomatic patients and the use of artificial intelligence].

Aortic valve diseases are among the most common structural heart diseases in adults. Aortic stenosis (AS), in particular, poses a growing challenge due to demographic trends, as it continues to be underdiagnosed. The current 2025 ESC/EACTS guidelines mark a paradigm shift: for the first time, early intervention is recommended for selected asymptomatic patients. At the same time, various AI tools support early patient screening. Also new, with a Class IIb recommendation, is that TAVI may be considered for patients with a bicuspid aortic valve and increased surgical risk, provided the anatomy is s

Surgical Explantation of a Balloon-Expandable Transcatheter Aortic Valve With Aortic Root Preservation for Prosthetic Valve Endocarditis: A Case Report.

Infective endocarditis following transcatheter aortic valve implantation represents an increasingly recognized and devastating clinical entity characterized by severe morbidity, high mortality, and formidable surgical complexity. As indications for transcatheter aortic valve procedures expand toward younger and lower-risk patient cohorts, late-onset endocarditis poses significant therapeutic dilemmas. Surgical management is technically challenging due to dense tissue integration, neointimal proliferation, and complete endothelialization of the expanded metallic frame into the aortic wall and s

Adenosine and ATP differentially control osteogenic gene expression patterns in a cardiac valve cell line.

INTRODUCTION: Calcific aortic valve disease (CAVD) involves the osteogenic transition of valve interstitial cells (VICs) and is influenced by purinergic signaling. ATP is considered pro-calcific, whereas adenosine (Ado) has been reported as anti- or pro-calcific. Extracellular conversion of ATP to Ado by ecto-enzymes (including ecto-5'-nucleotidase, CD73) may shape receptor signaling in VICs. MATERIAL AND METHODS: We quantified osteogenic and apoptotic markers in the murine valve-like cell line tsA58-AVM under osteogenic conditions after treatment with ATP or Ado, selective Adora1/Adora2A ago

Transcatheter Aortic Valve Replacement Requiring Modification of a Protruding RCA Stent.

BACKGROUND: Protruding ostial stents represent a unique hazard for transcatheter aortic valve replacement (TAVR). CASE SUMMARY: An 81-year-old woman with symptomatic severe aortic stenosis and recent right coronary artery stenting with a 10-mm segment protruding into the aortic root presented for TAVR evaluation. After multidisciplinary discussion, the decision was made to proceed with TAVR with stent modification. The right coronary artery was wired through the stent struts at the distal end of the protruded segment and was ballooned, creating a neo-ostium. The protruded segment was crushed

Deep Survival Modeling With Echocardiographic Myocardial Texture Radiomics for Prediction of Major Adverse Cardiovascular Events in Hypertrophic Cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) carries substantial risk of major adverse cardiovascular events (MACE), yet current tools do not fully exploit quantitative myocardial texture from routine echocardiography. We retrospectively studied 413 patients with HCM from two tertiary centers to develop and externally validate a deep survival model integrating transthoracic echocardiographic texture radiomics with clinical and conventional echocardiographic variables. Patients from one center (n = 308) were used for development and internal testing and those from the other (n = 105) for external validati

Estimated Left Ventricular Outflow Tract Diameter: Proposal of an Equation for Clinical Use in Adults from The ELSA Brasil Study data.

Left ventricular outflow tract diameter (LVOTd) is essential for estimating stroke volume and aortic stenosis severity, but technical limitations may affect its accuracy. A predicted LVOTd may overcome these issues, though it has not been validated in large populations. We studied the participants of the representative random sample and with ≥ 60 y-o from the Elsa-Brasil study with echocardiogram performed (n=3,267), divided into a derivation (70%) and a validation subgroup (30%). In the derivation cohort, independent determinants of LVOTd were identified, and a regression equation was derived

Association of Sodium-Glucose Cotransporter 2 Inhibitor Use With Outcomes in Patients Undergoing Transcatheter Aortic Valve Implantation: A Systematic Review and GRADE-Assessed Meta-Analysis.

BACKGROUND: Patients undergoing transcatheter aortic valve implantation (TAVI) remain at risk of adverse cardiovascular and renal outcomes despite procedural advancements. Sodium-glucose cotransporter 2 (SGLT2) inhibitors have demonstrated cardiovascular and renal benefits across various patient populations; however, their role in patients undergoing TAVI remains unclear. This study is aimed at evaluating the association between SGLT2 inhibitor use and clinical outcomes in this population. METHODS: We conducted a systematic review and meta-analysis comparing SGLT2 inhibitors versus non-SGLT2

IL-17-associated Pro-inflammatory Programs in Aortic Valve Interstitial Cell Osteogenic Differentiation: JUN as a Candidate Regulator.

Calcific aortic valve disease (CAVD) involves fibro-calcific remodeling, in which osteogenic differentiation of valvular interstitial cells (VICs) is a key process. Although inflammation is implicated, uncertainties persist regarding how defined pro-inflammatory pathways are transcriptionally coupled to the VIC osteogenic program at the cell-intrinsic level. We implemented an interleukin-17 (IL-17) pathway-guided transcriptomic strategy in a pure human VIC osteogenic differentiation model. RNA sequencing identified osteogenesis-associated differentially expressed genes and IL-17-related inflam

The Yield of Clinical Screening in Pediatric Relatives of Hypertrophic Cardiomyopathy With Negative Genetic Testing.

BACKGROUND: Serial clinical screening of first-degree relatives of individuals with hypertrophic cardiomyopathy (HCM) is recommended. A lower yield of screening in adult relatives with genotype-negative disease might mean a one-off assessment is appropriate, but it is unknown if these findings can be extrapolated to pediatric relatives. OBJECTIVES: This study aims to determine the yield of clinical screening in pediatric relatives of genotype-negative families. METHODS: Clinical and outcome data were collected from 404 children (≤18 years) from 227 genotype-positive (G+) families and 292 chi

Tirzepatide and risk of newly diagnosed aortic stenosis in patients with obesity: a multi-institutional real-world cohort study.

BACKGROUND: Obesity is a major risk factor for the development of degenerative aortic stenosis (AS), yet no pharmacologic therapy has been proven to prevent AS onset. Tirzepatide, a dual GIP/GLP-1 receptor agonist, provides greater metabolic and anti-inflammatory benefits than conventional GLP-1 receptor agonists (GLP-1 RAs). Whether tirzepatide reduces the risk of incident AS remains unknown. OBJECTIVES: To evaluate the association between tirzepatide use and the risk of newly diagnosed AS or aortic valve replacement (AVR) compared with GLP-1 receptor agonists in adults with obesity.
