Mid-term Outcomes of Transcatheter Aortic Valve Replacement vs. Surgical Aortic Valve Replacement in Low-to-Moderate Risk Patients with Severe Aortic Stenosis: A Systematic Review and Meta-analysis.

INTRODUCTION: Several clinical trials have demonstrated the non-inferiority of transcatheter aortic valve replacement compared with surgical aortic valve replacement in patients with severe aortic stenosis and low to intermediate surgical risk. However, mid-term results are still contentious. We performed this meta-analysis to compare the safety and efficacy of transcatheter vs. surgical aortic valve replacement in the mid-term in patients with aortic stenosis at low to moderate surgical risk. METHODS: We searched Embase, PubMed®, and Cochrane databases for randomized clinical trials that com

GLP-1 receptor agonists and clinical outcomes in patients with type 2 diabetes undergoing transcatheter aortic valve implantation.

AIMS: We evaluated clinical outcomes with GLP-1 RA use post-TAVI in patients with T2D. GLP-1 receptor agonists (GLP1-RAs) lower cardiovascular risk in type 2 diabetes (T2D), but their impact in patients with valvular heart disease, including those undergoing transcatheter aortic-valve implantation (TAVI), remained unexplored. METHODS AND RESULTS: We conducted a retrospective cohort study using the TriNetX network, including 25 184 adults (≥18 years) with T2D who underwent TAVI (2015-2023). Patients were stratified based on GLP1-RA initiation within 14 days post-TAVI. Baseline characteristics,

Advances in single-cell transcriptomics: unraveling the pathogenesis of calcific aortic valve disease.

Single-cell RNA sequencing (scRNA-seq) has revolutionized the study of calcified aortic valve disease (CAVD) by enabling the dissection of cellular heterogeneity, lineage differentiation, and intercellular crosstalk at an unprecedented resolution. It revealed original vistas into the mechanisms of CAVD and novel putative therapeutic targets. This review provides a narrative state-of-the-art overview of scRNA-seq applications in CAVD research, summarizing key findings on valvular cell populations, disease-associated phenotypic transitions, and molecular pathways driving pathological remodeling.

VAPB Promotes Osteogenic Differentiation in Aortic Valve Interstitial Cells via Activation of the SMAD Signaling Pathway.

OBJECTIVE: Prior studies have associated vesicle-associated membrane protein- associated protein B (VAPB) with various cardiovascular diseases, yet its role in calcific Aortic Valve Disease (CAVD) is not fully understood. This study aims to elucidate the relationship between VAPB and aortic valve calcification at both tissue and cellular molecular levels, exploring potential regulatory mechanisms. METHODS: The expression levels of VAPB in calcified valve tissues were assessed using immunohistochemical analysis. Osteogenic differentiation and calcification levels of valve interstitial cells (V

Non-cardiac surgery after transcatheter aortic valve implantation.

AIMS: There is a lack of data on perioperative outcomes for patients undergoing non-cardiac surgery (NCS) after transcatheter aortic valve implantation (TAVI). Hence, we aimed to determine the incidence, type of surgery, timing, and perioperative outcomes of individuals undergoing elective NCS after discharge for TAVI. METHODS AND RESULTS: Hospitalisations for TAVI were identified from the US National Readmission Database between 2012 and 2021, and patients who received NCS within 6 months were included for analysis. Incidence, type, and timing of planned readmissions for NCS were evaluated a

Interpretable machine learning using accumulated local effects to characterise predictors of subclinical leaflet thrombosis after self-expanding transcatheter aortic valve implantation.

OBJECTIVES: Subclinical leaflet thrombosis is an early form of bioprosthetic valve dysfunction after transcatheter aortic valve implantation. Predicting subclinical leaflet thrombosis remains challenging. We aimed to apply machine learning not only to identify predictors but also to interpret their dynamic, non-linear effects on subclinical leaflet thrombosis risk using Accumulated Local Effects curves, a robust method that accounts for variable collinearity. METHODS: A prospective cohort of 128 consecutive patients receiving a self-expanding transcatheter heart valve underwent multimodality

Accelerometry-defined physical activity and quality of life in hypertrophic cardiomyopathy.

BACKGROUND: Patients with hypertrophic cardiomyopathy (HCM) often reduce their physical activity due to concerns about sudden cardiac death. However, objective data on activity patterns in HCM, particularly in relation to clinical phenotype and quality of life (QoL), remain limited. METHODS: We assessed physical activity using 7-day accelerometry in 203 patients with HCM and 37 genotype-positive, phenotype-negative (G+/P-) individuals. Outcomes included daily step counts, time spent in moderate-to-vigorous physical activity (MVPA) and sedentariness. QoL was measured using the Kansas City Card

Evaluation of Risk Score for Isolated Surgical Aortic Valve Replacement and Transcatheter Aortic Valve Replacement-Results from the German National Quality Database.

OBJECTIVES: For the treatment of aortic valve stenoses, both surgical aortic valve replacement (SAVR) and transcatheter aortic valve implantation (TAVI) are available. We compared the frequently used Euroscore with the AKL-Cath- and the AKL-Chir Score, describing the mortality risk of the 2 different treatment methods. METHODS: Based on a retrospective cohort study using mandatory quality assurance data, we analysed the frequency and the outcomes (primary end-point: in-hospital mortality) of all patients treated in Germany between 2015 and 2020. The observed results were compared to the predi

Coronary access after TAVR with the 4th and 5th -generation self-expanding Evolut valves: The EPROMPT-CA study.

BACKGROUND: As transcatheter aortic valve replacement (TAVR) expands to lower-risk patients, optimizing post-TAVR coronary access and identifying its determinants have become increasingly important. OBJECTIVES: To evaluate coronary access and assess CT predictors of selective access post-TAVR with the Evolut FX/FX+ valves. METHODS: This single-center, prospective study enrolled patients undergoing transfemoral TAVR with Evolut FX/FX+ valves. Coronary access was attempted immediately post-deployment, with CT performed at 30-60 days. The primary endpoint was coronary access success. CTs were a

CircZBTB44-Encoded Peptide ZBTB44-342aa Alleviates Aortic Valve Calcification Via cGAS-STING Inhibition.

BACKGROUND: Circular RNAs are implicated in various physiopathologic activities and play a crucial role in calcific aortic valve disease (CAVD) progression. However, the role of coding circular RNAs in CAVD remains unclear. In this study, we aimed to characterize coding circular RNAs and explore their functions in CAVD. METHODS: Using a systematic approach from transcriptome sequencing to experimental validation, we identified circZBTB44, confirmed its translation into ZBTB44-342aa, and investigated the function and mechanism of this peptide in CAVD using both cellular and animal models. Aortic valve calcium score from computed tomography for signal localization by cardiac amyloid radionuclide imaging to predict aortic stenosis and assess prognosis in patients with transthyretin cardiomyopathy.

PURPOSE: Transthyretin cardiomyopathy (ATTR-CM) and aortic stenosis (AS) are frequently associated. The diagnosis of ATTR-CM can be made by performing a cardiac amyloid radionuclide imaging (CARI) with a computed tomography (CT) scan to localize the signal, which allows visualization aortic valve calcifications (AVC). The aim of this study was to determine whether AVC on the CT performed at the time of the CARI for the diagnosis of ATTR-CM were predictive of AS and had an impact on prognosis. METHODS: CT images of patients diagnosed with ATTR-CM by CARI were reviewed for the presence of AVC an

Personalized computational hemodynamic analysis in transcatheter aortic valve: investigation of long-term degeneration.

Introduced as an alternative to open-heart surgery for elderly patients, Transcatheter Aortic Valve Implantation (TAVI) has recently been extended to younger patients due to comparable performance with the gold-standard. However, the long-term durability of the bio-prosthetic TAVI valves is limited by Structural Valve Deterioration (SVD), an inevitable degenerative process whose pathogenesis is still unclear. In this study, we aim to computationally investigate a possible relationship between aortic hemodynamics and SVD development. To this aim, we collect data from twelve patients with and wi

Clampless transcarotid transcatheter aortic valve implantation.

The use of transcatheter aortic valve implantation (TAVI) has become the most popular technique of aortic valve intervention. Classically, TAVI is performed via femoral arterial access. However, some patients who have severe peripheral arterial disease do not have safely accessible femoral arteries. In such patients, the use of carotid access has been well described via the 'clamp-and-sew' technique. We describe a minimally invasive carotid access technique for TAVI deployment. The right common carotid is accessed by a 3 cm suprasternal incision. The carotid sheath is entered, and the surgeon

Mechanisms of Trifecta Valve Failure and Contemporary Approaches to Reintervention: Perceval vs Valve-in-Valve Transcatheter Aortic Valve Implantation.

OBJECTIVES: Aortic valve replacement with Trifecta valve provides a greater early haemodynamic performance with its externally mounted structure. However, failure rates in short-term post-implantation started to be reported, which brought great attention towards understanding the mechanisms of failure and optimal approaches for the replacement of the valve. METHODS: Fifty-seven studies published in PubMed/Medline, Cochrane Central Register of Controlled Trials (CENTRAL), Embase, and ClinicalTrials.gov were explored until January 2025. Original research articles or clinical trial reports that

Clinical and Transcriptomic Characteristics of Aortic Stenosis in Patients Undergoing Haemodialysis.

OBJECTIVES: Patients with aortic stenosis (AS) undergoing haemodialysis (HD) often experience more rapid disease progression and poorer prognosis than non-dialysis patients; however, the underlying mechanisms remain unclear. This study aimed to elucidate clinical and molecular differences between HD and non-HD patients with AS, focusing on transcriptomic profiling of resected aortic valves. METHODS: We retrospectively analysed 183 patients with severe AS who underwent surgical aortic valve replacement at the University of Yamanashi Hospital from February 2015 to May 2024. Among them, 34 patie

Opportunistic automated aortic valve calcification assessment in low-cost, screening CT calcium score exams.

Aortic valve stenosis (AS) is the most common valvular disease, with a growing impact in the aging population. AS can culminate in heart failure if left untreated. Early treatment with minimally-invasive transcatheter aortic valve replacement (TAVR) is being evaluated in clinical trials. We addressed an unmet clinical need for early detection, referral to echocardiography for AS evaluation, and possible treatments (e.g., lifestyle changes, drugs, or TAVR). As aortic valve calcification (AVC) is typically present in AS, we created a method to detect AVC in low-cost/no-cost non-contrast CT calci
