Balloon-Expandable Versus Self-Expanding Valves in Transcatheter Aortic Valve Replacement for Patients with Left Ventricular Systolic Dysfunction.

OBJECTIVE: Patients with severe aortic stenosis (AS) and left ventricular systolic dysfunction (LVSD) represent a particularly fragile subgroup undergoing transcatheter aortic valve replacement (TAVR). Comparative outcome data for balloon-expandable valves (BEV) and self-expanding valves (SEV) in this population remain scarce. METHOD: This retrospective single-center study evaluated 246 consecutive subjects with left ventricular ejection fraction (LVEF) < 50% who underwent transfemoral TAVR between January 2015 and June 2025. Clinical, echocardiographic, and procedural characteristics were co

Prosthesis-Patient Mismatch After Transcatheter Aortic Valve Implantation: The Prognostic Effect of Right-sided cardiopulmonary Remodelling.

BACKGROUND: Prosthesis-patient mismatch (PPM) after transcatheter aortic valve implantation (TAVI) for aortic stenosis (AS) is associated with adverse outcomes. In this study we evaluated the prognostic effect of PPM after TAVI in relation to baseline right-sided cardiopulmonary remodelling. METHODS: Consecutive patients who underwent transfemoral TAVI for severe symptomatic AS between 2015 and 2022 were identified (n = 1118). PPM was defined at discharge according to Valve Academic Research Consortium-3 criteria. Baseline right-sided cardiopulmonary remodelling was defined as either right ve

Prevalence of elevated lipoprotein(a) levels and associated atherosclerotic cardiovascular diseases in subjects with metabolic disorders: A real-world study in a lipid unit.

BACKGROUND AND AIMS: Lipoprotein(a) (Lp(a)) is a proatherogenic particle that is considered an important cardiovascular risk modifier due to its association with atherosclerotic cardiovascular disease (ASCVD) as well as calcific aortic valve stenosis (CAVS). Data on the clinical burden associated with elevated lipoprotein(a) levels in patients at high and very high cardiovascular risk remain limited. We evaluated the prevalence of ASCVD and LDL-C target achievement in subjects with high and very high elevated Lp(a) levels referred to a lipid unit. METHODS AND RESULTS: In this retrospective st

Peripheral Embolization of Amplatzer Device to External Iliac Artery: Do Asymptomatic Patients Require Intervention? A Case Report and Literature Review.

BACKGROUND Device embolization is a rare complication of percutaneous closure of paravalvular leaks after transcatheter, surgical aortic, or mitral valve replacement. Management typically involves surgical or endovascular intervention. However, the optimal management of asymptomatic patients with a delayed diagnosis of device embolization remains unclear, as evidence for watchful waiting and/or delayed intervention is limited. CASE REPORT We present a case of 73-year-old man who had undergone endovascular closure of a paravalvular leak following transcatheter aortic valve replacement. The pati

Severe Aortic Stenosis: ECG Abnormalities as the First Clue in Emergency Care.

Aortic stenosis (AS) is a progressive valvular disease that presents with subtle symptoms in the emergency department, leading to delayed diagnosis. While acute presentations of valvular pathology exhibit dramatic clinical signs, most cases are chronic and commonly discovered incidentally during routine evaluations. Compensatory adaptations, including cardiac remodeling and atrial augmentation of preload, temporarily sustain cardiac function and mask disease severity, increasing the risk of heart failure and sudden cardiac death. This case-based article details the diagnostic journey of a 59-y

Cryopreserved aortic homograft root replacement for supravalvular root stenosis in familial homozygous hypercholesterolemia.

Homozygous familial hypercholesterolemia is a genetic disorder resulting in severe elevations of the total cholesterol that is frequently associated with complex aortic root stenosis and small aortic valve annulus. The few reported surgical approaches included variations of mechanical valve/root replacement and annular enlargement. We report a rare case of supravalvular aortic stenosis (SVS) with severe atherosclerotic root dystrophic stenosis with coronary and valve involvement treated with a cryopreserved homograft using a hemi Cabrol technique.

Gamma-Oryzanol Attenuates Aortic Valve Interstitial Cell Calcification via Suppression of BMP2-SMAD and MAPK Signaling Pathways.

Calcific aortic valve stenosis (CAVS) is a progressive cardiovascular disease associated with oxidative stress-driven osteogenic differentiation of valvular interstitial cells (VICs), yet no pharmacological therapy can prevent its progression. γ-oryzanol (γ-ORZ), a rice bran-derived phytosteryl ferulate, exhibits potent antioxidative and anti-inflammatory activities that may counteract valvular calcification. Here, we show that γ-ORZ markedly attenuates PCM-induced intracellular ROS elevation, osteogenic differentiation, and calcium phosphate deposition in porcine VICs (pVICs). In addition, RT

TAVI Performance at a Single Center over Several Years: Procedural and Clinical Outcomes.

Background and Objectives: Transcatheter aortic valve implantation (TAVI) has become the mainstay of treatment for symptomatic aortic stenosis (AS) in patients over 70 years of age. It is also indicated for younger patients with significant comorbidities, for valve-in-valve interventions, and in selected patients with severe aortic insufficiency. We aimed to evaluate procedural and clinical outcomes of transfemoral TAVI performed over the course of 12 years by the same operators using different bioprosthetic valves. Materials and Methods: Between 2012 and 2023, 375 patients underwent TAVI in o

Transcatheter or sutureless/rapid-deployment aortic valve replacement? Contemporary insights from an individual patient data meta-analysis.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) and sutureless/rapid-deployment aortic valve replacement (SUAVR) have each undergone rapid technological advances and expanding indications. However, direct comparative evidence between the two, particularly for long-term outcomes, remains limited. METHODS: We conducted an updated meta-analysis of propensity score-matched studies using reconstructed individual patient data (IPD) to compare TAVR with SUAVR. The protocol was registered in PROSPERO (CRD420251046330). Published Kaplan-Meier survival curves were digitized, IPD reconstructed

Association between inflammatory hematological ratios and aortic valve stenosis: a propensity-matched analysis.

BACKGROUND: Relationship between systemic inflammation and aortic valve stenosis (AVS) has been well demonstrated. This investigation aimed to evaluate the link between various inflammation hematological ratios and patients with AVS. METHODS: Patients with AVS (n = 229) and control (n = 1716) were identified. Propensity score matching (PSM) was performed with the proportion of 1:1 using logistic regression based on the variables of age, sex, hypertension, diabetes, creatinine (Cr) and glutamate transpeptidase (γ-GT). The receiver operating characteristic (ROC) analysis was used to estimate th

Race and Ethnic-Specific Burden of Aortic Valve Calcium and Its Association With Long-Term Aortic Stenosis: Multi-Ethnic Study of Atherosclerosis.

BACKGROUND: Racial and ethnic differences have been reported for aortic valve calcium (AVC) and long-term aortic stenosis (AS). Whether these differences are due to differing risk factor profiles or the burden of AVC is unknown. METHODS: Baseline AVC was quantified using the Agatston method among 6812 MESA (Multi-Ethnic Study of Atherosclerosis) participants. AVC scores were not reported to participants. The primary outcome of long-term moderate or severe AS was adjudicated using standard clinical criteria. We calculated multivariable Cox proportional hazards with log-transformed AVC as a con

Prevalence and clinical implications of angiodysplasia in patients with aortic stenosis: a systematic review and meta-analysis.

Gastrointestinal angiodysplasia is frequently observed in patients with aortic stenosis and may present with bleeding and anemia. We conducted a systematic review and meta-analysis to estimate its prevalence in this population and to summarize outcomes after valve intervention. Following PRISMA 2020, we searched PubMed, Scopus, and Web of Science and registered the protocol in PROSPERO (CRD42024550839). Eligible observational studies reporting angiodysplasia, von Willebrand factor abnormalities, or both in aortic stenosis were appraised for quality and pooled using a random effects model; hete

Bidirectional nexus between metabolic disease and aortic valve calcification: from pathogenic mechanisms to integrated multimorbidity management.

INTRODUCTION: Calcified aortic valve disease (CAVD) imposes a severe global health burden, and there is currently no effective pharmacotherapy, which urgently requires a new understanding of pathophysiology to guide treatment. AREAS COVERED: This review systematically evaluates the epidemiological association and common mechanistic pathways between metabolic diseases and aortic valve calcification (AVC) through the proposed 'calcification-metabolic axis' framework. We conducted a comprehensive literature search, covering the PubMed database up to December 2025, with a focus on studies related

Importance of digital media in patient education for transcatheter aortic valve implantation.

BACKGROUND: Patient consent discussion is an essential part of preoperative patient preparation. It is the main basis of decision making for the patient. Transcatheter aortic valve implantation (TAVI) has been established as the standard treatment option for symptomatic aortic stenosis-patients. Current guidelines recommend transfemoral TAVI for patients aged 70 years or older, independently of the surgical risk and for patients below 70 years of age if they are at high surgical risk. AIM: To evaluate beneficial aspects of an explanatory video in addition to conventional medical counseling.

Evidence-based guidelines for aortic stenosis: Focus on surgical aortic valve replacement versus transcatheter aortic valve implantation for young patients.

Transcatheter aortic valve implantation (TAVI) has revolutionized the management of aortic stenosis and is now an established alternative to surgical aortic valve replacement (SAVR) for all surgical risk categories. However, the extension of TAVI to younger, low-risk patients has raised important questions regarding the long-term outcomes, valve durability, and lifetime management strategies for these patients. This narrative review summarizes the current evidence from pivotal randomized trials and contemporary registries comparing TAVI and SAVR in young, low-risk patients, integrating the lat

Multidisciplinary team-guided management of severe aortic stenosis: 5-year outcomes following TAVI versus conservative treatment.

OBJECTIVE: Multidisciplinary team (MDT) meetings are central to treatment decisions in aortic stenosis (AS), particularly for borderline or high-risk patients. This study evaluates long-term, real-world outcomes according to MDT-selected management strategy within routine clinical practice in this clinically important patient group. METHODS: We conducted a retrospective cohort study of all patients with severe AS discussed at a transcatheter aortic valve implantation (TAVI) MDT at a tertiary UK centre between January 2014 and December 2016. Patients were categorised as TAVI or non-TAVI (conse

Sex differences in the prevalence and risk factors for aortic valve calcification in the general population.

BACKGROUND: Aortic valve calcification (AVC) is a disease process driven by inflammation and lipid infiltration, serving as a precursor to aortic stenosis. While male sex has been implicated as a risk factor for AVC, sex-specific differences, particularly among younger individuals in the general population, are not well characterised. METHODS: The Swedish CArdioPulmonary BioImage Study was used, comprising 30 154 individuals between 50 and 64 years, randomly selected from the general population. Study participants were part of a prospective cohort and underwent laboratory tests, clinical exam

Vessel-Oriented Analysis on the Relationship Between Quantitative-Flow Ratio and Mortality in Patients With Severe Aortic Stenosis and Intermediate Coronary Lesions.

BACKGROUND: Quantitative flow ratio (QFR) is effective in predicting mortality in patients undergoing transcatheter aortic valve replacement (TAVR). How QFR reclassifies coronary artery disease (CAD) at vessel-level compared to angiography and how this influences the risk of death, remains undetermined. METHODS: We calculated QFR from consecutive 280 TAVR patients with bystander coronary stenoses. All lesions were managed conservatively. Angiographic CAD was defined by a diameter stenosis ≥ 50%, functional CAD by a QFR ≤ 0.80. The outcome was mortality at 3 years. RESULTS: Overall, 635 lesio
