Impact of Virtual Reality on Transcatheter Aortic Valve Implantation: A Prospective Randomized Controlled Trial.

BACKGROUND: Accurate preprocedural planning is crucial for a successful transcatheter aortic valve implantation to ensure patient safety and valve longevity. Through 3-dimensional visualization, virtual reality (VR) offers the potential to enhance this process. The study investigated whether the inclusion of VR in preprocedural planning can improve the procedural preparation, impact intraprocedural parameters, and improve short-term patient outcomes. METHODS: This randomized, prospective, controlled study included 140 patients who underwent transcatheter aortic valve implantation at the Unive

Prediction of severe aortic stenosis using peripheral arterial waveforms during brachial-ankle pulse wave velocity testing.

With the increasing use of minimally invasive catheter-based therapies, early identification of severe aortic stenosis (AS) has become essential. The slow upstroke waveform (Tardus) observed in carotid artery wave analysis is a known feature of AS, and quantitative assessments of Tardus can estimate the severity of AS. Prolongation of ejection time (ET) and upstroke time (UT) is a characteristic of Tardus. However, the clinical relevance of ET and UT measured during brachial-ankle pulse wave velocity (baPWV) assessment remained unclear. This study aimed to develop a predictive model for screen

Validation of an Automated Artificial Intelligence-Driven Planner for Transcatheter Aortic Valve Replacement.

BACKGROUND: Preprocedural planning with multislice computed tomography (MSCT) is an essential prerequisite for successful transcatheter aortic valve replacement (TAVR). However, preprocedural planning is time-consuming and may contain clinically meaningful interobserver variability. Artificial intelligence (AI) based automated planning methods may provide anatomical MSCT measurements while reducing the time load required for the analysis. Previous studies on the validation of AI-based TAVR planning methods excluded anatomical variants or lacked data on actual TAVR prosthesis size agreement. Mi

Dapagliflozin After Transcatheter Aortic Valve Implantation According to Frailty in DapaTAVI Trial.

BACKGROUND: Frailty is common in patients with severe aortic stenosis undergoing transcatheter aortic valve implantation and portends worse outcomes. Whether the benefit of posttranscatheter aortic valve implantation dapagliflozin differs by frailty is uncertain. METHODS: In this post hoc analysis, we investigated the efficacy of dapagliflozin according to frailty status in the DapaTAVI (Dapagliflozin in Patients Undergoing Transcatheter Aortic Valve Implantation) trial. Frailty was assessed through the Fatigue, Resistance, Ambulation, Illnesses, and Loss of Weight scale (robust=0, prefrail=1

Discharge NT-proBNP predicts 2-year outcomes after TAVR but not after SAVR in preserved ejection fraction: a propensity-matched cohort study.

BACKGROUND: Discharge N-terminal pro-B-type natriuretic peptide (NT-proBNP) is a recognised prognostic marker after transcatheter aortic valve replacement (TAVR), but its value after surgical aortic valve replacement (SAVR) remains uncertain, possibly reflecting differences in perioperative physiology. AIMS: To compare the prognostic utility of discharge NT-proBNP after TAVR versus SAVR in patients with severe aortic stenosis and preserved ejection fraction, and to examine the association between NT-proBNP reduction and Left Ventricular Mass Index (LVMI) regression. METHODS: This propensity-

[Evidence-based treatment of valvular heart disease].

Valvular heart disease is a major cause of cardiovascular morbidity and mortality worldwide. As the prevalence of most valvular heart diseases increases with age, population ageing has led to a substantial increase in disease burden. Due to advanced age or significant comorbidities, many patients are considered unsuitable for conventional surgery. Whereas medical therapy was therefore often the only treatment option in the past, the rapid evolution of transcatheter interventions has fundamentally transformed the management of valvular heart disease over the past two decades. Today, effective c

Coronary artery disease management in patients with severe aortic stenosis undergoing transcatheter aortic valve implantation: a systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: The optimal management of coronary artery disease in patients with severe aortic stenosis undergoing transcatheter aortic valve implantation (TAVI) remains uncertain. AIMS: We aimed to compare clinical outcomes between routine coronary revascularisation and conservative management in patients undergoing TAVI and to assess the relative effectiveness of individual revascularisation strategies by performing a meta-analysis of randomised trials. METHODS: We conducted a pairwise meta-analysis comparing percutaneous coronary intervention (PCI) with conservative treatment and a network

SGLT2 inhibitor therapy following aortic valve intervention for aortic stenosis: a systematic review and meta-analysis of mortality, heart failure, rhythm, renal, and valve-related outcomes.

BACKGROUND: Residual mortality and heart-failure events remain common after aortic valve intervention for severe aortic stenosis. Sodium-glucose cotransporter-2 inhibitors (SGLT2i) improve cardiovascular and renal outcomes in heart-failure, diabetes, and chronic kidney disease populations, but their role following transcatheter or surgical aortic valve intervention remains uncertain. METHODS: PubMed, Scopus, ScienceDirect, Cochrane Library, citation tracking, and manually identified eligible studies were screened through June 2026 for randomized and observational studies comparing SGLT2i with

The Senokine Gas6 Promotes Aortic Valve Calcification Through CaMKII-DRP1-Mediated Terminal Scission of Mitochondrial-Derived Vesicles.

Calcific aortic valve disease (CAVD) is an age-associated valvular disorder in which calcification expands from discrete foci into adjacent tissue. We investigated whether mitochondrial-derived vesicles (MDVs) released by human valvular interstitial cells (hVICs) exert pro-calcific effects on recipient cells and how MDV release is regulated. Using ex vivo hemodynamic perfusion of human CAVD tissue, multi-omics profiling, live-cell single-organelle tracking, MDV transfer assays, and in vivo Gas6 deletion, we found that calcifying hVICs hypersecreted MDVs that exerted pro-calcific effects in rec

Successful surgical management of a rare left ventricular pseudoaneurysm following balloon aortic valvuloplasty in an infant.

Left ventricular pseudoaneurysm is an exceedingly rare, life-threatening complication following percutaneous balloon aortic valvuloplasty. We report a 1-month-old infant presenting with a large left ventricular pseudoaneurysm after successful valvuloplasty for critical aortic stenosis. Multimodal imaging confirmed the diagnosis. The patient underwent successful surgical resection and bovine pericardial patch repair. This case emphasises the importance of early surgical intervention for favourable outcomes.

Impact of prolonged balloon inflation on valve expansion and early clinical outcomes following contemporary balloon-expandable transcatheter aortic valve implantation.

BACKGROUND: Incomplete expansion of balloon-expandable (BE) transcatheter heart valves (THVs) after transcatheter aortic valve implantation (TAVI) may affect valve performance and durability. Routine THV post-dilatation (the "double-tap" technique) has been proposed to optimise device expansion; however, it may increase procedural complexity. AIMS: We sought to evaluate the impact of a prolonged balloon inflation implantation strategy on BE THV expansion, haemodynamic performance, and early clinical outcomes. METHODS: This single-centre study analysed consecutive TAVI patients treated for na

Low-dose juglone attenuates pressure overload-induced cardiac hypertrophy through Lys207-dependent FoxO1 activation and mitochondrial protection.

Pressure overload-induced cardiac hypertrophy is characterized by mitochondrial dysfunction and redox imbalance. Here, we investigated the efficacy, safety window, and redox-regulatory mechanism of juglone, a naturally occurring 5-hydroxy-1,4-naphthoquinone. In mice subjected to transverse aortic constriction (TAC), high-dose juglone induced cardiac, hepatic, and renal toxicity, whereas 1 mg/kg/day was well tolerated and attenuated cardiac hypertrophy. Juglone also suppressed Ang II-induced cardiomyocyte hypertrophy in vitro and preserved mitochondrial ultrastructure and mitochondrial membrane

Pelacarsen and Lp(a)HORIZON: Interpreting the Disconnect Between Lipoprotein(a) Lowering and Cardiovascular Outcomes.

Lipoprotein(a) [Lp(a)] is a genetically determined causal risk factor for atherosclerotic cardiovascular disease and calcific aortic valve stenosis. Pelacarsen, a hepatocyte-directed antisense oligonucleotide targeting apolipoprotein(a), was evaluated in Lp(a)HORIZON, the first large phase 3 cardiovascular outcomes trial specifically designed to test whether pharmacological Lp(a) lowering reduces cardiovascular events. Topline results announced in September 2026 indicated that pelacarsen lowered Lp(a) but did not reduce the primary composite cardiovascular endpoint in patients with established

Aspirin for primary prevention in patients with high lipoprotein(a): A promising therapeutic option while awaiting future therapies?

Elevated lipoprotein(a) [Lp(a)] is a genetically determined cardiovascular risk factor, linked to both atherosclerotic cardiovascular disease and aortic valve stenosis. Plasma Lp(a) levels between 30-50mg/dL suggest a moderate risk, while levels of 51-180mg/dL indicate high risk, and concentrations exceeding 180mg/dL are linked to a very high cardiovascular risk. Currently, there are no approved therapies specifically indicated to lower Lp(a). Recent evidences indicate a role of aspirin in reducing cardiovascular events in primary prevention in patients with high Lp(a). In this review, we disc

Lipoprotein(a): Particle vs mass and prevalent cardiovascular disease in the cardiovascular risk in Young Finns study population.

BACKGROUND: Lipoprotein(a) [Lp(a)] can be reported as mass (mg/dL) or particle concentration (nmol/L). Because Lp(a) acts at the particle level, molar concentration may better reflect biologically relevant exposure. OBJECTIVE: To compare molar- and mass-based Lp(a) in relation to cardiovascular outcomes, risk classification, LPA genetic variation, and apo(a) isoform phenotype in the Young Finns Study. METHODS: Molar Lp(a) was measured using a traceable Roche Tina-quant Gen.2 assay and mass using a nontraceable Thermo Fisher assay. Associations with cardiovascular outcomes and aortic valve st

Coronary microvascular dysfunction in patients undergoing transcatheter aortic valve implantation: A systematic review and meta-analysis.

BACKGROUND: Coronary microvascular dysfunction (CMD) is a complication of severe aortic stenosis. Its prevalence after transcatheter aortic valve implantation (TAVI) and prognostic significance are understudied, as are the post-TAVI trajectories of microvascular indices. AIMS: The aims of this study were to determine the pooled prevalence of CMD, the influence of the diagnostic modality, the post-TAVI coronary flow reserve (CFR) and microvascular resistance changes, and the prognostic impact of baseline CMD on major adverse cardiovascular events (MACE) in patients with severe aortic stenosis

Diagnostic Performance of a Deep Learning Model Using Phonocardiogram and Single-Lead ECG Signals for Detecting Moderate-or-Greater Aortic Stenosis: Multicenter External Validation.

BACKGROUND: Valvular heart disease, particularly aortic stenosis (AS), often remains asymptomatic until advanced stages, making early detection challenging. Although cardiac auscultation is a traditional screening tool, its diagnostic accuracy is limited. We aimed to externally validate a deep learning-based model (eAS model) using phonocardiogram (PCG) and single-lead electrocardiogram (ECG) signals to detect moderate-or-greater AS. METHODS: PCG and ECG recordings were acquired using a PCG device (Cardio-EGG). Transthoracic echocardiography served as the reference standard. The external vali

RNA-Based Therapies for Lipoprotein(a) Reduction: Antisense Oligonucleotides, Small Interfering RNAs, and Emerging Platforms.

PURPOSE OF REVIEW: Lipoprotein(a) [Lp(a)] is an independent, genetically determined, and causal risk factor for atherosclerotic cardiovascular disease (ASCVD) and calcific aortic valve stenosis. Elevated Lp(a) affects approximately 20% of the global population and is not meaningfully modified by lifestyle intervention or existing lipid-lowering pharmacotherapy. This review summarizes the biology of Lp(a) as a therapeutic target, evaluates the efficacy of currently available lipid-lowering agents against this target, and describes the mechanistic principles, clinical evidence, and unresolved qu

Restoration of frequency-dependent myocardial reserve after TAVI in severe aortic stenosis: an invasive hemodynamic study.

Severe aortic stenosis (AS) and chronic pressure overload may impair frequency-dependent myocardial reserve despite preserved left ventricular ejection fraction (LVEF). We investigated whether acute afterload reduction with transcatheter aortic valve implantation (TAVI) restores force-frequency relationships (FFR) and relaxation frequency relationships (RFR). In this prospective study, patients with severe normal-flow, high-gradient AS and preserved LVEF underwent invasive left ventricular pressure recording during incremental atrial pacing at 80 and 100 beats/min before and immediately after
