Incidence of Worsening Aortic Regurgitation After Mapping and Ablation in the Aortic Cusps.

BACKGROUND: Ventricular arrhythmias from the aortic cusps (ACs) represent a subset of left ventricular outflow tract arrhythmias. Although ablation in this region can be highly effective, concerns persist regarding aortic valve injury and iatrogenic aortic regurgitation (AR). Contemporary data on AR incidence after AC mapping/ablation remain limited. OBJECTIVES: This study sought to determine the incidence, severity, and clinical significance of AR in a multicenter cohort after mapping and/or radiofrequency ablation in the ACs or commissures. METHODS: We retrospectively analyzed patients at

Reoperation after previous aortic valve repair in children: Redo repair versus Ross procedure.

OBJECTIVE: The study objective was to compare the long-term outcomes of redo aortic valve repair versus the Ross procedure in children requiring reintervention after prior aortic valve repair. METHODS: We performed a retrospective, single-center cohort study of patients aged less than 18 years who underwent primary aortic valve repair between 1980 and 2024. Patients were included if their first aortic valve reoperation was redo repair or a Ross procedure. Outcomes of interest were survival free from heart transplantation, freedom from semilunar valve reoperation, and freedom from recurrent 2+

Bentall Procedure in a Patient with Loeys-Dietz Syndrome: A Case Report.

Loeys-Dietz syndrome (LDS) is a rare autosomal dominant connective tissue disorder characterized by aggressive aortopathy, craniofacial anomalies and systemic involvement. Rapid aortic root dilatation and valvular dysfunction mandate an early surgical intervention. We report a case of 15-year-old boy with LDS, rapidly progressive aortic root dilatation and severe aortic regurgitation (AR) who underwent successful Bentall procedure. The case highlights the need of early detection, multidisciplinary involvement and meticulous anesthetic care.

EACTS Expert Consensus Document on aortic valve repair and valve-sparing aortic root replacement procedures.

Aortic valve repair is a complex and evolving field that requires multidisciplinary teamwork among heart specialists to diagnose and treat various causes and mechanisms of aortic regurgitation and proximal thoracic aortic aneurysms across all age groups. From a surgical standpoint, aortic valve repair includes all procedures aimed at restoring or maintaining native valve function. This European Association for Cardio-Thoracic Surgery (EACTS) Expert Consensus Document critically evaluates the current evidence on the common causes and mechanisms of aortic regurgitation, the use of echocardiograp

Clinical Outcomes and Valve Performance of Intra-Annular Self-Expanding Transcatheter Aortic Valve in Valve-in-Valve Procedures: Insights From the NAViV Registry.

BACKGROUND: Most evidence for transcatheter aortic valve replacement (TAVR) for valve-in-valve procedures comes from balloon-expandable and supra-annular self-expanding valves. However, data on intra-annular self-expanding valves remain limited. The objectives of this registry were to evaluate the short- and midterm outcomes of ViV-TAVR using the intra-annular self-expanding valve Navitor platform in a real-world setting. METHODS: The NAViV (Navitor Valve-in-Valve) registry is a retrospective, multicenter study that included 200 patients who underwent valve-in-valve TAVR with the Portico/Navi

Rehabilitation Course Through Compassionate Use of JenaValve Trilogy Heart Valve System in a Patient With Left Ventricular Assist Device: A Case Study.

Aortic insufficiency (AI) develops in approximately one-third of patients 3 years after continuous-flow left ventricular assist device (LVAD) implantation. Transcatheter aortic valve replacement (TAVR) has recently emerged as a potential option. However, the use of TAVR in patients with LVADs is limited. There are currently only two transcatheter valves, J-Valve and Trilogy Valve, available under expanded access for aortic regurgitation, particularly for patients with LVAD. This case describes a 47 year old male with destination HeartMate 3 LVAD complicated by methicillin-susceptible Staphyloc

Quadricuspid Aortic Valve Presenting with Aortic Regurgitation in Adulthood: A Case Report.

Quadricuspid aortic valve (QAV) is a rare congenital cardiac anomaly characterized by four aortic cusps instead of the normal three. It is most commonly associated with progressive aortic regurgitation and is often diagnosed incidentally on echocardiography. A woman in her fifties presented with palpitations and exertional dyspnea (New York Heart Association Class II). Clinical examination revealed an early diastolic murmur at the left sternal border. Transthoracic echocardiography demonstrated a QAV with Hurwitz and Roberts Type F morphology and moderate aortic regurgitation with preserved le

Early Multicenter Outcomes of Dedicated Transcatheter Aortic Valve Replacement for Left Ventricular Assist Device-Associated Pure Aortic Regurgitation.

BACKGROUND: Aortic regurgitation (AR) is a frequent and clinically relevant complication in patients with left ventricular assist devices (LVAD), impairing circulatory support and worsening outcomes. Surgical aortic valve replacement often carries prohibitive risk in this population, while conventional transcatheter aortic valve replacement (TAVR) is limited by the lack of annular calcification. The JenaValve Trilogy is the first transfemoral TAVR system with regulatory approval for AR. Evidence on its use in LVAD patients remains scarce. METHODS: We conducted a retrospective multicenter regi

Early predictors of aortic valve surgery in patients with significant bicuspid aortic regurgitation without initial surgical indication.

BACKGROUND: Bicuspid aortic regurgitation (AR) is common in younger patients who often do not meet guideline-based criteria for aortic valve (AV) surgery at diagnosis. However, identifying early predictors of disease progression may aid in risk stratification and surgical timing. METHODS: From a single-centre registry of 1927 patients with bicuspid AV, we identified 335 patients with moderate or severe AR, excluding those with severe aortic stenosis (AS), endocarditis or other major valvular diseases. Among them, 199 patients (mean age 52±14.0 years; 80% male) did not initially meet the surgi

Impact of diastolic left ventricular strain rate on assessment of aortic regurgitation severity and timing of surgical intervention in patients with preserved left ventricular ejection fraction.

AIMS: In advanced chronic aortic regurgitation (AR), left ventricular (LV) volume/pressure overload leads to LV hypertrophy and heart failure. Echocardiography often reveals gradual and continuous enlargement of the LV throughout diastole as AR adds to LV inflow. The severity, cardiac overload, and timing of therapeutic interventions in AR patients remain controversial. Here, we investigated mid-diastolic LV strain rate (SRmin) as a measure of LV load due to AR, its relationship to conventional AR measures, and its impact on surgical intervention. METHODS AND RESULTS: This single-centre retro

Relationship between the left-to-right ventricular volume ratio and aortic regurgitation severity: an echocardiographic and cardiac magnetic resonance imaging study.

AIMS: Left ventricular (LV) enlargement in chronic aortic regurgitation (AR) is commonly assessed using diameters and volumes. However, these measures are influenced by body size, sex, and age. The left-to-right ventricular end-diastolic volume ratio (LV/RV ratio), assessed by cardiac magnetic resonance imaging (CMR) and known to remain close to 1 in healthy individuals, could provide a more individualized marker of LV remodeling in chronic AR. METHODS AND RESULTS: This bi-centre study included 258 patients with chronic AR (median age: 55 years, 18% women) who underwent echocardiography (Echo

Outcomes of moderate mixed aortic valve stenosis and regurgitation.

BACKGROUND AND AIMS: There are limited data on mortality in patients with moderate mixed aortic valve disease (MAVD), defined as the combination of moderate aortic stenosis (AS) and moderate aortic regurgitation (AR). Consequently, current guidelines lack specific recommendations for aortic valve replacement (AVR) in this population. This study aims to compare survival between moderate MAVD and isolated severe AS or severe AR, and to evaluate the impact of symptoms or left ventricular ejection fraction (LVEF) < 50%, as current criteria for AVR in severe AS or AR. METHODS: Overall, 1926 patien

Lipoprotein(a) and long-term structural valve degeneration of aortic bioprostheses.

AIMS: Structural valve degeneration (SVD) is the leading cause of late bioprosthetic valve failure. Lipoprotein(a) [Lp(a)] contributes to native aortic valve calcification, but its role in SVD is unclear. We investigated whether elevated Lp(a) is associated with SVD after bioprosthetic aortic valve replacement (AVR) and whether this differs between stenotic and regurgitant phenotypes. METHODS AND RESULTS: We studied 174 bioprosthetic AVR patients with available Lp(a) levels over a median echocardiographic follow-up of 7.3 years (1372 studies). SVD was defined by VARC-3 criteria, and associati

Natural history of asymptomatic moderate or severe aortic regurgitation: a systematic review and meta-analysis.

BACKGROUND: The natural history of asymptomatic moderate or severe aortic regurgitation (AR) remains uncertain, with conflicting reports about its progression and surgical timing. We aimed to quantify adverse outcomes under conservative management and evaluate the association of aortic valve replacement/repair (AVR) with mortality. METHODS: Systematic searches (inception-July 2025) identified cohort studies of asymptomatic moderate/severe AR. Random-effects models estimated pooled incidence rates of adverse events; fixed-effects models were used for hazard ratios (HRs) of AVR vs conservative

Fatal Aortic Regurgitation in Behçet's Disease: A Case Report Highlighting Pitfalls and Lessons in Preoperative Diagnosis.

INTRODUCTION: Behçet's disease (BD), a chronic multisystem inflammatory disorder, rarely involves the heart. Aortic regurgitation (AR) is the predominant valvular lesion. When AR precedes characteristic mucocutaneous symptoms, misdiagnosis and treatment delays often occur. CASE PRESENTATION: We, herein, report the case of a 37-year-old male presenting with isolated aortic regurgitation (AR) as the initial manifestation of Behçet's disease (BD). Initial echocardiography revealed severe eccentric AR with left coronary cusp prolapse and a vegetation-like lesion, raising suspicion of infective en

The role of CMR in the timing of aortic valve interventions and risk stratification in aortic regurgitation: a systematic review and meta-analysis.

AIMS: Aortic regurgitation (AR) is a prevalent valvular disease. Cardiovascular magnetic resonance (CMR) imaging is emerging as an accurate and precise method for assessing AR. However, its role in guiding interventions and risk stratification for outcomes remains to be fully defined. OBJECTIVE: This systematic review and meta-analysis evaluate the predictive utility of CMR-derived AR fraction (ARF) in determining intervention timing and clinical outcomes. METHODS AND RESULTS: A systematic search identified observational studies assessing CMR-derived ARF in AR prognostication. Hazard ratios

Evaluation of aortic regurgitation severity using flow convergence from the parasternal window: comparison with cardiac magnetic resonance and traditional Doppler parameters.

AIMS: To assess whether two-dimensional measurements of flow convergence (FC) from the standard parasternal long-axis (PLA) window relate to aortic regurgitation (AR) severity. METHODS AND RESULTS: Patients with chronic AR who underwent transthoracic echocardiography and cardiac magnetic resonance imaging (CMR) within 4 h were included. FCPLA was assessed using 3 parameters: the maximum length (Lmax), width (Dmax), and hemispheric area. Traditional echo/Doppler parameters of AR severity were measured per guidelines. FC and other echo/Doppler results were compared to CMR-derived regurgitant vo

Commissural Prolapse: Combining the Best From Two Aortic Valve Repair Techniques.

Aortic valve commissural avulsion is an exceptionally rare and often underdiagnosed cause of aortic regurgitation. We report a 30-year-old male with severe chronic aortic regurgitation and a mildly dilated aortic root, in whom intraoperative inspection revealed spontaneous avulsion of the right-noncoronary commissure. The patient underwent valve-sparing aortic root replacement using a combined approach: remodelling root replacement with external annuloplasty according to the Lansac technique, and targeted commissural reinforcement based on David principles. Postoperative and 6-month imaging sh
