[Cesarean section in severe double valvular disease].

During pregnancy haemodynamic changes are a stress to the cardiovascular system. Women with previously asymptomatic cardiovascular disease may develop life-threatening cardiac failure because of the extra demands of pregnancy. Early diagnosis, close control and treatment during pregnancy, delivery and the postpartum period are essential. We report a case where a woman with a mitral stenosis and insufficiency of the mitral and aortic valves gave birth to a child by caesarian section.

[Diagnostic value of various echo- and Doppler echocardiography methods in quantifying mitral valve stenosis].

UNLABELLED: Two-dimensional and Doppler echocardiography have been widely used for noninvasive determination of valve area in patients with mitral stenosis. Recent studies have indicated that the Doppler-echocardiographic pressure half-time method (PHT) does not accurately predict mitral orifice area (MOA). Therefore, applications of the continuity equation and the Gorlin formula have been used additionally to the PHT for Doppler-echocardiographic assessment of MOA. In a prospective study of 34 patients MOA determined by two-dimensional and Doppler echocardiography was compared with MOA measur

[Surgical treatment of traumatic rupture of the aortic valve].

Rupture of the aortic valve after a blunt chest trauma is rare. A 63-year-old man was injured in an automobile accident and was hospitalized elsewhere due to acute respiratory failure 4 days after the accident. On admission, he was intubated immediately. The patient did not respond to available medical treatments and was therefore transferred to our hospital. His condition was diagnosed as severe left heart failure, and an emergency operation was performed. The right coronary and non-coronary cusps were perforated and the commissure between the right coronary and non-coronary cusps was torn. T

[An evaluation of aortic root reconstruction using Carrel patch method with coronary anastomosis].

During the past 4 years, from January 1991 to December 1994, we experienced 12 cases of aortic root reconstruction utilizing Carrel patch method. The cases included annulo-aortic ectasia (AAE), root aneurysm with aortic regurgitation (AR), and aortic dissection with AR. The aortic valve and aortic root were resected, the coronary ostia were dissected free, mobilized, and then implanted to the composite graft. Coronary anastmosis was performed by mattress suture and reinforced by Tefron felt strips. It was necessary in one case to undergo coronary artery bypass grafting for myocardial ischemia

[Retrograde aortic dissection during cardiopulmonary bypass a case report successfully treated by contralateral femoral cannulation].

Retrograde aortic dissection is the most critical complication associated with femoral perfusion. It may be related to the trauma of femoral cannulation per se or to the jet of retrograde perfusion. We report the successful repair of a retrograde aortic dissection by composite graft replacement of the dissected ascending aorta using contralateral femoral cannulation. A 66-year-old man presented with aortic regurgitation due to annuloaortic ectasia. At the operation, the right common femoral artery was cannulated, and cardiopulmonary bypass was initiated. Within a few minutes after beginning th

[Pathophysiology and complications of aortic dissection].

The dissection is termed Type A according to the Stanford classification, if the ascending aorta is involved. It is termed type B, if the ascending aorta is not involved. Most patients with Type A aortic dissection die from intrapericardial rupture with cardiac tamponade, free pleural rupture, massive aortic regurgitation, or coronary or cerebral malperfusion (ischemic heart disease or stroke). Most patients with Type B dissection die from free pleural rupture or renal or visceral vascular complications. The resultant compromise of various aortic branches (inomunate, carotid, subclavian, spina

Increased risk of heart valve regurgitation after mediastinal radiation for Hodgkin's disease: an echocardiographic study.

OBJECTIVE: To assess by echocardiography the occurrence and degree of late cardiac sequelae after treatment for Hodgkin's disease by radiation and chemotherapy. PATIENTS AND METHODS: In Norway from 1980 to 1988, 129 patients < 50 years old with Hodgkin's disease had curative treatment with mediastinal radiation, with or without chemotherapy. 116 (90%) of these patients (mean (SD) age 37 (7) years, 67 males) were examined by echocardiography 5-13 years after treatment. 40 healthy individuals (mean (SD) age 40 (11), 20 males) were examined as controls. All those examined were in regular sinus r

The effect of heart rate on the slope and pressure half-time of the Doppler regurgitant velocity curve in aortic insufficiency.

The effect of the heart rate on the Doppler aortic regurgitant velocity curve was evaluated in 14 patients with aortic regurgitation. The heart rate was increased in two steps with either endocardial or transesophageal pacing in 12 patients and with atropine sulfate in 2 patients (increased from 66 +/- 7 to 82 +/- 4 beats per minute [step 1] to 100 beats per minute [step 2]) in all patients. The increased heart rate resulted in an increased regurgitant slope (from 3.3 +/- 1.2 to 4.5 +/- 1.7 m/s2 [step 1] to 5.8 +/- 1.9 m/s2 [step 2]; p < 0.01) and a shortened pressure half-time (PHT) (from 442

Color Doppler and two-dimensional echocardiographic determination of the mechanism of aortic regurgitation with surgical correlation.

The character of the color flow Doppler jet provides information on the mechanism and pathologic elements of valve dysfunction. This has been useful in the evaluation of mitral regurgitation, a repairable problem, but has not been described comprehensively for the aortic valve. The purpose of our study was to correlate the color flow Doppler characteristics of the aortic regurgitant jet and two-dimensional echocardiographic findings of valve mobility with surgical pathology. Prepump intraoperative echocardiography and color flow Doppler echocardiography were performed on 124 patients with aort

[Three cases of coronary to pulmonary arterial fistulae combined with valvular disease].

Three cases of coronary to pulmonary arterial fistulae (CAF) combined with valvular disease are reported. The first case was 53-year-old male admitted with mitral stenosis (MS) and left atrium thrombus. CAF originating from both coronary artery and draining to pulmonary artery were found by coronary angiography. Open mitral comissurotomy (OMC), thrombectory and ligation of CAF were performed. Case 2 was 52-year-old female suffered from CHF due to MS. Abnormal communication from left coronary artery to pulmonary artery was demonstrated. OMC and ligation of CAF were performed. Case 3 was 65-year

The Ross operation: applications to children.

The Ross operation is the operation of choice for children requiring an aortic valve replacement. It is associated with a low surgical mortality and late morbidity and does not require anticoagulation. It allows a growing child to have a near-normal lifestyle with a limited risk of reoperation for autograft valve or homograft valve failure.

Geometric mismatch of the aortic and pulmonary roots causes aortic insufficiency after the Ross procedure.

BACKGROUND: Geometric mismatch between the two semilunar valves can cause aortic insufficiency after the Ross procedure. Thus, whenever the aortic root is larger than the pulmonary root, surgical reduction of the aortic anulus or of the sinotubular junction (or both) to match the diameters of the pulmonary root is necessary to prevent late malfunction of the pulmonary autograft. METHODS: The Ross procedure was performed in 81 patients during the past 5 years. The diameters of the aortic and pulmonary roots were measured in 77 patients. Reduction of the aortic anulus and of the sinotubular jun

Vasoactive drugs in chronic regurgitant lesions of the mitral and aortic valves.

This review examines the results of vasodilator therapy in patients with chronic regurgitant lesions of the aortic and mitral valves. The analysis includes those studies which provide data on hemodynamic measurements, left ventricular systolic function, ventricular volumes and regurgitant flow. In patients with chronic aortic or mitral regurgitation, the short-term administration of nitroprusside, hydralazine, nifedipine or an angiotensin-converting enzyme (ACE) inhibitor produces salutary hemodynamic effects. The major difference in the response to combined preload and afterload reduction (i.

Myocardial sympathetic denervation prevents chamber-specific alteration of beta-adrenergic transmembrane signaling in rabbits with heart failure.

OBJECTIVES: The purpose of this study was to assess the effect of myocardial sympathetic denervation on the chamber-specific alteration of beta-adrenergic signaling in left ventricular failure in rabbits. BACKGROUND: Local abnormalities in sympathetic nerve terminals, including the neuronal reuptake of norepinephrine, are thought to be responsible for the chamber-specific regulation of beta-adrenergic signaling in heart failure. METHODS: Sixteen rabbits were given 6-hydroxydopamine, 25 mg/kg body weight intravenously on days 1 and 2 and 50 mg/kg intravenously on days 7 and 8. Another 16 rabb

Transesophageal echocardiographic evaluation of native valvular disease and repair.

Surgery for valvular heart disease corrects systolic or diastolic dysfunction of the mitral, aortic, or tricuspid valves. The intraoperative echocardiographic assessment of the native heart valve is aimed at defining the pathology of valve disease, determining the mechanism of valve dysfunction, and quantitating the degree (grade) of valvular stenosis or insufficiency.

Aortic valve replacement with a pulmonary autograft in infants with critical aortic stenosis.

Seven patients with critical aortic stenosis underwent aortic valve replacement with a pulmonary autograft (the Ross operation) between the ages of 5 weeks and 9 months. The operation was considered mandatory for survival because of continued severe heart failure or valve avulsion. Six of the patients had undergone unsuccessful previous palliations, such as commissurotomy, balloon dilation, and transventricular valvotomy, performed singly (n = 1) or in combination (n = 5). The other patient with a severely hypoplastic aortic valve ring underwent the Ross procedure as a primary operation. Two o

[Quadricuspid aortic valve: diagnosis by multiple transesophageal echocardiography].

The case of a quadricuspid aortic valve diagnosed in adult age is reported. A 67-year-old patient, who had no previous diseases or cardiovascular complaints, presented in the clinic for an embolic occlusion of the left retinal artery. Isolated moderate aortic regurgitation was diagnosed clinically and echocardiographically. For its further evaluation and for seeking embolic sources, multiplane transesophageal echocardiography was performed, which discovered a quadricuspid aortic valve as the cause of aortic regurgitation and major atherosclerotic lesions in the ascending aorta and the aortic a
