Multidisciplinary approach to prolonged in-hospital cardiac arrest (IHCA) managed by ECPR and emergent valve-in-valve transcatheter aortic valve implantation.

IntroductionExtracorporeal Cardiopulmonary Resuscitation (ECPR) is a lifesaving intervention for in-hospital (IHCA) and out-of-hospital cardiac arrest (OHCA). However, severe aortic regurgitation (AR) has traditionally been considered an absolute contraindication for veno-arterial extracorporeal membrane oxygenation (V-A ECMO).Case reportA 63-year-old man with aortic regurgitation experienced IHCA with 50 min of low-flow time. The patient received ECPR and emergent valve-in-valve (ViV) transcatheter aortic valve implantation (TAVI) through a rapid multidisciplinary approach. Despite transient

Ross Procedure After Arterial Switch Operation?

Arterial switch operation (ASO) has been the standard anatomic repair for D-transposition of the great arteries (d-TGA) since Jatene et al. first described it in 1975. While survival is excellent, some patients develop late complications requiring reoperation, including neo-aortic regurgitation (neo-AR) and root dilatation (ARD). Mechanisms of neo-AR are multifactorial, and management depends on whether the pathology is leaflet- or root-driven. When neo-AR is secondary to ARD, options include valve-sparing root replacement (VSRR) or composite root replacement (Bentall procedure) using either a

Late adverse cardiovascular events after aortic valve-sparing operations in patients with Marfan syndrome.

OBJECTIVES: This study examines the very long-term outcomes of aortic valve-sparing operations for the treatment of aortic root aneurysm in patients with Marfan syndrome at a high-volume center. METHODS: Among 215 patients with aortic root aneurysm associated with Marfan syndrome, 209 (97%) had aortic valve-sparing operations. Patients have been followed prospectively for a median of 14.6 years, with 26 patients remaining at risk at 25 years. All-cause mortality was estimated using the Kaplan-Meier survival method. Competing-risk models, along with the cumulative incidence function, were used

Dedicated versus non-dedicated transcatheter valves for pure native aortic regurgitation: a single-centre experience.

BACKGROUND: Aortic regurgitation (AR) is the third most common valvular heart disease in the Western world. Management of high-surgical-risk patients is challenging. The absence of calcifications in the aortic valve presents anchoring challenges for transcatheter aortic valve implantation (TAVI) in patients with severe native AR. The Trilogy device is a dedicated transcatheter heart valve for the treatment of AR and anchors by clipping onto the native aortic valve leaflets. Real-world data comparing dedicated vs. non-dedicated THVs remains scarce. METHODS: We evaluated 42 consecutive high- or

Ten-year outcomes of valve-replacing versus valve-sparing aortic root replacement in patients with Marfan syndrome: An observational multicenter study.

OBJECTIVE: We compared 10-year outcomes of aortic valve-replacing (AVR) and aortic valve-sparing (AVS) root replacement in patients with Marfan syndrome (MFS). METHODS: Patients who met strict Ghent diagnostic criteria for MFS and who underwent either AVR or AVS between March 1, 2005, and December 31, 2010, were enrolled in a 3-year prospective observational study. The follow-up, which was clinical and echocardiographic, was later extended to 20 years. Time-to-event analyses were performed with and without inverse probability weighting to adjust for baseline differences; comparison between gr

Novel coronary ostia-oriented classification for quadricuspid aortic valve: insights from a case report and literature review.

BACKGROUND: Quadricuspid aortic valve (QAV) is a rare congenital anomaly. While existing classifications (Hurwitz, Nakamura) describe cusp morphology, they fail to address coronary ostial involvement, a major determinant of surgical risk. METHODS: We present a 38 year-old woman with severe aortic insufficiency (AI) due to Type 3 QAV (Nakamura classification) who underwent aortic valve replacement. Preoperative echocardiography confirmed four equal cusps. Based on this case and literature review, we developed the University of Illinois at Chicago (UIC) classification system for QAV, focused on

Repair of a unicuspid aortic valve with root stabilization.

A unicuspid aortic valve morphology is a relatively rare variant and may require surgical correction early in life for regurgitation. In the majority of cases, a patch is required for repair, which degenerates and thus limits the durability of valve repair. The bicuspidization of a unicuspid aortic valve without the use of patch material is possible in cases with a symmetric cusp nadir. Unicuspid aortic valves are often accompanied by root enlargement and require root stabilization. Whenever possible, we repair these valves without the use of patch material and with annular stabilization by us

Ascending aortic dilation and aortic regurgitation - a neglected problem among single-ventricle patients after Fontan palliation.

BACKGROUND: The Fontan procedure revolutionized the management of univentricular heart physiology but is associated with long-term complications. Although pulmonary vascular disease, atrioventricular valve regurgitation, and ventricular dysfunction are well-recognized risks, aortic dilation and aortic regurgitation (AR) are emerging concerns. This study aimed to assess the prevalence and risk factors for aortic dilation and AR in adult post-Fontan patients. METHODS: We retrospectively analyzed 101 single-ventricle patients who underwent Fontan palliation and were hospitalized at NIKARD betwee

Contemporary Overview of Aortic Regurgitation in Young Individuals: Insights From Real-Life Evidence.

Aortic regurgitation (AR) in young adults (aged 18-44 years) is often under- or misdiagnosed and is not always benign. As in older populations, prognosis depends on the severity of regurgitation, the underlying cause, early left ventricular dysfunction, and the chosen intervention strategy. Prompt diagnosis and timely treatment improve outcomes, yet the optimal timing for surgery in asymptomatic chronic AR remains uncertain. Recent studies have proposed new thresholds for left ventricular structural and functional assessment to better evaluate the impact of AR on prognosis and guide interventi

Eosinophilic vasculitis of aortic valve in EGPA: a life-threatening onset of disease.

OBJECTIVE: Eosinophilic granulomatosis with polyangiitis (EGPA), formerly known as Churg-Strauss Syndrome, is an anti-neutrophil cytoplasmic antibodies (ANCA)-associated vasculitis that can manifest with a wide spectrum of clinical presentations. Indeed, EGPA is characterized by eosinophil-rich inflammation and systemic necrotizing vasculitis affecting small-to-medium-size vessels of different organs. Cardiac involvement is associated with a poor prognosis, and it is mainly described in the form of myocardial eosinophilic infiltration. Aortic valve involvement of EGPA is exceedingly rare. MET

Aortic insufficiency and right heart failure in patients treated with durable left ventricular assist devices after preoperative support with microaxial flow devices.

PURPOSE: Temporary mechanical circulatory support (tMCS) with microaxial flow pumps (mAFP) reverses the shock state and improves hemodynamics prior to durable left ventricular assist device (dLVAD) implant. Aortic insufficiency (AI) and right heart failure (RHF) are recognized sourced of late morbidity and mortality during dLVAD support. We examined the impact of tMCS with mAFP on AI and RHF progression given their transaortic deployment and unique interaction with the aortic valve and right ventricle. METHODS: The Society of Thoracic Surgeons Intermacs Database was queried for all fully magn

Valve-sparing Aortic Root Replacement: Defining High-volume Centres Using Prospective Data.

OBJECTIVES: Valve-sparing aortic root replacement (VSARR) is technically demanding and may vary in institutional procedural volumes, rendering the establishment of thresholds for centres of expertise challenging. This study aims to identify an annual case volume that could define a high-volume centre for VSARR using data from the Heart Valve Society Aortic Valve (HVS AV) database. METHODS: All consecutive elective patients undergoing VSARR within the HVS AV database were included. The primary early outcome was a composite of mortality, thromboembolic events, reinterventions, intraoperative co

Transcatheter Aortic Valve Implantation for Pseudoaneurysm of the Mitral-Aortic Intervalvular Fibrosa Causing Severe Aortic Regurgitation.

Pseudoaneurysm of the mitral-aortic intervalvular fibrosa (P-MAIVF) is a rare complication following infective endocarditis or valve surgery. We report the case of a 65-year-old male who developed a P-MAIVF following surgical mitral valve replacement for infective endocarditis. Shortly after the surgery, although the patient remained afebrile and blood cultures were negative, routine transthoracic echocardiography revealed new-onset severe aortic regurgitation. Multimodality imaging confirmed a P-MAIVF extending into the non-coronary sinus and the left ventricular outflow tract, causing valvul
