The favorable results of mitral valvuloplasty when compared with valve replacement have renewed the interest of many surgeons in aortic valve repair. However, these efforts have, for the most part, been unsuccessful. Also, the results of aortic valve replacement are usually better than those of mitral valve replacement. HISTORY AND CLINICAL FINDINGS: A 58-year-old man was admitted after he was found to have a huge aneurysm of the thoracic aorta. 38 months previously he had first experienced subacute pain in the right thorax. After this the aneurysm gradually increased in size. On admission a pulsating mass, 20 cm in diameter, was obvious, having broken through the right ventral thoracic wall. It was causing venous inflow congestion in the upper part of the body. The patient's general condition was poor: he had marked orthopnoea.

The Ross operation is physiologically the best approach for aortic valve replacement. At the Hospital de Santa Cruz 22 consecutive pulmonary autograft operations have been performed in patients with a mean age of 49 (range 17-65) years. Six patients had mitral valve disease, two had aortic aneurysms and one had a ventricular septal defect. Subcoronary implantation of the autograft was performed in 20 patients. We report a 35-year-old Marfan patient who underwent David's operation for annuloaortic ectasia and aortic regurgitation. BACKGROUND AND AIMS OF THE STUDY: Aortic insufficiency due to dilation of the aortic root is a common finding. Although combined replacement of the aortic valve and the ascending aorta is regarded as standard treatment, total replacement of the proximal aorta with a tube graft and resuspension of the valve is a new alternative.

BACKGROUND: Composite aortic root replacement is accepted treatment for aneurysms of the ascending aorta involving the root with aortic valve regurgitation, but controversy continues regarding the best technique of operation. We excise the aneurysm, implant a composite valve graft, directly attach the coronary arteries to the aortic graft, and make the distal anastomosis to the divided aorta.
METHODS: We reviewed the records and collected complete follow-up data on 110 consecutive patients having composite aortic root replacement with this technique from 1979 to 1995.

Between October, 1979 and September 1994, 18 consecutive patients with Marfan's syndrome underwent replacement of the ascending aorta and aortic valve with a composite graft. There were 11 men and seven women; the mean age was 38.7 years. All patients had annuloaortic ectasia with significant aortic regurgitation. Ten of the 18 patients had aortic dissection. We report an interesting case of aortic regurgitation. Phonocardiographically, the shape of the diastolic musical murmur in this case changed in each cardiac cycle despite being in sinus rhythm, in the same posture and in the same breathing phase. Two patients with haemodynamically significant aortic regurgitation and angiographically normal coronary arteries developed giant T wave inversion. The T wave abnormality partially resolved in both, following valve replacement in one and medical treatment in the other. This unusual electrocardiographic appearance may be related to myocardial ischaemia caused by increased left ventricular wall stress and changes in phasic coronary blood flow, abnormalities in cerebral perfusion associated with severe aortic regurgitation, or to coincidental neurological disease.
