Construction and implantation of a modified trileaflet polytetrafluoroethylene valved conduit for pulmonary valve replacement.

OBJECTIVES: Handmade expanded polytetrafluoroethylene (ePTFE) valved conduits have emerged as a durable and cost-effective option for pulmonary valve replacement. We have reported excellent results of trileaflet ePTFE valved conduit and the longest follow-up exceeded 10 years. Technical precision during valve construction and implantation is critical to optimize valve function and conduit geometry, and we recently modified the technique. We report our updated technique and clinical outcomes. METHODS: The conduit was constructed intraoperatively on a sterile back table while the patient was be

Pathogenic Variants in NKX2-5 Associated With Familial Atrial Septal Defect and Conduction System Disease.

BACKGROUND: Most atrial septal defects (ASDs) occur sporadically, but familial cases can occur and carry a 10% risk of transmission to offspring. CASE SUMMARY: We describe a 3-generation family with autosomal dominant secundum ASD and conduction system disease related to a novel NKX2-5 variant (c.300del; p.Asp101Thrfs∗75). The index patient, a 58-year-old woman with prior ASD repair and mitral valve replacement, presented with chronotropic incompetence, atrial fibrillation, left bundle branch block, and prolonged pauses, leading to pacemaker implantation. Her mother had an ASD, atrial fibrill

'Double-dunk' implantation of valveless ring-reinforced PTFE conduits for neonatal truncus arteriosus repair.

This case describes a neonatal truncus arteriosus repair using an 8 mm ring-reinforced, valved polytetrafluoroethylene conduit for right ventricular outflow tract reconstruction via the 'double-dunk' technique. The diagnosis was established prenatally in a patient with familial history of the disease. Anatomically, both pulmonary arteries arose adjacent to each other from the posterolateral common trunk; there was a mildly regurgitant quadricuspid truncal valve and an accidental finding of an intramural course of both coronary arteries.  Clinically, the neonate presented with mild cyanosis and

Direct SVC Access for Valve Replacement With Percutaneous Venotomy Suture System Hemostasis: A Way In.

BACKGROUND: Adult patients with congenital heart disease often have limited vascular access due to previous procedures and anatomic variations. CASE SUMMARY: This case describes how a percutaneously placed venotomy suture system allowed for direct percutaneous superior vena cava access for delivery sheath insertion for transcatheter pulmonary valve replacement. This system allowed for a novel access approach and the achievement of hemostasis in an otherwise noncompressible site. DISCUSSION: The availability of percutaneously placed venotomy suture placement systems allows for a more daring a

Radiographic Comparison of Pulmonary Lobar Vasculature Dimensions in Medically Managed and Drug-Naive Dogs With Left Heart Failure Secondary to Myxomatous Mitral Valve Disease.

Myxomatous mitral valve disease (MMVD) is the most common acquired cause of left heart failure (LHF) in dogs, and radiographic identification of pulmonary venous enlargement is traditionally used to support the diagnosis of LHF. However, the absence of venous enlargement has been anecdotally observed after initiation of therapy. Therefore, we aimed to compare radiographic pulmonary vessel size in LHF patients with ongoing medical management ± receiving acute rescue therapy. Dogs with echocardiographically confirmed MMVD and radiographic cardiogenic pulmonary edema were retrospectively selected

Advanced Rheumatic Heart Disease Complicated by Pregnancy, Mitral Valve Disease, Pulmonary Hypertension, and Left Atrial Thrombus: A Multidisciplinary Case Report from Resource-Limited Settings.

INTRODUCTION: Rheumatic heart disease (RHD) remains a leading cause of cardiovascular morbidity and mortality among pregnant women in resource-limited settings. Pregnancy induces significant hemodynamic changes that can decompensate pre-existing valvular heart disease, leading to life-threatening complications for both mother and fetus. CASE PRESENTATION: A 27-year-old pregnant woman (gravida 6, para 5) at approximately 34-38 weeks presented with progressive shortness of breath, palpitations. Based on the modified World Health Organization (mWHO) classification, she was categorized as Class I

Prognostic value of pulmonary artery pulsatility index for perioperative adverse cardiovascular events in patients undergoing surgery for mitral regurgitation.

BACKGROUND: The pulmonary artery pulsatility index (PAPi) demonstrates prognostic value across cardiopulmonary conditions, yet its utility in mitral regurgitation (MR) perioperative risk stratification remains undefined. This study aimed to evaluate whether the PAPi could independently predict perioperative adverse cardiovascular events in patients with MR, and to explore whether its addition to the European System for Cardiac Operative Risk Evaluation (EuroSCORE) II provided incremental value. METHODS: This ambidirectional cohort study enrolled 302 consecutive patients with moderate-to-sever

Temporary Left Ventricular Unloading With Impella as a Bridge to Emergency Surgery for Acute Mitral Bioprosthetic Failure.

Structural valve deterioration usually declares itself gradually, but a bioprosthetic leaflet that tears away from a stent post can produce catastrophic regurgitation without warning. In the mitral position, the left atrium has not had time to become compliant, and the resulting rise in pulmonary capillary pressure can flood the lungs within hours. A 68-year-old woman who had undergone mitral valve replacement with a bovine pericardial bioprosthesis 13 years earlier developed sudden dyspnea. Her oxygen saturation was 58%, the chest radiograph showed almost complete bilateral opacification, and

Assisted Reproductive Technology and Risk of Congenital Heart Defects in the Offspring: A Systematic Review and Meta-Analysis.

BACKGROUND: To date, >10 million children have been conceived via assisted reproductive technology (ART) globally. However, there are some concerns about potential adverse outcomes associated with ART-conceived pregnancies, including the risk of congenital heart defects (CHDs). This meta-analysis aimed to evaluate the association between ART and the risk of major CHDs as well as their specific subtypes. METHODS: Major electronic databases were screened for studies comparing the incidence of CHD following ART-conceived pregnancies versus spontaneous conception. The primary end point was study-

Successful staged anatomic repair for congenitally corrected transposition with both atrioventricular valves' regurgitation: a case report.

A 4-year-old boy with dextrocardia, congenitally corrected transposition of the great arteries, pulmonary valve stenosis, and ventricular septal defect demonstrated progressive hypoxemia and mild-to-moderate mitral and tricuspid regurgitation. Cardiac magnetic resonance imaging revealed insufficient right ventricular volume for Senning-Rastelli type anatomical repair. Given the elevated surgical risk associated with mitral regurgitation and inadequate ventricular volume, an initial palliative approach was undertaken at 2 years of age, consisting of systemic-to-pulmonary shunt placement and mit

Percutaneous balloon valvuloplasty in a small cat with pulmonary valve stenosis and concomitant tricuspid valve dysplasia.

A 14-month-old, male castrated, 3,9-kg Singapura cat was referred for treatment of severe pulmonary valve stenosis associated with tricuspid valve dysplasia. The cat was pretreated with atenolol and scheduled for pulmonary balloon valvuloplasty. Because the right femoral vein could not accommodate a 5 Fr introducer, vascular access was obtained via the left external jugular vein, followed by right ventricular catheterisation, pressure measurement and angiography. A 10 mm low pressure balloon catheter was advanced over a guidewire to the level of the pulmonary valve and inflated once, resulting

Development and validation of a prognostic model of postnatal circulation in fetuses with pulmonary atresia or critical pulmonary stenosis with intact ventricular septum: protocol for an observational cohort study with prospective validation.

BACKGROUND: Pulmonary atresia or critical pulmonary stenosis with intact ventricular septum (PA/CS-IVS) is a rare and heterogeneous congenital heart disease with highly variable postnatal outcomes. Existing prenatal prognostic models are limited by small sample sizes, heterogeneity in echocardiographic variables, and suboptimal validation. Robust prenatal prediction of postnatal circulation is essential for parental counselling, perinatal planning, and selection of candidates for potential fetal intervention. METHODS: This international observational cohort study will include non-intervened f

Severe pulmonary valve stenosis associated with advanced atrioventricular conduction disease: A rare case report.

Congenital pulmonary valve stenosis is a common cause of right ventricular (RV) outflow tract obstruction. Advanced atrioventricular (AV) block in congenital heart disease is usually associated with abnormalities of AV septation or conduction system malposition. An association between isolated pulmonary valve stenosis and advanced AV block has not previously been reported. A 16-year-old male presented with exertional dyspnea and recurrent presyncope. Electrocardiography (ECG) revealed 2:1 AV block. Transthoracic echocardiography demonstrated severe valvular pulmonary stenosis with a peak gradi

Critical Rheumatic Mitral Stenosis Unmasked During Pregnancy Complicated by Preeclampsia With Severe Features and Managed With Urgent Balloon Mitral Valvuloplasty.

Pregnancy imposes significant hemodynamic changes that can unmask previously undiagnosed cardiovascular disease and complicate management when concurrent obstetric conditions are present. We present the case of a 32-year-old immigrant woman at 21 weeks' gestation with a limited medical history due to a significant language barrier, who presented with hypertensive crisis and was diagnosed with preeclampsia with severe features. During her evaluation, she developed worsening dyspnea and acute respiratory decompensation. Further cardiac assessment revealed critical rheumatic mitral stenosis, acco

Transcatheter pulmonary valve implantation in patients with repaired tetralogy of Fallot: International evidence and the current status in Japan.

In repaired tetralogy of Fallot (TOF), chronic pulmonary regurgitation drives right ventricular (RV) dilation, ventricular arrhythmias, and heart failure. Surgical pulmonary valve replacement improves outcomes but requires redo sternotomy. Transcatheter pulmonary valve implantation (TPVI) has emerged as a less invasive alternative. This review summarizes contemporary evidence and the Japanese context for TPVI. International registries report high procedural success and favorable mid-term outcomes with both balloon-expandable and self-expanding platforms. In Japan, two platforms are approved-SA

Right ventricular function after repair of tetralogy of Fallot: an echocardiographic and clinical evaluation.

BACKGROUND: Right ventricular dysfunction is a major determinant of long-term outcomes after tetralogy of Fallot repair, particularly in the presence of chronic pulmonary regurgitation. Strain analysis may detect early right ventricular dysfunction more sensitively than conventional echocardiographic parameters. METHODS: This retrospective single-centre study included 63 patients after tetralogy of Fallot repair who underwent postoperative echocardiographic evaluation during mid-term follow-up. Right ventricular function was assessed using tricuspid annular plane systolic excursion, right ven

Characteristic flow of different right ventricular outflow tract morphology in repaired tetralogy of fallot patients studied by 4D-flow cardiac magnetic resonance images.

BACKGROUND: Right ventricular outflow tract (RVOT) morphology may influence hemodynamics and suitability for transcatheter pulmonic valve implantation (PPVI) in patients with repaired tetralogy of Fallot (rTOF). METHODS: We retrospectively analyzed 120 rTOF patients who underwent cardiovascular magnetic resonance (CMR) imaging with 4D-flow analysis. RVOT morphology was classified as tubular, pyramidal, or trapezoid, with inverted pyramidal and concave morphologies absent. Due to small numbers in minor groups, analysis focused on tubular (n = 86) and pyramidal (n = 22) RVOTs. Conventional CMR

Novel Bailout for Pulsta Valve Embolization: Microporous Endograft Fixation at the SVC-RA Junction in Repaired Tetralogy of Fallot With Severe Pulmonary Regurgitation.

This report describes a 47‑year‑old woman with a history of repaired tetralogy of Fallot presenting with a massively dilated native right ventricular outflow tract (RVOT) and enlarged pulmonary arteries. She underwent bilateral branch pulmonary artery valve implantation using self-expanding Pulsta valves. Early embolization of the left pulmonary artery valve necessitated a complex percutaneous rescue, including balloon anchoring, through-and-through wiring, and stepwise relocation. Following a subsequent embolization into the right ventricle resulting in tricuspid entrapment, emergent surgical

Pulmonary hypertension in patients with Noonan syndrome.

BACKGROUND: Noonan syndrome is a RASopathy inherited in an autosomal dominant manner, mainly caused by gain-of-function variants activating the RAS/mitogen-activated protein kinase signalling pathway. Pulmonary hypertension (PH) may occur in Noonan syndrome, but its mechanisms, clinical characteristics and outcomes remain poorly defined. METHODS: We analysed data from the French PH Network to characterise the phenotype of Noonan syndrome patients who develop PH, and conducted a systematic analysis of the literature. RESULTS: Seven patients were identified from the French PH Network (male/fem
