Survival and Risk Profile of Patients With Significant Tricuspid Regurgitation by Etiology.

BACKGROUND: Tricuspid regurgitation (TR) is a common valvular disorder that can affect patients' quality of life and survival. The impact of TR etiology on overall survival and the associated risk factors in each subgroup are not well studied. METHODS: Multisite, retrospective study evaluating survival based on TR etiology using Kaplan-Meier and Cox regression models. Stepwise approach was used to define TR etiology (≥moderate): primary (PTR; presence of a primary valvular pathology), lead associated (LTR; lead interfering with closure), and secondary TR (STR) in the setting of left-sided val

Hemodynamic and Metabolic-Inflammatory Phenotyping Across the Cardiac Index Spectrum in Moderate and Severe Tricuspid Regurgitation: Prognostic Implications.

BACKGROUND: Moderate and severe tricuspid regurgitation (TR) is associated with poor outcomes, yet current grading systems do not fully capture circulatory heterogeneity. We investigate the relationship of cardiac index (CI) with rest-exercise hemodynamics, metabolic and inflammatory profiles, and clinical outcomes in moderate and severe TR. METHODS: We prospectively enrolled 300 outpatients with atrial secondary, nonatrial secondary, and lead-associated moderate and severe TRs without ≥moderate left-sided valve disease. All underwent comprehensive laboratory profiling and ultrasound evaluati

Multivalvular Cardiac Involvement from Giant Hepatic Metastases of an Ileal Neuroendocrine Tumor.

Intestinal neuroendocrine tumors (NETs) are rare, slow-growing neoplasms arising from enterochromaffin cells, capable of secreting vasoactive substances that may cause carcinoid syndrome (CS) and clinically significant valvular heart disease. These tumors are often diagnosed at advanced stages due to nonspecific gastrointestinal symptoms, and distant metastases, particularly to the liver and lymph nodes, are common at presentation. Hormonal dysregulation can lead to chronic diarrhea, flushing, and bronchospasm, while carcinoid heart disease (CHD) contributes substantially to morbidity and mort

3D Transgastric Views in Transesophageal Echocardiography (TEE) to Diagnose Isolated Pulmonary Valve Endocarditis: When the Standard Views Fall Short.

Isolated pulmonary valve endocarditis (PVE) is a rare and diagnostically challenging form of right-sided infective endocarditis, often lacking the typical findings seen with left-sided or tricuspid valve involvement. We describe an 83-year-old man with end-stage renal disease on hemodialysis, who developed persistent Staphylococcus aureus bacteremia despite appropriate antibiotics and catheter exchange. Standard diagnostic methods, including physical examination, laboratory tests, and transthoracic echocardiography, were unremarkable. Initial transesophageal echocardiography (TEE) with convent

Applying the 2025 ESC/EACTS Recommendations for the Treatment of Severe Tricuspid Regurgitation to Real-World Practice.

BACKGROUND: According to the 2025 ESC/EACTS guidelines for the management of valvular heart disease, transcatheter tricuspid valve interventions (TTVI) have received a Class IIa recommendation (Level of Evidence: A) for the treatment of patients with severe symptomatic tricuspid regurgitation. However, in patients with severe left ventricular dysfunction (LVD) or right ventricular dysfunction (RVD) or precapillary pulmonary hypertension (pcPH), optimal medical therapy (OMT) is preferred because of the potential risk for futility. OBJECTIVES: The aim of this study was to evaluate clinical and

Transcatheter and surgical management of tricuspid valve disease: multidisciplinary lifetime management considerations.

Tricuspid valve disease (TVD) has long been underrecognized compared with left-sided valvular disorders, yet it represents a major determinant of cardiovascular morbidity and mortality. Tricuspid regurgitation (TR), the predominant manifestation of TVD, is being recognized with increasing frequency as populations age and the burden of left-sided heart disease rises. While mild TR is often physiological, moderate and severe TR are associated with adverse outcomes, independent of left ventricular function or pulmonary pressures. Secondary TR, driven by right atrial or right ventricular remodelin

Age-Stratified Performance of the TAPSE/sPAP Ratio as a Marker of Right Ventricular-Pulmonary Arterial Coupling in Chronic Kidney Disease.

BACKGROUND: Right ventricular (RV) dysfunction in chronic kidney disease (CKD) remains underrecognized. The tricuspid annular plane systolic excursion to systolic pulmonary artery pressure ratio (TAPSE/sPAP) reflects RV-pulmonary arterial coupling, but its relationship with CKD severity is unknown. To evaluate the association between TAPSE/sPAP ratio and CKD stage severity and assess age-related effect modification. METHODS: This cross-sectional study enrolled 120 participants: 40 patients with stage I-II CKD, 40 with stage III-IV CKD, and 40 healthy controls. All underwent echocardiography a

Brucella anthropi Endocarditis: An Unusual Pathogen.

BACKGROUND: The genus Brucella has expanded considerably in the 21st century. With the advent of advanced phylogenetic analyses, a close genetic relationship between Brucella and Ochrobactrum has been identified, leading to reclassification of Ochrobactrum species within the genus Brucella. Among these, Brucella anthropi (formerly Ochrobactrum anthropi) is increasingly recognized as a rare cause of invasive human infection. We report a clinically significant case of B. anthropi infective endocarditis and review the available literature. METHODS: We report a case of B. anthropi infective endoc

Extracorporeal life support in lung transplantation with dual-lumen cannulas: A systematic review.

BackgroundRefractory end-stage lung failure may benefit from extracorporeal life support (ECLS) as a bridge to lung transplantation. Venovenous (VV) extracorporeal membrane oxygenation (ECMO) is indicated in patients who have failed conventional medical treatment and mechanical ventilation. The Avalon Elite Bi-Caval Dual-Lumen Catheter (Getinge) and the Crescent Jugular Dual-Lumen Catheter (MC3, Inc., Medtronic) have been used for this configuration. Venoarterial (VA) ECMO may be used in patients with acute right ventricular (RV) failure, haemodynamic instability or refractory respiratory fail

Prehabilitation in frail patients undergoing cardiac valve intervention.

BACKGROUND: Management of valvular heart disease (VHD) has shifted from predominantly surgical approaches to widespread use of transcatheter interventions, including transcatheter aortic valve replacement (TAVR), mitral transcatheter edge-to-edge repair (M-TEER), transcatheter mitral valve replacement (TMVR), and emerging tricuspid transcatheter valve intervention (TTVI). Although ESC/EACTS and ACC/AHA guidelines recognize frailty as a major determinant of mortality, complications, and recovery, they offer limited guidance on how to modify frailty before intervention. CONTENT: Frailty is high

Differential effect of chronic kidney disease on right-sided extravalvular damage in low-flow, low-gradient aortic stenosis.

OBJECTIVES: In the transcatheter aortic valve replacement (TAVR) era, the presence of low-flow, low-gradient aortic stenosis (LFLG AS) itself does not preclude intervention. However, chronic kidney disease (CKD) and right-sided extravalvular damage (RSED) complicate procedural risk assessment. Both their association with each other and their joint impact on post-TAVR mortality in LFLG AS and across its subtypes remain incompletely characterized. METHODS: The authors performed a retrospective study of patients with LFLG AS who underwent TAVR between 2019 and 2022, stratified by CKD stage (from

International practices and variability in right heart echocardiography: results from the RVNet(Work) international survey.

BACKGROUND: The assessment of the right heart (RH) plays a central role in the diagnosis and management of cardiovascular and pulmonary diseases. Although current guidelines have improved standardization, there is anecdotal evidence suggesting that there is significant variability in clinical practice regarding acquisition and reporting persist. OBJECTIVES: This international survey evaluated the extent of variability in echocardiographic assessment of the RH. A simple method for the standardization of RH measurements is proposed. METHODS: The international anonymous survey consisted of 68 que

Prognostic Value of Tricuspid Regurgitation in Pulmonary Hypertension and Among the Different WHO Groups.

BACKGROUND: Secondary tricuspid regurgitation (STR) is frequently observed in patients with pulmonary hypertension (PH). Whether it independently confers mortality risk beyond the associated right ventricular (RV) dysfunction remains unknown. OBJECTIVES: The aim of this study was to evaluate the prognostic significance of STR in patients with PH other than related to left-sided valve disease or reduced ejection fraction and across the individual WHO (World Health Organization) groups of PH. METHODS: Patients with RV systolic pressures ≥50 mm Hg on echocardiography between 2010 and 2023 and m

Severe Iatrogenic Tricuspid Regurgitation.

Tricuspid regurgitation (TR) is a valvular heart disease mostly associated with structural heart disease or pulmonary hypertension (PH). Rare cases of TR secondary to exposure to specific medications, such as dopamine agonists, have been described. A 50-year-old woman presented with a pituitary microprolactinoma (without surgical indication) and a history of pulmonary embolism (PE) in 2021 (risk factors included bromocriptine, which was switched to cabergoline). An echocardiogram performed one year after the PE showed no significant abnormalities. She was maintained on rivaroxaban 10 mg due to

Rheumatic heart disease is not over: Cardiac cirrhosis and multivalvular sequelae in an endemic setting - A case series and review.

BACKGROUND: Rheumatic Heart Disease (RHD) remains a major cause of valvular morbidity in endemic regions despite declining incidence in high-income countries. While early disease manifests as acute rheumatic fever, contemporary presentations increasingly reflect advanced structural sequelae. Congestive hepatopathy and cardiac cirrhosis secondary to chronic right-sided failure remain under-recognized manifestations of advanced rheumatic valvular disease. OBJECTIVES: To describe three cases of advanced rheumatic multivalvular disease presenting predominantly with ascites and hepatic congestion

Pulmonary hypertension in dogs with myxomatous mitral valve disease.

Pulmonary hypertension (PH) is a clinically significant and increasingly recognized complication of myxomatous mitral valve disease (MMVD) in dogs, contributing to worsening morbidity and mortality. The development of PH in the context of MMVD is associated with increased symptom burden, including exercise intolerance, syncope, and respiratory distress, and has been correlated with reduced survival times. Echocardiography is the primary modality for detection and monitoring, relying on Doppler estimation of tricuspid regurgitation velocity, but limitations in sensitivity and accuracy remain. T

Tricuspid valve endocarditis in people who inject drugs: strategies to reduce mortality and recurrent infection.

Tricuspid valve infective endocarditis (TV-IE) in people who inject drugs (PWID) has become an increasingly important clinical and public health concern, driven largely by the expanding opioid epidemic. Although PWID with TV-IE are typically young and often lack major comorbidities, they experience disproportionately high rates of mortality, reinfection, and recurrent hospitalizations. Repeated intravenous injection introduces contaminated particulate matter and pathogens into the venous circulation, promoting microbial adherence to the tricuspid valve, with Staphylococcus aureus including met

When an Atrial Septal Defect Is No Longer Silent: A Case Report of a Large Symptomatic Defect Presenting in Adulthood.

Atrial septal defects (ASDs) are among the most common forms of congenital heart disease diagnosed in adulthood. Ostium secundum defects represent the majority of cases and may remain clinically silent for decades. Delayed diagnosis can lead to progressive right-sided cardiac chamber dilation, pulmonary hypertension, arrhythmias, and heart failure. The choice between percutaneous and surgical closure depends on anatomical characteristics such as defect size and the adequacy of surrounding septal rims. We report the case of a 49-year-old woman with hypertension, type 2 diabetes mellitus, and hy
