The Ross Renaissance: Trends and Short-Term Outcomes of the Ross Procedure in the United States.

BACKGROUND: The Ross procedure, involving pulmonary autograft replacement of the aortic valve, has remained underutilized. Recent technical advancements have prompted renewed interest in the Ross operation, particularly as an alternative to conventional surgical aortic valve replacement (SAVR). OBJECTIVES: The purpose of this study was to examine national utilization of the Ross procedure over the last decade and evaluate associated feasibility, safety, and cost, relative to SAVR. METHODS: We identified adult and pediatric patients undergoing elective aortic valve replacement within the 2010

Combined assessment of pulmonary artery pulsatility index and pulmonary arterial capacitance for risk stratification following transcatheter aortic valve implantation.

BACKGROUND: Right heart haemodynamics, including right ventricular function and pulmonary arterial capacitance (PAC), are important prognostic determinants in cardiovascular disease. However, their combined role in patients undergoing transcatheter aortic valve implantation (TAVI) remains unclear. This study aimed to determine whether the combined assessment using pulmonary artery pulsatility index (PAPi) and PAC can stratify clinical outcomes in patients with aortic stenosis (AS) undergoing TAVI. METHODS: This retrospective single-centre study included 77 patients with severe AS who underwen

20-Year Experience With Decellularized Pulmonary Homografts for Pulmonary Valve Replacement.

OBJECTIVES: Fresh, non-cryopreserved and non-seeded, decellularized pulmonary homografts (DPHs) were translated in 2005 into clinical practice. The aim of this current study is to summarize our 20-year clinical experience with DPH for pulmonary valve replacement (PVR) in congenital heart disease. METHODS: Prospective follow-up of all DPH implanted within 2 institutions. Indication for PVR according to current clinical guidelines was the key inclusion criterion without age limits; patients with active endocarditis were not included. RESULTS: From January 2005 to August 2025, 310 patients (188

[Infective endocarditis: spectrum of multidetector computed tomography findings].

Infectious endocarditis is an infection of the endocardium, typically involving the cardiac valves, and that presents with higher incidence in patients with degenerative and congenital valve disorders, prostheses and foreign bodies (e.g. defibrillator and pacemaker leads). Initial assessment include transthoracic echocardiography, however transesophageal echocardiography and other multiplanar imaging modalities may be needed to better delineate the presence of vegetations and other complications. Multidetector computed tomography (MDCT) is particularly useful in detection and assessment of som

Clinical Profile of a Series of Left-Sided Prosthetic Valve Endocarditis: Revisiting Surgical Indications.

Background/Objectives: Prosthetic valve endocarditis (PVE) carries a high morbidity and mortality. Surgery is classically indicated for heart failure, uncontrolled infection, or prevention of embolic events; however, evidence supporting surgery for "non-classical" indications-such as Staphylococcus aureus infection, non-HACEK Gram-negative bacteria, or early PVE-remains limited. This study aimed to update the clinical and prognostic profile of PVE and assess the impact of surgery, particularly in patients with non-classical surgical indications. Methods: We prospectively included all definite

Pulmonary Valve Replacement: Update on Timing and Ventricular Remodelling.

Chronic pulmonary regurgitation (PR) after the repair of tetralogy of Fallot (TOF) and other right ventricular outflow tract (RVOT) interventions leads to progressive right ventricular (RV) dilatation, altered ventricular-ventricular interaction, and an increased risk of arrhythmia and heart failure. Pulmonary valve replacement (PVR), whether surgical or transcatheter, effectively eliminates or reduces PR and is associated with short- and mid-term improvement in RV size, symptoms, and electrocardiographic markers. However, the optimal timing of intervention remains unresolved: operating late c

Performance of an Artificial Intelligence Convolution Neural Network Software for the Detection of Confirmed Heart Failure in Dogs and Cats.

An independent dataset was used to retrospectively test the ability of an artificial intelligence (AI) program to detect radiographic signs of right or left heart failure (HF) and combine these findings to make a radiographic assessment of HF in dogs and cats. Experimental groups included 105 confirmed cases of heart disease in failure and a control group of 40 confirmed cases of heart disease not in failure. The program had an 87.04% sensitivity and an 80% specificity for radiographically concluding HF in dogs. Compared to true findings, the program had a 98% sensitivity in detecting an inter

Black Patients Experience Higher Complication Burden and Distinct Valve Selection Patterns in Aortic Valve Replacement.

Background Young adults undergoing the Ross procedure for aortic valve replacement may have better outcomes than more conventional mechanical and bioprosthetic aortic valve replacement. This study evaluates racial disparities in access to the Ross procedure and postoperative outcomes, hypothesizing that Black patients may have differing Ross utilization due to constrained valve options, with persistent disparities in postoperative complications. Methods Cases of open aortic valve replacement (AVR) surgery between 2012 and 2021 from the American College of Surgeons National Surgical Quality Imp

Optimizing patient triage on the waiting list for transcatheter aortic valve replacement: the clinical utility of the cardiac damage staging system.

AIMS: Access to transcatheter aortic valve replacement (TAVR) is generally prioritized for symptomatic patients presenting with severe heart failure, syncope, angina, and decrease in left ventricular ejection fraction <40%. However, a substantial number of patients die while awaiting TAVR. In response to rising demand driven by an ageing population, it is essential to stratify interventions, as proposed by Généreux's Aortic Stenosis staging. METHODS AND RESULTS: We conducted a retrospective, single-center, longitudinal study in patients referred for TAVR. We specifically analysed the characte

Severe Congenital Pulmonary Valve Stenosis Diagnosed in Adulthood: A Case Report.

Pulmonary valve stenosis is a congenital heart disease that is usually diagnosed during childhood, while presentation in adulthood is uncommon and may lead to delayed diagnosis. We report the case of a 21-year-old patient with no significant medical history who presented with a six-month history of exertional dyspnea and constrictive chest pain. Electrocardiography showed right axis deviation with signs of right ventricular hypertrophy, and chest radiography revealed dilatation of the left pulmonary artery. Transthoracic echocardiography demonstrated severe valvular pulmonary stenosis with a p

Early outcomes and predictors of mortality in native aortic valve endocarditis: results from a national registry.

BACKGROUND: Native aortic valve endocarditis continues to present significant operative challenges, often complicated by heart failure, peri-annular extension and conduction disturbances. This study aimed to evaluate temporal trends, outcomes and predictors of mortality following surgery within a nationwide cohort. METHODS: A retrospective analysis was performed using data from the UK National Institute for Cardiovascular Outcomes Research registry. All patients undergoing aortic valve replacement (AVR) for infective endocarditis between 1996 and 2019 were included. The primary endpoint was i

Distribution, recognition and clinical implications of syndromic conditions in adults with congenital heart disease.

OBJECTIVE: Syndromic conditions are frequently associated with congenital heart disease (CHD). Their distribution, recognition and prognostic impact in adults with CHD (ACHD) remain poorly characterised due to limited assessment beyond childhood. METHODS: We retrospectively analysed 9826 patients with ACHD from a tertiary ACHD centre. Syndromic conditions were identified from clinical records and categorised as chromosomal, single-gene or other syndromes. Clinical characteristics, comorbidities and CHD complexity were compared between syndromic and non-syndromic patients. Multivariable Cox re

Transapical TAVI in Left Ventricular Apical Aneurysm.

Transcatheter aortic valve implantation (TAVI) was the preferred intervention for patients with severe aortic stenosis at high surgical risk and recently included intermediate and even low surgical risk patients. The transfemoral (TF) route is the standard approach, with alternative access reserved for patients with challenging peripheral artery disease. The transapical (TA) approach is rarely performed, especially in patients with left ventricular (LV) apical aneurysm due to risk of myocardial rupture and bleeding secondary to fibrosed myocardial tissue. We are presenting the first reported c

Pulmonic Valve Insufficiency After Repair of Tetralogy of Fallot: A Case Report.

Repair of Tetralogy of Fallot (TOF) has significantly improved long-term mortality. A consequence of repair is pulmonic valve insufficiency, leading to cardiac remodeling in adulthood, which can often remain asymptomatic for an extended period. This emphasizes the importance of consistent follow-up with an adult congenital heart disease physician to evaluate for these pathophysiologic consequences. We present a case of a young female patient with a history of TOF status post-repair who was lost to follow-up.

Beyond the Usual Suspects: IgG4-Related Disease as a Rare Culprit in Cardiac Valvular Disorders.

Cardiologists consider degenerative or infectious causes when evaluating valvular heart disease. However, the role of autoimmune disorders, though less frequent, remains clinically significant. This report describes a young male patient presenting with persistent coronary disease and a suspected valvular cusp perforation initially attributed to infective endocarditis, which ultimately proved to be a manifestation of IgG4-related disease. IgG4-related disease is a rare condition, more prevalent in Asian populations, that typically affects the pancreas, salivary glands, lacrimal glands, and the

One Health Insights into Pulmonary Hypertension: Bridging Human and Canine Medicine.

Pulmonary hypertension is a progressive syndrome characterised by pulmonary vascular dysfunction, inflammation, maladaptive remodelling, and progressive right-ventricular strain. Translational progress remains limited because experimental models reproduce only selected aspects of the complexity of human disease. This narrative review evaluates naturally occurring canine disease as a comparative and spontaneous model of human pulmonary hypertension within a One Health framework. To achieve this, we synthesise recent human and veterinary literature, international consensus statements, and key re
