Comprehensive multimodal imaging of quadricuspid pulmonary valves: morphology, function, and mid-term prognosis.

Quadricuspid pulmonary valve (QPV) is a rare congenital anomaly with limited reports. This study characterizes QPV morphology and function by multimodal imaging, evaluates predictors of moderate-to-severe pulmonary regurgitation (PR), and assesses mid-term prognosis. Retrospective search of the imaging database at a tertiary medical center identified patients diagnosed with QPV between October 2004 and September 2024. Multimodal imaging was utilized to assess the characteristics of QPV and associated abnormalities. Among 1,367,280 cardiac CT and MR exams, 16 patients were diagnosed with QPV (3

Early-Diastolic Right Ventricle-Pulmonary Artery Pressure Gradient by Doppler Echocardiography to Guide Cardiac Magnetic Resonance Assessment of Pulmonary Regurgitation after Right Ventricular Outflow Tract Reconstruction.

BACKGROUND: Pulmonary regurgitation (PR) after right ventricular outflow tract (RVOT) reconstruction is a major driver of right ventricular (RV) remodeling. Cardiac magnetic resonance (CMR) is the reference standard for quantifying PR but is not routinely feasible in many pediatric follow-up settings. We evaluated whether an echocardiographic early-diastolic RV-pulmonary artery (PA) pressure gradient (Echo-EaDPG), derived from early-diastolic peak PR velocity (ΔP = 4v2), can serve as a practical complementary hemodynamic marker to guide CMR assessment. METHODS: We retrospectively analyzed pat

The efficacy and safety of cerebral embolic protection devices in patients undergoing transcatheter aortic valve replacement: a systematic review and meta-analysis with trial sequential analysis of randomized controlled trials.

INTRODUCTION: Transcatheter aortic valve replacement (TAVR) is an established treatment for severe aortic stenosis but carries a risk of stroke and cerebral embolism. Cerebral embolic protection devices (CEPD), including filter-based and deflection-based systems (Sentinel and TriGUARD), aim to reduce embolic complications; however, their clinical benefit remains uncertain. This study evaluated the efficacy and safety of CEPD during TAVR using trial sequential analysis (TSA) and the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) framework. METHODS: Randomized contr

Isolated native pulmonary valve endocarditis in a child: Navigating diagnostic and therapeutic challenges with limited resources.

Isolated native pulmonary valve endocarditis (INPVE) is a rare form of infective endocarditis, accounting for <2% of cases. It typically affects the individuals with predisposing factors, including congenital heart defects, intravenous drug use, or indwelling catheters. Pediatric cases without such risk factors are extremely uncommon. Diagnosis is often delayed due to nonspecific symptoms that resemble pneumonia, sepsis, or heart failure and is further complicated by culture-negative results from prior antibiotic use. We present the case of a previously healthy 9-year-old boy with prolonged fe

Partial heart transplant in the setting of truncus arteriosus after several corrections and definitive contraindication for anticoagulation due to immune thrombocytopenia.

We report the case of a 12-year-old boy with a history of type I truncus arteriosus repaired in early infancy. He underwent initial correction at 2 months of age, conduit replacement between the right ventricle and pulmonary artery at 4 years, and truncal valve replacement with a bioprosthesis at 8 years. He was referred with severe aortic regurgitation, severe aortic stenosis, free regurgitation of the right ventricle-pulmonary artery conduit with moderate stenosis and preserved biventricular function. Long-term anticoagulation was formally contraindicated due to immune thrombocytopenic purpu

Minimally invasive treatment strategy for severe congenital pulmonary valve insufficiency and pulmonary artery aneurysmal dilatation: a case report.

Pulmonary valve insufficiency most commonly occurs following various precordial interventions, including repair of tetralogy of Fallot, balloon dilatation for pulmonary stenosis, and surgical or percutaneous treatment for pulmonary atresia with intact ventricular septum. Isolated congenital pulmonary valve insufficiency is rare, and insufficiency secondary to myxomatous degeneration of the pulmonary valve is even rarer. We report the case of a 16-year-old female with severe pulmonary valve insufficiency and aneurysmal dilatation of the pulmonary artery caused by myxomatous degeneration of the

Transcatheter Harmony pulmonary valve placement in the right pulmonary artery followed by stenting of a hypoplastic left pulmonary artery.

We report the case of a 60-year-old man with repaired Tetralogy of Fallot and long-standing severe pulmonary insufficiency, complicated by asymmetric pulmonary blood flow and anatomy unsuitable for standard right ventricular outflow tract valve implantation. Cardiac imaging demonstrated a pyramidal right ventricular outflow tract that did not meet the oversizing criteria for a Harmony TPV 25, with the lack of interference with the inflow part of the device. In addition, the left pulmonary artery was severely hypoplastic with proximal stenosis. To avoid jailing the left pulmonary artery and hig

[Echocardiography in cardiology-Basic tool, current developments and future perspectives].

Despite advances in computed tomography (CT) and magnetic resonance imaging (MRI), transthoracic echocardiography remains the central imaging method in cardiology and internal medicine. Key advantages are ubiquitous availability, repeatability and the direct linkage between findings and symptoms at the bedside. In daily practice it answers questions relevant to treatment with respect to pump function, pericardial effusion or tamponade, relevant valvular diseases, heart failure with preserved ejection fraction (HFpEF), pulmonary hypertension and the hemodynamic profile in shock, sepsis or circu

Minimally invasive cardiac surgery for tetralogy of Fallot: early outcomes from a single-center experience.

BACKGROUND: While minimally invasive techniques for simple septal defects have demonstrated comparable safety and efficacy to median sternotomy, clinical outcomes following minimally invasive repair of Tetralogy of Fallot (TOF) remain insufficiently characterized. METHODS: This retrospective single-center cohort included patients who underwent complete TOF repair through a modified right vertical infra-axillary thoracotomy (RVIAT) between January 2022 and December 2024. RESULTS: All 144 patients (mean age, 8.7 ± 7.8 months; mean weight, 7.6 ± 2.4 kg) underwent RVIAT repair without conversion

Preserved Ejection Fraction Despite Progressive Functional Decline in a Patient With Single-Ventricle Physiology Without Documented Successful Surgical Palliation.

Seventh-decade survival with single-ventricle congenital heart disease (CHD) without documented successful surgical palliation is exceedingly rare. The prognostic value of ejection fraction (EF) in this population remains poorly characterized, and validated staging tools are absent from current guidelines. We report a 62-year-old man with complex single-ventricle CHD who survived without documented successful surgical palliation and had a common atrium, systemic right ventricle, and severe common atrioventricular valve regurgitation. Despite a preserved EF of 55-60%, he was NYHA Class IV with

Possible Right Coronary Artery Supply to Pericardial Adipose Tissue Adjacent to an Abnormal Proximal Ascending Aortic Wall: A Case Report.

A 63-year-old male patient with chronic obstructive pulmonary disease (COPD) was examined using cardiac computed tomography (CT) and transesophageal echocardiography (TEE) to estimate the presence of chronic thrombi, such as left atrial appendage (LAA) thrombi and pulmonary vein thrombi (PVTs). The patient presented with a strange thrombus-containing vessel over the anterior surface of the heart, which did not appear to be a coronary vein. The abnormal wall of the ascending aorta (AAo) was difficult to differentiate from the AAo thrombi, and a dark (low echogenic) mass was observed in the cent

Localized Pulmonary Amyloidosis Associated With Sjögren's Syndrome, Coexisting Lymphoid Interstitial Pneumonia, and a Severe Double Aortic Lesion: A Case Report and Literature Review.

Amyloidosis is a protein-folding disease that results in the deposition of misfolded proteins into various tissues of the body, resulting in dysfunction of the affected organ. It is caused by a heterogeneous group of disorders caused by extracellular deposition of misfolded proteins as insoluble fibrils. Involvement of the lungs can present as nodular, tracheobronchial, or diffuse interstitial, and can be mistaken for cancer or infection on imaging. The combination of Sjögren's syndrome, chronic activation of B cells, and the risk of developing a lymphoproliferative disorder can lead to the ra

Unpredictable Virulence of a Benign Commensal: Streptococcus salivarius Bacteremia and Infective Endocarditis in an Immunocompetent Patient.

Streptococcus salivarius is a gram-positive viridans group streptococcus that primarily colonizes the oral cavity and gastrointestinal tract and is generally regarded as a benign commensal organism. Although invasive infections have been reported, they typically occur in immunocompromised hosts or following invasive gastrointestinal or spinal procedures. We present a rare case of S. salivarius bacteremia complicated by meningitis and infective endocarditis in an elderly immunocompetent patient following a routine dental cleaning. Unmasking the Stenosis: Simultaneous Mitral Valve Replacement and Scimitar Vein Rerouting in a Dextro-Rotated Heart.

Scimitar syndrome (SS) is a rare congenital condition marked by partial anomalous pulmonary venous connection, dextroposition of the heart, right lung hypoplasia, and systemic arterial supply to the right lung. Also known as congenital veno-lobar syndrome and other names, it has been classified into three types by Lupius et al. A significant number of cases are associated with congenital heart anomalies, while association with acquired cardiac disease is rare. We present the rare case of a 39-year-old woman with exertional dyspnoea during pregnancy, initially diagnosed with mild mitral stenosi

Unusual Coexistence of Takayasu Arteritis, Diffuse Coronary Aneurysms, and a Large Left Atrial Myxoma in an Elderly Male: A Case Report and Literature Review.

Takayasu arteritis (TA) is a form of vasculitis that primarily affects the large arteries of the body. Involvement of the coronary arteries is uncommon, whereas cardiac myxomas are benign growths within the heart that can cause problems by embolization or by obstruction. Both diseases are rare, and coexistence of both is exceptionally uncommon. An 84-year-old man with a 30-year history of TA (type V) on low-dose prednisone and weekly methotrexate was found to have a large left atrial myxoma in addition to diffuse coronary aneurysms on routine follow-up.
