INTRODUCTION/OBJECTIVES: Main pulmonary artery end-diastolic forward flow (EDFF) has been evaluated as an indicator of right ventricular (RV) restrictive physiology (RVRP) in humans. The authors theorized that dogs with conditions associated with RVRP, including pulmonic stenosis (PS) and precapillary pulmonary hypertension (PH), would have an increased EDFF magnitude compared to normal dogs and that EDFF velocity in dogs with PS or PH would be correlated with disease severity and RV remodeling.

OBJECTIVE: This study aimed to evaluate the diagnostic yield of fetal echocardiography (f-Echo) in detecting significant congenital heart disease (CHD) in pregnancies with a first-degree relative with a history of bicuspid aortic valve (BAV) and a normal level II obstetric ultrasound.
STUDY DESIGN: A retrospective review was conducted of all f-Echos performed between 2019 and 2023 for the sole indication of family history of BAV. Cases with additional indications or affected nonfirst-degree relative were excluded. Postnatal transthoracic echocardiography (t-Echo) data were reviewed when avail

PURPOSE: To report our findings in a patient with an atypical case of supertemporal iris and retinal coloboma, and to assess its systemic associations and potential genetic causes.
OBSERVATIONS: A premature female infant born at 31 weeks gestation presented a left eye superotemporal coloboma affecting both the iris and retina. Systemic abnormalities included a left multicystic kidney, hypoplastic left kidney, atrial septal defect, ventricular septal defect, and mild pulmonary valve stenosis. Whole genome sequencing and mitochondrial deoxyribonucleic acid (DNA) sequencing were performed, and b

We report the case of a complex congenital heart disease that accounts for unusual clinical presentation: subtle cyanosis and absence of murmur in a pediatric patient with double-outlet right ventricle, subaortic ventricular septal defect and severe pulmonary stenosis. Echocardiography revealed a large aorto-pulmonary window supplying the pulmonary arteries at systemic pressure.

INTRODUCTION: Balloon pulmonary valvuloplasty (BPV) has emerged as the standard of care for critical pulmonary stenosis (CPS) in newborns. The aim of this study was to describe BVP results for CPS in a North African country where data are scarce.
METHODS: A monocentric retrospective descriptive study was conducted on neonates who underwent BPV for CPS between 2000 and 2023, with a minimum clinical and echocardiographic follow-up of 12 months. Primary endpoint was immediate procedural success rate, and secondary endpoints were early additional procedures and late reinterventions.
RESULTS: Thi

We report a rare case of double outlet right ventricle with transposition of the great arteries, subpulmonary ventricular septal defect, pulmonary valve stenosis, and superior-inferior ventricles successfully managed by the Nikaidoh procedure. Preoperative 3-dimensional simulation clarified the complex spatial anatomy and confirmed feasibility without inflow compromise. Surgical results mirrored the simulation, with smooth recovery and excellent 4-year outcome. The Nikaidoh procedure is feasible in cases with superior-inferior ventricular arrangement and pulmonary stenosis, and 3-dimensional s

A 50 year old male with medical history of hypertension and carcinid heart disease was initially seen in valve clinic for planned tricuspid valve replacement. A repeat echocardiogram demonstrated flow acceleration accross the pulmonic valve concerning for pulmonic stenosis, which was not seen on prior echocardiograms. Follow up CT imaging confirmed pulmonic stenosis and the surgical plan was changed to include both tricuspid and pulmonic valve replacements. A high clinical index of suspicion is requried for evaluation pulmonic involvement in carcinoid heart disease.

Both infundibular pulmonic stenosis with a bicuspid pulmonic valve and a spontaneously ruptured tricuspid valve are infrequent cardiac anomalies to come across in day-to-day cardiology practice. We are writing about a middle-aged woman who presented with a spontaneous rupture of the tricuspid valve chordae, most probably secondary to right ventricular pressure overload from an infundibular pulmonic stenosis with a bicuspid pulmonic valve. A successful surgical repair relieved her of her symptoms.

Disorders of the pulmonic valve (PV) receive considerably less attention than other forms of valvular heart disease. Due to the dramatically improved survival of children with congenital heart disease over the last 5 decades, there has been a steady increase in the prevalence of adults with congenital heart disease, which necessitates that clinicians become familiar with the anatomy and the evaluation of right ventricular outflow tract and PV anomalies. A multimodality imaging approach using echocardiography, cardiac computed tomography, and magnetic resonance imaging is essential for a compre

Background: Totally endoscopic techniques have become increasingly popular in cardiac surgery, with minimally invasive mitral valve repair emerging as an effective alternative to median sternotomy. This approach could be particularly advantageous for patients with Noonan syndrome, who often present with structural thoracic anomalies and other comorbidities like bleeding disorders. Endoscopic mitral valve surgery is rapidly establishing itself as the new standard of care for mitral valve operations, demonstrating both safety and efficacy. Noonan syndrome is an autosomal-dominant multisystem dis

RAS proteins are critical in cellular signal transduction, influencing cell proliferation, differentiation, and survival. While extensively studied for their role in cancer, RAS gene mutations also contribute significantly to cardiovascular diseases, such as hypertrophic cardiomyopathy, pulmonary valve stenosis, and atrial septal defects. Despite their similar primary structures, RAS proteins exhibit distinct functions in cardiac biology: H-RAS regulates cardiomyocyte size, K-RAS governs proliferation, and N-RAS, less associated with cardiac defects, is understudied in cardiac cells. BACKGROUND: Supravalvular aortic stenosis (SVAS) is an autosomal dominantly inherited congenital cardiovascular disease caused by disruption of elastin gene (ELN), encoding elastin, an essential component of elastic arteries. It usually affects the middle layer of the wall of the aorta but also the pulmonary and coronary arteries may be affected.
METHODS: We report a family with six affected siblings who were closely followed up from infancy to early adulthood at a pediatric cardiology outpatient clinic. Whole-exome sequencing was performed using DNA of the index patient. Targeted variant tes

A 13-year-old male with a past medical history of Noonan syndrome, pulmonary valve stenosis status post balloon valvuloplasty and subsequent surgical valvotomy with main pulmonary artery augmentation, splenomegaly, and von Willebrand disease type 1 presents with chest pain, fever, and chronic, recurrent vomiting. At the outlying facility, computed tomography of the chest revealed a right upper lobe pulmonary cavitation. After an extensive workup, he was found to have a right-sided tracheal bronchus via bronchoscopy. Broad-range polymerase chain reaction analysis of the biopsied specimen demons

INTRODUCTION: Kallmann De Morsier syndrome (KS) is a rare genetic disorder characterized by congenital gonadotropic deficiency alongside anosmia or hyposmia, with a lower prevalence in females. Diagnosis relies on clinical and biological assessment, confirmed through pituitary magnetic resonance imaging (MRI). Cardiac involvement in this syndrome is uncommon, with only a few cases documented in the literature.
OBSERVATION: We reported the case of a 22-year-old female with a history of pulmonary valve stenosis for which she underwent balloon dilatation at the age of five years. She presented w

Though major complications during transcutaneous pulmonary valve replacement (TPVR) are rare, clinically-significant ventricular arrythmia (CSVA) has been reported following self-expanding valve placement. We assess whether alterations in valve frame dimensions and geometry within the right ventricular outflow tract (RVOT) post-implantation in patients who underwent TPVR with Harmony TPV25 or Alterra contribute to CSVA risk. A single center review was performed of patients who underwent TPVR with either Harmony TPV25 or Alterra Pre-stent between August 2019 and April 2023. Using post-procedura

BACKGROUND: Pulmonary stenosis (PS) is common in congenital heart disease and an integral finding in Tetralogy of Fallot (TOF). Pulmonary regurgitation (PR) is more commonly found following surgery in repaired TOF. We aimed to evaluate the haemodynamic effects of PS and PR on cardiac physiology in a porcine model using cardiac magnetic resonance-based feature tracking (CMR-FT) deformation imaging.
METHODS: CMR-FT was performed in 14 pigs before and 10-12 weeks after surgery. Surgery included either pulmonary artery banding to simulate PS (n = 7), or an incision to the pulmonary valve to simul

Pulmonary regurgitation is a common consequence following the repair of tetralogy of Fallot and can lead to heart failure. Early detection of right ventricular dysfunction remains challenging, and current clinical markers have limited predictive value to identify which patients are at risk for heart failure and require interventions. This study aimed to investigate the potential of ventriculo-arterial coupling as a marker of early right ventricular dysfunction in a porcine model of chronic pulmonary regurgitation following transannular patch repair of neonatal pulmonary stenosis. Neonatal swin

BackgroundResidual pulmonary valve disease (PVD) including pulmonary stenosis (PS), pulmonary regurgitation (PR), or both remains a known complication following repair of Tetralogy of Fallot (TOF). We sought to clarify prevalence, progression, and prognostic impact of residual PVD.MethodsWe identified consecutive patients who underwent repair of TOF at our institution between January 2004 and July 2023. Based on echocardiographic measurements following repair, residual PVD was defined: nonsevere PVD (PS < 25 mm Hg; PR<Grade3), isolated PS (PS ≥ 25 mm Hg; PR<Grade3), isolated PR (PS < 25 mm Hg;
