Outcomes After Neonatal Balloon Pulmonary Valvuloplasty: A Contemporary Multicenter Analysis.

Balloon pulmonary valvuloplasty (BPV) is the standard treatment for neonates with pulmonary stenosis (PS), but outcomes in this population remain undefined. We performed a retrospective study using Pediatric Health Information System data, including neonates undergoing BPV for isolated PS from 2016 to 2025. Patients with other congenital heart disease, except atrial septal defect or patent foramen ovale, were excluded. The cohort was divided into critical PS (requirement of Prostaglandin E1) and non-critical PS. The primary safety outcome was a composite of in-hospital mortality or cardiac arr

Impact of posttraumatic stress disorder on incident cardiovascular outcomes in women veterans: a retrospective longitudinal study.

BACKGROUND: Women veterans are a growing-yet understudied-population with a high psychiatric and cardiovascular disease (CVD) risk burden. This study evaluated the association of posttraumatic stress disorder (PTSD) with a wide range of incident CVD outcomes in a large, nationally representative cohort of women veterans. METHODS: In this retrospective longitudinal study, Veterans Health Administration (VHA) electronic health records were used to identify women veterans aged ≥18 years receiving care from 1/1/2000 to 12/31/2019 in the USA. Women with prior CVD at baseline were excluded. Partici

Reoperative Pulmonary Valve Replacement via Left Mini-Thoracotomy in a Young Woman With Absent Pulmonary Valve Syndrome: A Case Report.

Absent pulmonary valve syndrome is a rare congenital cardiac anomaly characterized by severe pulmonary valve insufficiency and progressive dilation of the right ventricular outflow tract and pulmonary arteries. Although primary surgical repair is typically performed during childhood, long-term outcomes are frequently limited by structural degeneration of bioprosthetic valves, often leading to the need for reintervention in early adulthood. We present the case of a 22-year-old woman with a history of pulmonary valve agenesis who underwent surgical correction during childhood, including implanta

Transcatheter Pulmonary Valve Implantation With the Alterra Adaptive Prestent and SAPIEN 3 Transcatheter Heart Valve: 3-Year Pooled Outcomes of the ALTERRA Trials.

BACKGROUND: The Alterra Adaptive Prestent provides a landing zone for implantation of the 29 mm SAPIEN 3 transcatheter heart valve (THV) in patients with a dysfunctional right ventricular outflow tract (RVOT) to treat pulmonary regurgitation (PR). Here, we report 3-year outcomes from a pooled analysis of patients who underwent Alterra/SAPIEN 3 THV implantation enrolled in the ALTERRA pivotal trial, Continued Access Protocol, and Pulmonic Delivery System Registry. METHODS: This multicenter, prospective trial enrolled patients with moderate or greater PR and RVOT/pulmonary valve anatomy suitabl

Preserved Ejection Fraction Despite Progressive Functional Decline in a Patient With Single-Ventricle Physiology Without Documented Successful Surgical Palliation.

Seventh-decade survival with single-ventricle congenital heart disease (CHD) without documented successful surgical palliation is exceedingly rare. The prognostic value of ejection fraction (EF) in this population remains poorly characterized, and validated staging tools are absent from current guidelines. We report a 62-year-old man with complex single-ventricle CHD who survived without documented successful surgical palliation and had a common atrium, systemic right ventricle, and severe common atrioventricular valve regurgitation. Despite a preserved EF of 55-60%, he was NYHA Class IV with

Possible Right Coronary Artery Supply to Pericardial Adipose Tissue Adjacent to an Abnormal Proximal Ascending Aortic Wall: A Case Report.

A 63-year-old male patient with chronic obstructive pulmonary disease (COPD) was examined using cardiac computed tomography (CT) and transesophageal echocardiography (TEE) to estimate the presence of chronic thrombi, such as left atrial appendage (LAA) thrombi and pulmonary vein thrombi (PVTs). The patient presented with a strange thrombus-containing vessel over the anterior surface of the heart, which did not appear to be a coronary vein. The abnormal wall of the ascending aorta (AAo) was difficult to differentiate from the AAo thrombi, and a dark (low echogenic) mass was observed in the cent

Localized Pulmonary Amyloidosis Associated With Sjögren's Syndrome, Coexisting Lymphoid Interstitial Pneumonia, and a Severe Double Aortic Lesion: A Case Report and Literature Review.

Amyloidosis is a protein-folding disease that results in the deposition of misfolded proteins into various tissues of the body, resulting in dysfunction of the affected organ. It is caused by a heterogeneous group of disorders caused by extracellular deposition of misfolded proteins as insoluble fibrils. Involvement of the lungs can present as nodular, tracheobronchial, or diffuse interstitial, and can be mistaken for cancer or infection on imaging. The combination of Sjögren's syndrome, chronic activation of B cells, and the risk of developing a lymphoproliferative disorder can lead to the ra

Unpredictable Virulence of a Benign Commensal: Streptococcus salivarius Bacteremia and Infective Endocarditis in an Immunocompetent Patient.

Streptococcus salivarius is a gram-positive viridans group streptococcus that primarily colonizes the oral cavity and gastrointestinal tract and is generally regarded as a benign commensal organism. Although invasive infections have been reported, they typically occur in immunocompromised hosts or following invasive gastrointestinal or spinal procedures. We present a rare case of S. salivarius bacteremia complicated by meningitis and infective endocarditis in an elderly immunocompetent patient following a routine dental cleaning. A 74-year-old woman with a history of atrial fibrillation, hyper

Unmasking the Stenosis: Simultaneous Mitral Valve Replacement and Scimitar Vein Rerouting in a Dextro-Rotated Heart.

Scimitar syndrome (SS) is a rare congenital condition marked by partial anomalous pulmonary venous connection, dextroposition of the heart, right lung hypoplasia, and systemic arterial supply to the right lung. Also known as congenital veno-lobar syndrome and other names, it has been classified into three types by Lupius et al. A significant number of cases are associated with congenital heart anomalies, while association with acquired cardiac disease is rare. We present the rare case of a 39-year-old woman with exertional dyspnoea during pregnancy, initially diagnosed with mild mitral stenosi

Unusual Coexistence of Takayasu Arteritis, Diffuse Coronary Aneurysms, and a Large Left Atrial Myxoma in an Elderly Male: A Case Report and Literature Review.

Takayasu arteritis (TA) is a form of vasculitis that primarily affects the large arteries of the body. Involvement of the coronary arteries is uncommon, whereas cardiac myxomas are benign growths within the heart that can cause problems by embolization or by obstruction. Both diseases are rare, and coexistence of both is exceptionally uncommon. An 84-year-old man with a 30-year history of TA (type V) on low-dose prednisone and weekly methotrexate was found to have a large left atrial myxoma in addition to diffuse coronary aneurysms on routine follow-up. The increased inflammatory markers, eryt

Extensive left atrial thrombus invading pulmonary veins and mitral valve in severe mitral stenosis.

Severe mitral stenosis is a valvular heart disease that obstructs blood flow from the left atrium to the left ventricle, leading to impaired ventricular filling, left atrial dilatation, atrial fibrillation, and thromboembolic events. We report the case of a female patient who presented with shortness of breath (NYHA Class-II) for six months. On examination, a low-pitched murmur was heard at the apex; other systemic examination findings were unremarkable. TTE/TEE revealed left atrial dilatation with LAVI of 120 ml/m2, a large left atrial thrombus, severe rheumatic mitral stenosis, and moderate

Giant Left Atrial Myxoma Causing Functional Mitral Stenosis: A Minimally Invasive Approach.

Cardiac myxoma is the most common primary cardiac tumor and typically arises in the left atrium. Large, mobile myxomas may prolapse through the mitral valve and produce functional mitral stenosis with secondary pulmonary hypertension. We report a 57-year-old woman with cardiovascular risk factors (hypercholesterolemia and obesity) who presented with New York Heart Association class III dyspnea and palpitations. First-line transthoracic echocardiography revealed a giant, mobile left atrial mass (4.7 x 4.8 cm) attached to the basal third of the interatrial septum, prolapsing into the left ventri

Valvular Heart Disease During Pregnancy: Outcomes from a Large Integrated Healthcare System.

Valvular heart disease (VHD) accounts for significant morbidity and mortality during pregnancy, with limited data to guide cardio-obstetric risks. This is a retrospective cohort study describing maternal, obstetric, and fetal outcomes of Kaiser Permanente Northern California members with VHD during pregnancies between 2010 and 2021. Adult patients with moderate to severe native VHD and/or previous valve interventions were included. There were 242,066 eligible pregnancies in the study period, 89 (0.04%) of which were complicated by clinically significant VHD: 25 with pulmonary stenosis/regurgit

4-Dimensional Intracardiac Echocardiography-Guided Antegrade TAVR Using a Novel CABLE-TAVR Technique.

BACKGROUND: An antegrade approach to transcatheter aortic valve replacement (TAVR) has become less common because of the complexity and chance of complication, but use of a large-bore sheath to protect the mitral valve has been described. CASE SUMMARY: We present a 75-year-old patient with severe symptomatic aortic stenosis who underwent antegrade TAVR with 4-dimensional intracardiac echocardiography under conscious sedation. Severe Aortic and Mitral Regurgitation Secondary to Infective Endocarditis Presenting as Acute Respiratory Failure.

Infective endocarditis (IE) typically presents with fever, a new or changing cardiac murmur, and embolic phenomena. Acute respiratory failure (ARF) as the dominant presenting feature, without overt cardiac signs, is uncommon and creates significant diagnostic challenges. We present a previously healthy 24-year-old man who attended the Emergency Department with fever, productive cough, and ARF requiring mechanical ventilation for bilateral pulmonary infiltrates. Cardiovascular examination on admission was unremarkable: there was no audible cardiac murmur and no peripheral stigmata of IE. Blood

A Unique and Extensive Constellation of Late Radiation Effects in a Single Patient Following High-Dose Radiation Therapy for Childhood Hodgkin's Lymphoma.

Childhood cancer survivors treated with large-field radiation may develop late secondary effects across multiple organ systems. We present a 60-year-old male whose PET/CT revealed an absent thyroid gland with multiple metastatic foci, a surgically absent spleen and bladder with an ileal conduit, severe pulmonary outflow tract calcifications, and transcatheter aortic valve replacement (TAVR), bilateral proximal humeral osteonecrosis, and bilateral total hip arthroplasties. Evaluation of the Venus P-valve for transcatheter pulmonary valve replacement in patients with right ventricular outflow tract dysfunction.

Right-ventricular outflow tract dysfunction is a common late sequela of congenital heart disease, particularly after tetralogy of Fallot repair. Balloon-expandable valves such as Melody and Sapien have transformed care for conduits and bioprostheses, but until recently most patients with large or irregular outflow tracts were not candidates for percutaneous therapy. This review appraises the Venus P-Valve, the first self-expanding transcatheter pulmonary valve for large right-ventricular outflow tracts, summarizing evidence on efficacy, safety, and regulatory status. We performed a narrative r

Acquired aortic and mitral stenosis characterized by valvular fibroelastosis in a dog.

Left-sided congestive heart failure was diagnosed in a 2.6-kg 13-year-old neutered male Yorkshire terrier by compatible clinical signs and findings on thoracic radiographs. Echocardiography revealed left atrium enlargement, left ventricular concentric hypertrophy, and thickening and stenosis of the mitral and aortic valves (AVs). A hypercoagulable state, chronic kidney disease, and a portal vein thrombus were identified. Medical management and balloon valvuloplasty of the AV provided good quality of life for nine months since diagnosis, but ultimately, recurrent episodes of pulmonary oedema an

One Anatomy, Multiple Valve Choices: Interobserver Agreement in Selecting Transcatheter Pulmonary Valve.

BACKGROUND: Transcatheter pulmonary valve replacement (TPVR) requires selecting the optimal device type, size, and position. Variability in anatomical shape may affect procedural planning. AIM: This study aimed to evaluate interobserver agreement (IOA) in TPVR device selection using 3D patient-specific anatomies and explore a framework for determining agreement. METHODS: Six experienced observers from three regions of the Americas independently assessed valve type (self-expanding [S-E], balloon-expandable [B-E], surgical approach [SA]), size, and position for 60 3D reconstructed anatomies (p
