Development of atrioventricular valve regurgitation following transcatheter aortic valve replacement for severe aortic stenosis.

BACKGROUND: Concomitant mitral (MR) and/or tricuspid regurgitation (TR) in patients with aortic stenosis (AS) is often regarded as a late-stage disease manifestation, resulting from left ventricular damage, pulmonary hypertension, and right heart dysfunction. However, we assume that atrioventricular valve regurgitation in the context of severe AS may often develop as an independent entity, but knowledge regarding these patients and their risk factors is scarce. OBJECTIVES: The aim of this study was to characterize patients who develop atrioventricular valve regurgitation after transcatheter a

Natural history of aortic valve disease after rheumatic mitral valve surgery: implications for concomitant aortic intervention.

BACKGROUND: Rheumatic heart disease (RHD) frequently affects both mitral and aortic valves. While mitral valve replacement (MVR) is often prioritized, the natural history of coexisting aortic valve disease particularly when mild or moderate is less well understood. OBJECTIVE: The objective of this study is to assess the progression of aortic stenosis (AS) and aortic insufficiency (AI) in patients undergoing isolated MVR for rheumatic mitral disease. METHODS: We conducted a single-center retrospective cohort study involving 403 adult patients with rheumatic mitral valve disease who underwent

Pulmonary Carcinoid and Left-Sided Heart Disease: When Carcinoid Goes Left.

BACKGROUND: Carcinoid heart disease (CHD) is a rare cause of valvular dysfunction due to exposure to vasoactive substances secreted by neuroendocrine tumors. Left-sided involvement of the mitral and aortic valves is uncommon. CASE SUMMARY: A 53-year-old woman presented with progressive dyspnea and severe mitral regurgitation with moderate stenosis and moderate aortic regurgitation. N-terminal pro-B-type natriuretic peptide level was elevated, while other biomarkers of carcinoid activity were negative. Echocardiography demonstrated characteristic valvular thickening and restricted leaflet moti

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Concomitant Aortic Valve Neocuspidization and Resection of a Cardiac Calcified Amorphous Tumor in a Hemodialysis Patient: A Case Report.

Cardiac calcified amorphous tumor (CAT) is an uncommon nonneoplastic intracardiac lesion associated with end-stage renal disease; mobile lesions carry an embolic risk and are generally resected. A 62-year-old man on maintenance hemodialysis was found to have a mobile verrucous mass 17 mm long on the anterior mitral leaflet and a separate immobile calcified lesion, 19 × 12 mm, at the base of the posterior mitral leaflet, together with moderate aortic regurgitation, left ventricular dilatation, a left ventricular ejection fraction of 41%, and pulmonary hypertension. Infective endocarditis was co

Neighborhood Economic Deprivation, Hispanic Ethnic Enclaves, and Congenital Anomalies in Texas.

BACKGROUND: Congenital anomalies are common, costly, and potentially life-threatening. The impacts of structural determinants of health on risk for these conditions have scarcely been studied. We evaluated associations between residence in economically disadvantaged neighborhoods or Hispanic/Latino enclaves and congenital anomalies. METHODS: We performed a registry linkage study of 7.25 million births and 332,935 cases with anomalies in Texas, 1999-2018. We defined exposure groups using maternal residential address at delivery or termination and tract-level Yost socioeconomic index and Hispan

When Time Is of the Essence: Multifocal Infective Endocarditis in a Young Patient.

Infective endocarditis (IE) is uncommon in adolescents but may cause severe morbidity, especially with congenital heart disease (CHD) or delayed diagnosis. A 17-year-old male with a presumed closed perimembranous ventricular septal defect (VSD) presented with five months of weight loss, malaise, fevers, cough and pleuritic chest pain. A new pansystolic murmur prompted transthoracic echocardiography (TTE), which was highly suggestive of mitral and pulmonary valve vegetations with evidence of severe valvular regurgitation. Blood cultures grew Streptococcus mitis and computed tomography (CT) demo

Acute Severe Mitral Regurgitation Following Blunt Chest Trauma and a Febrile Illness in a Young Adult With Degenerative Mitral Valve Disease: A Rare Case Report.

Acute severe mitral regurgitation (MR) in young adults is rare and is usually associated with infective endocarditis, trauma, congenital abnormalities, or connective tissue disorders. However, acute presentation of degenerative mitral valve disease is uncommon in young adults. A 23-year-old man developed acute pulmonary edema following blunt precordial trauma during a football match, with a brief febrile illness occurring several days later. Transthoracic echocardiography demonstrated acute severe eccentric MR due to flail of the P2 scallop resulting from the rupture of the chordae tendineae.

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

From Traditional to Cutting-Edge: Transforming Pulmonary Valve Assessment With Advanced Echocardiography Techniques.

The pulmonary valve (PV), although often less emphasized than other heart valves, is crucial for cardiac function and hemodynamics. Historically, the PV has been underrepresented in echocardiographic assessments due to its rare involvement in pathological conditions, particularly in adults. Additionally, the anatomical position of the PV makes it one of the most challenging valves to visualize using conventional echocardiography. Traditional two-dimensional (2D) techniques, while foundational, have limitations in capturing the full spectrum of valve pathology and dynamics. Recent advancements

Surgical and Bronchoscopic Lung Volume Reduction for Severe Emphysema: A Systematic Review and Network Meta-analysis.

BACKGROUND: Along with lung volume reduction surgery (LVRS), bronchoscopic lung volume reduction is a treatment option for end-stage emphysema. However, comparisons among interventions remain insufficient. METHODS: We searched on PubMed, CENTRAL, Embase, and Web of Science. We included randomized controlled trials with outcomes measuring mid-term mortality within 6 months, changes in forced expiratory volume in one second (FEV1), St. George's Respiratory Questionnaire (SGRQ), six-minute walk distance (6MWD) from baseline, adverse event related to procedures, and long-term mortality within 5 y

A 23-Year-Old Man With Multilobar Consolidation.

A 23-year-old man presented to the ED with a history of respiratory distress, cough, and fever for 10 days. He was evaluated in the ED, where he received a diagnosis of pulmonary edema, secondary to mitral regurgitation with mitral valve prolapse syndrome. He was treated with antibiotics and diuretics and discharged to home. Three months later, he returned to the ED with similar complaints, for which he was treated symptomatically and discharged. After 4 months, the patient once again appeared with worsening respiratory distress and cough with fever. The dyspnea was not accompanied by orthopne

Current Status of CT Imaging Before Common Transcatheter Interventions for Structural Heart Disease.

Background: Over the past decade, several trials and observational studies have validated the use of minimally invasive cardiac interventions as viable treatment options for various cardiac diseases. Transcatheter techniques for severe aortic valve stenosis have rapidly emerged as alternatives to surgical aortic valve replacement in certain patient populations. Additionally, non-surgical treatment options have expanded for conditions affecting other cardiac valves, such as the mitral valve. These emerging minimally invasive interventions complement already well-established endovascular techniq

Surgical techniques and prognostic nomogram for patients with supravalvular aortic stenosis.

BACKGROUND: An effective prognostic nomogram to predict the prognosis for supravalvular aortic stenosis (SVAS) patients is lacking. METHODS: A multi-center retrospective study of consecutive SVAS patients with surgery between 2002 and 2020 was conducted. Patients underwent McGoon repairs, Doty repairs, and other repairs. The primary outcome was the re-operation or restenosis at follow-up. The nomogram based on Cox regression and Kaplan-Meier method was used to show the risk factors of the primary outcome. The predictive accuracy was determined by the concordance index (C-index) and calibratio

Multimodality Imaging Evaluation of Diseases of the Pulmonic Valve and Right Ventricular Outflow Tract for the Adult Cardiologist.

Disorders of the pulmonic valve (PV) receive considerably less attention than other forms of valvular heart disease. Due to the dramatically improved survival of children with congenital heart disease over the last 5 decades, there has been a steady increase in the prevalence of adults with congenital heart disease, which necessitates that clinicians become familiar with the anatomy and the evaluation of right ventricular outflow tract and PV anomalies. A multimodality imaging approach using echocardiography, cardiac computed tomography, and magnetic resonance imaging is essential for a compre

Non-Canonical TERT Activity Initiates Osteogenesis in Calcific Aortic Valve Disease.

BACKGROUND: Calcific aortic valve disease is the pathological remodeling of valve leaflets. The initial steps in valve leaflet osteogenic reprogramming are not fully understood. As TERT (telomerase reverse transcriptase) overexpression primes mesenchymal stem cells to differentiate into osteoblasts, we investigated whether TERT contributes to the osteogenic reprogramming of valve interstitial cells. METHODS: Human control and calcific aortic valve disease aortic valve leaflets and patient-specific human aortic valve interstitial cells were used in in vivo and in vitro calcification assays. Lo

Clinical studies in Myxomatous Mitral Valve Disease dogs: most prescribed ACEI inhibits ACE2 enzyme activity and ARB increases AngII pool in plasma.

The hypertension patient population has doubled since 1990, affecting 1.3 billion globally and >75% live in low-and middle-income countries. Angiotensin Converting Enzyme Inhibitors (ACEI) and Angiotensin Receptor Blockers (ARB) are the most prescribed drugs (>160 million times in the US), but mortality increased >30% since 1990s globally. Clinical relevance of Myxomatous Mitral Valve Disease (MMVD) is directly linked to WHO group 2 pulmonary hypertension, with no disease specific therapies. Therefore, MMVD pet dogs with elevated systolic blood pressure treated with ACEI/ARB, were supplemented
