Distinguishing myocarditis from perimyocardial syndromes in children presenting to the emergency department: a biomarker-based approach.

INTRODUCTION: Myocarditis and perimyocardial syndromes share overlapping features but differ in management. Differentiating them in children presenting with chest pain or palpitations remains difficult, particularly in non-fulminant cases. This study evaluated the diagnostic value of inflammatory biomarkers, especially the Systemic Immune-Inflammation Index (SII), in distinguishing perimyocardial syndromes from isolated myocarditis in the pediatric emergency department (PED). METHOD: We retrospectively analyzed patients aged 1-18 years diagnosed with myocarditis, perimyocardial syndromes, or

Cardiac Metastasis Mimicking Lateral STEMI in Small-Cell Lung Cancer.

BACKGROUND: ST-segment elevation in patients with cancer may reflect malignant myocardial-pericardial infiltration rather than acute coronary occlusion. CASE SUMMARY: A 68-year-old man with active small-cell lung carcinoma receiving chemotherapy presented with acute stabbing chest pain. Electrocardiography showed ST-segment elevation in leads I, aVL, and V2 with inferior reciprocal depression. Transthoracic echocardiography demonstrated anterolateral hypokinesia, focal myocardial discontinuity, and adjacent pericardial effusion. Emergent coronary angiography revealed nonobstructive coronary a

Immune checkpoint inhibitor-related myocarditis: a comprehensive analysis of clinical manifestations and prognostic factors.

BACKGROUND: Immune checkpoint inhibitors-related myocarditis (ICI-myocarditis) is a rare but potentially lethal complication of cancer immunotherapy. Extensive information concerning risk factors and outcomes is still insufficient. METHODS: This multicenter retrospective study enrolled 161 patients diagnosed with biopsy-proven or clinically diagnosed ICI-myocarditis. We performed thorough studies of clinical characteristics and management approaches. Time-to-event analyses, encompassing landmark analysis, and competing risk models, were employed to identify early mortality predictors and eval

Cardiac Involvement in Patients With Multisystem Inflammatory Syndrome in Adults.

Multisystemic inflammatory syndrome in adults is a hyperinflammatory condition following (within 4-12 weeks) SARS-CoV-2 infection. Here, the dysregulation of the immune system leads to a multiorgan involvement often affecting the heart. Cardiac involvement in multisystemic inflammatory syndrome in adults has been described mainly in young men without other comorbidities and may present with different clinical scenarios, including acute heart failure, life-threatening arrhythmias, pericarditis, and myocarditis, with a nonnegligible risk of mortality (up to 7% of all cases). The heterogeneity of

Non-rheumatic Streptococcal Acute Myocarditis: The First Documented Sibling Case.

Myocarditis is an inflammatory disease of the cardiac muscle that manifests as chest pain, dyspnea, and other signs of heart failure. ST segment changes with elevated cardiac biomarkers mimic acute coronary syndromes. It is most commonly caused by viruses like the Epstein-Barr virus (EBV) and Coxsackie B virus, but it can also be due to cardiotoxic drugs like cyclophosphamide and cocaine or caused by a systemic infiltrative process like sarcoidosis or collagen vascular diseases. One relatively common bacterial cause of myocarditis is beta-hemolytic Group A Streptococcus, which is well known to

Immune checkpoint inhibitor-related myositis and myocarditis: diagnostic pitfalls and imaging contribution in a real-world, institutional case series.

BACKGROUND: Immune checkpoint inhibitors (ICIs) are reshaping the prognosis of many cancers, but often cause immune-related adverse events (irAEs). Among neurological irAEs, myositis is the most frequently reported. Our aim is to describe clinical and non-clinical characteristics, treatment and outcome of all irMyositis (skeletal limb-girdle and/or ocular myositis) and irMyocarditis cases in our reference center. METHODS: We retrospectively enrolled all irMyositis/irMyocarditis patients seen between 2018 and 2022. We reviewed demographics, clinical characteristics, biological, neurophysiologi

Akkermansia muciniphila participates in the host protection against helminth-induced cardiac fibrosis via TLR2.

Helminth Trichinella spiralis (Ts) is one of the major pathogens of human infective myocarditis that can lead to cardiac fibrosis (CF). The gut microbiota involved in this pathology are of interest. Here, we use mice infected with Ts as a model to examine the interactions between gut microbes and host protection to CF. Infected mice show enhanced CF severity. We find that antibiotics treatment to deplete the microbiota aggravates the disease phenotype. Attempts to restore microbiota using fecal microbiota transplantation ameliorates helminth-induced CF. 16S rRNA gene sequencing and metagenomic

Cardiac magnetic resonance findings in acute myocarditis after mRNA COVID-19 vaccination.

There is increasing evidence for myocarditis as a complication of the mRNA coronavirus disease 2019 (COVID-19) vaccination. We report the case of a 20-year-old previously healthy man who presented with fever and chest pain 2 days after the second dose of mRNA-1273 vaccine. Electrocardiogram and laboratory studies showed extensive ST-segment elevation accompanied by elevated cardiac biomarkers. Cardiac magnetic resonance (CMR) revealed late gadolinium enhancement (LGE) characteristics of myocarditis. The patient rapidly improved with conservative management and was discharged on hospital day 6.

Risk of myopericarditis following COVID-19 mRNA vaccination in a large integrated health system: A comparison of completeness and timeliness of two methods.

PURPOSE: How completely do hospital discharge diagnoses identify cases of myopericarditis after an mRNA vaccine? METHODS: We assembled a cohort 12-39 year-old patients, insured by Kaiser Permanente Northwest, who received at least one dose of an mRNA vaccine (Pfizer-BioNTech or Moderna) between December 2020 and October 2021. We followed them for up to 30 days after their second dose of an mRNA vaccine to identify encounters for myocarditis, pericarditis or myopericarditis. We compared two identification methods: A method that searched all encounter diagnoses using a brief text description (e

Paediatric multisystem inflammatory syndrome associated with COVID-19: filling the gap between myocarditis and Kawasaki?

Myocarditis and Kawasaki disease are common but usually distinct diseases in children. During the coronavirus pandemic (COVID-19), reports of a new form of myocarditis with clinical features of Kawasaki appeared. We investigated the place of this new disease in the spectrum encompassing Kawasaki disease and myocarditis.Thirty two consecutive children referred to our centre for a suspicion of Kawasaki or a diagnosis of myocarditis were included and eventually divided into four groups: 11 Kawasaki diseases, 6 Kawasaki syndromes (children with another diagnosis), 7 myocarditis without Kawasaki cl

All-trans Retinoic Acid-Induced Isolated Myocarditis: A Case Report and Review of the Literature.

All-trans retinoic acid (ATRA) is the mainstay of treatment in patients with acute promyelocytic leukemia. Despite being effective, it can lead to cardiac complications either as a component of ATRA syndrome or an isolated form denominated as ATRA-induced isolated perimyocarditis. We present a case of this complication and review the literature. (Level of Difficulty: Intermediate.).

A rare case of perimyocarditis induced by all-trans retinoic acid administration during induction treatment of acute promyelocytic leukemia.

Treatment by All-Trans Retinoic Acid (ATRA) combined with an Anthracycline Chemotherapy has been shown to improve disease-free survival in patients with Acute Promyelocytic Leukemia (APL). Although ATRA is generally well tolerated, occurrence of Retinoic Acid Syndrome (RAS) during the induction chemotherapy in some patients is recognized as a distinct complication and a potential life-threatening side effect. Isolated myocarditis as a result of RAS related to ATRA administration is uncommon and has been rarely reported in the literature. We report a very rare case of ATRA-induced perimyocardit
