Recurrent Large-Vessel Occlusive Stroke Requiring Repeated Mechanical Thrombectomy in Severe Dilated Cardiomyopathy With Extremely Low Left Ventricular Ejection Fraction and HIV Screening Positivity: A Case Report.

Severe dilated cardiomyopathy with markedly reduced left ventricular ejection fraction can rarely present as recurrent large-vessel occlusive stroke. This case demonstrates that timely mechanical thrombectomy and sustained anticoagulation are crucial for neurological recovery and secondary prevention, while HIV screening positivity adds complexity but should not independently preclude reperfusion therapy.

Perioperative Management of a Toddler With Tuberous Sclerosis and Dilated Cardiomyopathy Undergoing Open Abdominal Aortic Aneurysm Repair: A Case Report.

Toddler abdominal aortic aneurysms (AAA) are rare and lack defined perioperative guidelines. Aneurysms may be associated with tuberous sclerosis complex (TSC) but rarely occur with concurrent dilated cardiomyopathy. This case report describes the anesthetic and surgical considerations for a two-year-old male with TSC and dilated cardiomyopathy with low ejection fraction undergoing open infrarenal AAA repair. Our most important perioperative considerations included advanced hemodynamic monitoring, management of intraoperative blood loss, and the physiological changes associated with aortic clam

Case Report: When cardiomyopathy is the clue: late-presenting propionic acidemia manifesting as acute heart failure.

BACKGROUND: Propionic acidemia (PA) is a rare inborn error of metabolism that typically presents in infancy; however, late-onset variants may manifest with atypical features, including cardiac involvement. Dilated cardiomyopathy (DCM) is an uncommon but serious complication. CASE SUMMARY: A 15-year-old with no prior medical history but a significant maternal family history of cardiomyopathy of unknown etiology from an Amish background presented with altered mental status, increased work of breathing, and progressive edema in the context of preceding viral illness. Evaluation revealed severe b

Prognostic Value of Late Gadolinium Enhancement Findings in Isolated Left Ventricular Dilatation and Dilated Cardiomyopathy.

BACKGROUND: Non-ischemic dilated cardiomyopathy (DCM) and isolated left ventricular dilatation (ILVD) are defined as left ventricular (LV) dilatation, with reduced or preserved left ventricular ejection fraction (LVEF), respectively. We aimed to assess the prognostic value of late gadolinium enhancement (LGE) findings, including LGE extent, location, and number of areas to predict all-cause death in ILVD and DCM, separately. METHODS: Between 2008 and 2021, we conducted a bicentric retrospective study including all consecutive ILVD and DCM patients referred for cardiovascular magnetic resonanc

Risk stratification in early non-ischaemic cardiomyopathy: the interplay of genetics and cardiac imaging.

Preventing sudden cardiac death (SCD) in early non-ischaemic cardiomyopathy (NICM) remains a major clinical challenge. Traditional risk stratification centred on left ventricular ejection fraction (LVEF) poorly captures arrhythmic risk, particularly in early dilated cardiomyopathy (DCM) and non-dilated left ventricular cardiomyopathy (NDLVC). Increasing recognition of fibrosis-based and genotype-specific arrhythmogenic substrates has prompted a shift towards multimodal risk assessment strategies integrating imaging, electrocardiographic, and genetic data. This review examines contemporary appr

Decoding Atrial Fibrillation Heterogeneity: A Nationwide Phenome-Wide Association Study in England.

BACKGROUND: Age and sex influence atrial fibrillation (AF) prevalence and outcomes, yet comprehensive data on sex- and age-specific health care utilization in patients newly diagnosed with AF remain limited. OBJECTIVES: We aimed to address this gap using a phenome-wide association study (Phe-WAS) approach. METHODS: We conducted a retrospective cohort study using nationwide Clinical Practice Research Datalink data linked to Hospital Episodes Statistics. Adults diagnosed with AF between 1998 and 2016 (n = 199,433) were matched 1:1 by age and sex to non-AF control subjects. Primary and secondar

Silent Ears, Failing Heart: Dilated Cardiomyopathy in an Infant With LARS2-Related Perrault Syndrome Type 4 (PRLTS4).

We report a 19-month-old girl with molecularly confirmed Perrault syndrome type 4 (PRLTS4), due to a homozygous LARS2 variant (c.457A>C, p.Asn153His), who developed dilated cardiomyopathy (ejection fraction 21%) at 2 months of age despite a normal neonatal echocardiogram, alongside bilateral profound sensorineural hearing loss and global developmental delay. No second causative cardiac variant was identified. To our knowledge, this is the first reported case of dilated cardiomyopathy in molecularly confirmed LARS2-related PRLTS4, expanding the recognised phenotypic spectrum of this disorder an

Heart-reactive antibody levels differentially associate with ventricular function in hereditary cardiomyopathies.

BACKGROUND: gCMP are a major cause of cardiovascular morbidity and mortality. While agonistic anti-β1-adrenergic receptor antibodies (anti-β1AR-ECII) have a recognized pathogenic role in idiopathic dilated cardiomyopathy (DCM), the relevance of HRA in gCMP remains unclear. METHODS: One hundred gCMP patients with confirmed genetic variants in cardiomyopathy genes were compared with 100 age- and sex-matched healthy individuals. Antibody reactivities were quantified using bead-based flow cytometric assays against cardiac antigens: The second extracellular loop of the β1 adrenergic receptor (β1AR

Arrhythmic Risk Stratification in Non-Ischaemic Cardiomyopathy: The Role of Cardiac Magnetic Resonance Imaging.

PURPOSE OF REVIEW: This review aims to evaluate the contemporary and evolving role of cardiac magnetic resonance (CMR) imaging in arrhythmic risk stratification for patients with non-ischaemic cardiomyopathy, specifically focussing on dilated cardiomyopathy (DCM) and non-dilated left ventricular cardiomyopathy (NDLVC). This review synthesises current data on late gadolinium enhancement (LGE), parametric mapping, strain imaging and artificial intelligence (AI), and contextualises these among emerging genetic risk stratification approaches. RECENT FINDINGS: Left ventricular ejection fraction ha

Spiked Helmet Sign in the Early Post-Heart Transplantation Period: A Dramatic STEMI Mimic in a Structurally Normal Donor Heart.

A 36-year-old woman with severe dilated cardiomyopathy (LVEF 14%, NYHA IV) underwent heart transplantation. Postoperatively, she developed subarachnoid hemorrhage and septic shock. The ECG revealed a spiked helmet sign (SHS) pattern, with repolarization abnormalities and QT prolongation. Despite transient clinical improvement and resolution of the ECG changes after intensive support, the patient died due to infectious complications. This case highlights the occurrence of SHS in the early posttransplant period in a structurally normal donor heart, a previously unreported presentation, reinforci

Glyceamic variability is a pridictor of in-hopital and long-term mortality of patients with dilated cardiomyopathy: a 5-year retrospective cohort study.

BACKGROUND: Dilated cardiomyopathy (DCM) is a major cause of heart failure with high morbidity and mortality, yet reliable prognostic markers remain limited. Glycaemic variability (GV), reflecting fluctuations in glucose levels, has been linked to adverse cardiovascular outcomes, but its role in DCM remains uncertain. METHODS: Data were obtained from Beth Israel Deaconess Medical Center in Boston. We included patients with DCM as the primary diagnosis and ≥ 3 glucose measurements during hospitalization. GV was defined as the natural logarithm of the standard deviation-to-mean glucose ratio an

Utility of longitudinal and radial myocardial velocities assessed by color tissue Doppler imaging in dogs with dilated cardiomyopathy.

INTRODUCTION/OBJECTIVES: Dilated cardiomyopathy (DCM) is characterized by left ventricular (LV) dilatation and impaired myocardial systolic function. Tissue Doppler imaging (TDI) has been developed to assess myocardial wall velocities and might therefore be expected to be a useful tool in the diagnosis of DCM. The aim of the study was to ascertain whether TDI variables differ between DCM dogs and normal control dogs. ANIMALS, MATERIALS AND METHODS: Eighty-three client-owned dogs, 37 with DCM and 46 control dogs, were included in the study. A prospective observational study was conducted. RES

Progressive Dilated Cardiomyopathy in a 24-Year-Old Man: A Rare Manifestation of Propionic Acidemia.

BACKGROUND: Adult onset propionic acidemia (PA) is exceedingly rare, particularly when manifesting as dilated cardiomyopathy. CASE SUMMARY: A previously healthy 24-year-old man presented with cardiac dilatation and acute progressive heart failure. His condition was refractory to guideline-directed medical therapy, deteriorating to cardiogenic shock within 6 weeks. Plasma acylcarnitine profile revealed elevated propionylcarnitine and an increased propionylcarnitine-to-acetylcarnitine ratio. Urinary organic acid analysis showed elevated methylcitric acid. Whole-exome sequencing identified a hom

Discordance Between Gated Single-Photon Emission Computed Tomography Myocardial Perfusion Imaging and Coronary Angiography in Dilated Cardiomyopathy.

Background Gated single-photon emission computed tomography myocardial perfusion imaging (SPECT-MPI) provides functional information that may help distinguish ischemic from nonischemic dilated cardiomyopathy (DCM). However, its classification may differ from that of coronary angiography (CAG). Examining these discordant cases may improve SPECT-MPI interpretation in patients with DCM. This study aimed to assess the pattern of discordance between gated SPECT-MPI and CAG in patients with DCM and describe the features of false-positive and false-negative SPECT-MPI findings. Methodology This cross-

A Common CD36 Variant and the Genetic Landscape of Dilated Cardiomyopathy in Individuals of African Ancestry.

IMPORTANCE: Dilated cardiomyopathy (DCM) is a major cause of heart failure that disproportionately affects individuals of predominantly African ancestry, among whom familial clustering of disease is more pronounced relative to those of predominantly European ancestry. Truncating variants in TTN, which encodes the sarcomeric protein titin, are the most common established monogenic cause of DCM. A common CD36 nonsense variant enriched among individuals of African ancestry has also been shown to account for a substantial burden of DCM. OBJECTIVE: To compare the contribution of a CD36 nonsense va

Achieving Reliable Peripheral Nerve Blockade in Cardiogenic Shock With Distorted Femoral Anatomy: A Case Report.

Patients with cardiogenic shock and severe structural heart disease may poorly tolerate both general and neuraxial anesthesia. In selected cases, peripheral nerve blockade may reduce hemodynamic disturbance, but the block must be reliable when pain, sympathetic stimulation, or emergency conversion to general anesthesia could be poorly tolerated. We report a 62-year-old woman with cardiogenic shock, low-flow, low-gradient severe aortic stenosis, advanced dilated cardiomyopathy, pulmonary edema, orthopnea, and active femoral bleeding who required urgent femoral artery and vein repair. Doxorubicin-induced Cardiotoxicity in Dogs: A Narrative Review.

Doxorubicin, an anthracycline widely used in both human and veterinary oncology, is highly effective against a variety of malignancies in dogs, including lymphomas, sarcomas and carcinomas. Despite its efficacy, doxorubicin exposure can be associated with cardiotoxicity, which may present acutely or chronically, potentially leading to severe cardiac arrhythmia and congestive heart failure. Mechanisms underlying cardiotoxicity are multifactorial, including oxidative stress, mitochondrial dysfunction, DNA damage via topoisomerase IIβ, inflammation and dysregulated cell death pathways such as apo
